ObjectiveTo evaluate the effectiveness of surgical technique improvement in thyroid lobectomy for prevention of recurrent laryngeal nerve (RLN) injury. MethodsThe clinical data of 85 patients of thyroid disease undergoing 126 lobectomies from Jan 2010 to Jan 2011 were retrospectively analyzed.All patients undergone accurate thyroid lobectomy with accurate envelop anatomic dissection,skeletonized blood vessels. Recurrent laryngeal nerve was all dearly exposed.ResultsNo-bleeding surgical area was achieved.The RLN was exposed.Unilateral temporary RLN injury occurred in 2 cases.The rate of RLN injury was 1.59％(2/126). There was no bilateral and perpetual RLN injury.ConclusionsCarrying through the accurate thyroid lobectomy,the incidence of RLN injury is low.

Objective To explore the application value of indocyanine green (ICG)in sentinel lymph node biopsy(SLNB)for breast cancer.Methods A total of 160 patients with breast cancer in our hospital from January 2013 to December 2015 were analyzed,who were randomly divided into methylene blue group(n =65)and methylene blue plus ICG group(n =95).The success rate of detection,accuracy and false negative rate between two groups were compared.Results The success rate of detection and accuracy in methylene blue plus ICG group was significantly higher than thoase in methylene blue group,with statistically significant difference(P <0.05).The false negative rate of methylene blue plus ICG group was significantly lower than that of methylene blue group,with statistically significant difference (P <0.05).The number of sentinel lymph node in methylene blue plus ICG group was (2.9 ±0.9)/case which was more than (1.9 ± 0.5)/case in methylene blue group ,with statistically significant difference(P <0.05).The length of opertion of methylene blue plus ICG group was obviously shorter than that of methylene blue group.Conclusion Indocyanine green used in the sentinel lymph node detection for breast cancer can provide a reliable basis and reduce the false negative rate,who improves the level of diagnosis of breast cancer.

Objective:To explore the influence of disease severity and effectiveness of antituberculosis treatment on helper T-cells in peripheral blood from patients with pulmonary tuberculosis.Methods:Th1 cells and Th2 cells of peripheral blood of 62 pulmonary TB patients and 30 controls were marked by intracellular cytokine(INF-? and IL-4 representative of Th1 and Th2 subsets respectively) and counted by Flow Cytometry.The levels of Th1 and Th2 cell count from patients with different disease severities radiologically before antituberculosis therapy were compared and their correlation with effectiveness of different antituberculosis treatment radiologically was analyzed either.Results:The baseline levels of Th1 cell count and Th2 cell count in pulmonary tuberculosis group before antituberculosis therapy were significantly lower than those in the control group(P0.05,t test).The levels of Th1 cell count in the stable group were still significantly lower than those in the control group(P

Objective To explore the effect of transcription factor E2F1 on the apoptosis of small cell cancer line H446. Methods Plamid vector- mediated E2F1 small hairpin RNA (shRNA) was used to silence E2F1 in H446 cell. RT-PCR and western-blot assay were used to detect the expressions of E2F1 and Bcl-2. The apoptosis rate in H446 cell line was detected by flow cytometry assay. Result E2F1 protein was suppressed in shRNA1-modified H446 cell. Sgnificant difference of the apoptosis was shown between E2F1 shRNA1 group and the other two groups. Additionaly, the expression of Bcl-2 protein increased in E2F1 shRNA1-modified cell line. Conclusions E2F1 is highly expressed in H446 small cell lung cancer cell line. E2F1 promotes apoptosis of H446 through upregulating Bcl-2 expression.

Objective To observe the effect of DNA methyltransferase 1 (DNMT1 ) gene silencing by RNA interfering technology on the proliferation and apoptosis of HeLa cells. Methods Recombinant plasmid pshRNA-DNMT1-A, B and C were respectively transfected into HeLa cells by lipofectamine 2000, while cells transfected plasmid vector pSilencer3.1-HI and cells untreated as control groups. RT-PCR was adopted to select the recombinant plasmid which showed the most optimal inhibition effect. RT-PCR and western blotting was used to detected the mRNA and protein expression of DNMT1 in HeLa cells transfected for 24, 48 and 72 hours. Cell counting kit-8 (CCK-8 ) assay was used to investigate the proliferation of the HeLa cells after transfection, while apoptosis was detected by flowcytometry(FCM ) method. Results Three DNMT1-targeted short hairpin RNA (shRNA) A,B and C were successfully inserted into the plasmid vector PShRNA, and the coding sequences of the obtained shRNA were consistent with the designed fragments. The results indicated that both recombinant plasmid pshRNA-DNMT1-A and B could effectively knock down the expression of DNMT1 gene in human cervical cancer cells, of which pshRNA-DNMTI-B was the better choice. While no effect of pshRNA-DNMTI-C was seen. BT-PCR results showed that the relative mRNA expression of DNMT1 gene in Helm cells transfected with pshRNA-DNMT1 for 24, 48 and 72 hours were 0.406±0.057,0.191±0.036 and 0.104±0.015, which were significantly lower than that in Helm cells transfected by empty vector and non-transfected cells (0.520±0.020, 0.537±0.041, respectively, P ＜ 0.05 ). The western blotting analysis manifested that the relative expression of DNMT1 protein of Helm cells transfected by pshRNA-DNMT1 for 24, 48 and 72 hours were 0.197±0.024, 0.075±0.015, 0.040± 0.013, which were significantly lower than that in transfected cells by empty vector and non-transfected cells (0.273±0.010, 0.283±0.016, respectively, P ＜0.05). The CCK-8 results showed that the cell survival rates of HeLa cells transfected by pshRNA-DNMT1 for 24, 48, 72, 96 and 120 hours were 70.8%, 64.8%, 51.6%, 45.3% and 38.0%, there were statistically different compared with cells transfected by empty vector and non-transfected cells at different time-points (P ＜ 0.01 ). The results of FCM indicated that the apoptesis rate of HeLa cells trandected with pshRNA-DNMTI for 24, 48 and 72 hours were (17.7± 1.3 ) %, (35.3±1.3 ) %, (47.6±1.6 ) %, which were significantly higher than empty vector transfected cells and non-transfected cells [(4.9±0.5 ) %, (5.1±0.7 ) %, respectively, P ＜ 0.05]. Conclusions DNMT1 can be successfully silenced by RNA interfering in cervical Helm cells. Downregulation of DNMT1 can inhibit cervical cancer cells proliferation and induce cell apoptosis.

Objective:To elucidate the phenotypic and functional characteristics of CD3+ CD4+ and CD3+ CD8+ T in pleural fluid of patients with tuberculous pleurisy.Methods:PBMCs and PFCs were isolated. The frequency of CD3+,CD3+ CD4+,CD3+ CD8+ T cells and the ratio of CD4/CD8 in PFCs were analyzed by FACS. The concentration of IFN-?,IL-2 and TNF-? in the pleural fluid were analyzed by ELISA.Production of IFN-?,IL-2 and TNF-? by CD3+ CD4+ and CD3+ CD8+ T cells was detected by FACS following stimulation with BCG.Results:Higher frequencies of CD3+ CD4+ T cells and CD4+ T/CD8+ T ratio were demonstrated in PFCs when compared with PBMCs from normal donors and TB patients.ELISA analysis demonstrated that significantly high levels of IFN-? and TNF-? were detected in pleural fluid.Further analysis by FACS indicated that IL-2,TNF-? and IFN-? were predominantly secreted by CD3+CD4+T cells but not by CD3+ CD8+ T cells.Conclusion:The frequencies of CD3+ and CD3+ CD4+ T cells are increased in PFCs. Large amounts of Th1 cytokines are detected,which are produced by CD3+ CD4+ T cells,and might play important roles against TB infection.

Objective To study the operative technique and effect of temporary balloon occlusion of the abdominal aorta for preventing intraoperative bleeding during cesarean for patients with pernicious placenta previa and placenta accreta. Methods Retrospective analysis was conducted on the intraoperative situation of forty-one cases and information of follow-up twenty-nine cases, which were pernicious placenta previa and placenta accreta and delivered in the First Affiliated Hospital of Zhengzhou University from May 1, 2013 to June 30, 2014. Diagnosis was confirmed by line of color Doppler ultrasound and MRI for all patients before operations. An interventional physician performed right femoral artery puncture and preset the abdominal aortic balloon catheter in the digital subtraction angiography operation room before cesarean. At the same time of fetal delivery, 10 ml normal saline was injected into the balloon immediately, which results in filling of the balloon and blocking of the aorta. According to the area and depth of placenta implantation and implantation or penetration of the posterior bladder wall, placenta separation, partial resection of the uterine wall and partial bladder resection and repair were performed correspondingly. Meanwhile, saline in the balloon was pumped out gradually until empty. Condition of placenta implantation, blood loss and blood transfusion volume during the operation, intraoperative and postoperative complications, the duration and dose of fetal radiation exposure, and Apgar score of neonates were analyzed. Results Among the 41 cases, penetrative placenta and implanted placenta were observed in five cases and 36 cases, respectively. The latter 36 cases including 28 cases of bladder posterior wall accreta and eight cases of bladder posterior wall penetration. For all cases, the average operation time was (68.5±15.3) min, the mean blood loss in the operation was (1 058±960) ml, among which eight received blood transfusion with an average of (600±400) ml, and the mean hospital stay was (8.2±2.3) d. Uteruses were reserved in all cases. The mean duration and dose of fetal radiation exposure was (8.1±3.6) s and (5.2±2.9) mGy, and the Apgar score of neonates was 8.7±0.5 at 1 min and 9.5±0.3 at 5 min, respectively. The patients were followed up until October 31, 2014. Among them, six were lost, six were still in puerperium, 18 were breast-feeding, and the menses of 11 had returned. Conclusion Preset abdominal aortic balloon catheter in pernicious placenta previa and placenta accrete patients might effectively reduce the blood loss during cesarean section as well as the risk of hysterectomy through temporary occlusion of the abdominal aorta.

The abdominal aortic aneurysm with a diameter of 3.0-5.4 cm is called small abdominal aortic aneurysm.Generally asymptomatic,there is no evidence-based medical evidence that surgery or endovascular repair is better than follow-up observation.Small abdominal aortic aneurysms are managed by regular CT or ultrasound surveillance until they grow to a diameter threshold (commonly 5.5 cm) at which surgical intervention is considered.But these patients without surgical intervention remain risk of rupture.We briefly summarized the current situation and progress of treatment on sAAA.

INTRODUCTION: Development of carcinoma on oral tongue may cause bilateral cervical lymph node metastasis, rapid invasion and growth of the cancer cells due to rich blood supply in muscle tissues. It is not only difficult to develop an animal experimental model, but also to proceed follow-up research after the development of such model as the induction of cancer lead to difficulty in taking nutrition for the experimental animals that often causes early death. MATERIALS AND METHODS: IIn this study, author have transplanted YD-10Bmod cells into nude mouse oral tongues with different cells number (5x10(4), 5x10(5), 5x10(6) cells/mouse) and observed the development aspect of oral tongue cancers. RESULTS: The cancer developed from orthotopic transplantation of YD-10Bmod cells into nude mouse oral tongue show invasion and central necrosis of the tumor, similar to the cancers developed human oral tongue cancer. The difference in tumor size and the time of central necrosis development depending on the number of transplanted tumor cells shows the feasibility of extending the survival period of the nude mouse by limiting the transplanted tumor cells to <5x10(4) cells/mouse or under per nude mouse. CONCLUSION: This nude mouse model could be used effectively in developing effective chemotheray agent and establishing an animal experimental model that can be used to study the mechanism of cervical lymph node metastasis of the oral tongue cancer.

Objective To analyze driver genes status and its clinical characteristics of advanced lung adenocarcinoma, then evaluate the status of first-line treatment in a single centric real-world. Methods EGFR, ALK, ROS-1 gene in 204 advanced lung adenocarcinoma tissue were tested by ARMS-PCR method. And the relationship between driver genes status and clinical characteristics was analyzed as the first line treatment in real clinical practice. Results The positive rate of driver genes status in 204 advanced lung adenocarcinoma was 53.9％ (110/204) , including EGFR mutation 46.1％ (94/204) , ALK positive 6.4％ (13/204) and ROS1 positive 1.5％ (3/204). The driving genes status was significantly correlated with gender, smoking history, tumor staging and serosal invasion (P < 0.05). There were significantly differences among the proportion of first-line standard treatment in different subgroup (P = 0.000) , the first-line standard treatment rate of EGFR mutation, ALK/ROS1 positive and drive gene negative were 77.7％, 37.5％, and 46.8％ respectively. And the ratio of using 1 st generation EGFR-TKIs in all patients is 70.6％ (60/85). Conclusion More than half of advanced lung adenocarcinoma have driver genes changes, and EGFR-TKI first-line treatment has higher acceptability in real-word.
