The authors give an account of the changes in both internal and external environments that hospitals confront in the new century, emphasizing that it is imperative for hospital managers to manage their hospitals with new concepts and means so as to meet the needs of the times. The paper is focused on 10 ways of management, namely, knowledge management, human resources management, operational management, efficiency and benefit management, new medical quality content management, fuzzy management, information management, image management, humanistic management, and hospital culture management.

With the development of the times and the constant change of the forms of labor, it is imperative to explore the value of labor and principle of distribution of medical workers. The authors first describe a number of relevant concepts, including instruments of labor, forms of labor, value of labor, and human resources. Then they analyze the value of labor of medical workers and their administrators, including the features and forms of the creation of value by medical workers and their administrators. Finally, they expound the importance of deepening reform in the distribution system of medical institutions. By analyzing the theoretical basis for readjusting the distribution mechanism, they put forward the principles of giving expression to the creation of the value of labor in readjusting the distribution system and taking efficiency, capital and production factors into account in distribution and offer suggestions respectively for the reform of the distribution system in profit and non profit medical institutions.

Firstly the authors expound the management attribute of hospital management ethics, clarifying the connotation of hospital management ethics, the basic issues in the study of hospital management ethics, and the eight functions, tasks and goals of hospital management ethics. Secondly they discuss the relationship between hospital management and hospital management ethics. Thirdly they offer a preliminary account of the application of hospital management ethics in hospital management: ①illustrating via a diagram the application of hospital management ethics in hospital decision-making; ②presenting ways of judging “good” and “evil” activities in the hospital; and ③describing the application of hospital management ethics to the monitoring and assessment of medical programs. Finally they propose some methods for cultivating the capability of the entire staff of the hospital to fulfill the obligations of moral excellence.

Objective To explore the effect of different diary method on daily affect of college students.Methods 75 numbers of under-graduates were randomly assigned into information recording group,feeling recording group and control group.Beck Depression Inventory (BDI) and Positive And Negative Affect Scale (PANAS) were used before and after two months intervention.Results For positive emotion,ANOVA results showed no significant (P＞0.05) main effect of time,a significant (P＜0.05) main effect of group(2.76±0.41,2.97±0.50,3.24±0.45),and the time × group had a significant interaction (P＜0.05).Simple effect analysis showed that after the intervention,the positive emotions of information recording group (2.47 ± 0.62) and feeling recording group (2.86±0.64) was lower than the positive emotions of control group(3.35±0.46) significantly(P＜0.05).For negative emotions,there was no significant difference (P＞0.05) in main effects of time,group and interaction;for depression,the main effect of the level of time (8.02± 5.65,5.38 ± 5.04,5.20± 5.51) was significant (P＜ 0.01),and main effect of group and the interaction were not significant(P＞0.05).Conclusion Different diary methods have different effects on the subject's daily affect.

Objective To study the influence of exposure factors on posttraumatic stress disorder( PTSD) and Posttraumatic Growth( PTG) in middle school students in disaster area four years after the Wenchuan earth?quake . Methods 1 526 students from four schools in Worst?Hit Areas were investigated with Self?compiled Earth?quake Exposure Factors Questionnaire,Posttraumatic Growth Inventory( C?PTGI) and Impact of Event Scale( IES?R). Data were analyzed by ANOVA and multiple linear regression analysis.Results The score of IES?R had sig?nificant difference between different levels of all exposure factors(F=5.75~89.10, P<0.05) ,and students with high exposure level((26.68±14.66),(26.80±15.56),(27.83±14.62),(29.02±15.36),(27.77±15.74),(26.74± 15.63),(25.43±14.32),(29.51±14.36)) had heavier symptoms of PTSD than those with low exposure level ((22.84±13.96),(23.98±13.99),(23.63±14.21),(23.53±13.96),(23.64±13.83),(24.24±14.15),(21.27± 14.35),(17.54±13.34)). Only exposure factors of having witnessed someone injured and having close friends se?riously injured or being killed could significantly influence the score of PTGI(F=11.82, P=0.001;F=6.23, P=0.013). Regression analysis showed that five exposure factors (grade,having felt scared,having family members being killed,having close friends seriously injured or being killed,having witnessed someone injured) had signifi?cant effect on IES(ΔR 2=0.141) ,but only one factor( having witnessed someone injured) had weak effect on PTG (ΔR 2=0.007).Conclusion Exposure factors can predict posttraumatic stress symptoms in middle school students in Wenchuan four years after the earthquake,and the emotion of fear is a strongest predictor,but they can not pre?dict posttraumatic growth.

Objective:To investigate the protective effects of Panax notoginseng on myocardial ischemia injury in dogs and study the spectrum-activity relationship of Panax notoginseng. Methods:Firstly, the HPLC fingerprint analytical method for Panax notogin-seng was established, and then the dog model of acute myocardial ischemia was established by left anterior descending coronary liga-tion. Bivariate correlation analysis and multivariate regression analysis were used to correlate the spectrum-activity relationship between the fingerprints and the anti-myocardial ischaemia activity, and the spectrum-activity relationship and efficacy material foundation of Panax notoginseng were determined. Results:The main effective components were Ginseng saponin Rg1 and Rb1 and notoginseng sapo-nins R1 etc. Notoginseng saponins R1 could significantly inhibit the increase of serum lactate, and ginseng saponin Rg1 could inhibit the increase of FFA in serum, which was the main component in Panax notoginseng for the treatment of myocardial ischemia. Conclusion:The effective substances in Panax notoginseng are obtained by investigating the relationship between the spectrum and efficiency, and a new method for the evaluation of spectrum-activity relationship for Panax notoginseng is established. It can objectively reflect the inher-ent quality of the drug and provide a new strategy for the further research of traditional Chinese medicines.

Objective To investigate the function of triggering receptor expresses on myeloid cells receptor-1 (TREM-1) in lymphocyte differentiation and regulation of Aspergillus infected immunosuppressed rats.Methods Cyclophosphamide (CTX) was intraperitoneally injected and Fumigatus spore suspension was inhaled by percutaneous tracheostomy to establish the immunosuppressive invasive pulmonary aspergillosis (IPA) rat model.After 24 h, 48 h, 72 h and 96 h inoculation, rats were sacrificed.Lung tissue specimens, bronchoalveolar lavage fluid (BALF) , and plasma samples were collected.Plasma and BALF sTREM-1, plasma T cell-specific transcription factor (T-box expressed in T cells, T-bet) and eomesodermin(Eomes) were detected by ELISA.Biopsy specimens of lung tissue were used for periodic acid-schiff (PAS) staining and culture.Results The mortality rate of immunosuppressed rats after Aspergillus inhalation for 96 h was as high as 54.4％.Biopsy of lung tissue suggested acute inflammatory cell infiltration, interstitial lung congestion, alveolar structural damage, and visible Aspergillus hyphae in alveoli.Compared with normal control group[(110.50 ± 7.70)ng/L], plasma sTREM-1 in study groups were significantly increased [IPA : (146.77 ± 10.41) ng/L;CXT + IPA at 24 h : (226.00 ± 11.88) ng/L;CTX + IPA at 48 h : (200.77 ± 10.63) ng/L;P ＜ 0.05], so were T-bet levels [IPA : (561.17 ± 7.23) μg/L;CXT + IPA at 24 h : (647.00 ± 33.03) μg/L;CTX + IPA at 48 h : (619.23 ± 87.44) μg/L;control group : (340.03 ± 26.32) μg/L;respectively, P ＜0.05].However, plasma Eomes levels in IPA group, CTX + IPA at 24 h and 48 h were significantly lower compared with that in normal controls [IPA : (7.96 ± 0.65) ng/L;CXT + IPA at 24 h : (3.97 ± 0.35) ng/L;CTX + IPA at 48 h : (4.00 ± 0.74) ng/L;control group : (8.38 ± 0.51) ng/L;respectively,P ＜0.001].Compared with those in CTX + IPA vaccination after 24 h and 48 h, plasma sTREM-1 [(106.67 ±7.64)ng/L;(133.27 ± 32.79) ng/L] and T-bet [(299.64±63.07)μg/L;(398.02 ± 109.22) μg/L] in CTX + IPA at 72 h and 96 h inoculation were significantly lower (P ＜ 0.001).While Eomes [(8.38 ± 0.54) ng/L;(8.40 ± 0.70) ng/L] raised significantly higher (P ＜ 0.001).Compared with the control group, sTREM-1 levels in BALF of IPA + CTX 24 h, 48 h, 72 h, and 96 h groups were consistently high (P ＜ 0.05).Pearson correlation analysis showed that sTREM-1 and T-bet had a significant positive correlation (r =0.91, P ＜ 0.001), yet Eomes was negatively correlated with them (r =-0.788, P ＜ 0.001).Conclusions sTREM-1 in rat plasma and BALF appears highly expressed in immune compromised Aspergillus infected rat model.Plasma sTREM-1 is closely correlated with T-bet and Eomes levels, which suggests that TREM-1 may be involved in lymphocytic regulation and differentiation during fungal infection.

Objective To investigate the career decision-making self-efficacy as well as the psychological well-being of medical students and to discuss the correlation between them.Methods A cross-sectional survey with cluster random sampling was performed anonymously among 714 students of different majors and different grades in an medical University.The contents of the survey included the career decision making self-efficacy scale (CDMSE) as well as the self-reporting inventory(SCL-90).All data in the two groups were analyzed with t test; comparison of data between groups were analyzed with One-Way ANOVA; multiple comparison was analyzed with S-N-K; Pearson and Partial correlation analysis were used to analyzed the correlation between the career decision making self-efficacy and wellbeing.Results Totally 659 students completed the inventory,with response rate being92.3％.Average score of the participants' career decision-making self-efficacy was (124.72 ± 26.76)points,situating at the middle and lower level.There were differences in the scores of career decision making selfefficacy among different sexes,sources,grades and being student cadres or not (P =0.019,0.000,0.027,0.026) ; No differences was found among students of different majors and whether being the only child(P =0.243,0.223).Results of the SCL-90 inventory demonstrated that medical students had some psychological problems including obsessive-compulsive,interpersonal sensitivity and anxiety.Correlation analysis showed that psychological well-being and career decision-making self-efficacy was negatively correlated.Conclusions Measures should be taken to improve the medical students' career decision-making self-efficacy according to their psychological characteristics and to improve the medical students' psychological status by the improvement of career decision-making self-efficacy.

Objective To study the effect of mixed purified autogenic and allogeneic hematopoietic stem cell transplantation for the treatment of systemic lupus erythematosus. Methods Thirty-six MRL/lpr mice were randomly divided into the control group, the study group,the mixed group ( the ratio of autogenic to hematopoietic stem cells, mixed in different proportions were infused intravenously after 60Co irradiation. The study group were treated with daily intraperitoneal infusion of dexamethasone 1 mg·kg-1·d-1, while the control group were treated with intraperitoneal infusion of equivalent volume of saline daily. The changes of serum creatinine level, the urine protein excretion of the mice and blood WBC count were compared. Repeat measures ANOVA was used for data analysis. ELISA was used for anti-nuclear antibody detection Light microscopy, electronic micros-copy, immunofluorescence were applied to detect the pathological changes in renal tissue. Results Serum creatinine and urine protein excretion levels increased with time in the ontrol group, while those of the transplant group and the study group decreased. The reduction in mixed transplantation group and the study group was more evident compared with that of the allogeneic group. The difference was statistically significant (P＜0.05), but there was no significant difference between the mixed transplantation groups and the study group (P＞0.05). The histopathologic damage was most serious in the control group as pathological injury score of most mice were in grade 3 or 4. The majority of the histopathologic damage of the allogeneic group was in grade 2. Most f pathological damage of the study drug group and the mixed transplantation group were grade 1 or 2. Conclusion Mixed hematoopoietic stem cell transplantation for the treatment of murine systemic lupus erythematosus can effectively correct heavy proteinuria in murine systemic lupus erythematosus so improve the renal damage. It is a safe and effectively way to treat murine systemic lupus erythematosus.
