ObjectiveTo discuss the clinical value of ultrasonically activated scalpel(UAS) in breast cancer patients who underwent 2-3 cycles of neo-adjuvant chemotherapy by comparing UAS and electric knives (EI) in modified radical mastectomy. MethodsFrom Jun.2009 to Aug.2011, 52 breast cancer patients taking 2-3 cycles of neo-adjuvant chemotherapy underwent modified radical mastectomy in our hospital. Among them, UAS group included 23 patients, and EI group included 29 patients.The 2 groups were compared in terms of the operation time, intraoperative blood loss, postoperative drainage tube duration, postoperative hospital stay, the number of lymph nodes retrieved, and the volume of subcutaneous hydrops.ResultsBetween the 2 groups, the difference of intraoperative blood loss and postoperative drainage tube duration had statistical significance(P ＜0.01 ).The difference of operation time, hospital stay, and subcutaneous hydrops had statistical significance ( P ＜ 0.05 ).The difference of the number of lymph nodes retrieved had no statistical significance ( P ＞ 0.05 ).ConclusionThere are good curative effects for breast cancer patients undergoing modified radical mastectomy by UAS and axillary lymph node dissection after neo-adjuvant chemotherapy, which is consistent with the conception of fast track surgery.

Objective To study the metastasis rule and influence factors of cervical central group (Ⅵ region) lymph node in patients with papillary thyroid microcarcinoma.Methods The clinical data of 195 patients with papillary thyroid microcarcinoma were analyzed retrospectively.All patients received bilateral thyroidectomy and bilateral cervical central group lymph node dissection.Results All patients died without surgery and hospitalization,the incidence of postoperative complications was 2.1％ (4/195).The total incidence of cervical central group lymph node metastasis was 35.4％ (69/195).Factors relating to cervical central group lymph node metastasis rate were:age (OR =0.924),gender (OR =0.213),the number of foci (OR =2.208) and the sum of the tumor diameters (OR =2.106).Conclusions Patients with papillary thyroid microcarcinoma may suffer from lymph node metastasis of the cervical central group,and lymph node metastasis rate increases significantly in young or male patients,and when the lesions are multifocal or the sum of the tumor diameter ＞ 0.5 cm.

Objective To sum up the experiences of diagnosis,treatment and medical evaluation of the anatomical abnormalities in nasal cavity(AANC,including deviation of nasal septum and turbinate hypertrophy) in aircrew.Methods Case data of 91 pilots with CANC were analyzed.The classification and typing of AANC and secondary barotraumas associated with typing of AANC were studied.Results There were 50 cases of class I AANC(with symptoms or signs on ground),41 cases of class II(no symptoms or signs on ground but with symptoms of secondary barotraumas in flight,including IIa 10 cases:with symptoms of secondary barosinusitis,IIb 26 cases:with symptoms of secondary barotrauma of ear,IIc 5 cases:with symptoms of secondary barosinusitis and barotrauma of ear after flying).For the typing of AANC,there were 41 cases of nasal septum deviation,29 cases of inferior turbinate hypertrophy,8 cases of middle turbinate hypertrophy,and 13 cases of nasal septum deviation and turbinate hypertrophy.85 cases were still in a qualified position to continue their flying job,1 pilot was transferred from fighter to transport pilot,2 pilots were temporarily grounded,and 3 were permanently grounded.Conclusions AANC is very common in aircrew.AANC in aircrew is classified and typed according to the effect of gas pressure adjustment functions of ear and nasal sinus.The classification and typing are of important implication in clinical aviation medicine.Medical workers must unify the qualification criteria in selection for air candidates,and also in diagnosis,treatment,medical evaluation of disease in aircrew.This is an important measure to improve the health quality and ensure combat effectiveness for the air force.

Objective To observe the effect of Zhi Qing Capsule on the plaque formation of hyperlipemia rabbits and to discuss its mechanism.Methods Forty-eight New-Zealand rabbits were randomly divided into normal control group,hyperlipemia group,Xuezhikang control group and three Zhi Qing capsule groups.Before and after 2-week treatment,the serum levels of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C) and high density lipoprotein cholesterol(HDL-C) in the serum were determined.After 4-week treatment,TC and TG contents in the liver,and nitric oxide(NO),thromboxan B2(TXB2) and endothelin-1(ET-1) levels in the serum were measured.At the end of the sixth week,all rabbits were killed to observe aorta pathological changes.Results Zhi Qing Capsule can obviously counteract the increase of serum TC,LDL-C and liver TC in the hyperlipemia rabbits,restrain the formation of aorta atherosclerotic plaques,increase the level of NO and decrease the levels of TXB2 and ET-1.Conclusion Zhi Qing Capsule has an evident effect on resisting the lipid metabolism disturbance and atherosclerosis.

Their differences were revealed. The amount of borne0l and isob-orneol in the samples were determined.

Objective To investigate the influence of smoking,drinking cognition and behavior among the officers and soldiers in South China War Zone after health management,and to explore the establishrent of appropriate health management model for the army.Methods Harmfulness cognition,rate,quantity,types of drinking and smoking were investigated and analyzed before and after health management among 1 102 officers and soldiers in South China War Zone by providing a set of health education,psychological counseling service,disease and lifestyle intervention of health management as a whole.Results The harmfulness cognition rate of smoking and drinking raised from 95.6％ and 94.1％ to 98.7％ and 97.8％,the smoking and drinking rate dropped from 64.7％ and 72.6％ to 37.2％ and 61.4％ after health management.The x2 values were 18,18.9,153,20.8,and had significant difference (P＜0.001).The rate of occasional smoking,a small amount of smoking,often smoking dropped from 28.3％,25.8％,45.9％ to 18.5％,12.2％,31.5％ as well as the rate of occasional drinking,a small amount of drinking,often drinking has dropped from 56.6％,32.3％,11.1％ to 49.8％,23.4％,4.1％.The x2 values were 26.8,62.1,31,9.61,8.06,19.4,and had significant difference (all P＜0.01).Conclusion The health management improves the harmfulness cognition of smoking and drinking,and decreases the rate and quantity of smoking and driking.It has a positive effect on improving the heahh literacy and developing health way of life in officers and soldiers.

A method of using Fourer　transform infrared spectroscopy (FTIR) to detect 36 Ganoderma lucidun products rapidly and undamaged is reported. As the result, each of the samples has its characteristic infrared spectrum. By the difference of the relative intensity of those absorption peaks, different kinds of Ganoderma lucidums products can be detected. It is very fast, simple, reliable and has no solution effects.

Objective:To evaluate the therapeutical effects of diplopore puncture of tympanum and intratympanic administration(DPTIA) on patients with acute secretory otitis media(ASOM).Method:One hundred and forty -one cases with ASOM were divided into two groups randomly,with 69 cases(80 ears) in group A(experimental group) and 72 cases(84 ears) in group B(control group).Group A was treated with DPTIA combined with medication, group B was treated with haplopore puncture of tympanum combined with medication. All patients accepted 1 to 3 courses of treament,and were observed during the period of treatment and 3 months after treatment.Result:The factors (including gender, age, side of ailing ear, course of disease, air-bone gap in the pure tone average,and acoustic immitance type in ailing ear) which may impact the prognosis were matched in two groups prior treatment(P＞0.05). While there was a significant difference in the cure rate and the total effective rate respectively between two groups after treatment(P＜0.05).No adverse reaction or complication was seen.Conclusion:DPTIA is a safe, reliable and effective way to manage patients with ASOM.
