1.Correlated study on preoperative health status and perioperative complication incidence of aged patients

Objective To explore the relativity of preoperative health status and perioperative com-plication incidence of aged patients. Methods The chnical data of 3108 old aged people (60 and above 60 years old)who were operated from January 2006 to November 2007 was retrospectively analysed. General e-valuated preoperative health status, and summarized perioperative complication incidence. Results Before operation, 68.7% patients had one or more coexist disease, 41.1% patients had two or more coexist dis-eases, 38.7% had three coexist diseases in the meantime. After operation,one or more complications oc-curred in 21.4% (666/3108)patients, 2.8% (88/3108) patients died because of heart,brain,lung, kidney function failure. Conclusions Aged patients have more coexist diseases, the complications and death rates are higher than those in younger patients. The complications are correlated with preoperative health status and coexist diseases. Some factors could influence the patients rehabilitation,such as heart,brain,lung, kidney function, water-electrolyte balance and blood glucose change.

3.Study on metabolism of tetramethylpyrazine in system of rat liver microsomes.

OBJECTIVEThe metabolic character of tetramethylpyrazine (TMPz) in rat liver microsomes was studied in vitro and in vivo to identify which isoforms of cytochrome P450 were responsible for TMPz metabolism in rats, offer the theoretical foundation for the fact that it is rational to use medicine in clinic.
METHODSet up UV- HPLC method of TMPz, determine concentration of TMPz and its formation in rat plasma and liver microsomes incubation solution, analyze the correlation between TMPz's metabolic eliminate rate and each inducer. Erythromycin( ERY) N-demethylase activity of each sample in rat liver microsomes was measured using N-demethylation reaction of ERY as probe. The correlation between the rate of TMPz metabolite formation and the demethylase activity was analysed. After the SD rats who had been treated with inducer, inhibitor, or untreated, received administration of TMPz in vein, the plasma concentration of TMPz were determined by HPLC. Pharmacokinetic parameters of TMPz were computed and compared.
RESULTThe disppearing rate of TMPz in the incubation solutions of the rats liver microsomes, which treated with DEX, were markedly quicker than that of control group (P < 0. 01) , while no obvious difference between P-NF group or PB and control group was observed (P > 0. 05). The activity of ERY-N-demethylase in DEX-induced group was corespondingly enhanced, was much higer than that in control group. The correlation between the rate of TMPz metabolic product formation and the activity of N-demethylase was significant. After using Ket, the CYP3A inhibitor, the metabolism of TMPz could be significantly inhibited the metabolism of TMPz in rat liver microsomes. In vivo, CL( s) were larger than that of the control group,t,/2 were smaller than the control group in DEX group; By contrary, CL(s) was smaller than the control group,t1/2 was larger than the control group in Ket group.
CONCLUSIONResults suggest that CYP3A plays a major role in TMPz metabolism in rats, TMPz lie in the possibility of Interaction among the medicines between TMPz and CYP3A inducers or inhibitors when they are used in clinic.

4.Therapeutic effect of amlodipine atorvastatin calcium tablet on hypertensive patients and its influence on blood pressure and blood lipids

Objective :To explore therapeutic effect of amlodipine atorvastatin calcium tablet on hypertensive pa-tients .Methods : A total of 112 hypertensive patients treated in our hospital were randomly and equally divided into amlodipine group (received amlodipine 5mg/d) and amlodipine atorvastatin group (received amlodipine atorvastatin calcium tablet 15mg/d) ,both groups were treated for eight weeks .Therapeutic effect and incidence of adverse reac- tions ,blood pressure and blood lipids before and after treatment were compared between two groups .Results : Com-pared with before treatment ,there were significant reduction in levels of blood pressure and blood lipids (except sig-nificant rise in HDL-C level) in two groups after treatment , P<0-05 or <0-01 ;compared with amlodipine group , there was significant rise in total effective rate (62-5% vs.96-4%, P=0-001) ,and significant reduction in inci-dence rate of adverse reactions (28-5% vs.3-5%) , P= 0-003 ;significant reductions in levels of blood pressure [ (147-5 ± 10-1)/(90-7 ± 8-2) mmHg vs.(133-2 ± 9-4)/(86-9 ± 8-2) mmHg] ,total cholesterol [ (5-7 ± 1-3) mmol/L vs .(5-2 ± 0-8) mmol/L] ,triglyceride [(2-5 ± 1-2) mmol/L vs.(1-9 ± 0-5) mmol/L] and low density lip-oprotein cholesterol [ (4-1 ± 0-7) mmol/L vs.(3-1 ± 0-6) mmol/L] in amlodipine atorvastatin group , P<0-05 or<0-01- Conclusion : Amlodipine atorvastatin calcium tablet can significantly improve blood pressure and blood lipids with few adverse reactions in hypertensive patients ,which is worth extending .
5.Prostate cancer cell vaccine transfected with 4-1BBL induces anti-tumor immunity in vitro.

OBJECTIVETo explore the anti-tumor immunity in vitro induced by prostate cancer cell vaccine transfected with recombinant adenovirus encoding 4-1BBL in mice.
METHODSThe replication-deficient adenovirus AdEasy-1 system was used to construct recombinant adenovirus Ad-m4-1BBL and Ad-eGFP. The prostate cancer cell RM-1 of mice was transfected with Ad-m4-1BBL and Ad-eGFP, and treated with mitomycin (MMC) to produce TCV, TCV-Ad-eGFP and TCV-Ad-m4-1BBL, followed by co-culture with syngeneic murine spleen cells. Then the cytotoxic activity of the lymphocytes against RM-1 cells was analyzed with CCK-8 solution, and IL-2 and INF-gamma were detected by ELISA.
RESULTSThe 4-1BBL protein was highly expressed in the TCV-Ad-m4-1BBL of the 4-1BBL-transfected mice. TCV-Ad-m4-1BBL significantly increased the expressions of IL-2 ([180.24 +/- 2.22] pg/ml) and INF-gamma ([1512.46 +/- 23.64] pg/ml) as compared with TCV and TCV-Ad-eGFP (P < 0.05), and induced higher RM-1 cell specific cytotoxicity ([34.24 +/- 2.64]%) than the latter two ([9.82 +/- 1.48]%) and ([14.65 +/- 3. 21]%), (P < 0.05). But none of them exhibited significant cytotoxicity against hepatocellular carcinoma Hepal-6.
CONCLUSIONThe m4-1BBL-expressing prostate cancer cell vaccine can effectively induce anti-tumor immune responses.

6.Comparison and evaluation of three thyroid imaging reporting and data systems for medullary thyroid carcinoma

Objective:To investigate the diagnostic performance of different thyroid imaging reporting and data systems (TI-RADS) in the diagnosis of medullary thyroid carcinoma (MTC) .Methods:A total of 160 thyroid nodules diagnosed as MTC by postoperative pathology from Aug. 2011 to Aug. 2022 at the First Affiliated Hospital of Zhengzhou University were included. Additionally, 160 papillary thyroid carcinomas (PTC) and 160 benign nodules were randomly selected as controls during the same period. Differences in gender, age, nodule diameter and various ultrasound features were observed. The nodules were classified according to American College of Radiology (ACR) TI-RADS, artificial intelligence (AI) TI-RADS and Chinese (C TI-RADS). Then, receiver operating characteristic curve (ROC) were plotted to calculate the diagnostic value. The Kendall concordance coefficient was used to evaluate the interobserver consistency of each TI-RADS system.Results:There was no statistically significant difference in gender among the three groups ( χ2=1.17, P=0.558). However, significant differences were observed in age and nodule diameter ( F=12.08,40.12, P<0.001 for both). The area under ROC (AUC) for diagnosing MTC and benign nodules using ACR, AI, and C-TIRADS were 0.762, 0.773, and 0.761, respectively, with no statistically significant differences ( Z=1.33, 0.01, 0.87, P=0.183, 0.994, 0.386). However, the sensitivity of C TI-RADS (87.5%) was lower than that of ACR and AI TI-RADS (both 95.0%) ( P=0.018). After combining the biopsy threshold, the false negative rate of C-TIRADS was lower than that of ACR (30.6% vs. 41.3%) ( P=0.048) and AI TIRADS (30.6% vs. 43.1%) ( P=0.020). The inter-observer diagnostic consistency of C-TIRADS was superior to ACR (0.884 vs. 0.819, P<0.001) and AI TIRADS (0.884 vs. 0.839) ( P<0.001) . Conclusions:AI and ACR TI-RADS have higher sensitivity in diagnosing MTC, while C TI-RADS has a lower puncture missed diagnosis rate. AI has similar diagnostic performance to ACR TI-RADS and can replace ACR TI-RADS.
7.Neurobehavioral development in preterm infants: a retrospective study of 181 cases.

OBJECTIVETo investigate the results of Gesell Developmental Scale in follow-up of preterm infants and to determine possible high-risk factors for poor long-term neurological outcome.
METHODSA preterm infants' questionnaire was designed, and a retrospective study was conducted on the clinical data of 181 preterm infants (corrected age 2-12 months) and their mothers. The developmental quotient (DQ) scores were determined by the Gesell Developmental Scale and statistically analyzed.
RESULTSCompared with those with a birth weight (BW) of ≥1 500 g, the preterm infants with a BW of <1 500 g had significantly reduced DQ scores of adaptability, gross motor movement, and fine movement (P<0.05). Compared with those with a gestational age (GA) of ≥32 weeks, the preterm infants with a GA of <32 weeks had significantly reduced DQ scores of adaptability, gross motor movement, fine movement, and social contact (P<0.05). DQ scores on five Gesell subscales were significantly positively correlated with GA and BW (P<0.05). The DQ scores on Gesell subscales showed a significant negative correlation with severe complications in neonatal period (P<0.001).
CONCLUSIONSFor preterm infants, BW <1 500 g and GA <32 weeks are high-risk factors for abnormal adaptability, gross motor movement, fine movement, and social contact, and this group of infants should be followed up closely. Severe complications in neonatal period may be associated with poor long-term neurological outcome and should be effectively prevented and treated.

8.Significance of high-risk human papillomavirus DNA in atypical squamous cells, cannot exclude high-grade squamous intraepithelial lesion.

OBJECTIVETo explore significance of high-risk human papillomavirus (HR-HPV) testing in atypical squamous cells, cannot exclude high grade squamous intraepithelial lesion (ASC-H).
METHODSPresence of HR-HPV DNA was examined in 45 patients with ASC-H using hybrid capture II (HC-II) test. Colposcopic examination and biopsy were taken all results were evaluated.
RESULTSOverall, 33 of 45 (73.3%) ASC-H cases were biopsy proven cervical intraepithelial lesion (CIN). 36 of 45 ASC-H cases were HPV-DNA positive, including 19 cases of HSIL and over lesion; whereas no HSIL or over was found in 9 HR-HPV negative cases. Sensitivity and negativity predictive value of HR-HPV in ASC-H with HSIL and over lesion were both 100%.
CONCLUSIONSASC-H strongly predicts the presence of HSIL, HR-HPV may serve as a predict select whether a patient with ASC-H should take colposcopic examination immediately, patients with positive HR-HPV should undergo immediate colposcopic examination, while negative HR-HPV is an excellent predictor of the absence of HSIL.

9.Environmental distribution characteristics of carbapenem-resistant Kleb-siella pneumoniae

Objective To analyze the medical environmental distribution characteristics of carbapenem-resistant Klebsiella pneumoniae (CRKP).Methods On April 25 and August 29,2016,random sampling was performed be-fore cleaning and disinfecting the surrounding of patients who isolated CRKP in a hospital,sampling were mainly object surfaces and medical devices.Distribution characteristics of CRKP were analyzed.Results The number of environmental sampling in surgical intensive care unit(SICU),respiratory ICU(RICU),and emergency ICU(EICU) were 90,41,and 56 respectively.CRKP was not found in SICU and RICU specimens;6 strains of CRKP were iso-lated in EICU,the isolation rate was 10.71%.In April 2016,103 specimens were taken,and 6 strains of CRKP were isolated,isolation rate was 5.83%,which was higher than in August of 2016(P <0.05).There were no sig-nificant differences in isolation rates of CRKP among different specimens and different responsible persons (all P >0.05).Conclusion Key departments,such as EICU,should be intensified cleaning and disinfection,so as to re-duce the spread of CRKP in the medical environment.
10.Evaluation of Circulating Endometrial Cells as a Biomarker for Endometriosis

Background: Circulating endometrial cells (CECs) have been reported to be present in the peripheral blood of women with endometriosis (EM), providing clear and specific evidence of the presence of ectopic lesions. In this study, we established a method with a high detection rate of CECs, assessed the diagnostic value of CECs for EM and compared with serum CA125, and proposed a hypothesis for the pathogenesis of EM from the new perspective of CECs. Methods: The participants were enrolled prospectively from October 2015 to July 2016. The peripheral blood samples were collected from 59 participants, and the blood cells were isolated for immunofluorescence staining via microfluidic chips. The cells that were positive for vimentin/cytokeratin and estrogen/progesterone receptor and negative for CD45 were identified as CECs. The serum CA125 level was tested with electrochemiluminescence immunoassay. Results: The detection rate of CECs reached 89.5% (17/19) in the EM group, which was significantly higher than that of the control group (15.0% [6/40], P < 0.001) and was independent of menstrual cycle phases. Furthermore, a positive CEC assay detected 4/5 cases of Stage Ⅰ–Ⅱ EM. In contrast, a positive CA125 test had limited value in detecting EM (13/19, 68.4%) and detected only one case of Stage Ⅰ–Ⅱ EM. Conclusion: CECs are promising biomarkers for EM with great potential for a noninvasive diagnostic assay.
