1.Observation of the effect of skull base meningioma communicate through the superior orbital fissure cranio-orbital approach of microsurgery

Objective To investigate the skull base meningiomas communicate through the superior orbital fissure cranio-orbital approach of microsurgical Methods and the efficacy.Methods 80 cases with skull base meningiomas were randomly divided into the control group and observation group,40 cases in each group.The control group was given contact microsurgery,and the observation group received communication through the superior orbital fissure cranio-orbital approach of micro-surgery.The improvement of symptoms and postoperative complications were compared and analyzed.Results (1)The operative time,blood loss,blood transfusion,Simpson grade (Ⅰ,Ⅱ,Ⅲgrade),DKPS,postoperative hospital stay and mortality rate in the control group were (9.28 ± 0.97)h,(1 222.38 ± 89.07) mL,(1 099.35 ±56.72) mL,14/17/9,3.21 (0,6.44),(29.78 ±3.29)d and 7.50% (3/40),which in the observation group were (7.02 ± 0.65) h,(877.34 ± 44.50) mL,(677.10 ± 32.28) mL,29/9/2,3.26 (-3.33,10),(20.00 ± 2.75) d and 5.00% (2/40).The differences of operative time,blood loss,blood transfusion,Simpson level,postoperative hospital stay between the two groups were statistically significant(t =4.209,4.997,5.823,x2 =4.011,t =5.711,P =0.033,0.027,0.018,0.022,0.013),but the D KPS and mortality between the two groups had no significant differences (t =0.433,0.096,P =0.089,0.317).The monocular prominent,eye movement disorder and vision improvement rates in the control group were 66.67%,72.22% and 75.68%,which were significantly lower than 92.31%,94.12% and 97.14% in the observation group (x2 =5.932,4.381,6.793,P =0.027,0.033,0.020).The postoperative eye movement disorder,on ptosis,eye abduction,subcutaneous fluid and intracranial infection rates in the control group were 10.00%,15.00%,30.00%,12.50% and 7.50%,those in the observation group were 7.50%,5.00%,10.00%,10.00% and 7.50%.The postoperative eye movement disorder,subcutaneous fluid and intracranial infection rates had no significant differences between the two groups(x2 =0.923,1.033,0.785,P =0.387,0.595,0.233).The incidence rates of postoperative ptosis,abduction eyeball had statistically significant differences between the two groups (x2 =6.299,7.889,P =0.018,0.009).Conclusion The skull base meningiomas communicate through the superior orbital fissure cranio-orbital approach of microsurgery has significant effect than the conventional contact microsurgery,which deserves to be promoted in clinical.
2.Clinical analysis of 326 cases of choledocholithiasis treated with endoscopic sphincterotomy

Objective To investigate the therapeutic efficacy of endoscopic sphincterotomy (EST) for treatment of choledocholithiasis. Methods The clinical data of 326 patients receiving EST from July 1990 to January 2009 were retrospectively analyzed. Long-term follow-up was performed in 195 of the 326 patients. Results The common bile duct stones were removed in 301 cases (92. 33%).The occurring rate of complications after EST was 6.13% (20/326). Of the 20 patients, 10 had pancreatitis (3.07%), 7 hemorrhage (2. 14%), 4 cholangitis (1. 220%) and 1 duodenal perforation (0.09%). Only 1 patient died. The follow-up showed that 20 patients had reflux cholangitis and 18 recurrence of common bile duct stones. Conclusion Therapeutic endoscopy is safe and effective for treatment of choledocholithiasis.
3.Features and treatment of gastroduodenal ulcer perforation in aged patients

Objective To ivestigate the clinical feature and therapeutic ways of the gastroduodenal ulcer perforation(GDUP) in aged patients.Methods The clinical data of 58 GDUP patients with age more than 60 gears treated in recently tweenty yesas in our hospital were analysed retrospectively. Results Among the 58 patients,56 patients underwent operation,46cured and 12 died.Of the 12 patients,dead of cardiorespiratory function failture in 5 ; septic shock in 4 ;and renal failturl in 3 .Conclusions Aged patients with GDUP would be operated as early as possible.Repair of the performed ulcer plus modified vagotomy is a better choice.
4.Resting-state functional MRI of regional spontaneous brain activity in classical trigeminal neuralgia patients

Objective To investigate the alterations of regional spontaneous activity in patients with classical trigeminal neuralgia (CTN) during resting state.Methods Twenty-seven patients with CTN (CTN group) and 27 healthy subjects (control group) were recruited and underwent a rest-state functional MRI.The regional homogeneity (ReHo) analysis was used to compare the differences of regional synchronization of spontaneous brain activity.And correlation tests were performed between ReHo values in the abnormal brain areas and clinical metrics (visual analogue scale and disease duration) of the disease.Results Compared with control group (P<0.05,Gaussian random field correction),ReHo increased in bilateral primary somatosensory cortex (S1) and primary motor cortex (M1),right supplementary motor area (SMA),inferotemporal cortex and cerebellum,left thalamus,limbic lobe,parahippocampa gyrus,middle and superior temporal gyrus in CTN group;ReHo decreased in bilateral insula,prefrontal cortex and orbitofrontal cortex,right frontal medial cortex and superior temporal gyrus,left anterior cingulate area,supramarginal gyrus and cerebellum in CTN group.ReHo values in right frontal medial cortex was negatively correlated with the course of disease (r=-0.45,P=0.03).The ReHo values of left primary sensorimotor cortex were positively correlated with the visual analogue scale scores (r=0.46,P=0.02).Conclusion CTN patients has abnormal functional homogeneity of spontaneous brain activity in regions involved in the pain processing,which can help understanding mechanism of CTN.
5.Alteration of cerebral regional homogeneity within sensorimotor network in patients with cervical spondylotic myelopathy after spinal cord decompression:a resting-state functional MRI study

Objective To assess the altered regional homogeneity (ReHo) of local intrinsic cerebral activity within sensorimotor network(SMN) in patients with cervical spondylotic myelopathy (CSM) before or after spinal cord decompression using functional MRI (fMRI). Methods Twenty-one CSM patients who would decompress spinal canal, and 21 healthy volunteers (age, gender and level of education matched) were enrolled from June 2013 to April 2014. All the patients underwent rs-fMRI examination before and 3 months after spinal cord decompression. ReHo measurement was performed statistically within a SMN mask. A second-level random-effect 2-tailed Student's t test was applied to compare the ReHo results between pre-and post-operation CSM patients and healthy volunteers. A second-level paired 2-tailed Student's t test was applied to compare the ReHo results between pre-and post-operation CSM patients. Pearson correlation analysis was performed to assess the correlations between the altered ReHo and clinical evaluation. Results Compared with healthy volunteers, pre-operation patients showed significantly lower ReHo in the left postcentral gyrus/precentral gyrus, together with enhanced ReHo in the right superior parietal lobule (GRF correction, P<0.05). Post-operation CSM patients showed significantly lower ReHo in the right superior parietal lobule comparing with healthy volunteers, as well as enhanced ReHo in the left postcentral gyrus/precentral gyrus comparing with pre-operation (GRF correction, P<0.05). Abnormal ReHo areas in CSM patients demonstrated no significant correlation with clinical measurements (P>0.05) between pre-operation and post-operation. Conclusions Myelopathy in cervical cord may affect intrinsic cerebral activity, as patients with CSM show disrupted regional homogeneity within sensorimotor network. The change of ReHo following decompression suggests that central plasticity may influence functional recovery.
6.The experience of the operation to different types of congenital choledochal cysts

Objective To summarize the experience of operative treatment of all types of congenital choledochal cysts (CCC).Methods Clinical data from 52 operated patients with CCC were analyzed retrospectively.(Operations) included excision of the cyst with Roux-en-Y hepaticojejunostomy in 38 cases,partial excision of the cyst with choledochojejunostpmy with Roux-en-Y loop in 2 cases etc. Results 52 cases were cured and left hospital . Among them, 45 were followed up and 42 cases had a good recovery. Conclusions (Resection) of the cyst with Roux-en-Y hepaticojejunostomy is recommended as the therapy of choice in selective operation.Adherence to the indications,the method of excision and the principle of Roux-en-Y (hepaticojejunostomy) are the key factors to increase the effect of the operation.
7.Application of fractional anisotropy entropy in cervical spondylotic myelopathy

Objective To quantitative analyze the injury degree of cervical cord in cervical spondylotic myelopathy (CSM) patients using fractional anisotropy (FA) entropy of diffusion tensor imaging (DTI).Methods Twenty-four CSM patients and well-match healthy volunteers underwent cervical cord DTI scanning.FA value was measured at each segment of spinal cord (including grey matter and white matter) of spinal cord level and intervertebral disc level.Further Shannon entropy of FA value was calculated in each segment to observe the disorder degree of cervical cord structure in CSM patients.After the analysis of homogeneity of variance,two samples t test analysis was used to identify FC's differences of FA and Shannon entropy of FA value between the two groups.Result In CSM group,the average FA value of whole cervical cord was 0.644 ± 0.056,while it was 0.672 ± 0.035 in the healthy control group.There was significant difference of FA values between the two groups (t=-2.049,P=0.046).The FA entropy of CSM patients' cervical cord was 0.687±0.043,while it was 0.854±0.027 in the healthy control group.The FA entropy of CSM patients' cervical cord was lower than that of control group.There was significant difference of FA entropy between the two groups (t=-12.100,P<0.001).The significant difference between the two groups was only found at C6 level for each level's FA value.The FA entropy of CSM patients' cervical cord was significantly lower than that of control group for every level.Conclusion This decreased FA entropy of cervical cord in CSM patients indicates that FA entropy can be used to detect the disorder of cervical cord structure in CSM patients.
8.Expression and significance of HSPC238 in cervical intraepithelial neoplasia and cervical cancer

Objective: To investigate the expression of HSPC238 in cervical intraepithelial neoplasia and cervical cancer.Methods:We collected 76 cases of cervical cancer,105 cases of CIN and 28 cases of normal cervical epithelial.Then we inves-tigated the expression of HSPC238 by using immunohistochemistry and compared the significant differences between them.Results:There was no significant difference in the expression of HSPC238 between the cervical cancer and normal cervical epithelial ( Z=-0.242,P>0.05).However,there was significant difference between the cervical intraepithelial neoplasia and normal cervical epithelial (χ2=19.159,P<0.01) and the expression of HSPC238 was correlated with the grades of CIN.The expression of HSPC238 decreased when the grade of CIN was increasing.( rs=-0.327,P<0.01 ).Conclusion:The low expression of HSPC238 might be correlated with the development of cervical neoplasia.
9.Role of transient receptor potential cation channel 6 (TRPC6) channel in mouse podocyte injury induced by TGF-β1

Objective To investigate the effects of transient receptor potential cation channel 6 (TRPC6) on the expression of nephrin, desmin and caspase 9 and on the apoptosis of podocytes in a mouse model of podocyte injury induced by TGF-β1.Methods Conditionally immortalized mouse podocytes were cultured in vitro and divided into four groups: control, TGF-β1 treatment, TGF-β1+PGPU6/GFP/Neo-TRPC6-mus-581 (TRPC6 knockdown) and TGF-β1+PGPU6/GFP/Neo-NC (negative control).Real-time RT-PCR and Western blot analysis were performed to detect the expression of nephrin, desmin and caspase 9 at mRNA and protein levels, respectively.Flow cytometry was used to analyze the apoptotic rate of podocytes.DAPI fluorescent staining was used to observe the morphological changes of apoptotic podocytes.Results Green fluorescent protein (GFP)-expressing podocytes at 48 hours after transfection were significantly more than those at 24 hours after transfection.The level of TRPC6 in mouse podocytes transfected with PGPU6/GFP/Neo-TRPC6-mus-581 was significantly decreased as compared with that of the control group (P<0.05).No significant difference in the expression of TRPC6 was observed between the negative control group and the control group.Compared with the control group, the TGF-β1 treatment group showed increased expression of desmin and caspase 9 at both mRNA and protein levels (P<0.01), but decreased expression of nephrin at mRNA and protein levels at 48 hours after TGF-β1 intervention (P<0.05).The up-regulated desmin and caspase 9 and the down-regulated nephrin induced by TGF-β1 could be inhibited by the means of TRPC6 knockdown.The apoptosis rate of podocytes in TGF-β1 treatment group was (14.0±2.1)%, while that in TRPC6 knockdown was (10.90±0.56)% (P<0.05).The apoptosis rate of podocytes in negative control group was higher than that in TGF-β1 treatment group (P>0.05).More apoptotic cells with typical morphological features of apoptosis were observed after exposure to TGF-β1 for 48 hours.Conclusion TGF-β1 could induce the apoptosis of podocytes, inhibit the expression of nephrin and enhance the expression of caspase 9 and desmin, the possible mechanisms of which may be related to TRPC6 signal pathway.These changes in TGF-β1-treated podocytes could be alleviated by inhibiting the expression of TRPC6, which might have a protective effect on podocyte injury.
10.Effect of FCN gene single nucleotide polymorphism on the susceptibility of pre-eclampsia in Han nationality pregnant women

Objective:To investigate the effect of single nucleotide polymorphism (SNP) of FCN gene on the susceptibility of pre-eclampsia (PE) in Han nationality pregnant women.Methods:A total of 274 PE pregnant women (PE group) and 154 healthy pregnant women (control group) admitted to Boai Hospital of Zhongshan, Affiliated Hospital to Southern Medical University from October 2020 to October 2022 were collected. The general information, medical history, reproductive history, blood pressure, body mass index and blood biochemical indicators before delivery were compared between the two groups. Twenty-three SNP loci of FCN gene family were genotyped by time-of-flight mass spectrometry, and the serum levels of ficolins (ficolin-1, -2 and -3) were detected by enzyme-linked immunosorbent assay.Results:(1) Compared with the control group, the body mass index, mean arterial pressure, gestational age at delivery, blood urea nitrogen, alanine aminotransferase, aspartate aminotransferase, direct bilirubin, albumin, and C-reactive protein in the PE group were significantly higher than those in the control group (all P<0.05). The levels of N-terminal pro-B type natriuretic peptide (NT-proBNP), placental growth factor (PlGF) and human soluble vascular endothelial growth factor receptor-1 (sFlt-1) were significantly different between the two groups (all P<0.05). (2) Among the 23 SNP loci in FCN gene family, 18 loci were in Hardy-Weinberg genetic equilibrium, including 5 loci in FCN1 gene, 10 loci in FCN2 gene, and 3 loci in FCN3 gene. Five loci that did not conform to Hardy-Weinberg genetic equilibrium were not included in the subsequent analysis. Compared with the control group, the genotype distribution of 3 loci of FCN2 gene (rs7872508, rs11103563, rs73664188) and 1 locus of FCN3 gene (rs3813800) in the PE group were significantly different (all P<0.05). After Bonferroni correction, only the genotype distribution of rs7872508 and rs73664188 in FCN2 gene were statistically different between the PE group and the control group (all P<0.05). Further analysis showed that for the rs7872508 locus of FCN2 gene, compared with GG genotype, genotype GT ( OR=3.025, 95% CI: 1.080-8.471) and TT ( OR=4.777, 95% CI: 1.758-12.979) both significantly increased the risk of PE (both P<0.05). For rs73664188 locus of FCN2 gene, compared with TT genotype, genotype TC ( OR=0.510, 95% CI: 0.334-0.778) significantly reduced the risk of PE ( P<0.05). (3) Compared with the control group, the serum levels of ficolin-1 and ficolin-2 in pregnant women in the PE group were significantly reduced (both P<0.05), while the level of ficolin-3 showed no significant change ( P=0.271). Correlation analysis showed that the serum levels of ficolin-2 in pregnant women in the PE group were significantly positively correlated with PlGF level ( r=0.321, P<0.001), and significantly negatively correlated with sFlt-1 level ( r=-0.187, P=0.002) and NT-proBNP level ( r=-0.392, P<0.001). Further analysis revealed that the serum levels of ficolin-2 in pregnant women of the PE group with GT and TT genotypes at rs7872508 locus of FCN2 gene were significantly reduced (both P<0.05), while the serum level of ficolin-2 in pregnant women of the PE group with TC genotype at the rs73664188 locus were significantly increased ( P<0.05). Conclusion:The SNP of FCN2 gene in FCN gene family might be related to the susceptibility to PE and have an effect on serum ficolin-2 level in PE pregnant women.
