1.Study of the Specificity of Lumbosacral Points in Regulating Unstable Bladder

Objective:To investigate the effects of acupuncture of Shenshu(BL 23),Huiyang (BL 35)and Shenshu(BL 23)plus Huiyang(BL 35)on the main symptoms of unstable bladder (USB)and explore the specificity of different points in treating USB through a clinical randomized controlled trial.Methods:Eighty-nine USB patients were allocated to three groups:Shenshu(BL 23),Huiyang(BL 35)and Shenshu(BL 23)plus Huiyang(BL 35).I-PSS was used as an observation index.The effects were evaluated and a comparison was made between the groups after three and six treatments.Resuits:I-PSS significantly improved in all Shenshu (BL 23),Huiyang(BL 35)and Shenshu(BL 23)plus Huiyang(BL 35)groups of USB patients (P<0.05).The effect was significantly beaer in the Huiyang(BL 35)and Shenshu(BL 23)plus Huiyang(BL 35)groups than in the Shenshu(BL 23)group(P<0.05).The efficacy rate was 50.0% in the Shenshu(BL 23)group,82.8% in the Huiyang(BL 35)group and 90.0% in the Shenshu(BL 23)plus Huiyang(BL 35)group.It was significantly higher in the Huiyang(BL 35)and Shenshu(BL 23)plus Huiyang(BL 35)groups than in the Shenshu(BL 23)group(P<0.05).Conclusion:Points Shenshu(BL 23),Huiyang(BL 35)and Shenshu(BL 23)plus Huiyang(BL 35)can all regulate voiding dysfunction and improve quality of life in USB patients.Shenshu (BL 23)plus Huiyang(BL 35)has the most marked therapeutic effect.
2.Dynamic Observation of Needling Huiyang(BL 35)on Ambulatory Urodynamics in Unstable Bladder

Objective:Needling lumbosacral acupoints could regulate unstable bladder (USB).The present study is to observe the ambulatory urodynamics changing of needling Huiyang (BL 35) on USB to provide scientific basis for the relative theory of acupoint and Zang-fu organs,and the rule of acupuncture treatment.Methods:Unstable bladder model were prepared in 80 Wistar rats,and then they were randomly divided into treatment group (n=40) and model group (n=40).The urodynamics were monitored respectively at 2 h,6 h,12 h and 24 h after acupuncture and the results were analyzed.Results:After acupuncture Huiyang (BL 35),the compliance of bladder were superior to that of model group at 2 h,6 h and 12 h,respectively (P<0.05),and the bladder volume were superior to that of model group at 2 h and 12 h,respectively (P<0.05).After acupuncture Huiyang (BL 35),the compliance and Volume of bladder at 12 h were superior to those at other time (P<0.05).Twenty-four hours later,the effect of acupuncture on urodynamics decreased.Conclusion:Needling Huiyang (BL 35) could improve the urodynamics of USB,including improving the compliance of bladder and increasing the volume of bladder.Acupuncture Huiyang (BL 35) had the most effect at 12 h.
3.Effects of electroacupuncture treatment on nitrergic neurotransmitter in bladder neck and detrusor of rats with unstable bladder

OBJECTIVE: To observe the effects of electroacupuncture treatment on nitrergic neurotransmitter in bladder neck and detrusor of rats with unstable bladder. METHODS: Rat models of unstable bladder were established by operation to induce urethral obstruction. Electroacupuncture treatment was given by acupuncturing Huiyang and Zhonglushu points for a week. Then the neuronal nitric oxide synthase (nNOS)-, endothelial nitric oxide synthase (eNOS)- and inducible nitric oxide synthase (iNOS)-positive cells in bladder neck and detrusor of the rats were observed. RESULTS: The nitrergic neurotransmitter in bladder neck and detrusor were obviously decreased in rats with unstable bladder. The electroacupuncture treatment could significantly increase the contains of NOS in bladder tissue. CONCLUSION: To promote the synthesis and secretion of nitrergic neurotransmitter in bladder tissue may be one of the mechanisms of acupuncture in adjusting bladder function.
4.A clinical analysis of myelodysplastic syndrome and aplastic anemia combined with autoimmune disease

Objective To investigate the characteristics of pathogenesis and therapeutics of myelodysplastic syndrome(MDS)and aplastic anemia(AA)combined with autoimmune disease(AID).Methods Retrospective analysis and follow-up visit of 111 patients with MDS and 56 patients with AA in our hospital were studied.Results There were 9(8.1%)of 111 patients with MDS and 2 (3.6%)of 55 patients with AA coexistent with AID.Autoimmune hemolytic anemia(36.4%)and Behcet Disease(18.2%)were common among those combined with AID.5 patients had AID proceeding to the occurrence of MDS/AA.4 patients had simultaneous occurrence of AID and MDS/AA.2 patients developed AID three years later after the diagnosis of MDS.Conclusion There was a certain intrinsic relationship between MDS/AA and AID.
6.Clinical analysis of related risk factors for delayed hemorrhage after pancreaticoduodenectomy

Objective:To invest the risk factors associated with delayed hemorrhage after pancreaticoduodenectomy occurred.Methods:A retrospective analysis was performed from January 2012 to June 2015 Shengjing hospital 511 cases of patients who accepted pancreaticoduodenectomy.Results:The incidence of PD postoperative delayed hemorrhage was 8.81% (45/511).Univariate analysis shows:Preoperative bilirubin levels,pancreatic fistula,biliary fistula,postoperative abdominal infection are all the meaningful related factors(P<0.05).With the application of multivariate logistic regression analysis,pancreatic fistula (OR=2.158) and abdominal infection (OR=3.051) were independent risk factors for postoperative delayed hemorrhages (P<0.05).Conclusion:Dealing with the postoperative complications of PD like pancreatic fistula,abdominal infection and others,early detection and rapid diagnosis and correct treatment can effectively reduce the incidence of delayed hemorrhage.
7.Value of circulating galactomannan screening for early diagnosis of invasive aspergillosis in hematological patients

Objective To detect the value of serum galactomannan (GM) for diagnosis of invasive aspergillosis (IA) in hematological patients. Methods We prospectively evaluated the diagnostic value of twice-weekly screening for circulating Aspergillus fumigatus with sanditch enzyme linked immunosorbent assay. Results On the basis of the analysis of 472 serum samples from 113 episodes of 92 patients, the sensitivity, specificity, positive and negative predictive values of the sandwich ELISA for proven and probable IA cases were 83.3 %, 91.1 %, 78.9 % and 93.1 %, respectively, when samples with 2 consecutive positive results≥0.7 were used. Furthermore, GM antigenemia was detected before the onset of radiologic signs with a median of 7 days (range, 1-14 days), before isolation of Aspergillus with a median of 4 days (range, 1-7 days),and before anti-fungal therapy with a median of 6 days (range, 1-15 days). Conclusion The sandwich ELISA for GM detection is a reliable method for early diagnosis of IA in patients with haematological diseases.
8.Influencing factors for delayed gastric emptying after pancreaticoduodenectomy and its prevention and treatment

Delayed gastric emptying(DGE)is a common complication after pancreatoduodenectomy. Due to its complex causes and difficul-ties in treatment,it has been a difficult problem in general surgery. By analyzing the improvement in the surgical procedure of pancreaticodu-odenectomy in recent years and the influence of the establishment of predictive scoring model on DGE,this article puts forward new direc-tions for the prevention and treatment of DGE. It is pointed out that effective preoperative assessment and reasonable selection of surgical pro-cedures may help to reduce the incidence rate of DGE after surgery.
9.Current status and influencing factors of alexithymia among elderly stroke patients

Objective:To explore the current status and influencing factors of alexithymia among elderly stroke patients.Methods:From August 2017 to April 2018, we investigated 200 elderly stroke inpatients in Neurology Department at a ClassⅢ Grade A hospital in Henan with the self-designed General Information Questionnaire, Toronto Alexithymia Scale 20 (TAS-20) , Social Support Rate Score (SSRS) and Hamilton Depression Scale (HAMD) .Results:Among those stroke patients, the score of alexithymia was (67.42±9.13) and the incidence rate was 51%, which was mainly manifested in extroverted thinking and recognition emotional disorders. Multiple stepwise regression analysis showed that the main influencing factors of alexithymia in elderly stroke patients were the age, hemiplegia, aphasia, social support and depression, which could explain 50.6% of the variation in alexithymia.Conclusions:Alexithymia of elderly stroke patients needs to be improved. While providing nursing care for elderly stroke patients, clinical nursing workers and family members of patients should pay attention to patients' emotional expression, thinking style and depression, especially for patients with advanced age, hemiplegia, aphasia, loss of self-care ability and so on. On the other hand, social support can also be used to improve patients' alexithymia so as to reduce complications, improve clinical outcomes and their quality of life.
