1.The role of myocardial metabolism activating drugs in children with severe angiocardiopathy

Myocardial metabolism activating drugs can increase the energy supply and decrease the energy consumption of the myocardial cells,playing a role in the nourishment and protection of cardiac muscles and the optimization of the energy metabolism.It is the adjuvant drug in treatment of myocarditis,myocardiosis,severe arrhythmia,acute or chronic heart failure and cardiosurgery,with an irreplaceable role in the salvage therapy of severe angiocardiopathy.Due to its safety and little side effects,it has been widely used in pediatrics.
2.Severe rigid kyphotic scoliosis treated with posterior spinal osteotomy

Objective: To evaluate the effect of severe rigid kyphotic scoliosis treated with posterior spinal osteotomy Methods: A total of 11 cases(average 11.2 years) of severe rigid kyphotic scoliosis treated with posterior spinal osteotomy from Mar.2005 to Sept. 2006 were retrospective. Of those cases, 3 were of neurofibromatosis, 5 of congenital scoliosis, 1 of poliomyelitis and 2of idiopathic scoliosis. The flexibility of all patients was less than 25%. They had an average follow-up period of 1.3 years (0.5-2 years) Results: There were 3 cases of one segment posterior wedge osteotomy, 2 of two segment posterior wedge osteotomy and 2 of three segment wedge osteotomy. Transpedicular osteotomy was performed in 2 cases, and the last 2 cases encountered vertebral resection and posterior column removed. The average kyphotic angle was 108 degrees (ranging from 87 to 135 degrees) and the average scoliosis angle was 97 degrees (ranging from 65 to 135 degrees) before operation. After operation the average kephotic angle was corrected to 49 degrees (the correction rate was 55%) and the average scoliosis angle was corrected to 37 degrees(the correction rate was 66%) . The height rose up by an average of 4.7 cm. No neurological complication happened in all the 11 cases. Conclusion: It is an effective method that severe rigid kyphotic spinal scoliosis is treated with posterior osteotomy.
3.The role of bone marrow micrometastasis in breast cancer in judging prognosis

Objective To investigate the relationship between bone marrow micrometastasis of patients with breast cancer and clinical pathological parameters, some molecular markers as well as prognosis.Methods The expression of hMAM mRNA in BM of patients with breast cancer was detected by RT-PCR.The expressions of ER, PR in cancer tissues were detected by immunohistochemical SP method. Results About 38.2 % positive expression rate of hMAM mRNA in 102 patients with stage Ⅰ -Ⅳ breast cancer was found.The expression of hMAM increased more in patients with T2-3 (>2 cm) tumors than T1 (≤2 cm) (P =0.001)and with stage Ⅱ ,Ⅲ than stage Ⅰ (P =0.001). The expression of hMAM in the BM of breast cancer with grade I was lower than that of grade Ⅱ or Ⅲ (P =0.014). The expression of hMAM in the BM was related to the pathological type (P =0.032) and the axillary lymph node metastasis (P =0.001). The expressions of hMAM in BM were much higher with ER negative in breast cancer tissues (P <0.05). There was a correlation between patients with positive expression of hMAM in BM and distant metastasis (P =0.009). Conclusion The micrometastasis in BM is correlated with some clinical pathological parameters and some tumor markers. The patients with positive expression of hMAM in BM have more chances with distant metastasis and poor prognosis. The detection of micrometastasis may be as one of targets to predict the prognosis of breast cancer.
4.Purification Method of Ultrasonic Emulsification Combined with Ultrafiltration Membrane Separation for Flavonoids in Cacumen Platycladi

Objective:To obtain flavonoids of Cacumen Platycladi with high purity by using the method of ultrasonic emulsification combined with ultrafiltration membrane separation.Methods:Seven compounds including myricetrin as the reference substances,an HPLC method was used for the content determination.Using the single factor experiments,ultrasound,microwave,reflux and ultrasonic emulsification were compared,and using the orthogonal experiments,the ultrasonic emulsification time,ethanol concentration,solid-liquid ratio and times were studied.Biomax-5 membrane was adopted to improve the membrane separation.Results:The single factor experiments showed the homogeneous extraction method with the highest contents.The orthogonal experiments showed the optimal extracting conditions as follows:the ultrasonic emulsification time was 15 min,the ethanol concentration was 50%,the material-liquid ratio was 1∶10,and the extraction times was 3.The conditions of ultrafiltration membrane separation were as follows:the flow rate was 1.5 L·min-1,and the membrane separation pressure was 0.8 kg.Conclusion:The combination of ultrasonic emulsification and ultrafiltration membrane separation is feasible in the extraction and purification of flavonoids in Cacumen Platycladi,and the product is with high purity,suggesting the method has good application prospect.
5.Clinical Observation of Shenmai Injection Combined with Insulin in the Intensive Treatment of Stress Hy-perglycemia in the Septicopyemia Patients

OBJECTIVE:To observe the efficacy of Shenmai injection combined with insulin in the intensive treatment of stress hyperglycemia in the septicopyemia patients. METHODS:The data of 156 septicopyemia patients with stress hyperglycemia was collected Retrospectively,according to the different medication divided into control group (78 cases) and observation group (78 cases).All patients determined the pathogenic bacteria and received related anti-infection treatment,improving ventilation state, adequate fluid resuscitation support,immune therapy,nutritional support and other conventional treatment;based on it,control group received different dosages of insulin [1 U/ml,intravenous micro pump,with speed of 0.1 U/(kg·h)] based on different blood glucose levels;observation group additionally received 40 ml Shenmai injection,adding into insulin mixed solution,intrave-nous micro pump. Endpoint events(mortality),recovering time of blood glucose,total dosage of insulin,basic indicators [Acute physiology and chronic health(24 APACHE-Ⅱ)score,mechanical ventilation time,hospitalization days in ICU and frequency of hypoglycaemia] and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total mortality rate in observa-tion group was significantly lower than control group,recovering time of blood glucose was significantly shorter than control group,the total dosage of insulin was significnatly less than control group,mechanical ventilation time was significantly shorter than control group,hospitalization days in ICU was significantly less than control group,the differences were statistically significant (P<0.05).before treatment,there were no significant differences in the 24 APACHE-Ⅱ score in 2 groups (P>0.05);after treat-ment,the 24 APACHE-Ⅱ score in 2 groups were significantly reduced,and observation group was lower than control group,the differences were statistically significant(P<0.05). And there were no obvious adverse reactions in 2 groups during treatment. CON-CLUSIONS:Based on conventional treatment,Shenmai injection combined with insulin shows good efficacy in the intensive treat-ment of stress hyperglycemia in the septicopyemia patients,it can decrease total dosage of insulin,reduce mortality rate and 24 APACHE-Ⅱscore,shorten mechanical ventilation time and decrease hospitalization days in ICU,with good safety.
6.Low resolutation fluorotyping of HLA-DRB1 locus

Objective To investigate and evaluate a novel fluorotyping procedure for HLA-DRB1 locus. Methods On the basis of low resolutation SSP technique,the fluorogenic probe was used to establish the fluorotyping procedure of HLA-DRB1 which was carried out for 46 samples. Results The fluorotyping has been successful for all the 46 samples and was completely coincident to the SSP results.As compared to the serological typing of DR locus,the coincidence rate was 66.3%(61/92) whereas the coincidence rate of both the 2 specific genes was 43.5%(20/46). Conclusions Fluorotyping is accurate,sensitive,reproducible,depends less on manual manipulation and eliminates the problems related to contamination.
7.The Value of Galactography in Diagnosis of Breast Diseases with Nipple Discharge: A Report of 70 Cases

Objective To explore the value of galactography in the diagnosis of breast diseases with nipple discharge. Methods X-ray features, clinical manifestations and pathological results in 70 patiens with nipple discharge proved by clinic and pathology were analyzed retrospectively. Results There were intraductal papilloma in 45 cases, ductal carcinoma in 5 cases, ductal dilatation in 17 cases, breast duct hyperplasia accompanied chronic mastitis in 3 cases .Conclusion Galactography plays important role in diagnosis and differentiation of the breast duct tumor and chronic mastitis diseases, and provides significant information for clinic.
8.The role of lumbar drainage in symptomatic cerebral vasospasm after aneurysmal subarachnoid hemorrhage

Objective To investigate the risk factors of symptomatic vasospasm after aneurysmal subarachnoid hemorrhage and evaluate the effect of lumbar drainage of cerebrospinal fluid on vasospasm.Methods In this retrospective controlled-cohort study, 175 patients with aneurysmal subarachnoid hemorrhage met our study criteria between January 2012 and December 2013.By multi-factor regression analysis, gender, age, Hunt-Hess grade, modified Fisher grade and lumbar drainage were analyzed.The outcomes were assessed by the presence or absence of symptomatic cerebral vasospasm and vasospasm-related infarction, and the mean days of hospital stay and score of Glasgow Outcome Scale at 1-month follow-up.Results Several factors affected the prognosis of subarachnoid hemorrhage.Lumbar drainage was a protective factor of symptomatic cerebral vasospasm (OR =0.243, 95% CI: 0.119-0.497) and vasospasm-related infarction (OR =0.305, 95% CI: 0.154-0.604).The patients with lumbar drainage had higher score of Glasgow Outcome Scale (P < 0.05).But the patients with lumbar drainage had longer hospital stay (P < 0.05).Conclusions Lumbar drainage of cerebrospinal fluid after aneurysmal subarachnoid hemorrhage markedly reduced the risk of symptomatic cerebral vasospasm and improved outcome.
9.Effect of radix paeoniae rubra on expressions of p38MAPK,NF-κB and iNOS in a rat model of li-popolysaccharide-induced acute lung injury

Objective To investigate the effect of radix paeoniae rubra (RPR) on expressions of p38MAPK, NF-κB and iNOS in ltpopolysaccharide (LPS)-induced acute lung injury (ALI) and explore its protective mechanism. Methods Forty Wistar rats with ALI were divided randomly into five groups: saline control group (Group A) , LPS group (Group B), RPR for treatment group (Group C), RPR for prevention group (Group D) and SB203580 group (Group E). The effects of RPR on protein content and the ratio of neutrophiles in bronchoalveolar lavage fluid (BALF), contents of malondidehyde (MDA) and serum NO in lung tissue were observed. Arterial blood was collected for blood gas analysis. Expressions of p38MAPK, NF-κB and iNOS in rat lung tissues were detected. Results Compared with Group A, expressions of p38MAPK, NF-κB and iNOS were significantly increased (P <0.01 orP< 0.05) , the protein content and the ratio of neutrophiles in BALF, contents of MDA and serum NO were significantly higher (P<0.01 or P<0.05) in Groups B, C, D and E. There was a significant decrease in the level of arterial bicarbonate and partial pressure of oxygen in Groups B, C, D and E (P<0.01 or P<0.05). Compared with Group B, the expressions of p38MAPK, NF-κB and iNOS were significantly lower, and the protein content and the ratio of neutrophiles in BALF, contents of MDA and serum NO were significantly decreased, while the levels of arterial bicarbonate and partial pressure of oxygen were significantly higher in Groups C, D and E (P<0.05 or P<0.01). Compared with Group B, the lunginjury of Groups C, D and E was significantly alleviated, while there was no statistical difference among Groups C, D and E in the indices of lung injury. Conclusion Protective effect of RPR on LPS-induced ALI is closely related to the inhibited expressions of p38MAPK, NF-κB and iNOS.
10.Optimization of the intranasal powders of Panax Notoginseng Saponins by the central composite design-response surface methodology

AIM: To determine the optimized intranasal powders formulation of Panax Notoginseng Saponins(PNS). METHODS: According to the indexes of the in vitro release rate in 1h and 12h of Rb1 and Rg1, bioadhesive intensity and the ciliary toxicity in situtoad palatal mucosa of the preparations, the optimized formulation was determined by the central composite design-response surface methodology. RESULTS: When the correspondent percentage of PNS, microcrystalline cellulose (MCC) and moderate viscosity hydroxypropyl cellulose (H-HPC) was 31%, 60% and 9%, respectively, the release rate in 1h and 12h of Rb1 and Rg1 was 6.82%, 53.25% and 83.4%, 95.09%, respectively. The adhesive time was 78min, and the lasting time of ciliary movement after the rinsing of the PNS powders was 94.84% to the physiological saline group which suggested the powder was almost no toxicity. CONCLUSIONS: The optimized formulation of the PNS intranasal powder was obtained quickly and conveniently by the central composite design-response surface methodology.
