1.EFFECTS OF NIDUS VESPAE PROTEIN ON PROLIFERATION OF HUMAN LEUKEMIA K562 CELLS

Objective To study the mechanism of protein extracted from nidus vespae protein(NVP) on anti-leukemia and provide experimental and theoretical evidence for exploring new anti-leukemia drugs. Methods The effect of the NVP on cell growth and morphological changes of K562 cells was observed by using cell culture and transmission electronic microscopy,the effect of NVP on apoptosis-associated signal transduct protein NF-? Bp65,?-catenin and iNOS of K562 cells was detected via immunohistochemical method and image analysis system. Results(1) The saturation destinity of K562 cells in NVP group was decreased which suggested that the proliferation was inhibited.(2) In the NVP group typical apoptosis morphological changes were observed in the K562 cells under transmission electronic microscope.(3) The result of immunohistochemistry showed that the expression of NF-? Bp65,?-catenin and iNOS of K562 cells in the NVP group decreased.The optical density(A) values of NF-?Bp65,?-catenin analysed by the image analysis system were decreased in the NVP group.Conclusion The NVP can inhibit the proliferation of K562 cell prominently,and the mechanism probably is that the apoptosis is induced via regulating the expression of apoptosis-associated signal transducted factors NF-?Bp65,?-catenin and iNOS.
2.The effect of oral chemotherapy in patients of adenocarcinoma of the head of the pancreas after radical resection

Objective To study the effect of oral chemotherapy on patients of pancreatic carcinoma undergoing Whipple′s procedure. Methods48 cases undergoing radical pancreaticoduodenectomy with regional lymphadenectomy were divided into two groups. 24 cases received oral chemotherapy consisted of 5′ DFUR and calcium folinate for a course of 6 months. ResultsThe clinicopathological features were similar between the two groups. During the period of follow up,5 cases in chemotherapy group died including motality caused by liver metastasis in 2 cases whereas 7 cases died of liver metastasis in the control group( P
3.Current situation and requirements of quality control of blood disease laboratory in Chongqing:a cross sectional study

Objective To understand the current situation and requirements of laboratory quality control of blood diseases in Chongqing .Methods A self‐designed questionnaire was designed ,and the investigation was carried out in 60 hospitals of Chongqing district .The main contents of the investigation were related to the instruments of work environment ,the regulations ,the conditions of the personnel ,etc .Results A total of 60 questionnaires were distributed ,57 questionnaires were valid .In the study ,only 14 .04%laboratories were the blood disease specialist laboratory ,most hospitals could complete the basic inspection items .66 .67% hospitals had all the experimental procedures and SOP documents .Most of staffs were with intermediate professional titles and undergraduate education .A total of 35 .08% experimental equipments with the value of over 5 million RMB ,45 .61% equipments had a special in‐spection records .About 45 .61% test reports would refer to the clinical performance ,and had a strict signing guideline .Most of the research objects reflected the problems ,including the backward equipments and the lack of personnel ,and hoped to set up telemedi‐cine indemnity system and carry out further education .Conclusion The overall quality of the laboratory for blood diseases in Chongqing is better ,it is recommended to strengthen the full quality control ,to establish emergency system ,and to improve the quality and level of laboratory .
4.Multiple types of hematopoietic growth factors secreted spontaneously by human umbilical cord blood stromal cells in vitro

Objective To investigate the potential of stromal cells from human umbilical cord blood cultured in vitro to secrete hematopoietic growth factors. Methods The expressions of hematopoietic growth factors of stromal cells from human umbilical cord blood and bone marrow were detected by ELISA kits at different time points. Results Stromal cells from human umbilical cord blood could secrete thrombopoietin (TPO), granulocyte-macrophage colony stimulating factor (GM-CSF) and stem cell factor (SCF). The peak levels of TPO and SCF were found at 7 d after culture in vitro, but then the levels of these factors decreased gradually. Stromal cells from human umbilical cord blood secreted more SCF and GM-SCF, but less TPO than human bone marrow stromal cells. Conclusion Our results demonstrate that stromal cells from human umbilical cord blood could sustain hematopoiesis by secreting multiple types of hematopoietic growth factors such as TPO, GM-CSF, and SCF.
5.Comparison of parameters of blood flow of grafts of right or left internal mammary artery to the left anterior descending artery

Objective:To analyze and compare difference of ultrasonic blood flow of left internal mammary artery(LIMA) and right internal mammary artery(RIMA)as graft to left anterior descending artery(LAD) in patients undergoing off-pump coronary artery bypass grafting(OPCABG).Methods:From October 2017 to October 2019, a total of 363 patients who underwent OPCAB including 329 patients in LIMA-LAD group and 34 patients in RIMA-LAD group, were included in this study. Transthoracic ultrasound examination on IMA was performed before OPCABG. The blood flow , the value of PI(pulsation index) and diastolic flow(DF) of LIMA and RIMA to left anterior descending were measured and recorded by intraoperative TTFM. Patients underwent coronary artery CT examinations at 1 week after OPCABG to discover the patency of grafts.Results:The pre-operative diameter of RIMA was larger and the blood flow and pulsation index of RIMA was better than that of LIMA before OPCABG( P<0.001). But there was no significant difference in the flow, pulsation index and DF value of graft between the two groups after IMA were anastomosed to LAD( P>0.05). In situ skeletonized graft of RIMA did not show the advantage of increased blood flow. Compared with the preoperative parameters of flow of internal mammary artery, both the left and right grafts of IMA were significantly better in blood flow and pulsation index( P<0.001). Considering the remarkable difference in age between the two groups, Flow, PI and DF were compared again after propensity score matching, and there was still no statistical difference between the two groups( P>0.05). A total of 313 patients in LIMA-LAD group completed coronary CTA examination one week after surgery, among which 4 grafts had different degrees of problems. A total of 34 patients in RIMA-LAD group completed coronary CTA examination, one case indicated that the CTA imaging in distal segment was not clear, and the other results had no obvious abnormality. There was no significant difference in postoperative rate of coronary CTA completion( P=0.381) and rate of graft failure( P=0.405) between the two groups. Conclusion:Compared with the preoperative parameters of blood flow of internal mammary artery, both the left and right IMA grafts are significantly better in flow and pulsation index. The pre-operative diameter of RIMA is larger and the blood flow and pulsation index of RIMA is better than that of LIMA before OPCABG. But there is no significant difference in the flow, pulsation index and DF value of graft between the two groups after IMA are anastomosed to LAD. 6.Treatment of long-segment fracture in middle-up part of femoral with long proximal femoral nail antirotation

Objective To investigate the effects of long proximal femoral nail antirotation (PFNA-long) in treating long-segment fracture in middle-up part of femoral. Methods From June 2006 to December 2013, we treated 139 cases long-segment fracture in middle-up part of femoral with long proximal femoral nail antirotation (PFNA, 320-380 mm) by minimally invasive per-cutaneous fixation and autogenous iliac bone graft. Data of 139 patients (104 male and female 35;ages 18 to 86 years, with an aver-age age of 48.8 years). Type IA 58 cases (41.73%, 58/139), type IB 25 cases (17.99%, 25/139), type IC 4 cases (2.88%, 4/139), type II 28 cases (20.14%, 28/139), type IIIA 12 cases (8.63%, 12/139), type IIIB in 5 cases (3.60%, 5/139), type IV 7 cases (5.04%, 7/139). The duration from injury to surgery was 3 hours to 12 days, with an average of 4.2 days. The postoperative follow-up was at 1, 2, 3, 6, 9, 12 months, and annually at least. The clinical efficacy was evaluated with Harris hip function score. The postoperative pain was evaluated with visual analogue scale (VAS). Results The operative time of 139 cases was from 35 to 90 minutes, averaged 45 minutes and the blood loss during surgery averaged 78.6 ml (ranged from 30 to 200 ml). All patients walked by walking aid appliance after 4 to 10 days postoperatively, apart from 16 cases with serious long grinding fracture. All patients had been followed up for 3 to 37 months (mean 19 months). No complications, such as infection, lower limb vein infarction, cut-out and breakage of the implants, occurred. However, there were 4 patients with nail broken and 6 patients with hip internal rotation who were treated by secondary operation. There were 5 cases with slight hip external rotation and 8 cases with hip internal reverse-lamb shortening malformation. All fractures healed after 2.8-6.8 months (mean 3.9 months). According to Harris criteria, the clini-cal results were best in 108 cases, better in 22 cases, good in 8 cases, and bad in 1 case. The excellent and good rate was 93.53%(130/139). The score for VAS was 0-6 (mean 0.8), 93 cases in which no pain, mild pain in 33 cases, moderate pain in 13 cases, 25 cases occasionally need to take non-steroidal analgesics. Conclusion Closed reduction or limited open reduction with PFNA-long is an effective treatment for long-segment fracture in middle-up part of femoral, with higher strength in fixation, higher rate of fracture union, early functional recovery and lower rate of complications.
7.Clinical features and surgical management of duodenal stromal tumors

Objective To investigate clinicopathological features、diagnosis and surgical treatment of duodenal stromal tumors. Methods A retrospective clinical analysis was made in 23 cases of duodenal stromal tumors admitted from 1995 to 2004. Results Melena and abdominal pain was the most common presenting symptom. Complete resection was achieved in all cases, including pancreatoduodenectomy in 13 cases, by pylorus-preserving pancreatoduodenectomy in 3 cases, and tumor enucleation in 7 cases. The mean size of the tumor was 6. 6 cm (2 - 21 cm) , among them 10 cm in 6. Pathologically tumors less than 5 cm were all of potential malignancy, whereas those larger than 5 cm were malignant. Follow-up of 2 months to 9 years found all cases alive and healthy except for one who died of liver metastasis one year postoperatively. Conclusion Duodenal stromal tumors were low malignancy tumors. Aggressive surgery is the choice of treatment. Favorable prognosis is expected after complete surgical excision.

AIM: To investigate the effect of vascular cell adhesion molecule-1(VCAM-1) modified human umbilical cord blood stromal cells(hUCBSCs) on the expansion of hematopoietic stem cells/progenitor cells in vitro.METHODS: After the VCAM-1-pcDNA3.1(+) eukaryotic expression vector was constructed successfully,the recombinant vector was transfected into hUCBSCs via liposome mediation.The expression of the VCAM-1 was detected by semiquantitative RT-PCR and immunocytochemistry.The expansion abilities of hematopoietic cell in vitro under different conditions were compared,including cytokine stimulation,hUCBSCs feeder layer and VCAM-1 modified hUCBSCs feeder layer etc.The effect on hematopoietic reconstitution of different expanded hematopoietic cell groups was compared after CFU-GM,CFU-E and CFU-Mg semi-solid culture were proceed,respectively.RESULTS: The positive clone and NeoR gene detection proved that recombinant vector had been transfected into hUCBSCs.The results of semiquantitative RT-PCR and immunocytochemistry manifested that VCAM-1 was high expressed not only at transcriptional level in the stable-transfected hUCBSCs(P
9.Human endostatin gene recombinant adenovirus for pancreatic cardnoma in nude mice

Objective To construct a human endostatin adenovirus vector and investigate its inhibitory effect on pancreatic carcinoma in nude mice.Methods Animal model of pancreatic carcinoma bearing nude mice was established by subcutaneous injection of SW1990 cells.All mice were randomized into Ad-hEnd group,Ad-LacZ group and control group with 8 mice in each group.The endostatin gene recombinant adenovirus were intratumorally injected every two days for 4 times.The rate of tumor growth was observed.lmmunohistochemical staining was employed to investigate the expression of vascular endothelial growth factor (VEGF) and micro-vessel density (MVD).TUNEL in situ was used to examine tumor cell apoptosis.Results The tumor formation rate was 100%.4 weeks later,the volumes of the tumors were (921.9±279.7 )mm3,(2804.4±553.5 )mm3 and ( 3040.6±487.6 ) mm3 in Ad-hEnd group,Ad-LacZ group and control group,respectively;the weights of the tumors were (1.19±0.18 ) g,( 2.38±0.42 ) g and ( 2.41±0.47 ) g,respectively;the VEGF positive rates were (36.3±7.1 )%,(81.2±6.6)% and (79.4±6.2)%,respectively;the levels of MVD were 12±4,27±5 and 25±6,respectively;the apoptotic rates were (31.2 ±5.4) %,( 9.4±4.9 ) % and ( 8.5±3.7 ) %,respectively.Compared with Ad-LacZ group and control group,the parameters in Ad-hEnd group were statistically different (P <0.01 ).The difference betweon Ad-LacZ group and control group was not statistically different.Conclusions Human endostatin gene mediated by recombinant adenovirus could inhibit tumor growth,angiogenesis and promote cell apoptosis of pancreatic carcinoma and could be used as geue therapy for pancreatic carcinoma.
10.Isolation of superior mesenteric artery and resection of mesentery root is beneficial for radical pancreaticoduodenectomy in the treatment of pancreatic ductal adenocarcinoma

Objective To improve the prognosis and radical resection of the extended pancreaticoduodenectomy for patients with pancreatic cancer in the ucinate process involving mesentery mot. Methods From Jan. 2004 to Dec. 2007, a total of 23 ( 14 male and 9 female, aged between 30 and 72 years old) patients with pancreatic cancer in the ucinate process involving mesentery root were treated in our department. Curative resection was performed for all patients by the extended pancreaticoduodenectomy with superior mesenteric artery (SMA) isolation and mesentery root resection. The surgical procedure, the safety and prognosis were analyzed retrospectively. Results 12 patients underwent the procedure, among them 11 also underwent combined SMV partial resection and reconstruction. The operation time was (4.2 ± 1.1 ) hours, and the blood loss was ( 1 635 ± 1 362) ml with the blood transfusion of ( 1 609 ± 1 462 ) ml. There was no operation related death in this case series, and mild to severe diarrhea occurred in 6 cases. The post-operative stay ranged 9 to 30 days. The pathological examination showed that the tumor size was (5.3 ± 1.4) cm. 13 patients (57%) had one or more lymph nodes metastasis. 20 patients (87%) had nerve involvement. Among 11 patients with SMV partial resection and reconstruction, 10 patients had endangium involvement. 22 patients had negative surgical margins for all specimens. Rapid intra-operative frozen pathological examination showed negative surgical margins in one patient, however, post-operative paraffin section pathological examination revealed nerve involvement between SMA and celiac trunk. After a follow-up of 5 to 42 months, liver metastasis occurred in 4 patients, and local recurrence occurred in 3 patients. The 1-year and 2-year accumulated survival rates were 77.2% and 42.5%, respectively. Conclusions Isolation SMA and the mesentery resection in extended pancreaticodudenectomy were safe and useful. Using this modified technique, Radical operation resection could be achieved in the treatment of pancreatic cancer in uncinate process.
