3.Study on Pharmacodynamic of Ketamine Hydrochloride for Intranasal Administration by Adding Different Absorption Enhancers

OBJECTIVE: To optimize the formulation of ketamine hydrochloride for intranasal administration that was of effective anesthetic effect.METHODS: Using ketamine hydrochloride injection as basic preparation,various nasal absorption enhancer or viscosity modifiers(0.5%,1% and 2% azone,EDTA,carbomer,2-hydroxypropyl-?-cyclodextrin,carboxymethyl cellulose) were added to make different nasal drops.After intranasal administration,the anesthetic effect was evaluated to find the optimum formulation.The behavior of anesthetic induction period and anesthetic index were used to evaluate the anesthetic effect.RESULTS: The perfect anesthetic effect was achieved when the formulation was prepared by adding 1% azone in ketamine hydrochloride injection(at a dose of 10 mg?kg-1).The anesthetic time was long.The muscle relaxing time,eyelid reflecting disappearing time,pain disappearing time and righting reflecting disappearing time were(21.4?7.1) min,(11.4?4.6) min,(15.6?5.7) min and(12.0?4.8) min,respectively.CONCLUSIONS: Optimal formulation of ketamine hydrochloride for intranasal administration was obtained when 1% azone was added as an absorption enhancer.
4.Application of diffusion tensor imaging in the neurodevelopmental evaluation of premature cerebral white matter damage

Cerebral white matter damage is the most common form of brain injury in premature infants,which is the main cause of neurological and mental injury and cerebral palsy.Imaging examination plays an important role in the diagnosis and follow-up of white matter damage.Because of the safe and accurate characteristics,nuclear magnetic resonance has become the most common imagine form.Different from the traditional nuclear magnetic resonance technique,diffusion tensor imaging (DTI) can get quantitative analysis of cerebral white matter fiber bundle in vivo.This technology has become a powerful tool for the evaluation of white matter damage.The article summarizes the application of DTI in the neurodevelopmental evaluation of cerebral white matter damage in premature infants.
5.Application of 3D printing teaching model in spine surgery teaching

Objective:To explore the application effect of 3D printing teaching model on spinal surgery teaching.Methods:A total of 50 eight-year clinical medical students of Batch 2015 were selected and randomly divided into two groups. Traditional teaching mode was used in the control group and 3D printing teaching model was used in the experimental group. The performance of graduation from the department, teaching experience indicators and teaching satisfaction were compared between the two groups. SPSS 22.0 was used for t test. Results:The performance of the experimental group was higher than that of the control group ( P < 0.05). The scores of teaching experience indicators in the experimental group were all higher than those in the control group ( P<0.05). The satisfaction rate of the experimental group was higher than that of the control group ( P < 0.05). Conclusion:3D printing teaching model can significantly improve students' theoretical knowledge assessment and surgical design evaluation, and improve the teaching effect and students' satisfaction.
7.Risk factors of acute organophosphorus pesticide poisoning

Objective To explore the risk factors of acute organophosphorus pesticide poisoning(AOPP). Method The patients with acute organophosphorus pesticide poisoning,admitted from September 2004 to August 2005,were retrospectively analyzed.The 87 patients were divided into groups according to the presence or absence of combined conditions including hypotension,hypoxemia or metabolic acidosis.Acute physiology and chronic health evaluationⅡ(APACHEⅡ)score was calculated.The chi-square test was used to examine the mortality between those groups.Results The total in-hospital mortality of 87 acute organophosphorus pesticide poisoning patients was 21.8%.The mean APACHEⅡscore was(7.58?5.32)in the 68 survivors and(21.17~9.46)in the 19 dead,there were significant differences between the survivors and the dead(t=9.25,P20 was 65.2%(15/23),andit was 6.3% (4/64)in patients with APACHEII score
8.Study on expression,puriifcation and properties of recombinant human Chymotrypsin

Objective To study the prokaryotic expression, puriifcation and properties of recombinant human Chymotrypsin. Methods The protein was highly expressed in E.coliBL 21 (DE 3) as inclusion body. After refolding and activated with trypsin, the activated protein was obtained and purified with ion-exchange chromatography(CM-FF), some properties of the recombinant human chymotrypsin was investigated. Results The molecular weight of chymotrypsinogen was about 30 KD in SDS-PAGE, total activity recovery rate of CM-FF puriifcation was 93.7%. The recombinant chymotrypsin kept stable from pH 3 to pH 5, and owned good temperature stability. Km was 0.067 mmol/L with BTEE as a substrate. The UV maximum absorption wavelength was 281 nm.Conculsion The recombinant human enzyme was expressed successfully, and a feasible production method to get a high activity of the recombinant human chymotrypsin was established.
9.Retroperitoneal fibrosis:a clinical and outcome analysis of 33 cases and review of literature

Objective To investigate the clinical features and outcome of retroperitoneal fibrosis (RPF).Methods Thirty-three cases of RPF treated in the Department of Rheumatology and Immunology were analyzed retrospectively,including the clinical characteristics and laboratory data.Results RPF occurred predominantly in elderly men with untypical clinical manifestations such as back pain,abdominal pain and lower limb edema.In laboratory tests,the inflammatory parameters such as erythrocyte sedimentation rate and C-reactive protein increased significantly,and renal function failure was frequent finding in patients with urethral obstruction.All patients had retroperitoneal soft tissue image or urethral obstruction on computed tomography or magnetic resonance imaging,four of which had histological diagnosis of idiopathic RPF.Twenty-three patients received surgical interventions.Eighteen patients received medical treatment alone including corticosteroids,immunosuppressants,and tamoxifen.Nine patients received corticosteroids after surgical intervention.Surgery and medical treatments are effective for idiopathic RPF.Conclusion Image examination is valuable in diagnosis and differential diagnosis of PRF.Pathological diagnosis is the golden standard.Long-term low dose corticosteroids and immunosuppressants may prevent relapse of idiopathic RPF.
10.Clinical observation of different concentrations of ropivacayin combined with dezocine for Epidural analgesia

Objective To investigate the analgesic effect and motor block of ropivacayin combined dezocine in lower abodiminal operation.Methods Sixty patients of ASA Ⅰ - Ⅱ were divided into three groups randomly(n =20 for each group).They were treated with dezocine 5 mg +0.125% ropivacayin/100 ml( group DI ),dezocine 5 mg +0.175% ropivacayin/100 ml( group D2),dezocine 5 mg +0.225% ropivacayin/100 ml (group D3 ).The initial dose was 5 ml,and the maintenance dose was 2 ml/h,with lock-out time of 20 mins,and the duration of patient-controlled epidermal analgesia (PCEA) was 48 h.The analgesic effect was evaluted by VAS score at 2,4,12,24 and 48 h after surgery.Bromage score and the side effects such as motor ability,and vomiting after surgery were recorded.Results At 2,4,12 h after surgery,the VAS scores of group D2 and D3 were less than that of group D1 (2.54 ± 1.34,2.47 ± 1.05 and 3.62 ± 1.02 at 2 hrs,2.13 ± 1.30,2.15 ± 1.28 and 3.56 ± 1.10 at 4 hrs,1.03 ± 0.43,1.04 ± 0.51 and 1.92 ± 1.20 at 12 hrs,respectively) ( Ps < 0.05 ).As for the Bromage score among the 3 groups,there were no sighificant diffrences between group D1 and D2,wheras they were significantly lower than that in group D3 ( 1.21 ± 0.21,1.28 ±0.22 and 1.53 ± 0.31 at 2 hrs,1.15 ± 0.15,1.44 ± 0.25 and 1.61 ± 0.12 at 4 hrs,0.92 ± 0.14,0.99 ± 0.13 and 1.71 ± 0.22 at 12 hrs,0.85 ± 0.11,0.88±0.14 and (1.66±0.15) at 24 hrs,0.42 ± 0.10,0.55 ± 0.09 and 1.19±0.11 at 48 hrs,respectively,Ps <0.05).There were 2,2 and 3 patients affected with nausea and vomiting among the 3 groups,and there were no other side effects ( P > 0.05 ).Conclusion 0.175% ropivacayin combined with dezocine 5 mg via PCEA in lower abodiminal operation shows better analgesic effect and benefical to the restoration of lower limbs movements.
