1.Early apolipoprotein M expression in ischemia -reperfusion injury of rats

ObjectiveTo investigate the expression of hepatic apolipoprotein M (apoM) mRNA and serum apoM during early stage of hepatic ischemia reperfusion injury in rats. MethodsRat models of hepatic (ischemia)-reperfusion injury (IRI) were established. Forty healthy male SD rats were randomly divived into five groups(each group containing eight rats):Sham-operation group(control); IRI groups, animals (experienced) 1 hr ischemia and then followed by various reperfusion intervals(0.5,1.0,2.0 and 3.0 hrs, respectively). ResultsSerum alanine-aminotransferase (ALT) levels and the liver injury degree (histologically) were increased with the prolongation of hepatic reperfusion time. The apoMmRNA level in liver was markedly decreased in the group that had reperfusion for 0.5hr compared to the sham group. However, it increased gradually in groups that had reperfusion for 1hr to 3hrs. Serum apoM protein level showed a (similar) tendency with apoMnRNA, but it increased after reperfusion for 2 hrs. ConclusionsIt is concluded that apoM expression pattern shows rapid and significant changes during hepatic ischemia-reperfusion injury, which suggests that apoM might have the characteristic of acute phase reactive protein.
2.Cardiac resynchronization therapy with optimized dynamic AV delay for patients with congestive heart failure

Objective To evaluate the effects of cardiac resynchronization therapy with optimized dynamic AV delay.Methods The study included 63 patients who underwent implantation of biventricular pacemakers for congestive heart failure.The dynamic AV delay was optimized on the day 1 week,6 months and 12 months after pacemaker implantation.The echocardiography and ECG,6MHW,QOL and NYHA grade were evaluated at the same time.Results The best optimized AV delay was between 90 ms and 140 ms(110.45?19.47 ms).Comparing the effectiveness 1 week after CRT with the day before CRT:the LVEF increased from 18.53%?4.83% to 22.07%?6.77% with the QRS interval decreased from 138.25?28.00 ms to 129.40?6.65 ms.Comparing the effectiveness of the day before and in 6-12 months after CRT optimized AV delay,the LVFT increased from 293.27?24.79 ms to 347.57?26.21 ms.There was no difference in QRS intervals.The LVEDD was upgraded from 71.43?7.12 mm to 57.06?6.54 mm while the QOL was upgraded from 74.14?6.24 to 55.15?11.02 and the NHYA upgraded from 3.12?0.55 to 1.82?0.43.Conclusion Optimized dynamic AV delay of the pacemaker parameters enhanced the short-term and long-term efficacy of CRT.
3.Repair of rabbit radius defects by inductive artificial periosteum and allogenic bone grafts

Objective To investigate the feasibility of repairing the radius defect with inductive bone regeneration membrane combined with freezed allogenic bone. Methods Preparation PLGA/bBMP+IGF-Ⅱ+ bFGF complosite was prepared as artificial periosteum. Twenty seven rabbits with 1.0 cm long right radius segmental osteoperiosteum defect were werved as the animal model, and randomly assigned to 3 groups with 9 in eath group; Group A: the defects were connected with the PLGA/bBMP+IGF-Ⅱ+bFGF membrane and allogenic bone; Group B: the defects were treated with the PLGA/bBMP+IGF-Ⅱ+bFGF membrane; Group C: the defects were implanted only woth allogenic bone grafts. Animals were killed at 2, 4, 8 weeks postoperation respectively. The healing was study with radiological, histological and immunological methods. Results The osteogenic ability in PLGA/bBMP+IGF-Ⅱ+bFGF group was superior to that in the other groups. The defects of group A ware fully repaired at week 8; bony bridging under remodeling was observed in group B; and the putrescence of allogenic bone which was packaged by mass of connective tissue was observed with group C; Group A caused lower titer of antibody than group C and inhibited lymphocytes reproduction in vitro; There was significant difference between group A and group C. Conclusion PLGA/bBMP+IGF-Ⅱ+bFGF artificial periosteum had provided a excellent osteoinduction condition. Artificial periosteum combined with freezed allogenic bone can effectively promote the rapair of radius defects of rabbit.
4.Change of hepcidin expression in patients with hyperlipemia after allograft renal transplantation

Objective To discuss the change and significance of hepcidin expression in patients with hyperlipemia after allograft renal transplantation.Methods Allograft renal transplantation recipients were divided into hyperlipemia group (n =35),and non-hyperlipemia group (n =25).Normal control group was divided into non-hyperlipemia subgroup (n =30),and hyperlipemia group (n =25).ELISA was used to detect serum hepcidin and IL6.Relevant indexes of hematology,biochemistry,and iron metabolism were examined.Results As compared with other three groups,hyperlipemia group after allogenic kidney transplantation had increased serum hepcidin,IL-6,and serum ferritin expression (P<0.05).Single factor analysis of each group revealed that serum hepcidin expression was correlated with serum ferritin and IL-6 (P < 0.05).Correlation analysis on all specimens demonstrated that serum hepcidin was correlated with glomerular filtration rate,total cholesterol,triglycerides,serum ferritin and IL6 expression (P<0.05).Conclusion Renal allograft transplantion recipients with hyperlipemia have higher serum hepcidin expression; There exists correlation between the expression of serum ferritin and expression of IL-6,which could reflect the hyperlipemia-induced atherosclerosis and its progress.
5.Role of Hepcidin in prostate cancer patients with bone metastasis

Objective To detect the expression and correlation of serum Hepcidin,IL-6,sTfR,BMP6 diversity and explore the role of serum Hepcidin in prostate cancer with bone metastasis.Methods From January 2012 to March 2012,serum Hepcidin,IL-6,sTfR,BMP6 diversity were tested by ELISA in 25 prostate cancer patients with bone metastasis,30 prostate cancer patients without bone metastasis and 30 patients with BPH were used as the control group.The mean patient's age was 67 years (range from 55 to 75 years).In prostate cancers with bone metastasis group,the mean PSA base line was 138.0 μg/L (ranged from 20.0-1500 μg/L),the prostate cancers without bone metastasis group,mean PSA was 10.2 μg/L(ranged from 3.5-28.2 μg/L),and in BPH group,the mean PSA was 3.7 μg/L (ranged from 0.3-14.2 μg/L).Venous blood samples were taken in fasting mornings,then stored 3 ml in EDTA anticoagulant vacuum tube and centrifuged at4 ℃ for 10 min,the isolated serum were then preserved in-80 ℃ refrigerator.The competitive in-phase enzyme-linked immunoassay (ELISA) was used to detect serum Hepcidin,IL-6 and sTfR and BMP6 levels.Results Serum Hepcidin expressions in three groups were 67.7 ± 40.6 μg/L,37.5 ± 15.3 μg/L and 34.3 ± 10.7 μg/L,respectively.For prostate cancers with bone metastasis group,serum Hepcidin expression were higher than control group (P < 0.05),and associated with IL-6(22.5 ±22.1 μg/L),sTfR (5.7 ± 2.6 μg/L),BMP6 (429.3 ± 188.4 μg/L),correlation coefficients were 0.972,-0.987,0.971 (P < 0.05).Conclusions Increased serum Hepcidin level might be a sensitive index for diagnosis and prognosis of prostate cancers with bone metastasis.
6.Surgery treatment and clinical research in renal transplant recipients with benign prostatic hyperplasia

Objective To study the clinical manifestations and surgical treatment in renal transplant recipients with benign prostatic hyperplasia (BPH). Methods From January 2013 to May 2015, 5 patients of kidney transplant recipients who suffered from BPH for transurethral resection of prostate (TURP) treatments were enrolled (kidney transplant recipients group) and 15 patients with the same surgical treatments during the same peoriod were enrolled too (common control group). The routine examination, urodynamic, sex hormone levels and and the clinical efficacy were observed. Results The levels of hemoglobin, total protein, urea nitrogen, creatinine, prostate volume , residual urine and maximum flow rate (Qmax) before operation between two groups had no significant differences (P>0.05). The level of testosterone in kidney transplant recipients group was significantly lower than that in common control group: (3.5 ± 1.2) mmol/L vs. (5.5 ± 2.2) mmol/L, P < 0.05. The level of estradiol in kidney transplant recipients group was significantly higher than that in common control group:(49.6 ± 13.8) mmol/L vs. (40.5 ± 16.4) mmol/L, P < 0.05. In kidney transplant recipients group, the levels of hemoglobin, total protein, urea nitrogen, creatinine before and after operation had no significant differences (P>0.05). After operation, the levels of hemoglobin, total protein, urea nitrogen, creatinine in two groups had no significant differences (P>0.05). In kidney transplant recipients group, the quality of life (QOL) score before operation was 5 scores in 4 cases and 6 scores in 1 case, and those after operation were zero in 3 cases and 1 score in 2 cases. Conclusions Under the sufficient preoperative preparation and assessment, the renal transplant recipients with BPH patients can improve the symptoms of urinary obstruction and obtain satisfactory clinical efficacy as common BPH patients.
7.Dendrobium nobile polysaccharides reduce cerebral ischemia reperfusion injury in rats

AIM To investigate the protective effects of polysaccharides from Dendrobium nobile Lindl.on cerebra ischemia/reperfusion injury in rats and its possible mechanism of action.METHODS One hundred and five SD rats were divided into sham group,model group,Nimodipine group (10 mg/kg) and treatment groups with low-,medium-,and high dose (50,100,and 200 mg/kg) of polysaccharides from D.nobile.The right middle cerebral artery of rats was occluded by inserting a.thread through internal carotid artery for 2 h,and was sampled after reperfusion for 24 h.The rats received gavage once a day for 7 d before operation.Neurological deficits score,brain index,brain water content and infarct size in rats were conducted at the end of reperfusion;the activity of superoxide dismutase (SOD),glutathione peroxidase (GSH-Px),myeloperoxidase (MPO) and the content of malondialdehyde (MDA) of brain tissue and blood serum were measured by chemical colorimetry;the infiltration of neutrophile granulocyte in rat ischemic cortex were detected with immune-fluorescence staining.RESULTS Sham group had no neurological deficit,but the model group showed severe neurological deficits,meanwhile,the infarct size,brain index and brain water content rose markedly,the content of MDA and the activity of MPO of the brain tissue and the blood serum increased remarkably,while the activity of SOD and GSH-Px apparently decreased;compared with the model group,neurological deficits of rats were improved significantly in polysaccharides from D.nobile dose groups,moreover,the infarct size,brain index and brain water content markedly declined,the MDA level and the activity of MPO significantly decreased,but the activity of SOD,GSH-Px increased remarkably;the infiltration of neutrophile granulocyte in the high dose group significantly decreased compared with the model group.CONCLUSION Polysaccharides from D.nobile has some neuroprotective effects on local cerebral ischemia/reperfusion injury,and that may be related to the remove of oxygen free radicals and the decrease in inflammation reaction.
8.Paraganglioma of urinary bladder (two cases reports and review of literatures)

Objective To study the clinical,histopathological,immunohistochemical features and the diagnosis and treatment of paraganglioma of urinary bladder. Methods Two cases of paraganglioma of urinary bladder were treated. The first case was a male with painless haematuria. The abdominal ultrasonography and CT scan showed a 2.5 cm× 2.0 cm mass in the right anterior wall of the bladder, and urine vanillylmandelic acid elevated to 17. 9- 31. 3 μmol/24 h (normal range 10- 35 μmol/24 h). The second case was a female who presented with 8 years history of headache and palpitation after voiding. Abdominal ultrasonography and CT scan showed a 2. 6 cm× 1.5 cm mass in the left wall of the bladder, and her urine vanillylmandelic acid was 35.3-43.3 μmol/24 h. Results One patient underwent transurethral resection (TURBT) and the other underwent partial cystectomy.The two cases were diagnosed as bladder paraganglioma by pathological examination. Immunohistochemically, the tumor cells were positive for CgA, Syn, NSE and S-100. No evidence of recurrence was detected during follow-up at 3 months and 3 years. Conclusions Paraganglioma of urinary bladder should be considered as a low grade malignancy. Partial cystectomy should be recommended. The diagnosis depends on clinical symptoms, pathological and immunohistochemical results.
9.Primary adrenal non-Hodgkin's lymphoma: report of 7 cases

Objective To discuss the diagnosis and treatment of primary adrenal lymphoma. Methods The clinical data of 7 adrenal primary lymphoma cases were retrospectively analyzed. Five cases were male,2 were female. Age ranged from 33 to 62 years,mean 48 years. Two cases presented with unilateral and 5 cases with bilateral masses. Two cases were found by regular health examination. Two cases had fever and weakness, with body weight loss for 3-4 months. One case had enlarged testis for 1 month. Two cases had lumbar pain accompanied by enlarged spleen. Abdominal ultra-sonography and CT showed adrenal neoplasms. All 7 cases had elevated serum lactate dehydrogenase (367-568 U/L, normal range 100-245 U/L) and β2 microglobulin (5.9-6.3 mg/L, normal range 2.4 mg/L). The CT showed irregular,inhomogeneous adrenal mass which was mildly enhanced. Results Four of the 7 patients were misdiagnosed before operation. Two patients were diagnosed as adrenal lymphoma by biopsy. One patient was diagnosed by testicular biopsy. One was T cell non-Hodgkin's lymphoma. Six cases were diagnosed as diffuse large B cell non-Hodgkin's lymphoma by pathology. Immunohistochemieally,the tumor cells were positive for CD3,CD45-RO, L26 and CD79a. Four patients had their adrenal mass removed and received chemotherapy afterwards. As follow-up of 2 years, 1 patient had no evidence of recurrence. Three patients died after 2,6,20 months after opera-tion. Three cases took chemotherapy and radiation therapy after diagnosed. They died 19,32, 38 months during follow up. Conclusions Because adrenal mass as the primary representation of prima-ry adrenal lymphoma has no characteristic clinical appearance, diagnosis could not be made preopera-tively. The principal treatment consists of adrenalectomy and adjuvant combination chemotherapy.
10.Combinations of intra-artery chemotherapy and bladder-preserving resection for primary T1G3 bladder cancer

Objective To evaluate the feasibility and effectiveness of bladder-preserving local resection combined with intra-artery chemotherapy for patients with T1G3 bladder cancer.Methods Thirty five cases with T1G3 bladder cancer were analyzed retrospectively. Patients were all treated by bladder-preserving local resection combined with intra-artery chemotherapy. Results Thirty five cases were followed up. The time of follow-up ranged from 7 to 116 months,and mean time was(66.0±18.3)months.The 5 year recurrence and bladder-preserving rate were 48.6 %(17/35)and 68.6 %(24/35),respectively.The overall and tumor specific survival at 5 years was 77.1% (27/35)and 82.9 % (29/35).The effect of bladder-preserving local resection plus intra-artery chemotherapy was satisfactory. Moreover, intra-artery chemotherapy had no obvious side effects.Conclusion Bladder-preserving local resection combined with intra-artery chemotherapy not only can retain normal bladder function, decrease in the recurrence, but also does not reduce the survival rate. It is an effective treatment for some patients with T1G3 bladder cancer instead of cystectomy over-treatment.
