2.Surgical treatment of major vessels invaded by tumor

Objective To study the surgical treatment of major blood vessels invaded by tumors in order to raise the resection rate and postoperative survival rate. Methods Clinical data were analyzed on 26 patients undergoing (resection) of tumors along with reconstruction of major blood vessels from October 1998 to February 2004. Results Tweety-three cases were followed up for 2~65 months with median 42.8 months. Doppler ultrosonography and CTA examination were performed respectively during the follow-up process.In 5 patients with carcinoma of the head of pancreas who had inferior vein cava replaced and in 1 patient with cholangiocarcinoma who had portal vein replaced,3 were complicated with carcinoma embolism formation at 3,12 and 15 months respectively after surgery,(postoperative) graft patency was 50%;all of the 6 patients had recurrence of the tumor at 3,6,8,12,24 and 31 months respectively after surgery.Seven patients with tumor in pelvic cavity or in posterior space of peritoneum were followed up for about 2 years,1 of them had tumor recurrence ,but the graft kept patent,2 patients with sarcoma in the limbs were followed up for 1 year without tumor recurrence or vessel obstruction.Eleven patients with tumors in the neck were followed up for 3 months to 5 years without tumor recurrence or vessel obstruction. Conclusions (Combination) of resection of neoplasm and reconstruction of major blood vessels is safe,effective and practical method for patients with major wessels invaded by tumor.
3.The contrast analysis of psychointervention for older hypertensives in community

ObjectiveTo explore the action of the psychointervension for older hypertensives in community.Methods50 older hypertensives were divided into the experimental group and the control group (with 25 cases in each group),the former received psychointervension combined body therapy and the later only received body therapy. A half of a year experiment period was made. Differences between two groups in blood pressure and SCL-90 before and after the experiment were compared and evaluated.ResultsThe mentation of older hypertensives is worse than the normal. There are distinct differences between two groups in the improvement of mentation and the stability of blood pressure (P<0.01). Conclusions It is an active effect to strengthen psychointervension for therapy of older hypertensives in community.
4.Application of nursing quality monitoring book in ICU

Objective To investigate the application and effect of nursing quality monitoring book in ICU.Methods Quality monitoring book was used,quality control management group was established,monitoring group leader was chosen and the writing method,content as well as tracking record method of quality monitoring book were all specified.Results After the use of quality monitoring book,the nursing quality rate inspected by nursing department increased from 94.54% to 97.41%,and the difference was statistically significant (x2 =7.788,P < 0.01 ).The nursing quality rate inspected by head nurse increased from 93.59% to 96.15%,and the difference was statistically significant ( x2 =5.270,P < 0.05 ).The incidence rate of nursing adverse events decreased from 2.5% to 0.8%,and the difference was statistically significant ( x2 =5.722,P < 0.05).Conclusions The application of nursing quality monitoring book has perfected the quality feedback system,can improve management efficiency and comprehensive quality,and reduce the occurrence of nursing adverse events.
5.Flavonol and monoterpene glycosides from Ligularia macrophylla

OBJECTIVE To investigate the chemical constituents of the roots and rhizomes of Ligularia macrophylla.METHODS The compounds were isolated through repeated column chromatography on silica gel, polyamide, RP-18 and Sephadex LH-20. And the structures were elucidated on the basis of spectroscopic data (UV, IR, MS, 1H NMR, 13C NMR, DEPT and 2D NMR). RESULTS Four flavonol glycosides and two monoterpene glycosides were obtained and determined as rhamnazin-3-O-β-D-rutinoside ( 1 ), rhamnetin-3-O-β-D-rutinoside ( 2), rutin ( 3), afzelin ( 4), betulalbuside A (5) and 3,7-dimethyloct-1 -en-3,8-diol8-O-β-D- glucopyranoside (6). CONCLUSION All the compounds were isolated from this plant for the first time.

7.Adverse drug reactions induced by Mailuoning injection: literature review on 162 cases.

To understand the adverse drug reaction (ADR) induced by Mailuoning injection. 162 ADRs due to the drug were retrieved from national medical journals of 1988-2007 for statistics. It was shown that there was no relationship between ADR and dosage, but ADR appeared mostly in middle-aged and old groups, and more in male than in female. The occurrence of ADR was commonly within 30 min after injection. It involved injuries of various systems and organs. As for the 123 cases of allergy, 38 were anaphylactic shock (accounting for 23.46%), two people died. ADR characterized by immediate type on initial use and tachy type. It should be paid more attention to the Mailuoning injection for the immediate hypersensitivity reaction (such as anaphylactic shock).

8.Influence of cationic cyclopeptide on microstructure and permeability of Caco-2 cell membrane.

The microstructure of cationic cyclopeptide (TD-34) treated Caco-2 cell membrane was observed, and we discussed the relationship between membrane structure and insulin transmembrane permeability. Atomic force microscope (AFM) was used to observe living cell membrane in air condition and tapping mode. Results showed that the surface of Caco-2 cell membrane treated with TD-34 lost its smoothness and nearly doubled its roughness. Apparent permeability coefficients (P(app)) of insulin in Caco-2 cell monolayers increased 2.5 times. In conclusion, AFM can be used to observe microstructure of cationic cyclopeptide treated cell membrane and cationic cyclopeptide enhanced insulin delivery across Caco-2 cell membrane by increasing membrane fluidity.

9.Impact of three to four cycles of neoadjuvant chemotherapy on survival of patients with N2-N3 nasopharyngeal carcinoma

Objective To evaluate the impact of three to four cycles of neoadjuvant chemotherapy (NACT) on the survival of patients with N2-N3 nasopharyngeal carcinoma (NPC).Methods The clinical data of 915 patients with T1-4N2-3M0 NPC from 2007 to 2010 were retrospectively analyzed.A total of 179 patients treated with 3-4 cycles of NACT (NACT≥3 group) were matched with 358 patients treated with 2 cycles of NACT (NACT=2 group) and 179 patients treated without NACT (NACT =0 group,concurrent chemoradiotherapy group) for age,N stage,pathological subtype,and NACT regimen.The Kaplan-Meier method was used to calculate overall survival (OS),disease-free survival (DFS),recurrence-free survival (RFS),and distant metastasis-free survival (DMFS) rates,the log-rank test was used for survival difference analysis and univariate prognostic analysis,and the Cox proportional hazards model was used for multivariate prognostic analysis.Results For the NACT≥ 3,NACT =2,and NACT =0 groups,the 5-year OS rates were 89.4%,81.6%,and 73.7%,respectively (P=O.000),the 5-year DFS rates were 83.2%,69.8%,and 64.2%,respectively (P=O.000),the 5-year RFS rates were 86.0%,76.0%,and 69.3%,respectively (P=0.001),and the 5-year DMFS rates were 86.6%,76.0%,and 68.3%,respectively (P=0.000).Three to four cycles of NACT was an independent protective factor for OS,DFS,RFS,and DMFS in patients with N2-N3 NPC.Conclusion Three to four cycles of NACT can significantly improve the survival of patients with N2-N3 NPC.
10.Screening for bioactive mutants with antitumor activity from an actinomycetic wild-type strain without antitumor activity by antibiotic-resistant mutation technique and by coupled with chemical mutagen-induced mutation

Objective To obtain antibiotic-resistant mutants producing metabolites with antitumor activity from wild-type actinomycete strains without antitumor activity. Methods An actinomycete strain L35-1 was used as an initial strain for obtaining antibiotic-resistant mutants, which is a marine-derived wild-type strain without antitumor activity with an inhibition rate of 2.8% at the 1000 μg/ml of high sample concentration on K562 cells. The antibiotic-resistant mutants both from auto-mutagenesis and chemical mutagen-induced mutagenesis were selected by single colony isolation on antibiotic-containing plates according to the method for obtaining drug-resistant mutants in ribosome engineering. The antitumor activity was assayed by the MTT method using K562 cells for the mutants with aqueous acetone extracts of the whole broth of their fermentation.Results A total of 114 neomycin-resistant (ner) and 68 streptomycin-resistant (str) mutants, all from auto-mutagenesis, was obtained on drug-containing plates. Among them, the 7 ner and 3 str mutants appeared to be bioactive with an inhibition rate above 20% at the 100 μg/ml sample concentration on K562 cells. On the other hand, 41 str and 32 ner mutants from DES-induced mutagenesis and 46 ner mutants from NTG-induced mutagenesis were obtained by mutagen-induced mutation coupled with the single colony isolation on antibiotic-containing plates, among which, one str mutant from DES-induced mutagenesis and one ner mutant from NTG-induced mutagenesis were bioactive with an inhibition rate over 20% at the 100 μg/ml sample concentration on K562 cells. Conclusions The present result has revealed that the wild-type actinomycete strains without bioactivity might become a great source initial strains to obtain bioactive mutants by drug-resistant mutation technique.
