1.Investigation on current situation and countermeasures of both Xinjiang minority and Han medical students' autonomous learning of ideological and political theory course

Objective To understand the status of Xinjiang medical students' autonomous learning of Ideological and Political Theory Course,and put forward corresponding educational countermeasures.Method By using stratified cluster sampling method 850 Xinjiang minority and Han medical students in a university were extracted as the subjects and were surveyed by college learning autonomy self-statements Questionnaire including two dimensions of attitudes and habits of learning ideological and Political Theory Course.Using SPSS 13.0 thex2-test was used for statistical analysis,the level of test=0.05.Results On the ideological and political theory course teaching attitude,positive attitude of the minority medical students and the Han medical students were 319/67.6% and 259/73.8% respectively(P=0.009 3),and the overall satisfaction rate reached 579/70.3%.In the habit of learning the ideological and political theory course,38/8.1% minority medical students selected the library to learn,while 132/37.7% of Han medical students chose Library(P=0.007 9、0.037 0).In the choice of place of study and the way to find information there was statistically significant difference between minority medical students and Han medical students.Conclusion According to the differences of minority students' and Han students' learning ideological and political theory course,we can carry out differentiated autonomous learning training strategies through the establishment of new education ideas,innovative education carrier and method to improve both the minority medical students'and Han medical students' autonomous learning ability of Ideological and Political Theory Course

OBJECTIVE To establish the basic quality control system for disinfection of medical apparatus and(instruments).in order to meet the national standards of the disinfection quality.METHODS On the basis of background investigation about the disinfection(management) of medical apparatus and instruments from 1997 to 2004 to implemeat the basic quality control system scientifically and continuously and to improve the management of disinfection quality in hospital.RESULTS The disinfection quality of commonly used medical apparatus and(instruments),first-aid devices,combat readiness materials,special instruments and sterilizer,etc was got to(improve) year by year,that created condition to control the emergence of hospital(infection).CONCLUSIONS To(avoid) the exogenous hospital infection due to unsuitable medical apparatus and(instruments).
3.Lentiviral vector mediated CGRP gene in vitro transfection and its effects on biological properties of MSC

Objective To explore in vitro mesenchymal stem cell (MSC) transfection of lentiviral vector mediated calcitonin gene‐related peptide(CGRP) gene and its effects on biological properties of MSC .Methods MSC were isolated ,cultured and identi‐fied .MSC were infected by lentivirus encoding recombinant enhanced green fluorescent protein (EGFP) gene and CGRP (Lv‐EG‐FP‐CGRP) .The transfection efficiency was determined by the inverted fluorescence microscope and flow cytometry .The expression levels of CGRP were detected in CGRP‐modified MSC by using real‐time PCR ,immunocytochemistry and enzyme‐linked immu‐nosorbent assay (ELISA) .The proliferation ,aging and differentiation ability of MSC were evaluated by MTT ,β‐galactosidase stai‐ning and inducing differentiation respectively .Results After 48 h of MSC transfection by Lv‐EGFP‐CGRP ,EGFP/CGRP could be expressed stably .When multiplicity of infection (MOI) was 30 ,the transfection efficiency reached more than 80% .Compared with the MSC group and the MSC‐EGFP group ,the mRNA and protein expression levels of CGRP in CGRP‐modified MSC(MSC‐CGRP group) were markedly increased(all P<0 .01) .The results of MTT ,β‐galactosidase staining and inducing differentiation assay dem‐onstrated that the transfected CGRP basically had no effect on the proliferation ,aging and endotheliocyte differerntiation of MSC . Conclusion MSC is a kind of ideal genetic vector cell ,which can serve as the target cell of CGRP gene transduction for the applica‐tion of gene therapy and lays the foundation for follow‐up in vitro and vivo experiments .
4.Status quo of care burden of caregivers of young patients with brain tumor and its correlation analysis with social support level

Objective:To investigate the care burden of primary caregivers of young patients with brain tumor and to analyze the correlation between the care burden and social support level, in order to improve the mental health of caregivers, reduce the care burden and improve the quality of care for patients.Methods:Using the convenient sampling method, a total of 173 primary caregivers of young inpatients with brain tumor who were admitted to Department of Neurosurgery of Henan Provincial People's Hospital from December 2018 to November 2019 were selected as the research objects. General data questionnaire, Caregivers Burden Inventory (CBI) and Social Support Rating Scale (SSRS) were used to collect general information of caregivers, the care burden and the level of social support, Pearson correlation analysis of the correlation between care burden of the primary caregivers and social support level of young inpatients with brain tumor and use of the Logistic regression analysis to explore the influencing factors of care burden of the primary caregivers.Results:A total of 173 primary caregivers of young inpatients with brain tumor were investigated, and their CBI total score was (34.42±7.38) , which was at the medium burden level. Among them, a total of 13 (7.51%) had severe burden, 79 (45.66%) had moderate burden and 81 (46.82%) had mild burden. The total score of SSRS of caregivers was (43.61±8.46) , which was in the middle level of social support. Among them, a total of 71 (41.04%) had high level of social support, 92 (53.18%) had medium level of social support and 10 (5.78%) had low level of social support. Pearson correlation analysis showed that CBI score of caregivers was negatively correlated with SSRS score ( P< 0.05) . Logistic regression analysis showed that age, family monthly income per capita, daily care time, with or without a co-care, depression and social support level were the influencing factors of the caring burden of the caregivers of young inpatients with brain tumor ( P<0.05) . Conclusions:The primary caregivers of young patients with brain tumor generally have different levels of care burden, and their burden degree is closely related to the level of social support. Age, family monthly income per capita, daily care time, with or without a co-carer, depression and social support level are the main factors affecting the burden of caregivers. Clinical nurses should take corresponding measures for the factors affecting the care burden and relevant institutions should actively build a policy support system for the primary caregivers, so as to reduce the burden of caregivers and improve the quality of care.
5.Clinical, skeletal muscle pathological and genetic characteristics of fatal infantile hypertonic myofibrillar myopathy

Objective:To investigate the clinical, skeletal muscle pathological, and genetic characteristics of fatal infantile hypertonic myofibrillar myopathy (FIHMM).Methods:The clinical manifestations, laboratory assessments data and gene sequencing results of 10 patients diagnosed with FIHMM in Shenzhen Children′s Hospital from February 2017 to April 2021 were retrospectively analyzed.Magnetic resonance imaging (MRI) of both musculoskeletal system and the brain, and electromyogram (EMG) were performed in 3 cases, while muscle biopsy was performed in 2 cases.Results:Among these 10 cases, 1 case was from Northeast China and 1 case from East China, while the rest 8 cases were from South China.Eight of the 10 patients were male, and the other 2 cases were female.They were all born normal and not related to each other.The age of onset varied from 2 to 12 months.The main clinical manifestations for all the patients were progressive rigidity of the rectus abdominis (8 cases), neck muscles (7 cases), rectus abdominis (2 cases) and intercostal muscles (1 case), resulting in respiratory failure.Mildly to moderately elevated serum creatine kinase level was detected (436-5 804 IU/L) (reference range: 24-229 IU/L). Complex repetitive discharges can be seen in the EMG, without any myotonic potential.Muscle fiber degeneration, necrosis, and vacuolar degeneration were noted in the histopathological examination of the vastus lateralis and rectus abdominis.An abnormal red granular deposit was observed in a portion of the field of the modified Gomory Trichrome staining.Immunohistochemistry showed substantial deposition of desmin.Under the electron microscopy, the sarcomere structure of the muscle fibers was seriously disordered, with the destruction of Z-bands and the presence of granular deposits.The whole-exome sequencing identified the same homozygous variation c. 3G>A, p.Met1? of CRYAB gene in all the patients, but heterozygous variation in their parents. Conclusions:Axial muscles involvement, such as rectus abdominis rigidity, is the main clinical characteristic of FIHMM.c.3G>A, p.Met1? mutation in the CRYAB gene is a hotspot mutation in Chinese children.
6.Clinical value of nucleic acid detection for hepatitis B virus screening in hospitalized patients

Objective:To explore clinical value of nucleic acid detection for hepatitis B virus (HBV) screening in hospitalized patients.Methods:This cross-sectional study collected and analyzed plasma samples from patients admitted to 10 domestic medical institutions from July 2021 to December 2021. Serological immunoassay and nucleic acid screening were used to simultaneously detect hepatitis B markers such as hepatitis B surface antigen (HBsAg), hepatitis B surface antibody (HBsAb), hepatitis B e Antigen (HBeAg), hepatitis B e antibody (HBeAb), hepatitis B core antibody (HBcAb),and HBV DNA. Statistical analysis was performed on the serology, nucleic acid test results and clinical information of the patients.Results:Of the 8 655 collected samples, HBsAg was positive in 216 (2.50%) samples,HBV DNA was positive in 238 (2.75%) samples ( P>0.05); 210 (2.43%) samples were positive for both HBsAg and HBV DNA, 28 (0.32%) were HBsAg negative and HBV DNA positive, 6 cases (0.07%) were HBsAg positive and HBV DNA negative. Conclusion:These results indicate that the HBV DNA testing is equally effective as hepatitis B virus serological detection for hepatitis B virus screening in hospitalized patients.
7.Cost-effectiveness analysis of nucleic acid screening for hepatitis B and C in hospitalized patients in China

Objective:To compare the cost-effectiveness of hospitalized Chinese patients undergoing nucleic acid screening strategies for hepatitis B and hepatitis C, immunological screening strategy, and no screening strategy under different willingness to pay (WTP). The results might aid to decision-making for the optimal strategy.Methods:In this study, nucleic acid screening, immunological screening and no screening were used as screening strategies, and China′s GDP in 2021 (80 976 yuan) was used as the threshold of WTP to construct a Markov model. After introducing parameters related to the diagnosis and treatment of hepatitis B and C in inpatients, a cohort population of 100 000 inpatients was simulated by TreeAge Pro 2021 software, the total cost, total health effects, incremental cost-effectiveness ratio and average cost-effectiveness ratio of different screening strategies were calculated, and cost-effectiveness analysis was conducted. Univariate and probabilistic sensitivity analysis were used to assess the impact of parameter uncertainty on the final results.Results:Compared with the non-screening strategy, the incremental total cost of the hepatitis B immunological screening strategy for cohort patients was 11 049 536 yuan, and the incremental cost-effectiveness ratio was 24 762 yuan/quality-adjusted life years (QALY), while the total incremental cost of nucleic acid screening was 19 208 059 yuan, and the incremental cost-effectiveness ratio was 29 873 yuan/QALY; the incremental cost-effectiveness ratio of nucleic acid screening and immunological screening was 45 834 yuan/QALY. Compared with the non-screening strategy, the incremental cost-effectiveness ratio of hepatitis C immunological screening strategy was 5 731 yuan/QALY, the incremental cost-effectiveness ratio of nucleic acid screening strategy was 8 722 yuan/QALY, the incremental cost-effectiveness ratio of nucleic acid screening and immunological screening was 45 591 yuan/QALY. The results of probabilistic sensitivity analysis showed that when the cost of nucleic acid testing exceeded 214.53 yuan, it was not cost-effective to perform hepatitis B nucleic acid screening under the WTP as 1 fold GDP. When the cost of nucleic acid testing exceeded 132.18 yuan, it was not cost-effective to conduct hepatitis C screening under the WTP as 1 fold GDP.Conclusions:Nucleic acid screening strategy can achieve more cost-effectiveness and is worthy of vigorous promotion. Compared with no screening, both the nucleic acid and immunological screening strategies are cost-effective, and hepatitis nucleic acid screening is the optimal strategy for hospitalized patients.
8.Toll-like receptor 3 (TLR3) regulation mechanisms and roles in antiviral innate immune responses.

Toll-like receptor 3 (TLR3) is a member of the TLR family, mediating the transcriptional induction of type I interferons (IFNs), proinflammatory cytokines, and chemokines, thereby collectively establishing an antiviral host response. Studies have shown that unlike other TLR family members, TLR3 is the only RNA sensor that is utterly dependent on the Toll-interleukin-1 receptor (TIR)-domain-containing adaptor-inducing IFN-β (TRIF). However, the details of how the TLR3-TRIF signaling pathway works in an antiviral response and how it is regulated are unclear. In this review, we focus on recent advances in understanding the antiviral mechanism of the TRIF pathway and describe the essential characteristics of TLR3 and its antiviral effects. Advancing our understanding of TLR3 may contribute to disease diagnosis and could foster the development of novel treatments for viral diseases.
9.Advances in the release mechanisms of bluetongue virus.

Bluetongue virus (BTV) causes Bluetongue (BT) of ruminants vectored by culicoides midges. It is also a classic model for studying the release mechanism of non-enveloped virus. This review begins with the infection and assembly of BTV, then summarizes the advances of different ways of releasing BTV. This includes BTV-induced autophagy and the release as extracellular vesicles via multivesicular bodies, BTV-induced apoptosis and the lytic release, as well as different pathways of release through budding via plasma membrane. The regulatory mechanisms of NS3 which is a key non-structural protein during the release of BTV are also discussed, providing a basis for further understanding the molecular mechanisms underpinning the infection, proliferation and release of BTV.

Bluetongue virus is an arbovirus that seriously harms ruminants such as sheep,this study aims to investigate the molecular mechanism of bluetongue virus infection and host cell interferon antiviral immune response.The study was conducted to characterize the mRNA expression of inter-feron pathway genes by real-time fluorescence quantitative PCR,as well as Western blot analysis of MDA5,TRAF3,RIG-Ⅰ,and TBK1 protein expression in BHK-21 cells induced by BTV with a multiplicity of infections(MOI)of 1 for 18,24,and 36 h.The results showed that the most pro-nounced changes in the expression of interferon signaling pathway genes were observed at 24 h of induction,the gene mRNA expression levels of the IFN-α,IFN-β,RIG-Ⅰ,TBK1,MDA5,VISA,and TRAF3 genes were upregulated.However,the mRNA expression levels of IKKε and TRAF6 genes were downregulated.At the protein level,MDA5 and TBK1 proteins were upregulated while RIG-1 and TRAF3 proteins were downregulated,which showed that BTV infection induces a typeⅠ interferon immune response in BHK-21 cells.This study lays the foundation for further exploring the antiviral immunity mechanism of IFN-Ⅰ signaling pathway regulatory genes in host cells infected with BTV infection.
