1.The Effect of Fentanyl on Proliferation and Cell Cycle of Human Breast Carcinoma Cell Line MCF-7

Objective To study the effects and its mechanism of fentanyl(Fen) on the proliferation and cell cycle of human breast carcinoma line MCF-7. Methods MCF-7 cells were cultured in the medium with Fen, naloxone(Nx) or both the medicines at different concentration for different time. MTT method was employed to evaluate the level of the cell proliferation. The distribution of the cell cycle was detected with the flow cytometry (FCM). The expression levels of p53 and p21/WAF1 in the cells were determined by SP immunocytochemical staining method. Results Fen at≥0.1?mol/L concentration inhibited MCF-7 cells proliferation in dose- and time-dependent manners, and its IC 50 for 72h was 0.81?0.02 ?mol/L. However, the antiprolifeative effect of Fen was not antagonized by Nx. Fen significantly enhanced the ratio of G 0/G 1 phase MCF-7 cells, and decreased the proliferation index of MCF-7 cells in dose-dependent manner. Fen also upregulated the expression of p53 and p21/WAF1 in MCF-7 cells. Conclusion The data suggested that the inhibitory effect of Fen on MCF-7 cell growth might be mediated by blocking cell cycle progression from G 1 to S phase, and upregulating the expression of p53 and p21/WAF1.
2.Distribution and clinical significance of EML4-ALK fusion gene in phase Ⅰ lung cancer

Objective To detect the mutation frequency of EML4-ALK fusion gene in lung cancer patients, and to inves-tigate the distribution of mutation character for EML4-ALK fusion gene in Ⅰ stage lung cancer patients and clinical features as well as provide a reference for the individual treatment of lung cancer .Methods 256 fresh tumor tissue specimens of lung cancer patients were screened from the specimen bank of our hospital and all the patients had accepted the surgical treatment from February 2013 to December 2014.Total RNA was extracted and then be transcribed into cDNA, the amplification-refrac-tory mutation system(ARMS) was used to detect mutation of EML4-ALK fusion gene.The results according to the positive con-trol, negative control and RNA quality control for EML4-ALK fusion type were analyzed.Results During the 256 patients ofⅠ stage lung cancer, there were 17 patients(6.64%) had mutations in EML4-ALK fusion gene.In lung adenocarcinoma mu-tation rate(16/207, 7.73%) was higher than that of lung squamous cell mutation rate(1/39, 2.56%), lung adeno-squamous mutation rate(0/4, 0) and large cell carcinoma(0/5, 0) of the mutation rate;young lung cancer patients( <63 years) of the mutation rate(14/139, 10.07%) was significantly higher than the high age of lung cancer patients(≥63 years old) mutation rate(3/117, 2.56%), P =0.009.EML4-ALK fusion with tumor invasion and visceral pleura group incidence (9/80, 11. 25%) was significantly higher than that of non-invasive and visceral pleura group incidence rate(8/176, 4.55%), P =0.045.Conclusion The occurence of EML4-ALK fusion correlates with patients’ age as well as whether visceral pleura is in-vaded, type 1 EML4-ALK fusion was detected more in phase I lung cancer patients.
3.Clinical Study on traditional Chinese medicine treatment of CSG and its impact on MTL, GAS and SS

Objective To discuss of the therapeutic effects of treating three TCM types of Chronic superficial gastritis (CSG)disharmony of liver and stomach, weakness of the spleen and the stomach, and damp heat in the spleen and the stomach with chinese medicine, and to discuss its impact on the changes of three gastrointestinal hormones-- motilin (MTL), gastrin (GAS), and somatostatin (SS). Methods The data of 150 CSG patients in Shenzhen Bao’an Hospital from Feb, 2011 to Mar, 2013 were selected. These patients belonged to three kinds of syndrome, which was disharmony of liver and stomach, weakness of the spleen and the stomach, and damp heat in the spleen and the stomach, each syndrome had 50 cases. Different Chinese medicines were applied to different syndrome of patients. After one month treatment, the therapeutic effects and the changes of MTL, GAS, SS in these patients were detected by radioimmunoassay. Results After treatment, the total effective rate of the damp heat in the spleen and the stomach syndrome was 94%, the weakness of the spleen and the stomach type was 88%, disharmony of liver and stomach type was 90%, the difference was not statistically significant among the three types(χ2=3.84, P>0.05);after treatment, the serum MTL, SS level of disharmony of liver and stomach, weakness of the spleen and the stomach, and damp heat in the spleen and the stomach type were [damp heat in the spleen and the stomach type were (240.25±7.29)pg/ml, (68.23± 5.97)pg/ml;the weakness of the spleen and the stomach type were (260.16±8.84)pg/ml, (75.35±4.65)pg/ml;disharmony of liver and stomach type were(258.32±15.23)pg/ml, (80.12±7.88)pg/ml] higher than before treatment [damp heat in the spleen and the stomach type were (87.26±7.58)pg/ml, (49.34±4.93)pg/ml;the weakness of the spleen and the stomach type were (67.11±7.81)pg/ml, (57.15±5.11)pg/ml; disharmony of liver and stomach type were (89.26±7.60)pg/ml, (40.63±7.26)pg/ml], and the levels of serum GAS [(156.38±17.36)pg/ml, (146.15±15.56)pg/ml, (140.26±13.37)pg/ml] was lower than before treatment [(274.59 ± 17.82)pg/ml, (320.83 ± 15.02)pg/ml, (216.15 ± 13.77)pg/ml], the difference was statistically significant (P<0.05 or 0.01). Conclusions The changes of three hormones-MTL, GAS, SS can be used as an objective basis for the diagnosis and the judgment of healing efficacy of CSG treated by traditional Chinese medicine.
4.Summary of best evidence for the management of eating behaviors in children with autism spectrum disorders

Objective:To search, evaluate and summarize the best evidence of intervention and management of eating behavior in children with autism, so as to provide evidence for medical staff and caregivers of children with autism to manage eating behavior problems.Methods:According to the evidence pyramid "6S" model, the clinical decisions, recommended practices, guidelines, evidence summaries, expert consensus, systematic reviews and randomized controlled trials on the intervention and management of eating behavior in children with ASD were systematically searched from domestic and foreign databases, related guide websites and professional association websites. The search period was from the establishment of the database to March 31th, 2023. The quality of the guide was independently evaluated by 4 researchers, and the remaining articles were evaluated by 2 researchers. Content extraction, evidence integration and grading of the included articles were carried out, and the level of evidence recommendation was formed through the expert meeting.Results:A total of 19 articles were included, including 3 guidelines, 1 expert consensus, 13 systematic reviews and 2 randomized controlled trials. This paper summarized 25 best evidence from 5 aspects, including the common eating behavior problems of children with autism, the assessment of eating behavior problems, causes, consequences and intervention methods.Conclusions:The best evidence summarized in this study provides a basis for the management of eating behavior problems in children with autism. It is suggested that clinical staff should fully consider the clinical context when applying the evidence, and formulate more detailed and feasible eating behavior intervention programs combined with the children and their families.
5. Correlation analysis between Dx-pH monitoring and proton pump inhibitor test in the diagnosis of laryngopharyngeal reflux disease

Objective:
The consistency of 24-hour oropharyngeal Dx-pH monitoring and proton pump inhibitor(PPI) test in the diagnosis of laryngopharyngeal reflux disease (LPRD) was investigated.
Methods:
Sixty patients with laryngopharyngeal reflux (LPR) related symptoms who had never received PPI treatment were assessed by reflux symptom index (RSI) and reflux finding score (RFS) between October 2017 and October 2018, including 28 males and 38 females, aged from 16 to 72 years, with a medium age of 38 years. Prior to treatment, all patients were evaluated with 24 hours oropharyngeal Dx-pH monitoring(Restech). After empiric therapy with PPI twice-daily for 8 weeks, the efficacy was evaluated according to posttreatment RSI score.The data was analysed with Kruskal-Wallis test, Student Newman Keuls test and consistency check.
Results:
(1)Among all 60 patients,13 patients (21.7%) had pathologic Ryan score and all resulted responsive to PPI;27 patients (45.0%) with a negative Ryan score were unresponsive to PPI; 20 patients (33.3%) despite a negative Ryan score resulted responsive to PPI therapy. Considering responsiveness to PPI therapy as the gold standard for the diagnosis of LPRD, the sensitivity, specificity, positive predictive value and negative predictive value of Ryan score were 39.4%, 100%, 100% and 57.4% respectively. This paper aims to discuss the ideas and experience about the radiology residency training system of Canada with a presentation of its base accreditation standards for five aspects, competency goals for seven roles, four stages of training arrangement, and two types of final assessment questions. Although the Canada's radiology residency program differs from China's standardized resident and specialist training programs for radiology, there are still several points that are worth referencing, including emphasizing the training priority of competency goals, providing a specific basis for the stratification of training, offering clear guidance for the implementation of training content, and improving assessment methods to focus on competency goals. These points are of great value for improving the standardized radiology resident and specialist training programs in China, so as to provide a reference for the training of excellent radiologists in China.
