1.A questionnaire survey of healthy quality of life in obstructive sleep apnea hypopnea syndrome and analysis of related factors

Objective:To investigate the health-related quality of life (HRQOL) of patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) through a questionnaire.Methods:The study period was from January 2019 to August 2019. A cross-sectional study was used to evaluate the quality of life of all 200 subjects diagnosed with OSAHS (OSAHS group) and 50 healthy controls (healthy control group) in Zhejiang Hospital by means of 36 summary health surveys (SF-36). Spirometry was performed to assess lung function, and the chronic lung disease (CLD) severity index was used to assess the severity of symptoms, measured patients′ neck circumference, and analyzed correlations between variables and patients′ quality of life.Results:The average disease duration of the OSAHS patients was (2.8 ± 0.5) years, the average Epworth sleep scale (ESS) score was (8.9 ± 0.8) scores, the CLD index was (30.5 ± 2.2) scores, and the average neck circumference was (41.5 ± 3.8) cm, respectively. Compared with healthy control group, the OSAHS patients had significant damage in all fields, and the difference was statistically significant ( P < 0.05). Lung function indicators including the percentage of forced expiratory volume in the first second occupied estimated value (FEV 1%), the ratio of forced expiratory volume in the first second and forced vital capacity (FEV 1/FVC) showed negative correlations with scores of SF-36 in all fields ( P < 0.05). The age, disease duration, ESS score, neck circumference and body mass index (BMI) were associated with poor HRQOL( P < 0.05). Patients′ living standard was significantly correlated with HRQOL ( χ2 = 71.25, P < 0.01). Conclusions:The HRQOL of patients with OSAHS is generally low. Poor lung function, increased disease duration, smoking, increased neck circumference and BMI, and increased daytime sleepiness can adversely affect HRQOL.
2.Analysis of the Application of Antifungal Drugs in Haematologic Patients from a Third Grade Class A Hos-pital during 2013-2014

OBJECTIVE:To provide reference for the formulation of correct antifungal treatment strategy,and to promote stan-dard use of antifungal agent. METHODS:A retrospective survey was conducted for 138 haematologic patients from May 2013 to May 2014 in a third grade class A hospital,of whom all had used antifungal drugs during hospitalization. We collected all patients' information and analyzed it statistically. RESULTS:Of 138 haematologic patients,3 were proven IFD (all were Candida infec-tion),6 were probable IFD,12 were possible IFD,and 117 were undefined IFD. The positive rates of fungi pathogenic detection, fungal smear,G-test,and GM-test were 15.3%,9.4%,6.4% and 23.4% respectively. 6 kinds of antifungal were used,and vori-conazole had the highest frequency,followed by fluconazole,itraconazole,amphotericin B,caspofungin and micafungin. 62.3%patients used only one kind of antifungal,but 15.9% patients used 2 or more kinds of antifungal. The average medication course was 20.5 days(1 day to 125 days). Irrational drug use showed improper drug selection,unreasonable dose,and replacing antifun-gal with insufficient basis. CONCLUSIONS:The antifungal use in haematologic patients in the hospital is consistent with the re-quirements of guidelines,but there are still some issues as insufficient antifurgal drug treatment course to be further standardized.
3.HHIE-S Scale for Hearing Screening in the Elderly Population

Objective To investigate the reliability and practicality of Chinese version the hearing handicap in-ventory for the elderly -screening version(HHIE -S) scale for elderly hearing screening in China .Methods We compared the HHIE -S scores with results of pure -tone audiometry test .A total of 840 Chinese individuals older than 60 years filled in the scale all by themselves first ,then refilled it according to explanations of each question translated by researchers .All participants finished pure -tone audiometry test with frequencies of 0 .25 ,0 .5 ,1 .0 , 2 .0 ,and 4 .0 kHz .Sensitivity ,specificity ,positive predictive value (PPV ) ,false-negative rate and false -positive rate were compared with HHIE -S cut -off points of >8 or >10 and pure-tone averages (PTAs) greater than 25 ,40 and 60 dB HL .Analysis the area under the curve (AUC) values of receiver operating characteristic (ROC) curves for each three PTA levels .Results Cronbach's α of the scale was 0 .85 .HHIE -S scale pre -explanatory score was 7 .6 ± 10 .6 ,and post-explanatory score was 10 .8 ± 16 .7(P<0 .01) .HHIE -S> 8 points higher than H HIE-S> 10 points in the sensitivity ,positive predictive value increase ,specificity ,false-positive rate ,false negative rate decreased ,paired samples t - test sensitivity and false -negative rate (P< 0 .05) .After the guide meanwhile HHIE-S cut-off points of > 8 when grouping with PTA> 25 dB HL ,PTA> 40 dB HL and PTA>60 dB HL ,conducted ROC curve analysis ,the AUC value was 0 .70 ± 0 .03 ,0 .84 ± 0 .01 and 0 .88 ± 0 .02 .Conclusion The score of HHIE-S scale>8 points can be used when PTA> 40 dB HL as goal for hearing loss of old people , it can be more accurate after explanations ,so HHIE-S scale can be used as a preliminary hearing screening method for Chinese elderly people .
