1.Sedative effect and safety of small doses of Sufentanil combined with Propofol in painless gastroscopy

Objective To investigate the Sedative effect and safety of intravenous Sufentanil combined with Propofol in painless gastroscopy.Methods 270 patients with ASA Ⅰ-Ⅱ were randomly divided into three groups:Group A (the Propofol group),Group B (the Fentanyl combined with Propofol group) and Group C (the Sufentanil combined with Propofol group).HR,MAP and SpO2 were monitored at three different time points:before placement of the gastroscope,during and 5 minutes after gastroscopy.The time required for consciousness recovery,the time required for regaining orientation,the operating time for gastroscopy and the dosage of Propofol were recorded.Results HR,MAP and SpO2 were obviously lower during operation than before or after operation in each group (P<0.05) and were significant different among the groups (P< 0.05).The times required for consciousness recovery and for regaining orientation,respectively,were significantly longer in Group A than in Group B or C,and there was a significantly difference between Group B and Group C (P<0.05).The total dose of Propofol in Group A was significantly larger than that in Group B or C (P<0.05).Conclusions Compared with Propofol alone or Fentanyl combined with Propofol,Sufentanil can be used in combination with lower does of Propofol in painless gastroscopy to produce rapid and effective Sedation,with fewer side effects and higher patient satisfaction.
2.Study on the relation between TCM constitution and familial hereditary in patients with type 2 diabetes

Objective This clinical research will compare the constitution of patients with positive and negative family history in order to study on the relation between TCM constitution and the familial hereditary in type 2 diabetes patients.Methods The data of family history,general conditions,Chinese medical constitutional types of 231 type 2 DM patients in Dongzhimen Hospital were collected and analyzed.Results Among patients with family history,the number of patients' mothers with diabetes was significantly more than that of fathers'(x2=22.319,P<0.05).There was a positive correlation between phlegm-dampness constitution r=0.215,blood stagnant constitution r=0.268 and female patients' family history(P<0.01).And there was a positive correlation between the familial hereditary of DM and the familial hereditary of hypertension r=0.328,hyperlipidemia r=0.223,obesity r=0.151 and coronary artery diseases r=0.215.Conclusion Among type 2 DM,female had stronger familial hereditary.It indicated that diabetic mothers had a stronger influence on following generations than diabetic fathers,especially those female patients of phlegm-dampness constitution or blood stagnant constitution.There was inner correlation between the familial hereditary of DM and the familial hereditary of hypertension,hyperlipidemia,obesity and coronary artery disease.
3.Therapeutic effect of ATP-sensitive potassium channel opener nicorandil combined with metformin in early T2DN

Objective To observe the therapeutic effect of ATP sensitive potassium channel opening agent nicorandil com‐bined with classical treatment drug metformin for treating type 2 diabetes nephropathy (T2DN ) .Methods Thirty patients with T2DN were selected and divided into the control group(14 cases) and the experimental group(16 cases) .The control group was giv‐en metformin 0 .25 g ,3 times daily for 26 consecutive weeks .The experiment group was given the same dose of metformin and nic‐orandil 5 mg ,3 times daily for 26 weeks .The fasting blood glucose ,total cholesterol ,triglycerides ,low‐density lipoprotein(LDL) , high density lipoprotein(HDL) ,blood urea nitrogen ,serum creatinine ,urine albumin ,IL‐6 and MMP‐9 levels before and after treat‐ment were measured in both groups .Results There was no statistically significant difference in fasting blood glucose level after treatment between the control group and the experimental group(P>0 .05);the LDL level after treatment in the experiment group was significantly lower than that in the control group with statistical difference(P<0 .05);blood urea nitrogen ,serum creatinine and urine albumin levels after treatment in the experimental group were significantly lower than those in the control group with sta‐tistical difference(P<0 .05);the levels of serum IL‐6 and MMP‐9 after treatment in the experiment group were significantly lower than those in the control group with statistical difference(P<0 .05) .Conclusion Metformin combined with nicorandil could delay the progression of T2DN .

5.Role of NK cells in mouse allogeneic bone marrow transplantation

AIM: To study the effect of natural killer(NK)-cells on graft-versus-host disease(GVHD), graft rejection, engraftment and reconstituting of hematopoiesis in mouse allogeneic bone marrow transplantation. METHODS: Lethally irradiated BALB/c(H-2 d) mice were transplanted with C57/6j(H-2 b) bone marrow containing donor peripheral T cells and/or NK cells. Recipients CD 34 + cell counts and the expression of H-2K b+ cell were detected by flow cytometry, peripheral white blood cell(WBC) was detected by auto-cytometry, and the recipients survival rates, GVHD, engraftment and hematopoiesis recovery were then observed. RESULTS: In the group of transplanted with NK cells infusion, the incidence of GVHD was evidently lessened, the survival rates, WBC and CD 34 + cell counts and the expression of H-2K b+ cell were significantly high than that without NK cells infusion. CONCLUSION: In mouse allogeneic bone marrow transplantation, alloreactivity NK cells prevent GVHD, reduce graft rejection, and promote engraftment and reconstituting of hematopoiesis. [
6.Simultaneous Determination of Phenolic and Salicylanilide Anthelmintics Multi-residues in Cattle and Ovine Tissues by HPLC-MS/MS

A high performance liquid chromatography-tandem mass spectrometric( HPLC-MS/MS) method was developed for the simultaneous determination of four phenolic and salicylanilide anthelmintics including nitroxinil, oxyclozanide, closantel and rafoxanide in cattle and ovine tissues. Muscle, liverand kidney were extracted with acetonitrile-acetone(60:40, V/V)and fat with 1% triethylamine in acetonitrile, then the extract was purified with MAX solid-phase extraction column. Qualitative and quantitative analysiswas achieved by HPLC-MS/MS undernegative multiple reaction monitoring ( MRM) mode. Good correlation coefficients were obtained (R>0. 99) in the concentration range of 1-100 μg/L. The limits of detection (LOD) and limits of qualification (LOQ) for the four compounds were 1 and 2. 5 μg/kg, respectively. The mean recoveries at the four levels of LOQ, 0. 5 maximum residue limit (MRL), MRL, 2MRL were between 71% and 112%,with the intra-day relative standard deviation(RSD)in the range of 1. 1%-14. 0%and inter-day RSD in the range of 6. 4%-14. 7%. Forty samples from the market were analyzed with the method, only two samples were found to show phenolic and salicylanilide anthelmintics residues.
7.Health care follow-up after chronic heart failture can reduce cardiovascular events and improve quality of life

Objective To observe the impatcts of health care follow-up after chronic heart failture on cardiovascular events and qualigy of life.Methods One hundred and sixty-six patients who were confirmed to have chronic heart failture in our hospital during February 2007 and December 2008 were enrolled in this investigation.The patients were then assigned to the health management group (n=65) or the non-health management group (n=101) and followed up for 5 years.All-cause mortality,readmission for heart failture and quality of life (SF-36 score) were evaluated.Results There was no significant difference of all-cause mortalitv between the two groups (health management group:12.30% (8/65); non-health management group:14.85% (15/101; x2=0.22,P>0.05),although a statistically significant difference of readmission rate was found between the two groups (health management group:12.30% (9/65); non-health management group:35.64% (36/101; x2=9.51,P<0.05).In health management group,the scores of SF-36 were significantly increased at 3 (114.11 ±2.76) or 5 years (116.07± 15.43) when compared with baseline (91.37 ± 2.62) (x2 =102.39,P<0.05).For the non-health management group,the scores of SF-36 were significantly decreased from 92.65±3.65 at baseline to 90.09±5.12 at 3 years or 89.08±5.71 at 5 years (x2=102.39,P<0.05).Conclusion Health care follow-up after chronic heart failture may not decrease all-cause mortality,although can decrease readmission rate and improve quality of life of the patients.
8.Effects of uric acid sodium salt on antibody response, dendritic cells and delayed-type hypersensitivity in BALB/c mice

AIM: To investigate the effects of uric acid sodium salt (UANa) as adjuvant on humoral and cellular immune response in BALB/c mice. METHODS: BALB/c mice were immunized with trichosanthin (TCS) as antigen together with UANa suspension as adjuvant. The antibody titers of IgG were detected by enzyme-linked immunosorbent assay. Dendritic cells (DC) were induced in vitro, the phenotypes of DC were analyzed by flow cytometry and the effect of UANa on DC maturity was evaluated. A delayed-type hypersensitivity (DTH) model was used to analyze the effect of UANa on cellular immune responses in vivo. The in vitro proliferation of lymphocytes was determined by ConA stimulation. RESULTS: Freunds adjuvant greatly enhanced the antibody response of mice to TCS, while UANa adjuvant failed to promote the antibody response but significantly reduced the antibody response as compared to TCS only. No effect of UANa on the expression of CD11c and CD83 in DC was observed by flow cytometry analysis. However, UANa significantly enhanced the expression of MHC II molecule. In the DTH model, UANa enhanced the degree of allergen-induced ear swelling and promoted the ability of lymphocyte proliferation in vitro. CONCLUSION: UANa suspension as adjuvant significantly enhances the cellular immune response but inhibits the humoral immune response to a certain degree, suggesting that UANa has potential application in the vaccine research.
9.The inhibitory effect of quercetin on in vitro activation of T lymphocytes

AIM: To investigate the inhibitory effect of quercetin on in vitro activation of T lymphocytes by polyclonal activators with CD69 expression as an activation marker. METHODS: After being separated from lymphoid nodes of a C57BL/6 mouse, the lymphocytes were exposed to polyclonal activators (PDB or Con A) with or without quercetin. Then they were harvested at 2 h, 6 h and 24 h, respectively. The expressional rates of CD69 on T lymphocytes were assessed by two-color immunofluorescent staining and flow cytometry, and the inhibitory rates of quercetin at different time points were estimated. RESULTS: Quercetin had no effect on the expressional rate of CD69 on T lymphocytes under resting states. After the stimulation with PDB or Con A, the expressional rates of CD69 on T lymphocytes in the present of quercetin (10 ?mol/L) showed significant decrease compared with those of control groups at different time points (P
10.The effect of dexmedetomidine and ketamine on tonsillectomy in children

Objective To observe and contrast the effects of dexmedetomidine and ketamine on the restlessness and analgesia during recovery period of anesthesia after tonsillectomy in children. Methods Sixty ASA Ⅰ-Ⅱ child patients underwent tonsillectomy and adenoidectomy were randomly divided into three groups, group P (appropriate amount of placebo was given in the operation), group D (dexmedetomidine) and group K (ketamine). Data of mean arterial pressure and heart rate of three groups were documented before anesthesia (T0), during extubation (T1), 5 min (T2), 10 min (T3), 15 min (T4) and 30 min (T5) after extubation were recorded. The analepsia time, adverse reactions, restlessness score and pain score were collected in three groups of patients. Results Compared with group P, values of mean arterial pressure and heart rate were more stable at T1, T2 and T3 in groups D and K (P<0.05). The restlessness score, incidence of restlessness and adverse reactions were lower in groups D and K than those in group P (P<0.05), and which were lower in group D than those of group K (P<0.05). Conclusion Both dexmedetomidine and ketamine can play an analgesic role in recovery period of anesthesia and reduce restlessness, adverse reactions and pain score in child patients. Moreover, dexmedetomidine is more effective on inhibiting restlessness and adverse reactions.
