1.Effect of PSG parameters of surgical and conservative treatment of obstructive sleep apnea hypopnea syn-drome

Objective To explore the surgery and conservative treatment for obstructive sleep apnea hypop -nea syndrome of PSG parameters .Methods According to the treatment method , 60 cases of obstructive sleep apnea syndrome patients for the study were divided into observation group and control group ,the control group were used for non-surgical treatment ,the research group were treated by surgery .PSG monitoring equipment were used to detect the AHI,AHTI,AHT,SAT90%,minimum and average SaO2 so SaO2.Results After treatment,patients were significant-ly lower SAT90%were (38.4 ±13.4)%,(17.5 ±12.5)%,the lowest Sa(O2) were(79.6 ±12.4)%,(83.7 ± 11.2)%,significantly higher average SaO 2 were (85.4 ±16.3)%,(90.8 ±12.4)%,increased significantly com-pared with before treatment difference was statistically significant (t=10.13,22.42,4.76,9.76,3.65,6.65,all P<0.05).And the control group improved more significantly (t=17.32,3.76,3.76,all P<0.05).After treatment,pa-tients were AHI,AHT and AHTI were (22.4 ±3.4)times/h,(14.5 ±2.5)times/h,(9.6 ±2.4)min,(5.7 ±1.2) min,(1.4 ±0.3)min,(0.8 ±0.4)min,significantly reduced compared with before treatment difference was statisti-cally significant(t=9.63,12.12,7.51,19.27,21.42,10.63,all P<0.05).And the control group decreased more significantly,the difference was statistically significant (t=8.97,9.76,6.57,all P<0.05).Conclusion Obstructive sleep apnea syndrome treated by surgery ,each of PSG parameters improved significantly superior to conservative treat-ment,which should be used as their preferred method of treatment .
2.Study on correlation between the gene ploymorphisms of CYP2C19 and the curative effects of clopidogrel in patients with acute cerebral infarction

Objective To observe the correlation between the gene ploymorphisms of CYP2C19 and the curative effects of clopidogrel in patients with acute cerebral infarction.Methods The gene polymorphisms of CYP2C19 were tested in 171 acute cerebral infarction patients by PCR-restriction fragment length polymorphism method. All participants were treated with clopidogrel for 2 weeks.The platelet aggregation rates before treatment and 1 d, 5 d after treatment were detected.NIHSS and activities of daily living ( ADL) were detected before treatment and 1 week, 2 weeks after treatment.The inhibition rate of platelet aggregation ( IPA) was calculated and analyzed.Results In the 171 patients, homozygous wild CYP2C19 genotype was found in 79 cases (A group), with CYP2C19 *1/*1;heterozygous wild and mutant CYP2C19 genotype was found in 71 cases ( B group ) , including 55 cases with CYP2C19 *1/*2 and 16 cases with CYP2C19 *1/*3; homozygous or heterozygous mutant CYP2C19 genotype was found in 21 cases (C group), including 11 cases with CYP2C19*2/*2, 9 cases with CYP2C19*3/*3 and 1 cases with CYP2C19 *2/*3.The differences of IPA among the 3 groups were statistically significant at 1 d, 5 d after treatment (all P<0.05).Compared with before treatment, the NIHSS scores of the 3 groups at 1 week, 2 weeks after treatment were significantly decreased, and the ADL scores were significantly increased ( all P<0.05 ) . Compared with the A group, the NIHSS scores of B group and C group at 1 week, 2 weeks after treatment were significantly increased, and the ADL scores were significantly decreased (all P<0.05).Multiple Logistic regression analysis showed that smoking (OR=1.584,95%CI:1.079-2.136,P=0.004) and CYP2C19 gene polymorphisms (OR=1.837,95%CI:1.106 -2.540,P=0.002) were independently correlated with IPA in patients with acute cerebral infarction.Conclusion CYP2C19 gene ploymorphisms have influence on the curative effects of clopidogrel in patients with acute cerebral infarction.

4.Severe lower limb injury

Objective To discuss the issue of the limb salvage versus the amputation of serious lower limb injury. Methods The history, the age, the clinical presentation, the treatment time and method of the 72 severe open fractures of the lower limb were studied. Following the patients admission, they were examined completely in emergency room and were sent to radiological department for taking X ray films. According to Gustilo and Andserson classification, there were 48 limbs of type ⅢB and 24 limbs of type ⅢC.16 limbs had multiple trauma as well as severe lower extremity injury. Among them, there were 7 (9.7%) limbs with head or spine injury, 5(6.9%) limbs had abdominal trauma, 4(5.6%)limbs had thoracic trauma. Results 16 limbs in 72 limbs were amputated, in which 7 limbs were type ⅢB and 9 limbs were ⅢC. 4 limbs in 8 limbs of more than 50 years old were amputated. The ratio of amputation was 50.0 %. 12 limbs in 64 limbs younger than 50 years old were amputated. The ratio of amputation was 18.8%.Their comparison has statistically significance(?2=4.018, P
5.Risk Factors of Lower Respiratory Tract Infection in Neurology Department

OBJECTIVE To find out some risk factors of lower respiratory tract infection in neurology(department) and provide controlling measures.METHODS Totally 883 inpatient cases of neurology(department) in 2005 were retrospectively surveyed and statistically analyzed.RESULTS From them 98 cases were with nosocomial(infection).The infection rate was 11.1%.Among the 98 cases,54 cases were with respiratory tract infection(55%).Lower respiratory tract is the popular part of nosocomial infection in(neurology) department.(CONCLUSIONS) The(treatment) and recovery of the patients are heavily influenced by the infection of lower(respiratory) tract.It is(necessary) to control and prevent main risk factors in order to avoid nosocomial infection.
6.Effect of sodium-glucose transporter 2 inhibitor combined with intensive insulin therapy on blood glucose fluctuation in patients with brittle diabetes

Objective:To analyze the effect and safety of blood glucose fluctuation in brittle diabetes treated with intensive insulin therapy combined with sodium-glucose transporter 2 inhibitor.Methods:Ninety-eight patients with brittle diabetes in Shenzhen Longhua District People's Hospital were selected from May 2017 to May 2019. They were divided into two groups by random number table, 49 cases in each group. The control group was treated with intensive insulin therapy combined with saglitine, while the observation group was treated with intensive insulin therapy combined with sodium-glucose transporter 2 inhibitor. Following up for 12 weeks, the plasma glucose and blood glycemic variations, insulin dosage, plasma 8-iso-prostaglandin F2α, hypersensitive C-reactive protein and safety related indicators were compared between the two groups before and after treatment.Results:Compared with the control group, the standard deviation of blood glucose [(1.8±0.5)mmol/L vs (6.5±1.4)mmol/L, t=7.235], large amplitude glycemic excursions [(6.5±1.1)mmol/L vs (17.3±4.7)mmol/L, t=13.446], postprandial glucose excursion [(1.2±0.4)mmol/L vs (9.2±2.0)mmol/L, t=8.921], inter-quartile range [(3.7±1.1)mmol/L vs (12.4±4.2)mmol/L, t=7.003], means of daily difference [(1.5±0.4)mmol/L vs (4.6±0.8)mmol/L, t=4.537] in the observation group were significantly decreased ( P<0.05), and the levels of plasma 8-iso-prostaglandin F2α [(7.8±1.2)ng/L vs (13.6±2.3)ng/L, t=4.882], hypersensitive C-reactive protein [(5.2±1.3)mg/L vs (8.7±1.3)mg/L, t=4.406], insulin dosage [(30.9±10.2)U/d vs (42.3±13.4)U/d, t=5.726] and body mass index [(18.3±1.2)kg/m 2 vs (21.0±2.3)kg/m 2, t=4.135] also decreased significantly ( P<0.05), and the incidence of hypoglycemic events [16.3%(8/49) vs 36.7%(18/49), χ 2=9.697] and severe hypoglycemia [0 vs 14.3%(7/49), χ 2=7.268] decreased significantly ( P<0.05). Conclusions:Combination of dapagliflozin and intensive insulin therapy can significantly improve glucose metabolism in brittle diabetes, and reduce insulin dosage and hypoglycemic events.
7.Limited significance of frozen section analysis in nephron sparing surgery

One hundred sixty-three cases with a mean age of 58 years (range 21 to 87) underwent nephron sparing surgery due to suspected renal tumors.Mean tumor size was 2.8 cm (range 1.0 to 4.0 cm).Frozen section analysis was routinely performed during surgery,positive tumor margins in frozen section analysis were found in 5 cases (3.1%),and immediate radical nephrectomy was followed.Among 158 cases,in which frozen section analysis showed negative margins,paraffin sections disclosed positive tumor margins in 6 cases (3.8%).Our results suggest that frozen section analysis during nephron sparing surgery has limited clinical significance and hence routine incorporation in urological practice should be reconsidered.
8.A model for evaluating risk grade of cardiovascular health in individuals undergoing health checkup

Objective To establish a model for evaluating risk grade of cardiovascular health in adults undergoing health checkups.Methods Medical record of individuals who took health checkup in 6 heahhcare centers from 2008 to 2010 was retrospectively reviewed.The study populations were then assigned to 3 classes by using Cluster analysis and ANOVA.An assessment model for cardiovascular health was then established by using discriminant analysis method.Results The evaluation model of cardiovascular health was built by using cluster analysis and diseriminant analysis.The model was good and the consistent homology rate was 97.5%.The model was well used for cardiovascular health evaluation in healthcare center (the Kappa value in 2009 and 2010 was 0.446 and 0.415,respectively) and the consistency of the model was good.Conclusions This evaluation model of cardiovascular health could be used for grading and assessing of the level of health management of cardiovascular health.
9.Validity and reliability of Chinese version of alcohol withdrawal scale(AWS)

Objective To evaluate the validity and reliability of the Chinese version of Alcohol Withdrawal Scale (AWS). Methods Totally 175 patients diagnosed as alcohol dependence according to the criteria of ICD-10 were studied. Intraclass correlation coefficient (ICC) analysis was applied for examining interrater consistency and Cronbach' s α for internal consistency. Factor analysis was used to examine the construct validity. Correlation analysis between AWS and CGI,Revised Clinic Institute Alcohol Withdrawal Syndrome Scale(CIWA-Ar) were conducted to evaluate the criterion validity. Based on clinical criteria,ROC curve was calculated so as to test the discriminative ability and establish the cut-off point of the scale. Results ( 1 ) Reliability: ICC value was 0.93,and Cronbach's α was 0.83,which indicated good interrater and internal consistency. (2) Validity:the correlation coefficients of the two subscale with the total scale score were 0.78,0.83 respectively. The correlation coefficients between the subscale were 0. 81 and factor analysis revealed that each item of the scale had relatively high load on the primary factor (0.409 ~0.926). At the time of admission,the total score of the AWS was positively correlated with that of CGI ( r = 0.71, P < 0.05 ). The total score of the AWS also was positively correlated with that of CIWA-Ar ( r = 0. 86, P<0. 05). As treatment went on,total score of the AWS showed a downward trend,at the end of the first week,the total score of the AWS was positively correlated with that of CGI ( r = 0.62, P<0.05). (3)The cut-off point of AWS for mild alcohol withdrawal state was determined as ≥3. With this cut-off point,AWS had both high sensitivity (92.1% ) and specificity (73.5% ) ,and the area under curve (AUC) was 0. 91. The cut-off point of AWS for moderate withdrawal state was determined as ≥7, and the sensitivity and specificity of AWS were 94.3 % and 89.7 % respectively, with the AUC of 0.94. The cut-off point of AWS for severe withdrawal state was determined as ≥ 10. With this cut-off point AWS had both high sensitivity (94. 9% ) and high specificity (92.6% ) .with the AUC of 0.93. Conclusion AWS has good reliability and validity and can reflect the change of the disease and the efficacy of treatment.
10.Research on risk factors for early hypoglycemia and on its relationship with prognosis of patients with cerebral infarction

Objective To investigate the risk factors for early hypoglycemia and its relationship with prognosis of patients with cerebral infarction.Methods A total of 273 patients with cerebral infarction were divided into the normal blood glucose(NBG) and severe hypoglycemia (SHG)and mild hypoglycemia(MHG) groups in our hospital.Biochemical indicators,the National Institute of Health stroke scale(NIHSS)and mortality were compared between the three groups.According to prognosis,patients were divided into death group and survival group.The NIHSS score,blood glucose concentration and incidence of hypoglycemia were compared between death and survival groups.Pearson relationship between hypoglycemia and NIHSS score,and spearman rank correlation between hypoglycemia severity and mortality were analyzed.Results Levels of lactic acid (6.3 ± 2.8) mmol/L,creatinine(268.7 ± 63.9) mmol/L,urea nitrogen (13.8 ± 3.7) mmol/L,albumin (25.6 ±4.9) g/L,alanine aminotransferase (150 ± 19.7) U/L,NIHSS (22.3 ± 9.2) scores,and mortality rates (38.1 %)were higher in severe hypoglycemia group than in both NBG group and severe hypoglycemia group[(lactic acid:4.7±2.3 mmol/L and 3.3±1.5 mmol/L),(creatinine 134.8±51.3 mmol/L and 78.7±40.8 mmol/L),(urea nitrogen 7.9±4.2 mmol/L and 7.7±3.3 mmol/L),(albumin 36.9±3.8 g/L and 35.6±4.3 g/L),(alanine aminotransferase 85.8± 18.3U/L and 46.3± 13.8U/L),(NHISS 14.6±5.9 scores and 10.5 ± 5.4 scores)and(mortality rates 20.8%,11.0%)] (all P<0.01).There was a negative correlation between hypoglycemia and NIHSS score(r=-0.45,P<<0.05).There was a positive correlation between hypoglycemic severity and mortality (r =0.41,P < 0.05).Multiple Logistic regression showed that creatinine and alanine aminotransferase were correlated with hypoglycemia and prognosis of patients with cerebral infarction(both P<0.05).Conclusions Early hypoglycemia in patients with severe cerebral infarction is closely correlated with the liver and kidney insufficiency,and a severe cerebral infarction combined with hypoglycemia often indicate a poor prognosis.
