2.Detailed research project management and boosting hospital disciplines

Since 2007,in order to comply with the hospital's strategic development and construction of the subjects,our hospital treat the research project management as the starting point,through the establishment of a sound project management and funding approaches; strengthen project process management,implement the total quality control; use of information technology tools to improve project management effectiveness.After several years of implementation,the number of our hospital's research projects above provincial and ministerial level has gradually increased,manyhigh level of SCI papers are emerging,and discipline construction has also been effectively carried out,all of which have promotedour research strength,the development of medical teaching and research.Therefore,we will continue to strengthen project management,and fully mobilize the initiative staff awareness of service,make full use of scientific information systems in future scientific research management to boost hospital disciplines.
3.Application of sub-Tenon's anesthesia for compound trabeculectomy with high intraocular pressure

AIM:To evaluate the efficacy and safety of Sub-tenon's anesthesia for compound trabeculectomy with high intraocular pressure.
METHODS: Forty - six eyes ( 46 cases ) of primary glaucoma received compound trabeculectomy under Sub-tenon's anesthesia, whose preoperative intraocular pressure were higher than normal after 24 to 48h of combined medication. Both efficacy and complication of the anesthesia were studied.
RESULTS: One minute after anesthetic injection, all cases were able to achieve the effect of analgesia and eye brake. During the operation, 0 level of anesthesia effect included 35 eyes ( 76%) , 1 level of anesthesia effect included 10 eyes ( 22%) , 2 level of anesthesia effect included 1 eye(2%). Only 1 case of these patients needed to add the surface anesthetic once, and other cases were successfully operatedunder Sub-tenon's anesthesia. The total effective rate was 98%. No anesthesia complications occurred in all cases.
CONCLUSION: Sub - Tenon's anesthesia is safe, effective, simple and quick for compound trabeculectomy with high intraocular pressure.

5.Preliminary implementation and effect of clinical pathways for chronic Keshan disease in endemic areas

Objective This present study explores and evaluates the effect of preliminary implementation in the clinical therapy programs for patients with chronic Keshan disease (CKD) in the disease seriously-affected endemic areas.Methods In 2010,seventy-six CKD patients with heart failure were chosen from Huangling and Xunyi Counties in Shaanxi Province,where incidences of CKD were high.Besides taking sodium selenite,all patients were given treatment with fixed prescription,which included angiotensin-converting enzyme inhibitor (captopril),β-blocker (propranolol),diuretics (hydrochlorothiazide,spironolactone) and cardiac (digoxin) for 4 months.The changes before and after treatment were analyzed,which included the changes of heart function by the United States of America New York Heart Association(NYHA) fractionation,cardiothoracic ratio,electrocardiogram,left ventricular ejection fraction(EF) and fractional shortening(FS).The therapeutic effect was subsequently evaluated.Results Seventy-four cases of the seventy-six CKD patients completed the treatment observation.The improvement rate of heart function was 81.1% (60/74) after treatment.The elimination rates of ectopic rhythm,conduction block and ST-T changes were 37.5% (9/24),2.7% (1/36) and 26.9% (7/26),respectively.The cardiothoracic ratios of heart function NYHA Ⅱ,Ⅲ and Ⅳ were 0.504 ± 0.051,0.572 ± 0.054 and 0.632 ± 0.063 before treatment.After treatment,the cardiothoracic ratios were 0.486 ± 0.048,0.538 ± 0.046 and 0.607 ± 0.048,which were reduced in all groups (t =2.643,6.641,3.005,all P < 0.05),while the D-value of cardiothoracic ratio changes before and after treatment was not significantly different(F =3.005,P > 0.05).Both the mild reduction group(35%≤EF < 50%) and the moderate-severe group(EF < 35%) EF were (43.62 ± 4.58)%,(27.57 ± 3.69)% before treatment and were (48.21 ± 10.01)%,(36.57 ± 6.60)% after treatment,EF were increased in the two groups,while the changes before and after treatment were significantly different(t =-2.911,-3.334,all P< 0.05).The EF D-value of the two groups was (4.59 ± 8.48)% before treatment and was (9.00 ± 7.14)% after treatment,which were not significantly different(P > 0.05).FS was higher compared with pre-treatment in FS reduction group(FS < 25%) and the changes before and after treatment[(19.75 ± 2.88)%,(21.92 ± 5.67)%] were significantly different(t =-2.297,P < 0.05).Conclusions This study shows that the feasibility of clinical treatment of patients with CKD is very promising.The treatment of fixed prescription is effective.
6.Facilitating coronary collateral recruitment by isometric exercise during acute coronary occlusion

Objective To determine whether isometric handgrip exercise can increase collateral flow to the ischemic myocardium in acute coronary occlusion patients with coronary artery disease (CAD).Methods Sixty-five patients with one-vessel CAD were randomly assigned to either an isometric exercise (IME) group or no-exercise (NE) group.Patients in the IME group performed isometric handgrip exercises (50% of the maximum voluntary contraction) during 1 min of coronary balloon occlusion.Patients in the NE group remained sedentary during the procedure.The collateral flow index (CFI),heart rare (HR),systolic blood pressure (SBP) and diastolic blood pressure (DBP) were determined prior to and at the end of 1 min of coronary occlusion.Results In the IME group the average CFI improved significantly more during the occlusion than in the NE group.The differences in HR,SBP and DBP were also significantly grcatcr in the CFI group than in the NE group controls.Conclusion Isometric exercise can induce significantly increased coronary collateral flow in CAD patients during acute vessel occlusion.
7.The role of inflammation and fibrosis cytokines in the pathogenesis of interstitial lung disease

Objective To study the role of tumor necrosis factor-alpha (TNF-α) and transforming growth factor-beta (TGF-β) in CTD-IDL. Methods mRNAs were extracted from lung tissues and perip-heral blood mononaclear cells (PBMCs) of CTD-ILD patients. RT-PCR was deployed to detect the percentages of TNF-α mRNA, TGF-β mRNAs; and the percentage of TNF-α, TGF-β and fibrosing parameters (hyaluronic acid, HA) was tested in serum by ELISA. The relationship between above laboratory data and image as well as pathological characteristics were analyzed. Results The percentage of TGF-β mRNA in lung tissue and PBMCs. As well as the serum TGF-β and HA increased significantly in those with interstitial lung diseases. On the other hand, the percentage of TNF-α mRNA of these patients did not increase in lung tissues and PBMCs. Conclusion TGF-β plays a role in lung interstitial fibrosis. It may be helpful to understand the degree of fibrosis in lung tissue by detecting lung tissue/PBMCs TGF-β mRNA, serum TGF-β and other fibrosis indicators.
10.Radiological evaluation of interstitial lung disease in diffuse connective tissue diseases

Objective To investigate the prevalence,radiological features and the association between HRCT changes and different diseases or clinical features in interstitial lung disease(ILD)caused by diffuse connective tissue diseases(CTD).Methods Retrospective analysis was performed based on the medical records of 412 in-patients with the diagnosis of diffuse connective tissue diseases from June 2003 to June 2005 in our hospital.268 cases were SLE,83 cases were SS,40 cases were DM/PM,21 cases were SSe.All patients had chest X-ray and the suspected cases had HRCT exam.The distribution of ILD among different diffuse CTD.the appearance between X-ray and HRCT appearances were compared.The correlation among HRCT appearance,different diseases and clinical features was studied.Results①In the 412 cases of CTD,ILD de- tected by HRCT was 9.7%(40 case in total),3,9% by X-ray.Among the 40 cases,DM/PM had the highest rate of ILD(25%),23.8% in SS,9.6% in SS and 6.3% in SLE.②The consensus rate between HRCT and X-ray was 40%.The rate of mis-diagnosis of ILD was 60% by X-ray.③The changes of interstitial involvement de- tected by HRCT were:ground glass change(41.0%),consolidation(32.8%),reticular change(16.4%)and hon- eycomb change(9.8%).The concomitant presence of two of the above changes were very common.④More level respiratory symptoms such as cough,panting,chest distress and chest pain and high level of complement were observed in patients with ground glass and consolidation changes,while high CRP level was seen in patients with consolidation and honeycomb changes(P
