1.Extracorporeal membrane oxygenation combined with ultrafiltration technique for treatment of kidney injury after severe hemorrhagic shock in rabbits

Objective To investigate the effect of extracorporeal membrane oxygenation (ECMO) combined with ultrafiltration in treatment of kidney injury induced by serious hemorrhagic shock in rabbits.Methods Models of pressure-controlled hemorrhagic shock was developed in 24 New Zealand white rabbits which were divided into unresuscitation group (n =8),ECMO combined with ultrafiltration group (combined resuscitation group,n =8),and fluid resuscitation group (n =8) according to the random number table.Heart rate was monitored via electrocardiograph and arterial pressure via fermoral artery catheter.Blood samples were collected pre-and post-shock and after resuscitation to measure levels of lactic acid,serum creatinine,IL-6,and TNF-α.Kidney samples were collected for measurement of histopathological changes via HE staining,expression of heat shock protein 70 (HSP70) via immunohistochemical staining.Results Arterial pressure was (53.1 ± 11.4) mmHg in combined resuscitation group,higher than (41.3 ± 11.1) mmHg in fluid resuscitation group and (25.9 ± 10.5) mmHg in unresuscitation group (F =41.425,P < 0.05).Hemorrhagic shock induced significant up-regulation of lactic acid,serum creatinine,IL-6,and TNF-α(P < 0.05),but all were lowered after resuscitation,especially in combined resuscitation group (P < 0.05).HE staining showed the degree of kidney tissue necrosis and inflammatory cytokine infiltration in combined resuscitation group alleviated notably compared with fluid resuscitation group.Median and interquartile values of HSP70 were 17 828.960 0 (15 779.865 0-21 751.980 0) in unresuscitation group,2 714.270 0 (1 339.215 0-7 616.950 0) in fluid resuscitation group,and 262.930 0 (198.820 0-538.195 0) in combine resuscitation group,with statistical differences among groups(P < 0.05).Conclusion ECMO combined with ultrafiltration is superior to conventional fluid resuscitation in improving hypoxia tissue injury and inflammatory reaction after hemorrhagic shock and is beneficial to attenuating kidney injury.
2.Investigation on newly emerging and re-emerging snail habitats in Anhui, 2017-2021

Abstract: Objective To understand the changes of newly emerging and re-emerging snail areas in Anhui from 2017 to 2021 and analyze its related influencing factors, so as to provide scientific basis for formulating snail control strategies. Methods According to the historical snail survey data, the information of newly emerging and re-emerging snails from 2017 to 2021 were collected, the causes for the formation of newly emerging and re-emerging snails flourish environments were investigated and analyzed retrospectively. Results During 2017-2021, in Anhui, the area with newly emerging snails was respectively 840.41 hm2, 559.14 hm2 (66.53%) in lake and marshland areas and 281.27 hm2 (33.47%) in hilly areas; the area of re-emerging snails was respectively 1 176.87 hm2, 669.39 hm2 (56.88%) in lake and marshland areas and 507.48 hm2 (43.12%) in hilly and mountainous areas. The sum of newly emerging snail area in Chizhou, Anqing and Wuhu accounted for 89.35% of all, the sum of re-emerging snail area accounted for 88.82%. In 2021, the areas with newly emerging and re-emerging snails peaked at 611.52 hm2 and 976.84 hm2, respectively. The newly emerging and re-emerging snail habitats were mainly distributed in the transmission interruption areas, accounting for 65.54% and 84.30%, respectively. The newly emerging and re-emerging snail habitats were mainly found in fluvial marsh, accounting for 65.54% and 52.12%, respectively. In recent 5 years, the longest time interval of re-emerging snails was more than 50 years. The main causes of newly emerging snails were natural factors such as flood disaster and snail drift along river system. Natural factors, such as flood disaster and snail drift along river system, were the main reasons for the newly emerging snail habitats, accounting for 71.44% and 21.75%, respectively. Human factors, such as soil extraction from snail habitats, construction of water conservancy facilities, farmland abandonment and seedling transplanting, could also lead to the newly emerging snail habitats. Flood disaster was the main cause of re-emerging snail habitats, accounting for 72.29%. In addition, the re-emerging snail habitats were caused by historical snail residue, soil collection in snail habitats for construction projects, land abandonment, seedling transplanting, prohibition of snail control in ecological protection areas, and construction of water conservancy facilities. Conclusions Flood disaster is an important factor for snail newly emerging and re-emerging. Human factors such as engineering construction and seedling transplanting are also easy to cause snail newly emerging and re-emerging. In order to timely detect and deal with newly emerging and re-emerging snail habitats and prevent snail diffusion, it is necessary to investigate snail distribution after flood disaster, the routine monitoring of historical snail habitats should be strengthened; in engineering construction and seedling transplanting, the disposal of soil with snails should be done well.
3.Effect and mechanism of jiannao yizhi decoction on learning and memory in rats with similar Alzheimer's disease.

OBJECTIVETo observe the effect of Jiannao Yizhi Decoction (JNYZD) on learning and memory in rats with similar Alzheimer's disease (AD), and to investigate its possible mechanism.
METHODSThe composite AD rat model was established by injecting aggregated Abeta25-35 into the lateral cerebral ventricle of senile rats, and all the modeled rats were divided into 5 groups, the model group, the Donepezil group, the high-, middle-, and low-dose JNYZD group. All rats, except those in the model group, were treated respectively with Donepezil and JNYZD at the daily dose of 0.525 mg/kg, 42.4 g/kg, 21.2 g/kg, 10.6 g/kg for 21 days. The ability of learning and memory of rats in different groups was tested using Morris water maze, and the activity of acetylcholine esterase (AchE) and butyrocholin esterase (BehE) in serum were determined, too.
RESULTSThe escape latent period was shorter in all medicated group than in the model group (P<0.01 or P<0.05), and it was insignificantly different among all medicated groups (P>0.05). A decreasing trend of AchE and BchE activity presented in the high- and middle-dose JNYZD groups, but insignificant difference was shown as compared these indexes respectively with those in the Donepezil group. Furthermore, the improvement of learning and memory in similar AD rats was insignificantly different between the Donepezil group and the JNYZD groups (P>0.05).
CONCLUSIONJNYZD can improve the learning and memory ability of similar AD rats by influencing the activity of cholinesterase.

4.Determination of trace elements in whole blood of patients with chronic Keshan disease and dilated cardiomyopathy

Objective To detect the levels of five trace elements in whole blood of patients with Keshan disease(KSD) and dilated cardiomyopathy(DCM) and explore their role in the pathogenesis of KSD.Methods One hundred and four patients with chronic KSD were selected from Keshan diseased areas in Shandong,Sichuan and Inner Mongolia.Thirty patients with DCM were selected from Qilu Hospital of Shandong University,Jinan Central Hospital,The First People's Hospital.Ninety-one healthy people from KSD endemic areas and 39 healthy people from Jinan were selected as endemic healthy controls and non-endemic healthy controls,respectively.Blood samples were collected to determinate the level of selenium (Se),copper (Cu),zinc (Zn),chromium (Cr) and manganese (Mn) with fluorescence method and atomic absorption spectrometry,according to the principle of informed consent.Results The level of Se,Zn and Cr of KSD group[(36.0 + 4.9)μg/L,(22.73 + 4.62)mg/L,(0.56 + 0.17)mg/L] was significantly lower than that of non-endemic healthy controls [(56.4 ± 6.8)lμg/L,(25.35 ± 4.44)mg/L,(0.71 ± 0.17)mg/L,all P < 0.05],but the level of Cu of KSD group[(0.95 ± 0.24)mg/L] was significantly higher than that of non-endemic healthy controls[(0.73 ± 0.13) mg/L,all P < 0.05].The level of Se and Cr of KSD was significantly lower than that of endemic healthy controls[(54.5 ± 5.4)μg/L,(0.87 ± 0.02)mg/L,P < 0.05],and Cu was significantly higher than that of endemic healthy controls[(0.66 ± 0.02)mg/L,P < 0.05].The level of Cu and Zn of KSD was significantly lower than that of DCM [(1.21 ± 0.23)mg/L,(27.09 ± 7.10)mg/L,all P < 0.01].The level of Se and Cr of DCM group[(39.6 ± 3.5)μg/L,(0.58 ± 0.14)mg/L] was significantly lower than that of non-endemic healthy controls(all P < 0.01),but Cu[(1.21 + 0.23)mg/L] was significantly increased (P < 0.01).Compared with non-endemic healthy controls,the level of Se of endemic healthy control group was significantly decreased (P < 0.01),while Cu was significantly increased (P < 0.01).Se,Zn and Cr level of KSD decreased gradually following elevated heart function level,but the level of Cu gradually increased.Conclusions The metabolism of Se,Cr,Cu and Zn is unbalanced in KSD patients,whose Se level is still lower than that of people in non-endemic areas.The change of Se,Cr,Cu and Mn level between KSD and DCM is consistent.
5.A retrospective analysis of clinical outcomes in 225 childhood with acute lymphoblastic leukemia.

OBJECTIVETo retrospectively analyze the outcomes in newly diagnosed children with acute lymphoblastic leukemia (ALL).
METHODSTwo hundred and twenty-five children with ALL (from Jan 2003 to Dec 2006) were divided into high-(HR) and standard-risk (SR) groups according to cytomorphology, immunology, cytogenetics and molecular biology (MICM) finding. SR patients were treated with conventional therapy, while HR patients with additional high-dose (HD) methotrexate (MTX) (5 g/m(2)) and early mid-dose cytarabine (Ara-C) (1 g/m(2), q 12 h x 3 d). All patients took maintenance chemotherapy of MM+VP. Respond rate (RR) and overall survival (OS) rate of patients were retrospectively analyzed.
RESULTSThe probabilities of 3-year and 5-year OS were 79.0% and 72.6%, respectively. The SR patients had a remarkably higher 3-year OS (87.6%) than that of HR patients (66.1%) (P = 0.002).
CONCLUSION(1) Early intensive therapy with mid-dose Ara-C and HD-MTX significantly improves the outcome of HR children with ALL. (2) The early intensive therapy in HR patients reduces the incidence of CNSL. (3) Periodic intensification and more re-induction therapies in SR patients remarkably improves the long-term survival.

6.Preliminary study on maturity of retinal vascularization in premature infants.

OBJECTIVETo study the extent of retinal vascular development and influencing factors at birth and the relation between retinal vascularization and retinopathy of prematurity (ROP).
METHODSFrom October, 2006 to December 2006, retinal vascularization was screened and evaluated in 84 neonates at different weeks of gestation and birth weights (BWs), had dilated fundus evaluation for zone of retinal vascularization by the 130 degrees lens of a digital fundus camera. The infants' pupils were dilated with 2.5% phenylephrine and 0.5% cyclopentolate eye drops. The study cohort was divided into subgroups depending on the weeks of gestation and birth weights. The control group consisted of healthy term infants. Maternal and neonatal factors were ascertained and analysed.
RESULTSVascularization up to zone I and II was considered to be immature retina; vascularization up to zone III or beyond was considered to be mature retina. In this study, 11 of 12 infants who were born at < 30 weeks of gestation, 12 of 26 infants who were born at < 31 approximately 33 weeks of gestation, 1 of 26 babies who were born at < 34 approximately 36 weeks of gestation and none of 20 babies who were born at < 37-40 weeks of gestation had immature retina; 12 of 15 babies at < 1500 g BW, 8 of 14 infants at 1500 g < BW < 1700 g, 4 of 11 infants at 1700 g < BW < 2000 g and of 44 infants at > 2000 g BW had immature retina. Those infants who were born at > 34 weeks of gestational age and at > 2000 g BW had mature retina. Infants who were born between 31 to 34 weeks of gestation and at 1501 to 2000 g BW had variable extent of retinal vascularization at birth. Vascularization was associated with postconceptional age (F = 31.9193, P = 0.000), birth weight (F = 32.4532, P = 0.000), anemia (F = 36.9391, P = 0.000), surfactant (F = 24.000, P = 0.0000), poor nutrition (F = 4.184, P = 0.041), RDS (F = 17.6191, P = 0.000), cesarean delivery (F = 10.972, P = 0.0022) and oxygen > 48 h (F = 22.076, P = 0.0000). Vascularization was affected mainly by the postconceptional age (95% CI = 1.57-261.728, P = 0.021). At last, 15/24 infants with immature retina developed ROP while none of the infants with mature retina developed ROP (chi2 = 45.1087, P = 0.000).
CONCLUSIONThere is considerable variability in the extent of retinal vascularization in infants who we born between 31 to 34 weeks of gestation. Modifiable maternal and fetal factors could influence extent of vascularization at birth. Immature retina is the critical factor of ROP. Gestational age is the main factor of the immature retina in premature infants.

7.Study on the resistance genes of carbapenem-resistant Enterobacteriaceae collected from 5 hospitals in Northeast China

Objective To investigate the antibiotic resistance and resistance genes of carbapenem-resistant Enterobacteriaceaes (CRE) isolated from 5 hospitals in Northeast China.Methods This study collected 85 CRE isolates during January 2013 to June 2015 from five hospitals in Northeast China.Drug sensitivities of 14 antimicrobial agents were determined by the broth microdilution method.The phenotypes of carbapenemases were screened by modified Hodge test and EDTA test respectively.The genotypes of carbapenemases and other extended spectrum β-lactamases (ESBL) were detected by PCR gene amplification and DNA sequencing method.Using the PCR result as gold standard, the performances of other two carbapenemase detection methods were evaluated.Results Among the 85 CRE strains collected in this study, Klebsiella pneumoniae was the most frequently isolated species (61/85,71.8%).The results of antimicrobial agent sensitivity showed that the 85 CRE strains had resistance rate of cephalosporin and β-lactams/enzyme inhibitor (piperacillin-tazobactam) over 80.0%.The resistance rate of carbapenem was high, with ertapenem 100.0% (85/85), meropenem 65.9% (56/85), imipenem 71.8% (61/85).There were 36 isolates resistant to both meropenem and imipenem.For fluoroquinolones, the resistance rates of levofloxacin and ciprofloxacin were 72.9% (62/85) and 65.9% (56/85), respectively.The resistance rate to fosfomycin and amikacin were 65.0% (55/85) and 54.1% (46/85), respectively.The resistance rate of colistin (21.2%, 18/85) and tigecycline (20.5%, 17/85) were low.Forty-nine strains were modified Hodge test positive and 12 strains were EDTA test positive.By PCR gene amplification and DNA sequencing method, 64 strains carried carbapenemase-encoding genes, of which KPC-2 was the main type (53/85, 62.4%), followed by IMP-4 (10/85, 11.8%), NDM-5 (7/85, 8.2%) and NDM-6 (1/85, 1.2%).At the same time, 85 CRE isolates had the ESBL gene detection and 47 isolates were CTX-M type ESBLs (47/85, 55.3%), with no TEM or SHV type.Conclusions Klebsiella pneumoniae is the majority of CRE strains from 5 large hospitals in Northeastern China.The CRE strains are resistant to most of antimicrobials.Most carbapenemases-producing isolates have the KPC-2 type.Nearly half of the carbapenemase-producing strains also carry ESBL genes, which makes the resistance mechanisms more complicated.
8.Clinical application research: color doppler flow image and three dimensional color power angiography in the diagnosis of thyroid nodules

Objective To evaluate the clinical value of color doppler flow image (CDFI) and three-dimension color power angiography(3D-CPA) in the diagnosis of thyroid nodules. Methods A total of 62 pathologically confirmed thyroid nodules of 30 patients were analyzed. All the patients underwent ultrasound examination and operation in the Second Affiliated Hospital of Harbin Medical University between January, 2008 and May, 2009. Both CDFI and 3D-CPA were performed before the operation. All the nodules were divided into three groups including nodular goiters, thyroid adenomas and thyroid cancers according to the pathological results. The hemodynamic features and the vascular morphology characteristics of nodules in different groups were compared. Results 3D-CPA showed that blood vessels of nodular goiters commonly distributed surrounding the mass and the inner vessels were thin and regular, thyroid adenomas were bulb-shaped and netted structure, and malignant thyroid nodules displayed distorted and irregular distributed vessels. Peak systolic velocity (PSV)of the three groups were (39.43±11.17a), (46.39±12.98) and (65.17±9.23)cm/s, respectively. Resistance index(RI) of the three groups were (0.32±0.08), (0.41±0.06) and (0.69±0.07)cm/s, respectively. Both PSV and RI in malignant thyroid nodules were higher than in nodular goiters and in thyroid adenomas and the difference were statistical significant (all P < 0.05). The blood flow grade of malignant nodules was also higher in malignant nodules than in other two groups(χ2 = 17.11, 12.79, 23.05, 15.41, P< 0.01). Conclusions CDFI and 3D-CPA could visually demonstrate the characteristic and distribution of the inner and outer blood vessels, display the vessels structures, and they are benefit the differential diagnosis of thyroid nodules.
9.The development of FSCLZLY--an ascites-ultrafiltration instrument.

It's known to all that the refractory ascites treatment has so far been a very difficult clinical problem. We have extracted much experience from the practical techniques used in the refractory ascites treatments of more than 1,000 cases, and have developed the ascites ultrafiltration & concentration therapeutic instrument--FSCLZLY-A. The clinical applications show that it is very effective. Its effective rate is about 72.08%. Therefore, it is a very useful and important medical device for refractory ascites, for the improvement of renal function, and for the prevention of the infection of abdominal cavity.

10.Spatio-temporal process and the influencing factors on influenza A (H1N1) pandemic in Changsha

Objective To analyze the spatio-temporal process on 2009 influenza A (HlNl) pandemic in Changsha and the influencing factors during the diffusion process. Methods Data were from the following 5 sources, influenza A (HlNl) pandemic gathered in 2009, Geographic Information System (GIS) of Changsha, the broad range of theorems and techniques of hot spot analysis, spatio-temporal process analysis and Spearman correlation analysis. Results Hot spot areas appeared to be more in the economically developed areas, such as cities and townships. The cluster of spatial-temporal distribution of influenza A (HlNl) pandemic was most likely appearing in Liuyang city (RR=22.70,P<0.01). The secondary cluster would include districts as Yuelu (RR=6A9,P< 0.01) , Yuhua (RR=81.63, P<0.01). Xingsha township appeared as the center in the Changsha county (RR=2.90, P<0.01) while townships as Yutangping (RR=19.31, P<0.01) , Chengjiao (RR=73.14,P<0.01) and Longtian appeared as the center in the west of Ningxiang county (RR= 14.43,P<0.01) and Wushan as the center in the Wangcheng county (RR= 13.84,P<0.01). As time went on, the epidemic moved towards the eastern and more developed regions. Regarding factor analysis, population, the amount of students, geographic relationship and business activities etc. appeared to be the key elements influencing the transmission of influenza A (H1N1) pandemic. At the beginning of the epidemic, population density served as the main factor (r=0.477, P<0.05) but during the initial and fast growing stages, it was replaced by the size of students to serve as the important indicator (r=0.831, P<0.01; r=0.518, P<0.01). However, during the peak of the epidemics, the business activities played an important role (r=-0.676, P<0.01). Conclusion Groups under high risk and districts with high incidence rates were shifting, along with the temporal process of influenza A(H1N1) pandemic, suggesting that the protection measures need to be adjusted, according to the significance of influencing factors at different stages.
