1.Effects of large dose gamma globulin on cardiac function and immune regulation in patients with dilated cardiomyopathy and heart failure

Objective: To explore effects of large dose gamma globulin on cardiac function and immune regulation in patients with dilated cardiomyopathy (DCM) and heart failure (HF).Methods: According to random number table, a total of 90 DCM + HF patients were divided into routine treatment group (n=45) and gamma globulin group (n=45, received gamma globulin based on routine treatment, 400 mg kg-1 d-1 intravenous drip for 5d).After three months, therapeutic effect, cardiac function, levels of cardiac troponin T (cTnT), cTnI, N terminal pro brain natriuretic peptide (NT-proBNP) and anti-cardiac myosin heavy chain auto-antibodies (AMHCA) IgG titer before and after treatment, incidence of adverse reactions were observed and compared between two groups.Results: After three-month treatment, total effective rate of gamma globulin group was significantly higher than that of routine treatment group (80.00% vs.57.50%, P=0.02);compared with routine treatment group after treatment, there were significant reductions in levels of cTnT [(0.54±0.18) μg/L vs.(0.31±0.12) μg/L], cTnI [(1.35±0.18) μg/L vs.(0.72±0.13) μg/L], NT-proBNP [(758.34±64.28) pg/ml vs.(708.34±60.22) pg/ml], and significant rise in left ventricular ejection fraction [LVEF, (39.35±6.23)% vs.(47.32±6.10)%] in gamma globulin group, P<0.05 all;compared with before treatment and routine treatment group after treatment, there was significant reduction in AMHCA IgG titer [(0.099±0.047), (0.092±0.045) vs.(0.069±0.032), P<0.05 both] in gamma globulin group;incidence rate of adverse reaction in gamma globulin group was significantly higher than that of routine treatment group (22.22% vs.6.67%, P=0.04), but the symptoms were mild, and they recovered after symptomatic treatment.Conclusion: Large dose gamma globulin can significantly improve cardiac function and possesses good immune regulation effect in patients with dilated cardiomyopathy and heart failure.
2.Reversal effects of carvedilol on multidrug resistance in bladder cell lines

Purpose:To study the reversal effects on multidrug resistance in multidrug-resistent bladder cell lines BIU-87/ADM. Methods:The degree of drug-resistence was detected by MTTmethod; the concentration of ADM in cells BIU-87 and BIU-87/ADM was detected after the cells were treated by carvedilol and ADM. Results:Pretreatment by carvedilol increased the concentration of ADM in BIU-87/ADM cells, ,and the concentration was much higher than that in those cells treated by ADM only. Conclusions:Carvedilol can enhance the cytotoxic effect of ADM,and can reverse the multidrug resistance in bladder cancer lines BIU-87/ADM.
3.Diagnosis of Papillary Carcinoma of Ampulla by Contrast-Enhanced Spiral CT after Low Tension and Drinking Water

Objective To explore the diagnostic value of contrast-enhanced spiral CT after low tension and drinking water in papillary carcinoma of ampulla. Methods CT manifestations of papillary carcinoma of ampulla comfirmed pathologically in 20 cases were analysed retrospectively emphasized in artery phase on contrast CT which was performed after injecting 654-2 20 mg and drinking 300~500 ml water.Results Of all the cases , only 2 cases showed tumor filling defect in the descending part of duedenum nearby pancreas on routine CT, but all the cases showed more or less tumor filling defect on contrast-enhanced spiral CT . The diameters of the tumor were between 0.8 to 2.6 cm,2 cases with head of pancreas affected,the diameters of the tumor were between 2.4 to 2.6 cm.There were 2 cases with lymph node metastasis nearby duedenum.All the cases showed expansion of gallbladder ,intrahepatic duct and choledochus expanding in the liver. Conclusion Contrast-enhanced spiral CT after low tension and drinking water is superior to routine CT in determining the size and morphosis of tumor.
4.Expression of TopoⅡ gene in human bladder cancer multidrug-resistant cells

Purpose:To study the expression of TopoⅡ gene in multidrug resistant cells of human bladder cancer. Methods:The degree of drug resistance was detected by MTT method ;the expression of TopoⅡ gene in cell lines BIU 87/ADM and BIU 87 was detected with reverse transcriptase polymerase chain reaction. Results:The cell lines BIU 87/ADM were 56.4 times more resistant to ADM than the cell lines BIU 87;the expression of TopoⅡ gene was poorly positive in BIU 87/ADM but strongly positive in BIU 87 cells. Conclusions: The decreased expression of TopoⅡ gene in BIU 87/ADM cells might contribute to the development of multidrug resistance of human bladder cancer.
5.Antigenic epitopes analyses and expression of P-glycoprotein

Purpose:To study antigenic epitopes and expression of P glycoprotein.Methods:The secondary structure and surface properties of P glycoprotein was studied by many methods , designed a pair of primers to amplify DNA with PCR. The product was inserted into a pGEM T vector, sequenced and cloned into pGEX 2T vector, established a high expression recombinant vector named pGEX Pgp,which was transferred into DH5? and expressed , identified and purified. Results:Antigenic epitopes in extracellular fragment of P glycoprotein were identified in amino acid residues 82 115(I),741 760(IV) and 800 816(V). SDS PAGE analysis indicated that the expressed protein was about 30?10 3 (30 kD).Conclusions:High level expression of the target gene fragment was established for preparation of its antibody and biospanning its specific peptide using random phage library , which became the basis for the targeting treatment of MDR. With the modernization of traditional Chinese medicine (TCM), classic extraction methods such as sol-vent extraction, steam distillation, squeezing method, sublimation method have become much more difficult in sat-isfying needs of the development of the society as a result of low extraction rate of effective components, low per-centage impurity clearance. Therefore, in recent years the new extraction techniques with the advantages of green environmental protection , conditional stability , high extraction rate , strong biological activity , have been widely used in TCM extraction. These new extraction techniques have already shown each of its strength. The application progress on new extraction technology of Chinese medicine polysaccharide, both in standalone application and in collaboration application, were discussed in this article, so as to make new technologies be applied more reason-ably and effectively in Chinese medicine polysaccharide components.
7.Prediction of the B cell epitope for MUC1 antigen

Objective To predict the B cell epitope for MUC1 antigen. Methods The secondary structure and surface properties of MUC1, such as physical and chemical characters, hydrophilicity, antigenicity, were analyzed with various methods. Results Many distinct antigenic epitopes in MUC1 were identified by computation: 1-10, 24-54, 65-77, 84-91, 108-134, 140-156, 159-174, 179-196, 199-210, 220-233, 237-265, 270-299, 316-337, 351-362, 369-396, 411-420, and 445-502. Conclusion Prediction of the B cell epitope for MUC1 can provide a base for the studies of structure and function of MUC1, construction of its mutants, and selection of new expression forms of MUC1.
8.Expression of fused P-glycoprotein in E. coli DH5?

Objective To study the expression of the extracellular fragment of P glycoprotein in the bacteria. Methods According to the P glycoprotein antigen analyses and mdr1 gene structure, a couple of primers were designed to amplify the extracellular DNA fragment of the protein with PCR. The product was inserted into pGEM T vector followed by sequencing. The sequencing verified vectors were cloned into pGEX 2T to get a highly expression recombinant vector pGEX Pgp. The recombinant was transferred into E. coli DH5?, and its expression, identification and purification were studied. Results The highest level of recombinant expression was achieved at 4 h after IPTG induction. SDS PAGE analysis indicated that the expressed protein was about 30?10 3. Conclusion The target gene fragment expression at high level is established for the preparation of its antibody and biospanning of its specific peptide by using random phage library, founding a basis for the targeting treatment of multidrug resistance.

To analyze the mortality risk of stroke in China by literature search.The mortality rate of stroke (age standardized) in China has declined in recent years.The mortality rate of stroke which increases with age is higher in rural areas than in urban areas,and is higher in men than in women in China.There are obvious regional differences and seasonal differences in the mortality of stroke in China.Different subtypes of stroke have different death characteristics.Low or high temperatures can increase the risk of mortality of stroke.With the change of epidemiological characteristics,the risk factors of mortality of stroke in China are also changing.Through the epidemiological study of stroke death,the mortality risk of stroke can be researched effectively,the epidemiological characteristics of stroke can be mastered,and the direction of prevention and treatment of stroke can be understood.Individualized and multi factor intervention for unhealthy lifestyle,unhealthy behavior and metabolic cardiovascular risk factors can promote the establishment of stroke prevention and control strategy in China.
10.Cost effectiveness evaluation of four kinds of surgical methods for hepatic echinococcosis

We analyzed the cost effectiveness of four different surgical methods (A,B,C and D) in treatment of hepatic echinococcosis.Totally 757 cases of hepatic echinococcosis surgery clinical data and cost information of hospitalized cases were collected from nine hospitals in Xinjiang during 2005-2013.The clinical effects,cost effectiveness ratio and incremental costeffectiveness ratio were analyzed and compared.The total complication rates were 15.8%,9.2%,0% and 2.9%;the recurrence rates were 7.3%,6.2%,0% and 0%;the cure rates were 77.8%,84.6%,100.0% and 97.1%;the cost (RMB) were 11 947.3,18 543.6,25 510.7 and 18 877.4,cost-effectiveness ratio were 153.6,219.1,255.1 and 194.3,incremental cost-effectiveness ratio /C//E were 964.4,610.4 and 358.0 respectively for group A,B,C and D.The results after adjusting of price factor were consistent with original cost effectiveness analysis.The complete resection of inner and outer capsule (D) is the most effective and economical way of clinical operation and may worthy be promoted in treatment of hepatic echinococcosis.
