1.Detection of Apoptosis by in Situ Labeling and Study on the Expression of bcl-2 in Lesions of Patients with Psoriasis

Objective To investigate the involvement of apoptosis in the lesions of patients with psoriasis. Methods The apoptosis was detected with terminal deoxynucleotidyl transferase mediated dUTP biotin nick end labeling (TUNEL), and the expression of p53, PCNA and apoptosis suppressing protein bcl 2 was assessed with immunoperoxidase technique in psoriatic lesions and normal skin. Results A large number of keratinocytes showing biochemical and morphologic features of cells undergoing apoptosis were observed in all the suprabasal layers of the psoriatic epidermis. The plaques from all patients analysed showed marked increase in the number of PCNA positive cells in the middle and basal keratinocytes, and a dramatic reduction in the number of bcl 2 positive cells in the basal cell layer. Conclusion The increased apoptosis of keratinocytes in the lesions of psoriasis might be a homeostatic mechanism to the hyperplasia of cells.

Congenital heart disease with left-to-right shunt easily lead to pulmonary hypertension.Currently agreed about its mechanism:shear stress induced by high pulmonary blood flow stimulus pulmonary endothelium,to start regulation of genetic transcription,to initiate a series of molecular biology and pathophysiology changes,and finally to lead to pulmonary vascular pathologic remolding.Nuclear factor(NF)-?B is a kind of nuclear factor with multiple biological effect and play an important role in pulmonary vascular remolding.NF-?B signal can be actiacted by high blood flow.Its target gene products,for example,vasoactive mediators,cytokines,make pulmonary vessels difficulty to maintain the normal structure and cause pulmonary vascular contraction and remolding,thus,pulmonary arterial pressure increases.
3.Liposuction-curettage under negative pressure combined with Xiaozhiling injection for the treatment of axillary bromhidrosis

Objective To evaluate the efficacy of liposuction-curettage under negative pressure combined with Xiaozhiling injection for the treatment of axillary bromhidrosis.Methods The operative field was marked along the border of hair-bearing area of axilla with a distal margin of 0.5-1 cm.Subcutaneous tissue was removed by stroke movement under negative pressure after tumescent anesthesia.Subsequently,Xiaozhiling injection was injected into the subcutaneous space.All the patients were followed for at least 3 months,and therapeutic efficacy was evaluated quantitatively.Results A total of 72 patients (144 sides) with axillary bromhidrosis were treated with this method from March 2009 to May 2012,including 16 males and 56 females aged 14-41 years.All the patients experienced primary healing with no necrosis or hematoma formation within one week after the operation.The follow-up lasted 3 to 18 months (average: 8 months).Among the 144 sides,133 (92.4%) were cured,11 (7.6%) were improved,with the response rate being 100%.Postoperative scar is invisible,and patients were satisfied with the treatment outcomes.Conclusion The combined therapy is a simple and effective approach to the treatment of axillary bromhidrosis with a fast recovery and less complications.
4.Determination of Ginsenoside Rg1, Ginsenoside Re and Ginsenoside Rb1 in Yi’ nianjin by UPLC

Objective:To establish a UPLC method for the determination of ginsenoside Rg1 , ginsenoside Re and ginsenoside Rb1 in Yi’ nianjin. Methods: The column was ZORBAX Eclipse Plus C18 (2. 1 mm × 50 mm,1. 8 μm), the flow rate was 0. 21 ml· min-1 , the column temperature was 30℃, the mobile phase consisted of acetonitrile-water with gradient elution, the detection wave-length was 203nm, and the sample size was 1μl. Results:The linear range of ginsenoside Rg1 was 0. 020 3-0. 303 9 mg·ml-1 ( r=0. 999 6), and the average recovery was 99. 05% (RSD=1. 3%, n=6). The linear range of ginsenoside Re was 0. 020 2-0. 302 7 mg·ml-1(r=0. 999 8), and the average recovery was 101. 31% (RSD=1. 1%, n=6). The linear range of ginsenoside Rb1 was 0. 020 3-0. 305 1 mg·ml-1(r=0. 999 8), and the average recovery of 100. 71% (RSD=0. 9%, n=6). Conclusion: The method is simple and accurate, and can determine the three components simultaneously. The method can be used in the quality control of Yi’ nianjin.
5.HBV Contaminated Blood Worktable Surface: Investigation and Analysis

OBJECTIVE To discuss the possibility of HBV contaminating environment and transmission through the polluted surroundings. METHODS On the basis of that ascertain the contaminated environment by HBV marker(HBVM) and the relationship between HBVM and HBV infectivity in the blood,to discuss the probability of HBV contaminating and be spreading through the circumstance. RESULTS The positive rate of HBsAg and HBeAg was 26.39% and 11.11%,respectively in these specimens.While all of samples in control group were negative.The HBV DNA positive rate was 73.72% and 100.00%,respectively in the positive blood specimens of HBsAg and HBeAg.The infectivity was 0-10~9 and 10~2-10~9 ID/ml,respectively. CONCLUSIONS The blood worktable surface is contaminated by HBVM.The pollution is from patients′ blood.So the probability exists that HBV is transmitted through environment pollution.
6.Changes of endothelial progenitor cells and nitric oxide in patients with pulmonary arterial hypertension secondary to chronic obstructive pulmonary disease

Objective To investigate the changing of endothelial progenitor cells and nitric oxide in patients with pulmonary arterial hypertension secondary to chronic obstructive pulmonary disease.Method Mononuclear cells from 20 healthy persons (the control group, n = 20) , COPD patients without PAH (COPD non-PAH group, n =30) and patients with PAH secondary to COPD (COPD and PAH group, n=30) were investigated. Total mononuclear cells were isolated from peripheral blood by Ficoll density gradient centrifugation. EPC number and migration were assayed by colony forming unit-EPCs (CFU-EPCs) assay and modified Boyden chamber assay , respectively. EPC adhesion assay was performed by replanting those on fibronectin-coated dishes , then adherent cells were counted. The concentration of NO was measured with method of nitrate reductase.Result The numbers of CFU and migration, adhesion activity of circulating EPCs in COPD[ (21.9±3. 9)CFU-EPC] and PAH group[ (14. 2 ±3. 5)CFU-EPC] were significantly lower than that in non-COPD group and COPD non-PAH group [ ( 24.9 ±4.1) CFU-EPC ]. It was also observed that a strong negative correlation between the levels of PAH and the numbers of CFU and adhesion, migration activity of circulating EPCs. The NO level in the PAH group [ (43. 6 ±4. 8)ng/ml] was significantly lower than that in control group [ ( 67. 17±4.9 ) ng/ml ] ( P < 0.01). The NO level was positively correlated with number and migration ability of EPCs( r =0. 77,0.71, P <0.01) , but not correlated with adhesion ability.Conclusion The number of CFU and migratory, adhesive activity of EPC in patients withPAH secondary to COPD was significantly decreased. These changes may be associated with low level of plasma NO.
7.B-Lynch suture in the treatment of postpartum hemorrhage due to uterine atony and impact on long-term fertility

Objective To evaluate the effects of B-Lynch suture for postpartum hemorrhage due to uterine atony,and to clarify its influence on long-term fertility and next pregnancy outcome.Methods We retrospectively collected the medical records of 150 women who underwent B-Lynch suture during cesarean section due to intra-operative bleeding caused by uterine atony from January 1,2006 to December 31,2013 in Peking University First Hospital.Follow-up was carried out to assess postoperative menstruation and subsequent pregnancy outcomes.Results All the cases underwent B-Lynch suture which successfully retained the uterus.Satisfactory hemostasis was achieved in 131 women (87.3%) who underwent B-Lynch suture only,and in eighteen women (12.0%) who had bilateral uterine artery ascending branch ligation at the same time;and in one patient (0.7%) who underwent B-Lynch suture and uterine artery embolization four hours later due to poor uterine contraction.Of the 141 (94.0%) cases followed up after surgery,menstrual cycle was assessed in a total of 104 women:no changes in the menstrual cycle and bleeding volume were noted in 67 cases (64.4%),prolonged menstruation was observed in five cases (4.8%),menorrhagia in ten cases (9.6%),and hypomenorrhea in 22 cases (21.2%).Eleven women (23.9%) became pregnant and delivered in two to four years after the surgery,and all were full-term abdominal delivery.Conclusions B-Lynch suture is a simple,safe,and effective treatment for postpartum hemorrhage due to uterine atony,without significant change of menstruation.B-Lynch suture is not associated with infertility,and the postoperative pregnant outcome is good.
8.Establishment of a rabbit model of oral soft tissue defects

BACKGROUND: In clinical, the research on the method and the material of the soft tissue defect of the operation area has been in depth. It wil have a positive impact on our exploration and research in this field through the establishment of reliable experimental animal oral soft tissue defect model. OBJECTIVE: To establish a rabbit model of oral soft tissue defects for oral treatment of soft tissue defects in-depth study. METHODS: Eighteen 3-month-old male New Zealand rabbits were enrol ed. A tissue ring cutting dril of 5 mm diameter was used to prepare round ful -thickness soft tissue defects in the front part of the hard palate and, respectively, from the back of the maxil ary incisors, about 2 mm from the hard palate mucosal edge. Morphology and histology were observed at 3, 7, 14, 21, 28 and 56 days after model establishment. RESULTS AND CONCLUSION: (1) Morphological observation: After 3 and 7 days, the wound’s inflammatory reaction was obvious. As the time goes, the inflammatory response subsided, the wound gradual y healed. Scar formation was observed at 21, 28 and 56 days after surgery. (2) Histological observation: 3 and 7 days after injury, many inflammatory cel s were infiltrating, and tissue necrosis area was larger. At 7 days after surgery, with the extensive formation of connective tissue proliferation and new blood capil aries, the wound gradual y shaping completely. (3) Results indicated that a rabbit model of oral soft tissue defects was successful y established, which was in line with the physical laws of wound healing and the characteristics of human oral soft tissue defects healing.
9.Analysis of clinical effect of McDonald cervical cerclage and the related risk factors

Objective To investigate the clinical effect of McDonald cervical cerclage and the affecting factors. Methods Between January 2002 to December 2013 in Peking University First Hospital we performed McDonald cervical cerclage for 116 single pregnant women. They were defined as the successful group who deliveried the live babies after 28 weeks after the cerclage and the failure group who deliveried in the second trimester. According to the surgical indications they were divided into preventive cerclage group and therapeutic cerclage group. Then we analyzed the curative effect and the affecting factors in the groups. Results (1) In the 116 cases, 12 cases (10.3%) failed, and 104 cases (89.7%) succeeded. In the successful group, 37 cases (35.6%,37/104) deliveried pretermly and 67 cases (64.4%) deliveried termly. And there were 56 cases of vaginal delivery (53.8%), and 48 cases (46.2%) of cesarean section. (2) Among the 116 cases, 48 cases (41.4%) were included in prophylactic cerclage group, the gestational age was (16.3± 2.2) weeks, 68 (58.6%) cases were included in therapeutic group, the gestational age was (24.0±2.2) weeks. The operation time was (22±9) minutes in preventive group and (24±13) minutes in therapeutic group,there was no statistical difference between the two groups (P>0.05). Live-birth rate between preventive cerclage group and therapeutic cerclage group was no statistically significant difference (P>0.05). The term birth rate (72.9%, 35/48) in preventive group was higher than that in therapeutic group (47.1%, 32/68), the difference was statistically significant (P<0.01). Neonatal hospitalization rate was lower in preventive group (14.6%, 7/48) than therapeutic group (36.8%, 25/68) , the difference was statistically significant (P< 0.01). (3) In the failure group placental pathology was examed in 7 cases. The placental tissue showed a large number of neutrophils infiltrating in 6 cases (6/7). In the successful group, 27 pregnant women deliveried between 28 to 33+6 weeks (26.0%,27/104), 10 pregnant women deliveried between 34 to 36+6 weeks 10 cases (9.6%, 10/104), 67 cases deliveried after 37 weeks (64.4%, 67/104). A lot of factors including maternal age, the previous cervix operation history, perioperative application of progesterone, operation time and preoperative invasive procedure were compared between the successful group and the failure group. Only maternal age and preoperative invasive proedcure were statistically significant (P<0.05) and the others had no statistical significance (P>0.05). (4) There were 68 cases in the therapeutic group, 7 cases failed, and 61 cases succeeded;the preoperative cervical os in failure group [ (21 ± 20) mm] was wider than that in successful group [(14±5) mm], the difference was statistically significant (P<0.05);and preoperative vaginal ultrasound measurement of cervical canal length were (18 ± 8) mm versus (19 ± 10) mm, there was no statistically significant difference (P>0.05). Conclusions The McDonald cervical cerclage for cervical incompetence is a simple, safe and high successful rate of intervention measures. The term labor rate of prophylactic cervical cerclage was higher than that of the therapeutic cerclage. Older maternal age and preoperative invasive procedure may be the risk factors for cerclage. The infection may play an important factor leading to the failure of McDonald cervical cerclage.
10.Study on the Influence of Humanistic Care on the Quality of Life and Negative Emotion of Patients with Liver Cancer Operation

Objective:To explore the effect of humanistic care on the quality of life and the negative emotion of patients with liver cancer operation. Methods: A total of 577 patients with liver cancer surgery were selected to participate in this study from January 2014 to December 2015 . From January 2014 to December 2014 , 288 cases of liver cancer patients were given routine nursing care as the control group. From January 2015 to December 2015, 289 patients were given humanistic care nursing as experimental group. The emotion of depression and anxiety were evaluated using Self-rating Depression Scale and Self-rating Anxiety Scale, the quality of life was evaluated with the core scale of life quality measurement system for cancer patients, while sleep quality with the Pittsburgh Sleep Quality Index. Results:Both depression and anxiety score of the subjects in experimental group were lower than those in the control group(P<0. 05). Physical function, cognitive function, social function, emotional function, role function, and overall health status of the subjects in the experimental group were higher than those in the con-trol group (P<0. 05), while pain, tiredness, nausea, vomiting, shortness of breath, insomnia, diarrhea, loss of appetite, constipation, economic difficulties and other score were statistically lower than those in the control group (P<0. 05). The subjects′quality of life of in the experimental group was better than those in the control group(P<0 . 05 ) , but the sleep index score was lower in the experimental group ( P<0 . 05 ) . Conclusion: Humanistic care nursing can effectively reduce the negative emotions of patients with liver cancer surgery, and thus to improve the quality of life.
