AIM: To find new methods for heavy metals pollution in traditional Chinese medicine(TCM) with macroporous chelating resins. METHODS: We emulsificated crude extracts of TCM and adjusted its pH to 7 first and the pretreated crude extracts was treated by three kinds of macroporous resins(D401,D402 and D001) respectively,then we detected the concentrations of 5 heavy metals(Cu,Pb,Cd,Hg,As) and flavonoids in the crude extracts with or without treatment. RESULTS: The concentrations of all heavy metals decreased significantly after treatment of D401 and D402.The concentrations(mg/L) of Cu,Pb,Cd changed from 0.500,0.521,0.078 to 0.117 and 0.236,0.174 and 0.165,0.024 and 0.045,respectively,and Hg and As became beyond the detection limit after the treatment,but the use of D001 affected a bit quantities of 5 heavy metals.Moreover,the concentration of flavonoids kept almost the same after treatment of such 3 resins.On the condition of room temperature,we found that a fluent velocity of 30 m/s and pH of 7 would bring the superior treatment of D401 to excessive heavy metals in TCM. CONCLUSION: Macroporous chelating resins(D401 and D402) can be employed in the treatment of excessive heavy metals in crude extracts of TCM.

Objective o investigate the prophylactic and therapeutic effects of montelukast,a cysteinyl leukotriene receptor-1 (CysLT1R) antagonist,on the delayed neuropsychological sequelae (DNS) in rat model of carbon monoxide (CO) poisoning and to explore the possible underlying mechanism.Methods A total of 90 rats were acclimated for one week prior to screening rat by Morris water maze test.Ten rats were randomly assigned to control group (Con group),and the remaining 80 rats were subjected to modified method of intraperitoneal injection of CO gas to establish animal model of acute CO poisoning,Thereafter,the survival rats randomized into CO poisoning group (Mod group),low-dose montelukast group (ML group),medium-dose montelukast group (MM group),high-dose montelukast group (MH group) (n =10 each).Montelukast was accordingly administered via intragastric tube at different intervals (30 min,4 h and 12 h) after CO poisoning,and then montelukast was administered every 12 hours for 7 consecutive days.The rats of control group and Mod group received equal volume of normal saline instead at given intervals.Twenty-one days after CO exposure,the average escape latency was measured by Morris water maze test to screen DNS rats followed by H-E staining to observe the pathological changes of cortex and hippocampal CA1 region and TUNEL was used to assess the apoptosis of neurons in cortex and hippocampal CA1 region after rats sacrificed.Results All CO-exposed rats exhibited cognition function lowered,and the escape latency (seconds) in Mod group (43.3 ± 15.5),ML group (31.5 ± 13.2) and MH groups (30.1 ± 12.2) was significantly prolonged compared with Con group (12.1 ± 3.0) (P ＜ 0.05),whereas the difference between MM group (15.0 ± 6.6) and Con group was statistically insignificant (P ＞ 0.05).Compared with Mod group,the escape latency in montelukast treatment groups was shortened,whereas the significant difference in escape latency only found between Mod group and MM group (P ＜ 0.05).Except for Con group,DNS was evident in CO-exposed groups,and the numbers of DNS rats in Mod,ML,MM and MH groups were 8,5,1,4,respectively,which made statistically significant differences to Con group (P ＜ 0.05) except MM group.The DNS incidence in MM group was lower than that in Mod group (P ＜ 0.05).Mod group exhibited severe histopathological injury to the brain,with evident apoptosis of neural cells,whereas in the groups with montelukast treatment,histopathological damage to the brain was mitigated and the number of apoptotic neuronal cells was diminished noticeably in MM group.Conclusion Montelukast can ameliorate the cognitive function of rats,decrease the incidence of DNS and reduce the apoptosis of neural cells as well as attenuate neuronal cell injury,thus exerting neuroprotection against DNS in rats with CO poisoning.

Objective To investigate the therapeutic effect of oxiracetam combined with dexamethasone for treatment of patients with severe acute carbon monoxide poisoning (ACOP).Methods Eighty-seven patients with severe ACOP admitted to Affiliated Hospital of Xuzhou Medical University from January 2013 to July 2016 were enrolled, and they were divided into observation group (47 cases) and control group (30 cases) according to random number table method. The two groups were given conventional nerve nutrition, hyperbaric oxygen and symptomatic treatment, while in the observationgroup, on the basis of conventional treatment the patients received intravenous drip of dexamethasone 5 mg (with addition of normal saline 100 mL), once a day for consecutive 3 days, and intravenous drip of oxiracetam 3.0 g plus 100 mL normal saline, once a day for consecutive 7 days. The changes of mini-mental state examination (MMSE), Barthel index and Glasgow coma scale (GCS) were observed after discharge for 30 days, and the differences of the incidence of delayed encephalopathy after acute carbon monoxide poisoning (DEACMP) and hospitalization time between the two groups were compared.Results After treatment the MMSE and GCS scores of the two groups were higher than those before treatment, and the Barthel score was lower than that before onset. After discharge for 30 days, MMSE score (26.93±2.92 vs. 24.20±6.82), GCS (14.41±1.32 vs. 13.98±2.13), Barthel (94.78±12.62 vs. 85.25±19.57) of the observation group were significantly higher than those of control group, and the incidence of DEACMP was obviously lower than that in the control group [8.5% (4/47) to 17.5% (7/40)], the differences between the two groups being statistically significant (allP < 0.05); the hospitalizationtime of the observation group was shorter than that of the control group (days: 10.81±5.49 vs. 13.05±6.84), but there was no significant difference between the two groups (P > 0.05).Conclusion Early combined application of oxiracetam and dexamethasone can significantly improve the cognition and self-care abilities and reduce the incidence of DEACMP in patients with severe ACOP after discharge for 30 days.

Objective To evaluate the clinical outcomes of double-port video-assisted thoracoscopic lobectomy. Methods We retrospectively analyzed the clinical data of 72 patients who underwent double-port video-assisted thoracoscopic lobectomy(DP group)from October 2014 to December 2015 in our hospital.A paired comparison was made with 72 patients who had the same lesion location and the nature(benign or malignant)and underwent three-port video-assisted thoracoscopic lobectomy(TP group)in the same period.The clinical outcomes included operation time, intraoperative blood loss,the number of lymph node, the chest tube time,postoperative hospital stay, incision pain VAS scores within three days and complications.Postoperative follow-ups were taken with telephone calls or out-patient reviews. Results No operative morality occurred in both groups.There were no statistical significances between the DP Group and the TP Group in intraoperative blood loss[(107.9 ±56.6)ml vs.(95.0 ± 46.8)ml,t=1.490,P=0.138],the number of lymph node(13.9 ±2.7 vs.14.5 ±2.6,t=1.358,P=0.177)and complication rate[11.1%(8/72)vs.9.7%(7/72),χ2=0.074,P=0.785].The DP group had significant longer operation time[(153.6 ± 22.6)min vs.(143.6 ±25.8)min, t=2.474, P=0.015], shorter chest tube time[(5.2 ±1.7)d vs.(6.4 ±1.1)d, t=5.029,P=0.000],shorter postoperative hospital stay[(6.1 ±1.6)d vs.7.6 ±1.2, t=6.364, P=0.000], and lower incision pain VAS scores within three days[(12.3 ±1.9)points vs.(14.4 ±1.8)points, t=6.808, P=0.000]as compared to the TP group.No short-term complication was noticed in both groups during a follow-up time ranged from 6 to 20 months. Conclusions Double-port video-assisted thoracoscopic lobectomy is safe and feasible.It is a preferred surgical mode for selected cases.

A rapid detection method of 16 preservative residues in vegetables for export was developed by gas chromatography_mass spectrometry ( GC_MS ) after pre_treatment method of dispersive solid phase extraction ( SPE) . Samples including leafy, eggplant fruit, bean, melon and tuber vegetables were extracted with acetonitrile and detected by GC/MS with external standard method after being cleaned up with dispersive SPE. The results showed that the method had a good linearity over concentration range of 0 . 02-2 . 0 mg/L preservatives with correlation coefficients of 0 . 9990-0 . 9997 . The limits of detection ( LODs ) were in the range of 0. 004-0. 965 μg/kg. The recoveries of the preservatives were in the range of 76. 7%-120. 0% at the spiked levels of 0. 01, 0. 02 and 1. 00 mg/kg, and the relative standard deviations ( RSDs) were in the range of 2. 1%-10. 6%. Compared with the GB/T 19648_2006, etc, the method is rapid, simple, highly sensitive, and can meet testing requirements of 16 preservative residues in vegetables for export.
