2.Therapeutic Observation of Contralateral Needling plus Point-toward-point Method for Hemifacial Spasm

ObjectiveTo observe the efficacy of contralateral needling plus point-toward-point method in treatinghemifacial spasm, and to seek the optimal treatment protocol by combining clinical data.MethodNinety-four patients with hemifacial spasm were randomized into a treatment group and a control group, 47 cases in each group. The control group was by acupuncture at the affected side, while the treatment group was by contralateral needling plus point-toward-point method. The therapeutic efficacy was analyzed by comparing the changes of symptoms and the recovery of the spastic facial tissues.ResultThe symptoms in the treatment group were effectively mitigated, and the effective rate was 97.9%, significantly higher than 83.0% in the control group (P<0.05).ConclusionContralateral needling plus point-toward-point method is significantly effective in treating hemifacial spasm, and it benefits the recovery of the facial muscle function.
3.The clinical evaluation of the expression of the PCNA and cell nuclear area in the different layers of oral dysplasia epithelium

Objective: To study the expression of the proliferating c el l nuclear antigen (PCNA) and cell nuclear area in the different layers of oral d ysplasia epithelium. Methods: S-P immunohistochemical method an d morphometric technique(computer image measurement method) were used to examine the PCNA and cell nuclear area in the different layers of the normal oral epith elium(NOR) in 8 cases and abnormal oral epithelium in 46 cases including mild d ysplasia(LD), moderate dysplasia(MD), severe dysplasia(SD) and well-differentia ted squamous cell carcinoma(SCC). Results: The positive PCNA sta ining cells in normal epithelium was found mainly in the basal layer. The prolif erating index(PI) in the whole layer of abnormal oral epithelium were increased (P05). PI in the basal la yer of abnormal epithelium was higher than in NOR (P0.05). PI in the spi nosum layer and superficial layer (granulosum and lucidum layer) increased with the development of the lesions, The cell nuclear area in the basal and spinosum layer of the abnormal epithelium increased(P
4.Expression and antitumor activity of fusion protein RGD-TRAIL in Pichia pastoris.

To compare the activity of RGD-TRAIL in different expression systems, RGD-TRAIL in both Escherichia coli (E.coli) and Pichia pastoris was constructed and expressed. In vitro activity of RGD-TRAIL from Pichia pastoris expression system was also analyzed. Genetic engineering techniques were used to construct recombinant plasmid pET30-rgd-trail and pHBM-rgd-trail. The recombinant protein RGD-TRAIL was purified with Ni ion affinity chromatography after induction. MTT assay, ELISA, scratch wound healing, transwell migration assay and Hoechst 33342 staining were performed to detect the effects of RGD-TRAIL on proliferation, binding activity, migration and apoptosis. The expression of apoptosis-associated proteins was detected by Western blotting. Recombinant protein RGD-TRAIL was successfully expressed in a form of inclusion body in E.coli, while expressed secretorily in Pichia pastoris. It possessed more potent cytotoxicity than RGD-TRAIL in E.coli by MTT assay. The RGD-TRAIL expressed by Pichia pastoris showed powerful binding affinity with cancer cells expressing α(v), DR4, DR5 and highly potent cytotoxicity through inducing apoptosis of cancer cells. Nuclear fragmentation was examined by Hoechst 33342 staining. Cleaved PARP and caspase-3 were also detected after incubation with RGD-TRAIL. Additionally, RGD-TRAIL inhibited migration significantly in A549 and HT1080 cells. The results demonstrate that Pichia pastoris expression system is more suitable for the recombinant protein RGD-TRAIL. Its binding affinity and antitumor activity might make RGD-TRAIL a promising candidate for cancer therapy.

5.Observation of medium-term therapy of bilateral and unilateral lunmbar puncture plasma coblatiom nucleoplasty in treatment of lumbar disc herniation

Objective To compare medium-term therapeutic effect by bilateral and unilateral puncture plasma coblation nucleoplasty(PCN) in the lumbar disc herniation patients. Methods 90 cases confirmed no significant rupture of annulus fibrosus of lumbar disc herniation by CT or MRI were randomly divided into two groups. Bilateral and unilateral puncture plasma coblation nucleoplasty groups were respectively divided into group A and group B, and compared the JOA scores. Results 82 patients were followed up for 10 ~ 32 months,the average follow-up time was 16.2 months. Preoperative JOA scores of the two groups were not statistically significant(P > 0.05) but the postoperative follow-up JOA scores were higher than preoperative(P <0.05) and the JOA scores of group A were higher than group B(P <0.05). Conclusion The medium-term therapeutic efficacy of bilateral puncture coblation nucleoplasty was superior to unilateral therpy.

7.Medial leg flap containing posterior tibial vessels for reconstructio n in children's limb injuries by high-energy

[Objective]To report the clinical results of free medical leg flap containing posterior tibial vessels (FMLFPTV) in repairing of serious limb trauma caused by high-energy in children and discuss the flap's unique superiorities.[Method]From Jan.1999 to Mar.2006,FMLFPTV were obtained under tourniquet and applied in ten children suffered from serious limb and extremity injuries complicated by soft tissue defects,meanwhile,vessels and nerves were repaired with the flaps' corresponding tissues,moreover,broken bones and tendons were reconstructed.[Result]Average blood loss was 183ml and mean operative time was 208 min.All flaps were transplanted successfully with excellent results including satisfactory limb appearance.Follow-up ranged 14 to 67 months (averaged 35 months).Sensation has recovered to more than S3 in all cases.In those who were treated with this innervated flap to repair defects of weight-bearing area of sole,no sore or ulcer occurred and normal gait recovered.Bone union took 4~9 months(averaged 6.7 months).Through investigation forms filled by parents and clinical examination,no blood supply problem or morbidity was discovered in donor limbs.[Conclusion]FMLFPTV is easy to dissect under tourniquets with relative short operative time and less blood lost which especially benefits children.The flap is thin and has almost necessary related tissues for all reconstructive needs,for example,to bridge vessel and nerve defects,and offer excellent results.It is an easy and reliable method.
8.Immediate reconstruction of Gustilo grade-Ⅲ long bone fractures caused by high energy:29 patients

[Objective]To report the clinical results of immediate reconstruction of open grade Ⅲ long bone fractures caused by high-energy.[Method]From Jan.2001 to Jul.2006,vascular flaps were applied in 29 patients suffering from massive limb trauma,complicated by Gustilo grade-Ⅲ open fractures(11 were type B,and 18 type C) and massive soft tissue defects,caused by high-energy,through an emergency procedure.Meanwhile,vessels and nerves were repaired,broken bones and tendons were reconstructed.The mean MESS(Mangled extremity severity score) score was 6.27(6.27?2.43).[Result]All limbs were salvaged and all the flaps survived without protracted course to obtain soft tissue coverage.The mean hospital stay was 23 days(12-67 days) and the follow-up ranged 14 to 58 months(mean 26 months).No severe complication occurred,including wound infection,bone nonunion,necrosis and morbidity.The ultimate functional results of the salvaged limbs were at least better than artificial limbs.[Conclusion]The immediate reconstruction of severely injured limbs complicated by Gustilo type Ⅲ fractures caused by high energy has the advantages over traditional methods.This technique enables controlling bone,soft tissue and other problems together through an emergency procedure with a relatively shortened in-hospital stay and less fibrosis,thereby favoring the functional outcome.Early wound healing favors antibiotic diffusion and local blood supply.It may be an alternative for fast fixation and better bone healing.
9.Clinical characteristics of retinal detachment after vitrectomy

?AIM:To investigate the causes and characteristics of retinal detachment after vitrectomy.?METHODS: Retrospective analysis of patients in our hospital in recent 4a who underwent primary, standard, three -port pars plana vitrectomy ( PPV ) . Retinal detachment occurred in 24 eyes ( 23 cases ) , 24-67 years old, average 49.0±12.0 years old.There were male in 17 cases, female in 6 cases, 6 right eyes, 18 left eyes.?RESULTS:Eighteen (75%) eyes without any symptoms were found during a routine check, 6 eyes ( 25%) with decreased visual acuity or in front of shadow.It took 1-223d from PPV to retinal detachment, average 40.7 ± 54.2d, of which retinal detachment occurred in 10 (42%) eyes within 1wk, 14 (58%) eyes within 1mo.The direct cause of retinal detachment was new retinal break ( a total of 27 holes ) in 19 eyes, proliferative vitreoretinopathy ( PVR) in 4 eyes, 1 eyes with primary hole not closed.All retinal detachment were ultimately completely reset.?CONCLUSION: The occurrence of new retinal breaks is the main cause of retinal detachment after vitrectomy, attention should be paid to the early postoperative peripheral retinal examination;after a timely surgery, the retina can be reset.
10.Correlation between HLA gene and Takayasu arteritis in the Chinese

Objective To determine the correlation between HLA gene and Takayasu arteritis(TA) in the Chinese. Methods Forty four TA patients and 135 healthy controls were examined for HLA DQA 1 and HLA DQB 1 locus alleles by PCR/SSOP typing and mass genetics techniques. Results The frequency of DQA 1 0301 allele(14.8%) in TA patients was significantly decreased compared with the controls( 30.7% )(P0.05). Conclusions In Chinese, people with HLAⅡ DQB 1 0601 gene may be susceptible to TA, while people with HLAⅡ DQA 1 0301 gene may be resistant to TA.
