1.Effect of txiplenygium wilfordii hook f on contrasuppressor T cells in SLE nephritis patients

Contrasuppressor T cell were analysed in 19 cases of lupus nephritis patients(6 cases weretreated with Txiplenygium Wilfordii Hook F 30mg/d,7 cases were treated with prednisone30mg/d,6 cases were untreated).Results showed that the content of CD_8~+VV~+ cells in lupusnephritis patients were higher than that in normal controls,and after taking Txiplenygium Wil-fordii Hook F the content lowered significantly,there existed significant differenec between thepatients treated with Txiplenygium Wilfordii Hook F and the patients untreated.We speculatedthat to lower the content of CD_8~+VV~+ cells may be one of the pharmacological effects of Txi-plenygiu Wilfordii Hook F.

The CCN family involves a group of six secreted proteins that specifically associate with the extracellular matrix.Cyr61 is the first cloned gene of the CCN family.CYR61 can promote cell growth,adhesion,migration,differentiation,apoptosis and extracellular matrix production.The synthesis of CYR61 is highly inducible by serum growth factors,cytokines,and environmental stresses.CYR61 can also modulate the activities of several growth factors and cytokines,including transforming growth factor-β(TGF-β),tumor necrosis factor-α(TNF-α),vascular endothelial growth factor(VEGF),bone morphogenetic proteins(BMPs)and Wnt proteins.Cyr61 is an important regulator in angiogenesis,osteogenesis,and chondrogenesis,and plays an important role in embryonic development,wound healing,tumor formation and development.It is reported that CYR61 plays a role in the course of ocular neovascular diseases.It can promote the proliferation,migration and tube formation of retinal endothelial cells in vitro.Expression of Cyr61 is upregulated in the retinas of diabetic rats and the vitreous of proliferative diabetic retinopathy (PDR)patients.CYR61 is likely to be involved in the pathogenesis of diabetic retinopathy(DR).Research progress on CYR61 in recent years is reviewed in this article.
3.S-ROM prosthesis total hip replacement for osteoarthritis secondary to developmental dysplasia of hip: Follow-up analysis of 57 cases with 63 hips

BACKGROUND: The optimal total hip replacement can be prepared by assembling S-ROM prosthesis individually. OBJECTIVE: To evaluate the short-time effect of S-ROM prosthesis total hip replacement for osteoarthritis secondary to developmental dysplasia of hip. METHODS: Totally 57 adult patients (63 hips) with osteoarthritis secondary to developmental dysplasia of hip treated by S-ROM prosthesis for primary total hip arthroplasty from November 2002 to February 2007 were selected. The preoperative and postoperative Harris score were compared. The prosthesis loosening, fracture and subsidence, bone absorption and fracture were followed-up with radiographic results. RESULTS AND CONCLUSION: All patients were followed up from 6 months to 5 years, mean 3.5 years. One patient suffered temporary common peroneal nerve palsy, and recovered after 4 months. One patient suffered hip dislocation, and recovered after manipulative reduction and traction. There were no complications in the rest patients. The Harris hip score was 70.20-92.10, mean 88.30, which was obvious increased than that of before operation (P < 0.05). No loosening, fracture and subsidence of prosthesis, bone absorption or fracture was observed. The results demonstrated that S-ROM prosthesis total hip replacement can receive an excellent short-time effect in treating osteoarthritis secondary to developmental dysplasia of hip.
4.Experimental observation of a highly sensitive method for nerve pathway tracing

BACKGROUND: Nerve fiber degeneration and horseradish peroxidase antrograde tracing methods are commonly used to study nerve pathway. However, both the methods cannot mark the area clearly and lack of sensitivity so that they cannot fully present the projection area of nerve fibers. It needs further efforts on developing new nerve pathway tracing methods to effectively research nerve pathway.OBJECTIVE: To look for a highly sensitive method of nerve pathway tracing in order to provide a simple, objective and reliable method to study regeneration.DESIGN: A randomized and controlled trial based on experimental animals.SETTING: Department of Internal Medicine of a hospital and the Experiment technique teaching section of a university.MATERIALS: The experiment was completed in the Experiment Technique Teaching Section of Chengdu Medical College. Six healthy male SD rats provided by Experimental Animal Centre of West China Medical University were used.METHODS: Using CB-HRP as tracer while using sodium tungstate as stabilizer of TMP to antrograde tracing the projection of optic nerve fibers on anterior region of hypothalamus and retrograde tracing the distribution of hypoglossal nerve fibers on the dorsal side of medulla.MAIN OUTCOME MEASURES: Distribution and dyeing of neuron bodies and nerve fibers.RESULTS: The fibers of optic nerve projected to the dorsomedial side of suprachiasmatic nucleus and the fibers of hypoglossal nerve projected on the hypoglossal nucleus on dorsomedial portion of the medulla oblongate. The result shows that the neurons and axons are very clear.CONCLUSION: It is a highly sensitive method to trace nerve pathway by using CB-HRP as tracer and sodium tungstate as stabilizer of TMB.
5.TCM Bushen Shugan Herbs Combined with Clomiphene Citrate Therapy of Promoting Small Follicular Development

This study was aimed to observe the therapeutic effect of Bushen Tiaogan of traditional Chinese medicine (TCM) herbs combined with clomiphene citrate therapy of promoting small follicular development. A total of 70 small follicle ovulation infertility cases were randomly divided into the control group of 35 cases with the treatment of clomiphene citrate of 50 to 100 mg for 5 days, and 35 cases in the treatment group with TCM herbal medicine of Radix Polygoni Multiflori, Lycium c hinense , eucommia, bupleurum root, turmeric, Fructus aurantii to nourish the kidney and regulate the liver. The clinical efficacy and maximum follicular diameter (MFD) were observed. The results showed that the cycle pregnancy rate in the treatment group was 43 . 59%, and 35 . 13% in the control group. The curative effect of treatment group was obviously superior to the control group. There was statistical significance in the curative effect ( P < 0 . 05 ) . The incidence of LUFS was 7 . 69% in the treatment group , and 29 . 73% in the control group . There was statistical significance ( P < 0 . 01 ) . The MFD value ( 18-28 mm ) in the ovulation cycle, the cycle pregnancy rate was 44.44% in the control group and 55.17% in the treatment group. There was statistical significance difference ( P < 0 . 05 ) . The early abortion rate was 18 . 75% in the control group and 6.25% in the treatment group. There was statistical significance difference (P < 0.01). It was con-cluded that Bushen Shugan Y angyin TCM herbs combined with clomiphene citrate can promote small follicular development with obvious clinical curative effect .
6.Value of ~(99m)Tc-MDP bone imaging in early rheumatoid arthritis

Objectives To investigate the clinic value of 99 Tc-MDP in the diagnosis of rheumatoid arthritis(RA).Methods The whole body bone imaging of 99 Tc-MDP were performed in 31 patients with RA and 20 controls with other arthritis(AR group) and 19 normal controls.X-ray,rheumatoid factor(RF),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),joint tender index and joint swollen index were detected in RA patients.Results Abnormal bone images were observed in 29 out of 31 patients, with a 93.5 % of coincidence in RA group, 80% and 10.5% of in AR group and normal group. 201 out of 226 (88.9%) patients with joint symptoms showed abnormal bone images. 95% abnormal bone image rate and 20% abnormal X ray rate were found in 20 early RA patients. The joint radiopharmaceutical uptake score correlated positively with JTI, JSI, CRP and ESR. Conclusions 99m Tc MDP bone imaging is a sensitive, functional, noninvasive but unspecific method in detecting RA. And in combination with clinical symptoms, signs and serological changes, it is of value to the diagnosis of early RA.

8.Role of fast-track recovery in pancreatic neoplasm surgery

Fast-track recovery emphasize applicating a series of effective measures confirmed in the perioperative care of patients,the optimized combination generates synergy effect to maximize alleviating various physical and mental stress reaction in order to expedite patient recovery.Most successful example is the application of fast track colon surgery program.Its safety and effectiveness have been widely confirmed,but the report on pancreas surgery is limited.FTRP is aiming to improve medical and health work efficiency,reduce hospitalization time and hospitalization expenses,improve life quality of patients.Based on the application of FTRP in pancreatic cancer,the relation of morbidity,re-admission and peri-operative mortality rates and clinical significance are reviewed.
9.Experience in treatment of post-ERCP peripancreatic and retroperitoneal abscess formation with non-operation management

Objective To investigate the technique and clinic value of non-operation management for ERCP-related peripancreatic and retroperitoneal abscess. Methods Five patients with post-ERCP peripancreatic and retroperitoneal abscess were reviewed. Guided by ultrasonic or CT, all the 5 pa-tients underwent puncture and the drainage tube was disposed to the lowest place of abscess. Non-op-eration managements for patients also included the use of anti-inflammatory drugs and enzyme activity inhibition drugs. Results All patients responded to the draining treatment and discharged from hospi-tal after complete recovery. There was no conversion to surgical intervention. Mean draining duration was 52. 4(20-90)d and average hospital stay was 91.8(35-165)d. Conclusion Puncture and drainage management is an effective and safe approach for post-ERCP peripancreatic and retroperitoneal ab-scess. It has advantages of less trauma, less pain, fast recovery and low rate of complications. Punc-ture point should be situated at the bottom or lowest position of abscess and drainage can achieve the best results.
10.The effects of bisphosphonate on the peroxisome proliferator-activated receptor γ expression of glucocorticoid-induced osteoporosis

Objective To investigate the effects of bisphosphonate on the proliferator-activated receptor γ(PRARγ)expression of glucocorticoid-induced osteoporosis. Methods ① Twenty-two cases of systemic lupus erythematosus(SLE)patients with glucocorticoids(>0.5 mg·kg-1 ·d-1)were divided into 2 groups: 9 cases in the treatment group in which patients received alendronate 70 mg qw., 13 cases in the control group without anti-osteoporosis treatment. All of the patients had bone marrow puncture after 24 weeks, bone tissues were embedded, sliced, and the value of average optical density of PPARγwas tested with immunohistochemistry. ② Human bone mesenchymal stem cells were cultured in the same conditions and stimulated to adipocytes. During the adipogenic process, the cells were divided into four groups, three of which were experimental groups treated with 10-5 mol/L(high-dose group), 10-7 mol/L(medium-dose group)and 10-9 mol/L(low-dose group)bisphosphonate respectively, while the other was the control group without bisphosphonate in the medium. The adipocytes were identified by oil Red O stain seven days later. The adipocyte rate was measured by light absorption at 515 nm. The expression of PPARγ was measured by quantitative PCR. Comparisons between groups were tested by One-Way ANOVA analysis and t test. Results The average optical density of PPAR-gamma immunohistochemistry in the treatment group was less than the control group(P<0.05). Seven days later, the absorbance between adipogenic induction group and low-dose group was not significant(P=0.167). but the adipogenic absorbance between the high-dose group, middle dose group and the control groups(P values were 0, 0.041). There was significant difference in the quantitative PCR results between the high-dose group, middle dose group and the expression of PPARγ adipogenic induction group(P value was 0, 0.01), but there was no significant differences between the low-dose group and adipogenic induction group(P>0.05). Conclusion Effective concentrations of bisphosphonate can inhibit the expression of PPARγ which in turn lead to the suppression of hMSCs to the adipocytes. The glucocorticoid-induced osteoporosis is also depressed.
