1.Effect of Pharmaceutical Care on the Treatment Outcomes of Epilepsy

OBJECTIVE: To investigate the effect of pharmaceutical care on the treatment outcome of epilepsy and to discuss the necessity for pharmacists' participation in clinical medication and the correlation between the participation of clinical pharmacists and curative effect.METHODS: The epileptic patients treated in our hospital from Jan.2001 to Jan.2007 who accepted serum drug concentration test were included in the study.The treatment outcome data were subjected to statistically analysis.RESULTS & CONCLUSION: The medication guidance provided by pharmacists is highly correlated with the treatment outcome of epilepsy.Reasonable pharmaceutical care provided by pharmacists is much needed in the treatment of epilepsy.
2.Immunophenotype diversity of leukomonocytes cultured with fludarabine in vitro

BACKGROUND:Recently,some overseas in vitro researches and animal models confirmed that donor leukomonocyte cultured by fludarabine can significantly reduce acute graft-versus-host disease,but the mechanism of action is not clear.OBJECTIVE:To explore immunophenotype changes of donor leukomonocyte cultured with fludarabine in vitro.DESIGN,TIME AND SETTING:The observational cytology experiment was performed at the Department of Hematology,First People's Hospital from July 2007 to January 2008.MATERIALS:Leukomonocytes were collected from four hour healthy persons as donors of allogenic peripheral blood hematopoietic stem cell transplantation.Fludarabine was offered by Schering AG,Germany.METHODS:Peripheral blood hematopoietic stem cells collected by COBO SPECTRA were washed with RPMI1640 twice.These cells were cultured in RPMI1640 containing 0.10 volume fraction of fetal bovine serum till cell density of 2?109 L-1.1 mL of hematopoietic stem cells were washed with phosphate-buffered saline,and added CD3,CD4,CD8,CD44mAb 10 ?L,and added Mouse Ig-G1-FITC/PE as control group.These cells were attenuated with phosphate-buffered saline after incubated for 30 minutes at a low temperature,then detected by flow cytometry.Other cells were put in 24-well cultivate plate,added 8 mmol /L fludarabine as a final concentration for 24 hours(5% CO2,37 ℃).MAIN OUTCOME MEASURES:Studying immunophenotypic changes of donor leukomonocytes by flow cytometry.RESULTS:There was no significant change in the ratio of CD4+CD44+T cells before or after donor leukomonocytes cultured with fludarabine for 24 hours,and proportion of CD8+CD44+T cells declined significantly(P
3.Effects of eicosapentaenoic acid combined with etoposide on proliferation and apoptosis of human lung cancer cell line A-549

Objective To study the effects of eicosapentaenoic acid (EPA) combined with etoposide on the proliferation and apoptosis of human lung cancer cell lines A-549 in vitro.Methods The A-549 cells were cultured,respectively,with culture fluids containing 40 μg/ml EPA,10 μg/ml etoposide,40 μg/ml EPA + 10 μg/ml etoposide,20 μg/ml etoposide,or 40 μg/ml EPA + 20 μg/ml.The effects of EPA and/or etoposide on the proliferation of the cell line were detected by MTT assay.The morphological changes of the cells were observed using annexin V-fluorescein isothiocyanate/propidium iodine and acridine orange/ethidium bromide double staining under fluorescence microscope.The apoptosis and the changes of cell cycle were detected by flow cytometry.Results The inhibition rates of EPA plus etoposide groups on the proliferation of human lung cancer cells A-549 were significantly higher than those in other etoposide groups (all P < 0.05).Cell staining showed that the amount of apoptotic cells in EPA plus etoposide groups was significantly larger than that in etoposide groups.The percentages of cells in S and G2/M phases of EPA plus etoposide groups were significantly higher than those of etoposide groups (all P < 0.05).Conclusion EPA may enhance the effect of etoposide on inhibiting proliferation and promoting apoptosis for human lung cancer cells A-549.
4.Preparation of danshensu microcapsules by spray drying and study on its in vitro release behavior

Objective To prepare sustained-release danshensu microcapsules by spray drying.Methods Encapsulation efficiency of different coating materials was compared.The spray process was optimized using a central composite design for two variables to obtain microcapsules with microcapsules yield.Furthermore, in vitro release of danshensu microcapsules was determined.Results PVA was chosen as coating material and the ratio of coating material to core material was 3:1.The inlet air temperature was 190℃,the feed flow rate was 36 r/min,encapsulation efficiency was 85.0%,and microcapsules yield was 56.1%.The particle size of more than 70%microcapsules was between 6—12?m.Conclusion The optimized conditions of spray drying are reasonable,danshensu microcapsules can be used as sustained-release preparation.

Objective To investigate the effects of eicosapentaenoic acid (EPA) or EPA plus carboplatin on the proliferation and apoptosis of human lung cancer cell line A-549.Methods A-549 cells were cultured by 100 μg/ml carboplatin,80 μg/ml EPA,and their combination for 48 hours.MTT assay was used to determine the effect of EPA,carboplatin,or their combination on the proliferation of human lung cancer cell line A-549.The morphological changes of human lung cancer cell line A-549 were observed using HE staining,and apoptosis of A-549 cells was analyzed by flow cytometry.Results MTT assay showed that the proliferation inhibition rate of A-549 cells cultured with EPA plus carboplatin was 85.20% ± 5.00%,which was significantly higher than that of cells cultured with 80 μg/ml EPA (32.85% ± 3.00%,P =0.0001 ) or 100 μg/ml carboplatin (53.25% ±3.00%,P =0.0013 ).HE staining showed apoptosis in all three groups,whereas the apoptosis rate reached 17.05% ± 4.00% in the combination group,which was significantly higher than that in carboplatin group (9.49% ± 1.00%,P =0.0252).Conclusion EPA may enhance the effects of carboplatin in inhibiting the proliferation and promoting the apoptosis of human lung cancer cell line A-549.
7.Expression of EZH2 mRNA and protein in human prostate carcinoma and its relationship with cell proliferation

Purpose To investigate the expression of EZH2 mRNA and protein, and its relationship with tumor cell proliferation in human prostate carcinoma (PCa).Methods A tissue microarray was constructed, which contained 68 dots of formalin-fixed,paraffin-embedded tissue samples, including 48 cases of human PCa.Immunohistochemical markers, including EZH2 and Ki-67,were used on the tissue microarray sections by the immunohistochemical staining method.In situ hybridization (ISH) using an EZH2 oligonucleotide probe was also performed on the tissue microarray sections.Additional 15 cases of benign prostate hyperplasia (BPH) and 12 cases of high grade prostate intraepithelial neoplasia (HGPIN) were used as controls.Results The positive rates of EZH2 protein and mRNA expression were 87.50% and 81.25% in the PCa, 16.67% and 16.67% in the HGPIN,13.33% and 6.67% in the BPH,respectively. There were statistical difference between PCa, HGPIN and BPH, respectively (P<0.05).The expression of EZH2 protein and mRNA had no statistical difference (P>0.05).The expression of EZH2 protein was related to Gleason score, TNM stage (P<0.05), but not to age and serum prostate-specific antigen (PSA) level (P>0.05). The expression of EZH2 mRNA was related to TNM stage (P<0.05),but not to age, PSA and Gleason score (P>0.05).The staining intensity of EZH2 was positively correlated with the Ki-67 indexes (r=0.746,P<0.05).Conclusions Over-expression of EZH2 which is involved in accelerating proliferation may play an important role in the pathogenesis and progression of human prostate carcinoma. As a molecular marker of prostate carcinoma, EZH2 may serve as a new index for estimating the level of malignancy and progression of the prostate cancer.
8.Variation of serum ferrin, folic acid and vitamin B_(12) levels in patients with acute promyelocytic leukemia

Objective:To observe the dynamic variation of serum ferritin (SF), folic acid, and vitamin B_(12) levels in patients with acute promyelocytic leukemia (APL) at different disease stages. Methods:Serum SF, folic acid and vitamin B_(12) levels were successively tested in thirty-six patients with primary APL every 1 to 3 months by using chemiluminescence analysis. Five different disease stages were selected as dynamic observation time points: first diagnosed, first complete remission (CR1), six months after CR1, relapsed stage,and CR1 for three years. Results:There were 75.0%(27/36)of patients with abnormal high levels of SF, 77.8% (28/36)of patients with abnormal low levels of folic acid, and 100%(36/36)of patients with increased vitamin B_(12) levels in first diagnosed stage. The number of patients with abnormal variations of SF, folic acid and vitamin B_(12) level was decreased in CR1 stage compared with those in first diagnosed stage (SF: P<0.05;folic acid and vitamin B_(12): P<0.01). The serum SF, folic acid and vitamin B_(12) levels tended to recover step by step with chemotherapy. At six months after CR1 the three parameters of most patients recovered to normal levels. APL was relapsed in 4 patients after 1-year CR. Both SF and vitamin B_(12) levels were increased and the folic acid level was decreased compared with those before replase, but the difference had no significance (P>0.05). The serum SF, folic acid and vitamin B_(12) levels were in normal ranges in the patients who had 3-year CR. Conclusion:The serum SF, folic acid and vitamin B_(12) levels had dynamic variation in APL course. Increase in serum SF and vitamin B_(12) as well as decrease in folic acid are related with the active degree of APL and its tumor load.
9.Primary renal neuroendocrine tumors:report of five cases and review of the literature

Objective To analyze the features of diagnosis, treatment and prognosis of primary renal neuroendocrine tumors, and to improve the understanding of primary renal neuroendocrine tumors. Methods From January 2008 to June 2015, 5 cases of primary renal neuroendocrine tumors were hospitalized and their data was analyzed retrospectively, with 1 male 4 females, aged 40-73 years with the middle age of 48 years.Tumors were all located in the left kidney, whose diameters arranged from 4 to 9 cm, with an average of 6.5cm.One case presented with hematuria, one case was identified because of abdominal pain, and the other three cases were identified via physical examination.Four cases underwent a renal contrast-enhanced ultrasound, which indicated a medium-hypoechoic mass in three cases and a hyperechoic mass in one case.All five cases underwent CT scan, presenting irregularly shape and density. Calcification was found in three cases on plain scanning.Significantly heterogeneous enhancement was found in three cases and moderate heterogeneous enhancement was found in one case on enhanced scanning. Results All five cases underwent operations, with two cases undergoing radical nephrectomy and three cases undergoing partial nephrectomy.Pathological examination showed three cases of tumor cells arranged in a nest slug or ribbon-like infiltrative growth, with no or rare mitosis and no necrosis, which is consistent with renal carcinoids.Three out of 4 renal hilar lymph nodes containing tumor cells were identified in one case. The tumor cells in another case exhibited a ribbon-like arrangement, with some round nuclei, no significant atypia, and rare mitosis, which is consistent with renal atypical carcinoid.Two out of two renal hilar lymph nodes containing tumor cells were identified in this case. The tumor tissue of one case showed morphologically uniform, medium-sized cells arranged in nest slug form with necrosis, a high nuclear cytoplasm ratio, an obvious allotype and frequent mitosis, which is consistent with renal small cell carcinoma.The immunohistochemistry of the five cases indicated synaptophysin( Syn) and chromogranin A ( CgA) positive in varying degree.One case of renal carcinoid relapsed approximately 78 months after partial nephrectomy, following with radical surgery.The other two cases were followed up for 8 or 27 months and no recurrence or metastasis was detected.One case of renal atypical carcinoid was followed up for 4 months after radical nephrectomy and no recurrence or metastasis was detected.One case of renal small cell carcinoma died of multiple organ failure 11 months after radical nephrectomy plus hepatic metastatic carcinoma radio-frequency ablation approximately.Conclusions Primary renal neuroendocrine tumors are rare clinically. Renal neuroendocrine tumors may be expressed as carcinoid, atypical carcinoid or small cell carcinoma, and the clinical manifestations, pathological characteristics and prognosis varied.Primary renal carcinoids may be treated by surgery with a nice prognosis.Patients with primary renal small cell carcinoma require comprehensive treatment, and their prognosis is poor.
10.Clinical significance of serum cholesterol levels in multiple myeloma

Objective To investigate the relationship between serum cholesterol levels and immunoglobin types and clinical stages in the patients with multiple myeloma (MM). Methods We retrospectively analyzed the blood lipid levels in 65 patients with MM at diagnosis, including total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), apolipoprotein Al (apo-Al) and apolipoprotein B (apo-B), and explored relationship between lipid parameters and immunoglobulin types or clinical stages in patients with MM. Thirty healthy persons were served as controls. Results Of the 65 MM patients, 53.85% were IgG type, 63.1 % were at stage Ⅲ. The levels of TC, HDL-C, LDL-C, apo Al and apo B in the patients with MM were significantly lower than that in the controls (P <0.05), and TG in MM patients was no difference with that in the controls (P >0.05). Except one case of IgD type, the levels of TC, HDL-C, LDL-C, apo Al and apo B in Ig G and Ig A types of patients were significantly lower than that in the light chain type among other 64 cases (P <0.05), and TG levels in different immunoglobulin types was found no statistical differences. The levels of TC, HDL-C, LDL-C and apo A1 in the patients with stage Ⅲ were lower than that of stage I and controls (P <0.05), furthermore, the level of LDL in stage Ⅱwas lower than that in stage Ⅰ. Conclusion Hypocholesterolemia are seen in the patients with MM and serum cholesterol levels are related to MM staging.
