2.Repair and reconstruction of breast after mastectomy

Objective To explore the procedures of breast repair and reconstruction after mastectomy with three methods for optimal appearances of the breast.Methods Three methods were adopted in this study:(1)Two-stage breast reconstruction operation with ampliate latissimus dorsi myocutaneous flap after mastectomy;(2)One stage breast reconstruction operation with transverse rectus adbominis myocutaneous(TRAM)flap after mastectomy;and(3)Two-stage breast reconstruction and nipple repair operation with tissue expander/implant after mastectomy preserving pectoralis major museulus or implants along under pectoralis major musculus.Results All of the flaps survived well and were well-shaped in total 10 patients(10 breasts)with a flap breast reconstruetion,in which the superior ratios were 83.33%.The appearance and volume were not satisfactory in 1 patiant treated with the method of tissue expander/implant,because the volume of tissue inflation was smaller than another one and 100 ml implant was applied.The partial flap necrosis occurred in 1 case with the method of TRAM flap.Conclusions With the development of plastic surgery technique,the appearances and shapes of reconstructed breast achieve the satisfactory effects.After the ampliate latissimus dorsi myocutaneous flap is applied,the appearance and volume of reconstructed breast are similar to the healthy one.The two-stage nipple repair operation is applied with breast reconstruction operation by the tissue expander/implant after mastectomy preserving pectoralis major musculus.According to the different methods of mastectomy to choose suitable procedures of breast reconstruction,the satisfactory appearances and psychotherapy will be achieved.
3.Plastic treatment of giant neurofibromatosis on body surface

Objective To study the treatment method of giant neurofibromatosis on body surface.Methods Replantation of the tumor skin,full-thickenss skin grafting and transferring of expanded flap were used to reconstruct the appearance and function after the excision of giant neurofibroma.Results Sixteen cases gained primary healing while other 2 cases with delayed healing after onestage or fractionated resection of giant neurofibroma on body surface.All the 18 patients resumed selfcare and working ability.Conclusions Giant neurofibromatosis has significant damage to the appearance and body shape and also greatly limits the patient's life and work.Patients can resume physical and psychological rehabilitation by surgical resection and plastic surgery treatment.
4.Current advance in the research of related influencing factors of myopia

Myopia is not only a global public health problem, but also a significant socio-economic problem. There are various hypotheses about the pathogenesis of myopia, which is basically the result of the combination of environmental and genetic factors. Although a large number of epidemiological studies have been carried out on the influencing factors of myopia, most of them are cross-sectional studies, longitudinal cohort studies are relatively few. This paper will summarize the influencing factors of myopia at homeland and abroad in recent years.
5.Metabolites of sphingolipids as targets for cancer therapy

Sphingolipids are structural components of cellular membranes.Their metabolites,such as ceramide,sphingosine,and sphingosine 1-phosphate are important bioactive molecules,which act as the first or second messengers regulating various cellular activities,including proliferation,survival,migration and neovascularization in vitro and in vivo tumor models.Sphingosine kinases 1(SPK1) is a critical regulator of the balance between ceramide and S1P.Recent studies show that ceramide,SPK1 and S1P can regulate many of the hallmarks of cancer.These studies suggest that these metabolites can serve as novel targets for cancer therapy.
6.Study on oral absorption enhancers of astragalus polysaccharides.

Astragalus polysaccharides was lounded to 4-(2-aminoethylphenol), followed by labeling the APS-Tyr with fluorescein-5-isothiocyanate (FITC) at the secondary amino group. The absorption enhancement effects of low molecular weight chitosan and protamine on astragalus polysaccharides were evaluated via Caco-2 cell culture model. The results show that the fluorecent labeling compound has good stability and high sensitivity. On the other hand low molecular weight chitosan and protamine also can promoted absorption of the astragalus polysaccharides without any cytotoxity, and the absorption increase was more significant with increasing the amount of low molecular weight chitosan and protamine. At the same time, the low molecular weight chitosan has slightly better effect. The transepithelial electric resistance (TEER) of Caco-2 cells show that absorption enhancers could improve its membrane transport permeability by opening tight junctions between cells and increasing the cell membrane fluidity.

8.Clinical study on low tidal volume ventilation in the treatment of children with acute hypoxia respiratory failure

Objective To explore the efficacy of low tidal volume ventilation strategy in children with acute hypoxia respiratory failure (AHRF).Methods A total of 79 hospitalized children with AHRF from Aug 2006 to Jul 2011 in PICU of Kunming Children's Hospital were enrolled in this study.The observation group in-cluded 55 children who received low tidal volume ventilation strategy (6-8 ml /kg),while the other 24 children (control group)were given traditional mechanical ventilation (10-12 ml /kg).Oxygenation situations such as PaO2 ,PaCO2 ,PaO2 /FiO2 ,oxygen index and blood gas pH value,organ function,mechanical ventilation complica-tions,hospitalization days and expenses in PICU and the mortality were observed.Results (1)PaO2 ,PaO2 /FiO2 and oxygen index in the observation group were better than those in control group after 24 h mechanical ventilation [(68.51 ±7.53)mmHg(1 mmHg =0.133 kPa)vs.(61.64 ±9.28)mmHg,(162.9 ±21.84)mmHg vs.(152.1 ± 19.03)mmHg,and 18.85 ±4.1 vs.26.53 ±5.2,respectively],and there were significant differences between two groups (P ﹤0.05);and there were also significant differences between two groups in the results after 48 h and 72 h mechanical ventilation.(2)The PaCO2 was (47.48 ±10.52)mmHg after 24 h in observation group,while the PaCO2 in control group was (30.17 ±6.59)mmHg,and it suggested excessive ventilation.(3)Mechanical venti-lation time (7.6 ±3.1)d and hospitalization days (12.8 ±3.6)d were shorter in observation group(P ﹤0.01). Barotrauma (7.3%)and mortality (20.0%)in observation group was significantly lower than those in control group (29.2%,41.6%;P ﹤0.01).The number of damaged organs in observation group was lower than that in control group (P ﹤0.05).Conclusion Low tidal volume ventilation with appropriate positive end expiratory pressure could improve oxygenation,prevent alveolar collapse,reduce complications and mortality for children with AHRF,it should be applied for the treatment of children with AHRF.
9.Comparison of overall development stages on traditional Chinese materia medica patent technology in China and abroad

Objective To analyze the overall developmental stages of traditional Chinese materia medica patent technology in China and abroad and the causes of the differences. Methods In this study, the Shanghai Intellectual Property Information Platform, Thomson Innovation, Derwent Innovations Index were utilized and graphic illustration method for patent technology life cycles was used to determine the overall developmental stages of traditional Chinese materia medica patent technology in China and abroad. Results The numbers of applications for patent traditional Chinese materia medica and their applicants were obtained. With information of the two parameters, the text made a preliminary prediction of overall trend for the number of applications. Conclusion The patent technology of traditional Chinese materia medica progressed into developing stages in China and appeared the feature of mature or recession stage abroad.
