1.Clinical effect analysis of locking compressing plates internal fixation under arthroscopy combined with Zhuyu Tongluo method in the treatment of distal radius fractures

Objective:To explore the effect of locking compressing plates (LCP) internal fixation under arthroscopy combined with Zhuyu Tongluo method in the treatment of distal radius fractures (DRF) and its effects on joint function, hemorheology, serum basic fibroblast growth factor (bFGF) and bone morphogenetic protein 2 (BMP-2).Methods:A total of 78 patients with DRF admitted to the Second Hospital of Tangshan City from June 2017 to January 2020 were selected and grouped according to the random number table method, with 39 cases in each group. The control group was treated with LCP internal fixation under arthroscopy, and the study group was treated with Zhuyu Tongluo method on the basis of the control group. The recovery, hemorheology (plasma viscosity, whole blood viscosity high cut, whole blood low cut, hematocrit), serum bFGF, BMP-2 levels, before and after the operation were compared between the two groups. They were followed up for 6 months and the range of motion of the wrist joints before and after the operation and the excellent and good rate of wrist joint function between the two groups were compared.Results:The swelling subsidence time, pain disappearance time, ecchymosis disappearance time, and fracture imaging healing time in the study group were shorter than those in the control group: (10.37 ± 3.13) d vs. (14.62 ± 3.38) d, (12.39 ± 2.97) d vs. (17.14 ± 4.02) d, (19.15 ± 2.35) d vs. (24.36 ± 3.27) d, (68.21 ± 7.12) d vs. (80.07 ± 8.24) d, and the differences were statistically significant ( P<0.05). The plasma viscosity, whole blood viscosity high-cut, whole blood low-cut, and hematocrit in the study group were lower than those in the control group at 1 and 2 weeks after operation ( P<0.05). The levels of serum bFGF and BMP-2 in the study group were higher than those in the control group at 1 and 2 weeks after operation: at 1 week after operation: (356.27 ± 46.29) μg/L vs.(236.51 ± 37.42) μg/L, (614.28 ± 61.47) μg/L vs. (487.39 ± 48.24) μg/L; at 2 weeks after operation:(502.07 ± 113.21) μg/L vs. (381.59 ± 82.75) μg/L, (910.77 ± 97.20) μg/L vs. (731.35 ± 92.13) μg/L, and the differences were statistically significant ( P<0.05). The palm flexion, back extension, radial deviation, and ulnar deviation in the study group were better than those in the control group at 6 months after operation ( P<0.05). The excellent and good rate of wrist joint function in the study group was higher than that in the control group at 6 months after operation: 89.5%(34/38) vs. 70.3%(26/37), and the difference was statistically significant ( P<0.05). Conclusions:LCP internal fixation under arthroscopy combined with Zhuyu Tongluo method in the treatment of patients with DRF can increase the expression of serum bFGF and BMP-2, reduce inflammation, improve blood rheology, promote fracture healing, and improve wrist range of motion and wrist function.
2.Analysis of NRAS gene mutations and prognostic factors in patients with acral melanoma

Objective To detect NRAS gene mutations in patients with acral melanoma, and to analyze their relationship with the prognosis of acral melanoma. Methods Clinical and pathological data were collected from 55 patients with pathologically diagnosed acral melanoma. DNA was extracted from paraffin?embedded specimens from lesions of the 55 patients and 15 patients with nevus. PCR and direct DNA sequencing were performed to detect NRAS gene mutations. Univariate and multivariate analyses were performed using the Cox′s proportional hazards regression model. Results Of the 55 patients, 6(10.9%)carried the Q61R mutation in codon 61 of the NRAS gene. No mutations were found in exon 1 or 2 of the NRAS gene in any of these paraffin?embedded specimens, and none of the pigmented nevus specimens harbored NRAS gene mutations. Of the 6 patients carrying NRAS gene mutations, 4 had lymph node metastasis. Multivariate Cox regression analysis showed that independent factors of poor prognosis included advanced clinical stage(RR = 2.54, 95% CI: 1.062- 6.066, P < 0.05), not receiving surgical resection(RR = 2.98, 95% CI:1.316- 3.525, P < 0.05), and carrying NRAS gene mutations (RR = 2.73, 95% CI: 0.932- 3.257, P < 0.05). Conclusions NRAS gene mutations may be associated with lymph node metastasis in patients with acral melanoma. The prognosis of acral melanoma may be associated with clinical staging, treatment strategies and NRAS gene mutations. Additionally, NRAS gene mutations may serve as a new index for predicting prognosis of acral melanoma.

The methamphetamine(METH)addiction is a compensatory adaptation of the central nervous system after the long-term exposure to METH in the molecular,cellular,neuronal loop function and brain structure.As a new type of post transcriptional regulatory molecules and regulatory factors,miRNAs are a large number of regulatory factors in the central nervous system.Studies have shown that miRNAs play an important role in the regulation of METH addiction.This paper is to review and summarize the expression features and the regulatory role of miRNAs in METH addiction,as well as the different expression of miRNAs in different tissues and organs.The aim of this paper is to provide a reference for further study on the role of miRNAs molecules in METH addiction and the discovery of new drug targets.
4.Study on distribution of drug resistance gene and integron and analysis of genetic relationship of 20 isolates of Proteus mirabilis

Objective To investigate the prevalence and resistance mechanisms of Proteus mirabilis in the ward of neurology de‐partment of our hospital .Methods For a total of 20 clinic isolates of Proteus mirabilis ,PCR were used for the detection of AmpC , ESBLs ,KPC and MBLs and then DNA sequencing was performed .The integrons were also detected by using PCR and then sequen‐cing was carried out .The genetic relationship between isolates were detected and analysed by pulsed‐field gel electrophoresis(PF‐GE) .The results of drug sensitivity tests were analysed .Results TEM‐1 and CTX‐M‐14 gene were found in all the 20 isolates ,the 10 isolates of Proteus mirabilis were also found carrying CMY‐2 gene .Class Ⅰ integrons were amplified from 19 strains carrying gene cassettes aacA4+cmlA1,dfrA12+orfF+aadA2and dfrA32+ereA+aadA2 respectively .PFGE analysis revealed that the 20 isolates were grouped into 11 PFGE types P1-P11 ,the 12 isolates of P1-P3 were same clones .The sensitive rates of the i‐solates to Meropenem ,Amikacin ,Aztreonam ,Ceftazidime and Tazocin were high .Conclusion Nosocomial transmission of the same clone of Proteus mirabilis was appeared in the ward of neurology department of our hospital .The predominance drug‐resistance genes were CTX‐M‐14 andCMY‐2 .The incidence of carrying class Ⅰ integrons was high ,and the major gene cassettes wereaacA4+cmlA1and dfrA12+orfF+aadA2.The 20 isolates were all sensitive to Meropenem ,Amikacin and Aztreonam .Other Clinical departments should also pay attention to the nosocomial infection caused by Proteus mirabilis and strengthen the infection control measures .
5.Study on the gene polymorphism of Auberger antigens in Chinese population.

OBJECTIVETo study the gene polymorphism of the Auberger antigens in Lutheran blood group system in Chinese population and establish a stable, accurate molecular method detecting Auberger antigens.
METHODSPeripheral blood samples from 162 randomly collected and unrelated volunteer blood donors were directly sequenced for the exon 12 at the gene locus of Auberger antigens. PCR products with novel nucleotide were further investigated by restriction fragment length polymorphism (RFLP) analysis.
RESULTSAuberger genotypes in the 162 Chinese individuals were obtained: Au(a+ b- )(nt1615A) was found in 119 individuals, Au(a+ b+ ) (nt1615A/G) in 40 individuals and Au(a- b+ ) (nt1615G) in 3 individuals. The allele frequencies of the Au(a) and Au(b) were 0.8580 and 0.1420, respectively. An individual with homozygous Au(a) genotype had a nucleotide mutation (1595 G to T). The mutation was confirmed by digesting the DNA with Hha I.
CONCLUSIONThe distribution of gene polymorphism of Auberger antigens in a Chinese population was investigated and obtained. And a molecular method determining the Auberger antigen was established. A novel Lutheran allele was deposited in GenBank (accession number EU260043).

6.A single person operation procedure and experience of tail vein injection in conscious tree shrews

Objective To introduce a single person operation procedure for tail vein injection in conscious tree shrews,and to improve the success rate of injection. Methods The tree shrew was fixed by a canvas glove and a clamp. The tail of the tree shrew was fixed by the operator's left hand, and the drug was injected by the right hand with a 1 mL disposable syringe. Results This method had the advantages of simple operation,economy and practicality,good fixation effect,high matching degree of animals and high success rate of drug administration. Conclusions Compared with other methods,this method has obvious advantages such as single person operation, convenient, time-saving and labor-saving. The cost is low and the effect is good,thus significantly improving the success rate of injection.
7.Parallel CAR⁃T cells targeting HER2 and CLDN18. 2 reduce the off target effect on gastric cancer

Objective :
To investigate whether dual⁃receptor chimeric antigen receptor T ( CART) cells targeting claudin⁃18 isoform 2(CLDN18. 2) and human epidermal growth factor receptor 2(HER2)can reduce the off⁃target effect and inhibit the growth of gastric cancer cells.
Methods :
Western blot and flow cytometry were used to detect the expression of CLDN18. 2 and HER2 in tumor cells. Tumor cell lines overexpressing CLDN18. 2 and HER2 were constructed by lentivirus infection. CART cells were constructed by lentivirus infection. Flow cytometry was used to detect the positive rate and subtype distribution of CART cells. Lactic dehydrogenase (LDH) assay was used to detect the cytotoxicity of CART cells against target tumor cells. The tumor suppressive efficiency of CART cells in vivo was measured using nude mouse xenograft model. Immunohistochemistry was used to detect the infiltration of CD3 + T cells in tumor tissues.
Results :
Gastric cancer cell lines with high or no expression of CLDN18. 2 and HER2 were successfully screened, and gastric cancer cell lines with single or simultaneous expression of CLDN18. 2 and HER2 were constructed. CART cells Cz, Hbb and CHbbz were successfully constructed. Both Cz and CHbbz had specific dose⁃dependent cytotoxicity against CLDN18. 2 + and CLDN18. 2 + HER2 + tumor cell lines in vitro ( P < 0. 001) . In the nude mouse transplanted tumor model, only CHbbz could produce significant tumor suppressor activity and T cell tumor infiltration against CH⁃AGS tumor cells (P < 0. 001), while only CHbbz could produce significant tumor suppressor activity and T cell tumor infiltration against CLDN18. 2 + HER2 + tumor cells ( P < 0. 001) .
Conclusion
Dual receptor CART cells targeting CLDN18. 2 and HER2 can reduce the probability of off-target toxicity and inhibit the growth of gastric cancer, which is a potential therapeutic strategy for gastric cancer.
