Journal of Clinical Medicine in Practice 2017;21(20):21-24
Objective To explore the application of evidence-based nursing in the prevention of hypothyroidism with obstructive sleep apnea hypopnea syndrome (OASHS).Methods A total of 100 patients with OASHS in our hospital were randomly divided into observation group and control group. The control group was given routine care, the observation group given evidence-based care, body mass index, anxiety, clinical symptoms and sleep suspension were compared. Results After the treatment, the BMI and SAS scores of the two groups were lower than the control group, but the observation group decreased significantly, the difference was statistically significant (P < 0.05);The incidences of hangover, dizziness, memory loss and depression in the observation group were significantly lower than that in the control group (P < 0.05);The mean blood oxygen and the lowest blood oxygen in the control group were significantly lower than that in the observation group, and the longest apnea time was significantly higher than that of the observation group (P < 0.05).Conclusion Effect of evidence-based care for OASHS patients with hypothyroidism is better, and it can significantly reduce anxiety, improve clinical symptoms, reduce the incidence of sleep pause, promote patient rehabilitation process,so it is worthy of promotion.

Objective:To synthesize the influencing factors of oral nursing practice among ICU nurses, so as to provide reference for clinical nursing practice. Methods:The quantitative, qualitative, and mixed methods research on the influencing factors of ICU nurses' oral nursing practice was systematically retrieved in PubMed, Web of Science, Embase, CINAHL, Cochrane Library, China National Knowledge Infrastructure, WanFang Data, VIP, China Biomedical Database and so on. Data based convergent synthesis design was used for comprehensive analysis of included literature. Results:A total of 20 articles were included. A total of 34 influencing factors were extracted from the oral nursing practice of ICU nurses, including 16 promoting factors and 18 obstacle factors, which were summarized as knowledge, skills, beliefs about consequences, goals, environmental background and resources, and social influence areas. Conclusions:The influencing factors of ICU nurses' oral nursing practice are diverse and complex. Future research can develop specific intervention plans based on the promotion and obstacle factors of ICU nurses' oral nursing practices, so as to promote the development of ICU basic nursing practices.

The rapidly aging society gives rise to sizable demands for chronic disease nursing care. With IT development as backup, the hospital has developed a platform for regional collaborative nursing informationization within a medical alliance and a health management platform for home care of the elderlies. This practice can build an extended nursing care model, featuring the three main service lines of " hospital-family, hospital-medical alliance and hospital-community hospital-family". The model provides diversified, personalized, disease specific, informationization and multi-disciplinary coverage for nursing care, providing the primary level with high quality nursing resources.Medical alliances and corresponding communities share information, for a full-range, full region and full timeframe coverage of nursing health supervision and scientific guidance, thus effectively minimizing the rate of revisit to upper-level hospitals by patients and implementing the hierarchical medical system.

Objective:To investigate the status of caring burden, psychological resilience and psychological distress among spouses of dyskinesia stroke survivors and to explore the path relationship among three aspects. Methods:A total of 200 spouses of dyskinesia patients for 4 hospitals were investigated with Zarit Caregiver Burden Interview, Connor-Davidson Resilience Scale and Distress Thermometer from Jan 2017 to Dec 2017.Results:The overall caring burden was middle level, and 26.2%(49/187) was high, the score of Connor-Davidson Resilience Scale was 57.31±12.37, 70.6%(132/187) of all spouses with psychological distress. There was a significant difference in three aspects with different education level ( P<0.01). Whether there was a minor only child has an influence on the psychological resilience of the spouses( P<0.05). The score of Zarit Caregiver Burden Interview and Connor-Davidson Resilience Scale was negatively correlated( r value was -0.832, P<0.01). The score of Connor-Davidson Resilience Scale and Distress Thermometer was was positively correlated( r value was 0.867, P<0.01). The score of Zarit Caregiver Burden Interview and Distress Thermometer was negatively correlated( r value was -0.975, P<0.01). Furthermore, psychological resilience effected as a mediator in caring burden and psychological distress. Conclusions:The detection rate of care burden and psychological distress in the stroke patients with limb movement disorder are high, and the psychological resilience plays a mediating factor between them. The nursing staff should raise the awareness of the patient′s psychological nursing care while paying attention to the patients themselves. It can improve the physical and mental health and care ability of the spouse, improve the quality of life of the family.

Objective To explore the effects of positive psychological intervention based on PERMA model of wellbeing on stigma, adaptation to ostomy and quality of life in patients with permanent enterostomy. Methods A total of 83 patients with permanent enterostomy were recruited from October 2022 to September 2023 in a tertiary hospital in Liaoning Province. They were randomly divided into an experimental group and a control group. While the control group received routine nursing, the experimental group received nursing intervention based on PERMA.The intervention lasted 8 weeks, and data were collected before and after the intervention. The scores of the Social Impact Scale(SIS),Ostomy Adjustment Inventory-20(OAI-20)and Stoma Quality of Life(Stoma-QOL)in the 2 groups were compared. Results Experimental group and control group were 40 cases each completed the study. After the intervention, the SIS score of the experimental group was(44.70±9.10),which was significantly lower than(62.15±12.43)of the control group, and the difference between 2 groups was statistically significant(t=-7.163,P<0.001).The OAI-20 score of the experimental group was(52.28±5.80),which was significantly higher than(41.53±9.00)of the control group, and the difference between the 2 groups was statistically significant(t=6.349,P<0.001).The Stoma-QOL score of the experimental group was(54.08±8.29),which was significantly higher than(42.95±9.19)of the control group, and the difference between the 2 groups was statistically significant(t=5.688,P<0.001).Conclusion Nursing interventions based on PERMA are beneficial in reducing stigma of patients with permanent enterostomy, improving their ostomy adjustment and postoperative quality of life.
