1.Clinical Study of Diaphragmatic Muscle Fatigue in Children with Pneumonia

Objective To explore the occurrence of diaphragmatic muscle fatigue(DMF) in children with pneumonia and its relationship with respiratory failure.Methods One hundred and twenty-six cases of pneumonia(pneumonia group) and 60 healthy children(healthy control group) were selected,and 2K-V1 type intelligence electric impedance respirograph was used to measure diaphragmatic function of the children in both groups to find if the ribcage-abdomen motion was synchronic and to calculate M levels.The abdomen impedance values as X-axis were calculated,which were regarded,and the chest impedance values as Y-axis were calcaulated,which described two-dimensional chart of ribcage-abdomen motion curve and the ? angle which was between connection of maximum X value to the smallest X value and the X-axis were calculated.According to the changes of chest-abdominal curve in one-dimensional chart and the changes of M value and ? angle,the degree of the diaphragmatic muscle fatigue was judged.Results There were 98 cases of DMF in the 126 children with pneumonia,and 68 cases of which were type Ⅰ DMF,and 30 cases were type Ⅱ DMF.One-dimensional chart showed contradictory chest-abdominal respiratory motional curve,and two-dimensional chart moved in a clockwise direction.M value was(46.1?8.4)%,? angle was(136.7?12.0) degrees in children with type Ⅰ DMF;One-dimensional chart showed that the peak value of chest-abdominal respiratory motional curve dislocates,and two-dimensional chart moved in a counter-clockwise direction.M value was(17.2 ? 3.2)%,? angle was(48.2 ? 9.5) degrees in children with type Ⅱ DMF;the control group had M value(4.3 ? 1.0)%,? angle was(31.7 ? 5.2) degree.There were significant differences in M value and ? angle between type Ⅰ DMF children,type Ⅱ DMF children and healthy children(Pa
2.Relative Factors of Glucose Metabolism Disorders in Newborn Infants

Objective To investigate the relative risk factors of glucose metabolism disorder in newborn infants.Methods Clinical information of 791 newborns suffered from glucose metabolism disorders who had been hospitalized in NICU from Jan.2004 to May 2007 were analyzed retrospectively.Four hundred and thirty-nine cases presented with hypoglycemia,275 cases presented with hyperglycemia,and 77 cases presented with both hypoglycemia and hyperglycemia.Data of risk factors were processed with both ?2 test and multiple Logistic regression analysis.Results The statistic analysis showed that low birth weight[258 cases(58.77%)],asphyxia[217 cases(49.43%)],acidosis[146 cases(33.26%)],hypothermia[128 cases(29.16%)],maternal gestational hypertension[83 cases(18.91%)],pneumonia[63 cases(14.35%)],anomaly of placenta[35 cases(7.97%)],maternal diabetes[17 cases(3.87%)] and septicaemia[10 cases(2.28%)]were significant hypoglycemia risk factors(according to the level of morbidity).Pneumonia[98 cases(35.64%)],asphyxia[129 cases(27.23%)],hypoxemia[61 cases(22.18%)]and septicaemia[24 cases(8.73%)]were significant hyperglycemia risk factors.Acidosis[33 cases(42.86%)],pneumonia[27 cases(35.06%)]and maternal diabetes[6 cases(7.79%)] were significant risk factors for neonates with both hypoglycemia and hyperglycemia.Conclusion Dynamic monitoring of blood glucose concentration and reasonable adjustment is recommended for neonates with risk factors to lower morbility and mortality.
3.Image characteristics of renal cysts with high density

Objective To discuss the imaging characteristics of renal cyst of high density.Methods Nine cases (5 women and 4 men; mean age,48 years; age range,31-79 years) with high density renal mass(>30 HU) were reviewed.For all patients,the symptom was intermittent lumbar pain in 2 patients,gross hematuria with lumbar pain in 1 patient,gross hematuria without pain in 1 patient.The other 5 patients were detected during routine physical examination without symptom.Preoperative examination included CT,MRI and DSA.Results All the patients had solitary and unilateral high density renal cysts,which size ranged from 2 to 5 cm in diameter.The cysts displayed predominantly hypoechoic or irregular internal echoes on sonography.Excretory urography showed an unenhanced round mass without further distinguishing characteristics.Their cystic features were con-firmed on nonenhanced and contrast-enhanced CT scans.MRI showed a round mass with long T1 and long T2 signal.Angiography revealed lack of vessels around the mass.All patients were operated by partial nephreetomy.There was no recurrence in all patients during the follow-up for 1-5 years.Conclusion Renal cyst of high density has many special features of imageology.

5.Comparison of electrophysiological features in peripheral vertigo and central vertigo

Objective To compared the electrophysiological features in peripheral vertigo and central vertigo. Methods The electronystagmograph ( ENG ) and brainstem auditory evoked potentials ( BAEP ) were applied in peripheral vertigo group(85 cases) and central vertigo group(61 cases).Result ENG abnomal was in 67 cases (78.8%) in peripheral vertigo group.Overshoot or undershoot of dysmetria test was in 6 cases ( 7.1%);spontaneous nystagmus was in 5 cases(5.9%);abnormal of gaze test was in 16 cases(18.8%); eye tracking test typeⅠwas in 42 case(49.4%), typeⅡwas in 17 cases(20.0%), and typeⅢwas in 8 cases(9.4%); bilateral asymmetry of optokinetic nystagmus test was in 19 cases(22.4%);positioning nystagmus was in 51 cases(60.0%);abnormal of cold and hot test was in 31 cases(36.5%).ENG abnomal was 42 cases(49.4%) in central vertigo group.Overshoot or undershoot of dysmetria test was in 19 case(31.1%);spontaneous nystagmus was in 13 cases (21.3%);abnormal of gaze test was in 23 cases(37.7%);eye tracking test typeⅠwas in 35 cases(57.4%), typeⅡwas in 13 cases(21.3%), and typeⅢwas in 8 cases(13.1%);bilateral asymmetry of optokinetic nystagmus test was in 33 cases(54.1%); positioning nystagmus was in 2 cases(3.3%); abnormal of cold and hot test was in 6 cases(9.8%).Compared with peripheral vertigo group, the abnormal rates of optokinetic nystagmus test, gaze test, eye tracking test, optokinetic nystagmus test in central vertigo group were significantly increased, and the abnormal rates of positioning nystagmus, cold and hot test in central vertigo group were significantly decreased (all P<0.05). There were 32 cases(37.6%) in peripheral vertigo group with BAEP abnormal, and 31 cases(50.8%) were in central vertigo group with BAED abnormal.Compared with central vertigo group, the latency ofⅠwave andⅠ-Ⅲwave latency delayed in peripheral vertigo group were significantly increased, the latency ofⅤwave andⅠ-Ⅴwave latency delayed were significantly decreased ( all P<0.05 ) .Conclusions There are high sensitivity of optokinetic nystagmus test, gaze test, eye tracking test, optokinetic nystagmus test of ENG to the diagnosis of central vertigo. There are high sensitivity of positioning nystagmus, cold and hot test to the diagnosis of peripheral vertigo.The positive rate of BAEP is relatively lower, but it can provide objective foundation for location of vertigo patients.

8.Clinical efficacy and safety of glaucoma filtration surgery combined with the application of 5-fluorouracil

Abstract?AlM: To observe the clinical efficacy and safety of glaucoma filtration surgery combined with the application of 5 - fluorouracil for clinical treatment to offer the reference.?METHODS:A total of 91 cases of patients with glaucoma treated in our hospital were randomly divided into three groups, 30 cases of group A were given mitomycin in surgery, 31 cases of group B were given 5-fluorouracil, 30 cases of group C were given cyclosporine. They were given 12 -month-follow-up after operation, the operation success rate were recorded, the incidence of 1 -year-adverse reactions, visual acuity, intraocular pressure, low intraocular pressure, shallow anterior chamber, corneal damage index were recorded.?RESULTS: lntraocular pressure ofgroups A, B, C were respectively (20.5±8.6)mmHg,(15.2±5.5)mmHg,(20.0± 8. 1 ) mmHg, by the analysis of variance, group B was significantly lower than other two groups,P<0. 05;patients of groups A, B, C whose visual acuity increased ratios after 1y were respectively 22. 5%, 38. 1%,and 23. 1% , by the Chi. square test, the increased ratio of group B was higher than other two groups , P<0. 05; The operation success rates after 1y of operation of groups A, B, C were respectively 67. 5%,88. 1%,71. 8%,by the Chi square test, the increased ratio of group B was higher than other two groups, P<0. 05;The general incidence of adverse reaction were respectively 22. 5%,9. 5%,20. 5%,by the Chi square test , the increased ratio of group B was lower than other two groups, P<0. 05.?CONCLUSlON:Glaucoma filtration surgery combined with the application of 5-fluorouracil can improve the operation success rate, decrease the occurrence rate of adverse reaction, and safety can be improved.
9.STUDY ON THE BACTERIAL SEPTICEMIA OF CARASSIUS AURATUS OF PENZE(♀)? CYPRINUS ACUTIDORSALIS(♂)I——ISOLATION AND IDENTIFICATION OF THE PATHOGEN

The bacterial strain CSS-4-2 isolated from the spleen of Carassius auratus of penze(♀) ? Cyprinus acutidorsalis (♂) with septicemia was proved to b e a pathogen by artificial infection. This bacteria has short rod-shaped cells , Gram negative and motile, with single polar flagellum, and with not capsule. No spore was formed , and it was resistant to 0/129. It could utilize glucose, mal t ose ,mannite, sucrose and inositol, but could not utilize malonate, lactose, xyl ose, raffinose, sorbin and adonitol. Oxidase and arginine dihydrolase was posit ive.While urease, lysine decarboxylase and ornthine decarboxylase was negative .Bacteriological identification by VITEK-AMS-60 showed the strain CSS-4-2 w as Aeromonas caviae. Drug sensitivity assay showed that this strain was sen sitive to s treptomycin, tobramycin, chloromycetin, rifampicin, norfloxacin and nitrofuranto in, but resistant to penicillin G.

The growth property of pathogenic bacterium Aeromonas caviae strain CSS-4-2 which was isolated from the spleen of Carassius auratus of penze(♀) ? Cyprinus acutidorsalis (♂) that diseased septicemia was studied.The growth curve of CSS-4-2:lag phase was in 0~3h,log phase was in 3~30h, stationary phase was in 33~36h,decline phase was after 36h when CSS-4-2 was cultured shaking in tryptone liquid medium. It's optimum growth temperature ranges from 25℃ to 30℃, optimum growth pH ranges from 5 to 10, and optimum salinity ranges from 0% to 1% Nacl concentration .
