3.Effect of vitrectomy timing on long-term efficacy in patients with vitreous hemorrhage due to proliferative diabetic retinopathy

AIM:To investigate the timing and efficacy of vitrectomy for patients with vitreous hemorrhage(VH) due to proliferative diabetic retinopathy(PDR).METHODS:Retrospective analysis.Patients who presented to our hospital between Feburary 2012 and May 2014 with VH secondary to PDR treated with vitrectomy were included.All patients were divided into three groups according to the duration of VH.A group was less than 1mo for 22 eyes, B group was 1-3mo for 23 eyes, C group was more than 3mo for 25 eyes.All patients underwent intravitreal injection of ranibizumab 1-2wk before vitrectomy, and supplemented or finished panretinal photocoagulation (PRP) intraoperatively or postoperatively.Patients with cataract accepted phacoemulsification and intraocular lens implantation.Eyes filling silicone oil were implanted intraocular lens in the second phase.All patients were followed up 24 to 42mo (mean:28.7mo).We assessed the intraoperative complications such as hemorrhage, iatrogenic retinal hole, and postoperative complications such as vitreous hemorrhage, neovascular glaucoma.Macular edema and best corrected visual acuity were observed at every follow-up.RESULTS:There was no significant difference for other baseline data (P>0.05) but DR stage between three groups (P=0.033).There was significant difference of last follow up visual acuity between three groups (P<0.001).The significant difference can be seen between group A and B (P=0.03).The same outcome showed between Group A and C(P<0.001).There was no significant difference between Group B and C (P>0.05).The percentage of visual acuity was 0.5 and above in the three groups were:41%, 23%, 0 respectively.The patients with visual acuity of less than 0.1 were 5%, 26% and 40% respectively.Silicone oil filling rate of three groups were:9%, 26%, 40% respectively and there was no significantly difference between three groups on postoperative complications (P>0.05).CONCLUSION:Patients with VH due to proliferative diabetic retinopathy undergoing early vitrectomy may get better visual acuity than who accepting delayed vitrectomy.
4.Application of Element and Metabolism Balancing in the Production Phase of Human-like Collagen

On the base of element and metabolism balancing,the mathematic model of the human-like collagen expression phase with recombinant Escherichia coli BL21 was developed and the unknown parameters in the model were estimated with the method of nonlinear optimization.The model was in agreement with the growth kinetics and the metabolic kinetics,and the key calculated parameters of ?h,?p and mx were 1.173 mol?C-mol-1,293.814 mol?C-mol-1 and 17.878 mol?C-mol-1?h-1 respectively.This model could preferably predict the macroscopic reaction rates,and in the synthesis phase of human-like collagen,the specific growth rate should be controlled at 0.04 h-1 with controlling glucose feeding rate to gain the highest specific production rate of human-like collagen.
5.Significance of visual electrophysiological techniques in the early diagnosis of glaucoma

Glaucoma is a group of diseases which can threaten and damage the optic nerve and its visual pathway, leading to visual impairment as a result. Glaucomatous optic neuropathy is a chronic disease accompanied by apoptosis of retinal ganglion cells ( RGCs ) , visual field defect and cupping of optic nerve head. The gold standard in functional glaucoma evaluation is standard automated perimetry ( SAP) , but it is often limited to the subjective feelings of the patients. Still, visual electrophysiological techniques cannot replace the conventional inspection, but with its rapid development, it has provided a new strategy for the early diagnosis of glaucoma as a supplement because it can show changes in amplitude and latency before visual field defect. Here we review three special electroretinograms and multifocal focal visually evoked potentials in the early diagnosis of glaucoma.
6.Observation on clinical efficacy of activating renal blood circulation and ovarian stimulation formula in treating ovulation failure infertility.

OBJECTIVETo discuss the clinical efficacy of the activating renal blood circulation and ovarian stimulation formula in treating ovulation failure infertility.
METHODEighty-six cases were randomly divided into two groups: the treatment group and the control group. The treatment group is administered with the activating renal blood circulation and ovarian stimulation formula (composed of 15 g Cuscutae Semen, 15 g Dipsaci Asperoidis Radix, 15 g Lycii Fructus, 15 g Spatholobi Caulis, 10 g Ligustri Lucidi Fructus, 15 g Lycopi Herba, 10 g Typhae Polleu, 10 g Angelicae Sinensis Radix, 15 g Cyathulae Radix etc.), whereas the control group was given clomiphene.
RESULTThe treatment group showed a pregnancy rate of 58.14%, with an ovulation rate of 68.6%. While the control group showed a pregnancy rate of 36.67%, with an ovulation rate of 70%.
CONCLUSIONThe comparison between the two groups showed that the activating renal blood circulation and ovarian stimulation formula was significantly different from clomiphene in statistical analysis (P < 0.05), without notable difference in the ovulation rate. Before and after the treatment, there is no significant difference in diameter of dominant follicles between the two groups, with remarkable difference in endometrium (P < 0.05).

7.Clinical observation of the effect of Xuesaitong soft capsule on post-hepatitis fibrosis.

OBJECTIVETo study the effect of Xuesaitong soft capsule (XST) on liver fibrosis criteria in patients with post-hepatitis fibrosis.
METHODSSixty-four patients with such fibrosis were randomly divided into the treated group and control group. They were treated with XST and Dahuang Zhechong pill (NFDA1) for 3 months respectively. Their liver fibrosis criteria were examined before and after treatment.
RESULTSThe levels of serum procollagen III, hyaluronic acid, collagen IV, laminin in the two groups were significantly lower (P < 0.01) than those before treatment. The differences between the two groups were insignificant (P > 0.05).
CONCLUSIONXST could recover liver dysfunction and had anti-liver fibrosis function.

8.Phospholipase Cγ1 and NF-κB are required for cell-matrix adhesion of colorectal cancer cells

Objective: To investigate the function and mechanism of phospholipase Cγ1 (PLCγ1) in cell-matrix adhesion in colorectal cancer. Methods: Highly metastatic colorectal cancer cell line LoVo and lowly metastatic colorectal cancer cell line SW480 were subjected to cell-matrix adhesion assay. U73122 (a specific inhibitor of PLC) and pyrrolidine dithiocarbamate (PDTC) (an inhibitor of NF-κB) were used to study the effect of PLCγ1 and NF-κB on cell-matrix adhesion. Furthermore, Western blot and gel electrophoresis mobility shift assay (EMSA) were performed to detect the mechanism of PLCγ1 in colorectal cancer cell adhesion to matrix. Results: Inhibition of PLCγ1 or NF-κB resulted in reduction of cell-matrix adhesion in a dose-dependent manner in LoVo cells(P<0.05), but had no marked effect on SW480 cells. Western blot analysis showed that epidermal growth factor (EGF) stimulated the phosphorylation of PLCγ1 in LoVo. The results of EMSA indicated that inhibition of PLCγ1 signaling pathway also down-regulated the activity of NF-κB while EGF reversed the function. Conclusion:These data suggest that PLCγ1 plays a pivotal role in the EGF-induced cell-matrix adhesion of highly metastatic colorectal cancer cells and that NF-κB is also functional in this signaling pathway.
9.Lung cancer screening with low-dose CT: a study on risk factors and establishing the high-risk model for lung cancer

Objective To establish standards of high-risk populations for low-dose CT(LDCT)lung cancer screening projects by analyzing lung cancer risk factors. Methods LDCT was performed in 6 990 subjects undergoing physical examinations in Changzheng Hospital from September 2013 to September 2016, including 4 567 males and 2 423 females. Mineralization nodes, solid nodules, pure ground glass nodules and partial solid nodules were defined as positive results.All cases of lung cancer were confirmed by pathological examination. The morbidity and risk factors of lung cancer were analyzed using t-test, chi-square test and logistic regression analysis.And there were 13 risk factors involved,including age,sex, body mass index(BMI),smoking,the family history of lung cancer and other malignant tumors,the history of chronic bronchitis or chronic obstructive pulmonary disease,the history of pulmonary fibrosis,the history of tuberculosis, the history of other malignant tumors, the history of cardiovascular disease, the history of second hand smoke exposure,the history of exposure to radon and the history of exposure to asbestos.The cut point of risk factors was analyzed according to the receiver operating characteristic (ROC) curve. Results Sixty-nine cases of lung cancer and 85 malignant nodules were detected in this screening.There were 34 males and 35 females among the lung cancer patients and their age ranged from 24 to 88 years with an average age of(60±16)years.In all the factors involved in this study,sex(χ2=7.937),age(t=19.509,χ2=29.991) and cardiovascular disease (χ2=5.333) were proved to be the risk factors of lung cancer by single factor analysis.Logistic regression analysis showed that sex(OR=0.478,95%CI:0.297-0.769,P=0.002)and age(OR=1.024,95%CI:1.024-1.060,P=0.001)were independent risk factors of lung cancer.ROC analysis showed an area under the curve(AUC)of 0.62 for age,0.587 for age of males and 0.659 for age of females. According to ROC curves, people over 56.5 years old (sensitivity 55.1%, specificity 75.0%, accuracy 74.8%),males over 56.5 years old(sensitivity 52.9%,specificity 73.4%,accuracy 73.3%)and females over 57.5 years old(sensitivity 57.1%,specificity 80.5%,accuracy 80.1%)were high-risk groups.Conclusions In this study,sex and age were proved to be the risk factors of lung cancer.Males over 56.5 years old and females over 57.5 years old are more likely to suffer from lung cancer.However,this study is a single center study,so that other risk factors of lung cancer are not ruled out completely.
10.Efficacy of pegylated interferon-alpha-2a plus ribavirin for patients aged at least 60 years with chronic hepatitis C.

BACKGROUNDIn China, patients with hepatitis C virus (HCV)-associated liver disease are getting older, and thus the number of deaths due to such disease is increasing. The efficacy of combination therapy with ribavirin and interferon for chronic HCV infection in elderly patients has not been fully clarified. The aim of the present study was to evaluate the efficacy and tolerability of the combination therapy in the elderly patients.
METHODSSixty-eight chronic hepatitis C patients, who received the combination therapy, were classified into two age groups: elderly group ((3)60 years, n = 25) and non-elderly group (< 60 years, n = 43). Rapid virological response, complete early virological response, sustained virological response, relapse, non-response rate, and safety were compared between the elderly group and non-elderly group.
RESULTSOverall sustained virological response was lower in the elderly group than non-elderly group (44% vs. 75%, P = 0.012, OR = 0.270, and 95%CI 0.095 - 0.768). Among patients with HCV genotype 1, sustained virological response was lower in the elderly group than non-elderly group (45% vs. 77%, P = 0.015, OR = 0.247, 95%CI 0.078 - 0.781). The proportions of dose reduction due to laboratory abnormalities were significantly higher in the elderly group than non-elderly group (60.0% vs. 32.6%, P = 0.027). Multiple binary Logistic regression analysis confirmed that patient age was an associated factor for sustained virological response.
CONCLUSIONAmong patients with HCV genotype 1, the elderly patients had lower sustained virological response than non-elderly patients during pegylated interferon-alpha-2a plus ribavirin combination therapy.
