1.Observation of intravesical chemotherapy effect in papillary urothelial neoplasm of low malignant potential urinary bladder after transurethral resection

Objective To observe the effect of intravesical chemotherapy in papillary urothelial neoplasm of low malignant potential (PUNLMP) urinary bladder after transurethral resection of bladder tumor (TUR-Bt). Methods 44 cases of PUNLMP confirmed by pathological examination from Jan 2004 to Dec 2013 were retrospectively analyzed. Clinical manifestation: 24 asymptomatic, 19 gross hematuria and 1 abdominal discomfort. All cases were treated by TUR-Bt and immediately administered by intravesical instillation of 40 mg HCPT once, then 32 cases (A group) underwent additional intravesical instillation of HCPT once, and 12 cases (B group) underwent instillation qw for 8 times. Results PUNLMP cases accouted for 2.28 %(44/1 926) of bladder neoplasms admitted during the same period. There were 32 males and 12 females. 39 patients had multiple tumors, and 5 had single. All of cases were followed up with the medial follow-up time of 82 months (16-136 months), only one case in B group recurred after 14 months, and postoperatively without pathological progression. The difference of recurrence rates between two groups had no statistical significance (P=0.273). Conclusions PUNLMP has low incidence rate and is more common among middle-aging males. Doctors should consider PUNLMP when patients with bladder neoplasm are asymptomatically detected. Immediate instillation combined with additional one postoperatively could reduce the recurrence rate of PUNLMP obviously with satisfying prognosis. However, maintenance instillation does not enhance effect.
2.Analysis of Compatibility Laws for Acupoint Selection of Acupuncture in Treating Diabetic Gastroparasis.

OBJECTIVETo summarize and analyze the compatibility laws for acupoint selection of acupuncture in treating diabetic gastroparasis (DGP) in modern literatures of clinical researches.
METHODSRetrieved were literatures related to treating DGP by acupuncture or acupuncture combined other therapies from PubMed, CNKI, and WF from 1982 to 2014. Analyzed were frequency of acupoint use, meridians selected acupoints belonged to, regions selected, association laws of selected compatible acupoints.
RESULTSRetrieved were 35 with compatibility frequencies more than 15 listed as follows: compatibility frequency of Zusanli (ST36) and Zhongwan (CV12) was 33 (94.29%); compatibility frequency of Zusanli (ST36) and Neiguan (PC6) was 23 (65.71%), compatibility frequency of Zhongwan (CV12) and Neiguan (PC6) was 22 (62.86%), compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Neiguan (PC6) was 22 (62.86%); compatibility frequency of Zusanli (ST36) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zhongwan (CV12) and Sanyinjiao (SP6) was 16 (45.71%); compatibility frequency of Zusanli (ST36), Zhongwan (CV12), and Sanyinjiao (SP6) was 15 (42.86%). Meridians selected acupoints belonged to were sequenced as Foot Yangming Stomach channel, Ren channel, Foot Taiyang Bladder channel, and so on. Acupoints selected were mainly in lower limbs, chest and abdomen, waist and back. The compatibility of Zusanli (ST36) and Zhongwan (CV12) was the most often used with the highest frequency.
CONCLUSIONSThe compatibility laws for acupoint selection of acupuncture in treating DGP were mainly dominated as upper-lower selection, three regions selection, local selection, anterior-posterior selection. The compatibility laws for acupoint selection of acupuncture along meridians were mainly dominated as the convergence points and exterior-interior meridian points.

Aim To evaluate the effects of pantoprazole on various experimental acute ulcer inrats and mice. Methods The model of a gastric ulcer of rats or mice was caused bystree- induced ulcer and ligatel pylurus-induced ulcer. Results & Conclusions At adose of 5, 10, 20 mg? kg-1 of Pantoprazole can markedly decrease the ulcer index ofstree-induced ulcer. Pantoprazole(4, 8, 16 mg? kg -1 ) significantly decrease the areaof ligated pylorus-induced gastric ulcer. It was also found that pantoprazole caninhibit the output of basic gastric acid.
4.Multi-state model in the evaluation of outcome on mild cognitive impairment to Alzheimer' s disease

Objective The aim of this study was to introduce the multi-slate Markov model for the prediction of mild cognitive impairment (MCI) to Alzheimer' s disease (AD) and to find out the related factors for AD prevention and early intervention among the elderly.Methods MCI,moderate to severe cognitive impairment,and AD were defined as state 1,2 and 3,respectively.A three-state homogeneous model with discrete states and discrete times from data on six follow-up visits was constructed to explore factors for various progressive stages from MCI to AD.Transition probability and survival curve were made after the model fit assessment.Results At the level of 0.05,data from the multivariate analysis showed that gender (HR=I.23,95%CI:1.12-1.38),age (HR=I.37,95% CI:1.07-1.72),hypertension (HR=l.54,95% CI:1.31-2.19) were statistically significant for the transition from state 1 to state 2,while age (HR=0.78,95% CI:0.69-0.98),education level (HR=1.35,95% CI:1.09-1.86) and reading (HR=1.20,95% CI:1.01-1.41 ) were statistically significant for transition from state 2 to state 1,and gender (HR=1.59,95% CI:1.33-1.89),age (HR=1.33,95% CI:1.02-1.64),hypertension (HR=l.22,95% CI:1.11-1.43),diabetes (HR=1.52,95%CI:1.12-2.00),ApoEε4 (HR=1.44,95%CI:1.09-1.68) were statistically significant for transition from state 2 to state 3.Based on the fired model,the three-year transition probabilities during each state at average covariate level were estimated.Conclusion To delay the disease progression of MCI,phase by phase prevention measures could be adopted based on the main factors of each stage.Multi-state Markov model could imitate the natural history of disease and showed great advantage in dynamically evaluating the development of chronic diseases with multi-states and multi-faetors.

Objective To study the feature of regional cerebral metabolism rate of glucose (rCMRglc) in patients with Alzheimer’s disease (AD) and the value of positron emission tomography (PET) scanning in diagnosis of AD. Methods 13 AD patients and 13 health controls ,Who matched in age,sex and education years,were scanned with PET. Results (1) Watching by naked eyes,there was mild decreasing of rCMRglc at parietal lobe in healthy elders. While in AD patients,there was widely decreasing of cerebral metabolism rate of glucose. The most significant region was parietal lobe, the next was temporal lobe,and the last was frontal lobe. 6.Supraorbital keyhole approach for microsurgical treatment of aneurysms in the anterior circulation.

OBJECTIVETo study the method with endoscope-assisted microsurgical technique through the supraorbital keyhole approach to treat aneurysms in the anterior circulation.
METHODSAccording to preoperative diagnostic imagings, to work out of the individual operation planning. Skin incision was made in the eyebrow, and the diameter of supraorbital craniotomy was about 2 cm, endoscope-assisted microsurgical technique was used to clip aneurysm. The technique was used in the most recent 12 consecutive patients.
RESULTSFive different kinds of aneurysms in the anterior circulation were clipped through this method and 12 patients were cured. One patient, intraoperative accidental aneurysm rupture occurred. There were no approach-related complications.
CONCLUSIONSThis endoscope-assisted microsurgical technique via supraorbital keyhole approach is a safe, minimal invasive and effective way for the treatment of aneurysms in the anterior circulation, and there is more sufficient operating space.

7.Endoscope-assisted supraorbital keyhole approach for excision of suprasellar region tumor.

OBJECTIVETo study endoscope-assisted microsurgical technique with supraorbital keyhole approach for the treatment of suprasellar region tumor so as to maximize tumor removal and minimize operative-trauma.
METHODSAccording to high resolution CT and MR images before operation, individual operation schemes were worked out. Skin incision was made in the eyebrow, and the diameter of supraorbital craniotomy was approximately 2 cm. Endoscope-assisted microsurgical technique was used to resect lesions in 16 consecutive patients.
RESULTSTotal Tumors were removed in all of the 16 patients via the supraorbital keyhole approach. No postoperative complications occurred.
CONCLUSIONSSupraorbital keyhole approach may diminish tissue injury considerably and has proven to provide sufficient operating space in the suprasellar region for tumor removal.

8.Effects of oligomeric grape seed proanthocyanidins on isoproterenol-induced cardiac remodeling in rats.

The purpose of this study is to evaluate the effect of oligomeric grape seed proanthocyanidins (GSP) on isoproterenol (ISO)-induced cardiac remodeling in rats. ISO was given subcutaneously (5 mg x kg(-1), sc, 7 days) to induce cardiac remodeling in rats. Therapeutic groups were given GSP (50, 100, and 150 mg x kg(-1)) after ISO treatment. After 2 weeks intervention, heart rate (HR), left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP), rate of rise of left ventricular pressure (+/- dp/dt(max)) were examined. The myocardial hypertrophy index was expressed as heart weight/body weight (HW/BW) and left ventricle weight/body weight (LVW/BW), the histological changes were investigated by HE and Van Gieson stain. SOD activity and MDA content in serum, contents of hydroxyproline (Hyp) in the left ventricular tissue were assayed by xanthinoxidase method, thiobarbituric acid (TBA) method and alkaline hydrolysis method, respectively. After the onset of ISO-treatment, GSP therapy potently improved cardiac function, inhibited myocardial hypertrophy, improved cardiac pathology change, decreased the myocardial cross-section area (CSA), collagen volume fraction (CVF) and perivascular circumferential collagen area (PVCA), reduced the content of Hyp in the left ventricular tissue, inhibited the decrease of SOD activity and increase of MDA content in serum. GSP possess protective effect against ISO induced cardiac remodeling in rats, this may be related to reducing the oxidative stress and improving antioxidant capacity.

9.Associations of the genetic polymorphisms in CYP11B2 gene with nonfamilial structural atrial fibrillation

Objective To investigate whether polymorphisms in CYP11B2 gene are associated with nonfamilial structural atrial fibrillation(AF) in Chinese Han population. Methods A free-designed pair-matched hospital based ease-control study was performed in 297 cases and 297 controls. We investigated two tagging single nucleotide polymorphisms (tSNPs)-rs4545, rs3802228 in CYP11B2 gene by using GenomeLab~(TM) SNPstream technique. Results Two tSNPs were consistent with Hardy-Weinberg expectations in case and control groups. Compared with controls, the left atrial diameter of cases was significantly higher(P<0.0001). No significant difference in genotype or allele frequencies of tSNPs in CYP11B2 gene was observed. However, at the site of rs3802228 in 3' UTR of the case group, the left atrial diameter in AF patients with GG genotype was significantly higher than others. After adjusted for covariates age, smoking, Body mass index and hypertension, we did not observe the association of rs4545, rs3802228 with AE Conclusion Our result suggested that polymorphisms of rs4545 in CYP11B2 gene might not be associated with atrial fibrillation but polymorphism of 3' UTR rs3802228 locus in CYP11B2 gone might be associated with atrial structural remodeling.
10.Clinical study on treatment of severe retroperitoneal infection under "frozen abdomen" status

Objective To investigate the therapeutic method of extensive abdominal adhesion and organ edema ("frozen abdomen" status) with severe retroperitoneal infection. Methods The treatment measures, therapeutic course and characteristics of 21 patients with retroperitoneal infection complicated with sepsis under "frozen abdomen" status admitted to Rizhao Hospital of Traditional Chinese Medicine from 2004 to 2017 were retrospectively analyzed, inductive therapeutic measure and characteristics and the clinical efficacy and prognosis were also observed. Results In 21 cases, 19 cases were cured clinically, 1 case died of massive hemorrhage and 1 case died of multiple organ failure (MOF). In the 8 cases, abdominal cavity opening were performed, no exposure of intestinal fistula occurred, although the abdominal wall hernias were left behind in 6 cases and pancreatic pseudo-cyst was formed in 1 case. The patients total time of hospitalization was (61.0±10.5) days, and the stay in Intensive Care Unit (ICU) was (28±5) days. Conclusion In the comprehensive treatments, the multi-form flushing drainages were the core of effective means to control the severe retroperitoneal infection.
