1.The activity of gemcitabine in advanced non-small-cell lung cancer

Gemcitabine is a new active drug in the treatment of advanced non small cell lung cancer (NSCLC). It is validated that the response rate of the single agent gemcitabine was consistently above 20%, median survival was 34 weeks and it could improve the disease related symptoms with well toleranced toxicities by many clinical studies. This report reviews the clinical studies of gemcitabine in the patients with advanced NSCLC.
2.Application of arginine stimulation test in evaluating function of pancreatic islet beta cell

Objective To investigate the value of intravenous arginine stimulation test (AST) in evaluating function of pancreatic islet beta cell response in patients of diabetes mellitus. Methods Twentyone patients with type 1 diabetes mellitus (DM1, DM1 group) and 113 patients with type 2 diabetes mellitus (DM2, DM2 group) were recruited in this study. DM2 patients were divided into two sub-groups, DM2a group (duration of no more than 1 year, 58 patients) and DM2b group (more than 1 year, 55 patients). The serum levels of C-peptide (CP) were determined at fasting and 2,3,4,5 minutes after intravenous injection of 5 g arginine. Results In DM1 group, the level of CP after injection of arginine was. similar to the fasting level (P> 0.05 ). In DM2 group, the peak level of CP appeared at 3th minute (CP3) during AST, and was significantly higher than fasting CP level(P < 0.01 ). The level of fasting and arginine-stimulated CP in DM2a group was significantly higher than that in DM2b group, and the level of fasting and arginine-stimulated CP in DM2b group was significantly higher than that in DM1 group. The patients of DM1 group whose level of CP3 < 600 pmol/L all needed insulin injection to control hyperglycemia, and the coincident rate was 100.0%.In DM2 group, there were 91 patients whose CP3 ≥600 pmol/L, among which 85 patients could be well controlled with diet or oral hypoglycemic agents, and the coincidentrate was 93.4%; there were 22 patients whose CP3 < 600 pmol/L, among which 19 patients need insulin injection to control hyperglycemia, and the coincident rate was 86.4%. Conclusions AST is valuable in assessing the function of pancreatic islet beta cell in patients with diabetes mellitus. The level of CP3 ≥600 pmol/L can be considered as a reference in diagnosis and treatment of diabetes mellitus.
3.Application of Polysorbate 80 in Microbial Limit Test

Objective:To provide appropriate test liquid preparation methods for antibacterial or water-insoluble drugs, and estab-lish a convenient and effective method for microbial limit test. Methods:Using function of polysorbate 80 as neutralization, emulsifica-tion, solubilization and microbial protection, We carried out the liquid preparations were prepared and microbial limit test validation was carried out for 11 kinds of hospital drugs. Results:The bacteria recovery rate and the control bacteria test were in accordance with the requirement of pharmacopoeia. Conclusion:Polysorbate 80 can be used in the microbial limit test of the drugs.
4.Value of dipeptidyl peptidase 4 inhibitor in the treatment of type 2 diabetes mellitus

Objective To investigate the efficacy and safety of dipeptidyl peptidase 4(DDP-4)inhibitor in the treatment of diabetes mellitus.Methods Eighty-six cases patients with poor glycemic control in type 2 diabetes mellitus in Shunde First People's Hospital of Foshan from May 2015 to May 2016 were selected as the research objects and divided into two groups according to random number table method,each group with 43 cases.The control group with acarbose(50 mg/times,3 times/d,with the same as the 3 meals,sustained medication for 12 weeks)to control blood sugar,while the observation group used DPP-4 inhibitors sitagliptin(100 mg/times,1 times/d,sustained medication for 12 weeks),compared the blood glucose,blood lipid indexes and adverse reactions of two groups before and after treatment.Results After treatment,FPG,2 hPG,HbAlc level of observation group were respectively(6.71±0.65)mmol/L,(8.10±0.17)mmol/L,(7.12±0.41)%,significantly lower than control group((7.86±0.72)mmol/L,(9.20±0.65)mmol/L,(7.51±0.52)%,the differences were statistically significant(P<0.01).After treatment,there were no significant differences in terms of TC,TG,HDL-C,LDL-C between two groups(P>0.05).Treatment for 12 weeks,the blood glucose compliance rate of observation group was 88.37%(38/43),the control group was 67.44%(29/43),the observation group was significantly higher than the control group(P=0.019).After treatment,the HOMA-IR value of the observation group was 4.42±0.17,significantly lower than the control group(4.91±0.24),HOMA-beta value was 88.20±6.31,significantly higher than that of the control group(80.21±5.67),the differences were significant(P<0.01).Conclusion DDP-4 inhibitor in the treatment of type 2 diabetes,can effectively reduce the level of blood glucose and glycosylated hemoglobin,and have no significant adverse reactions,is effective and safe hypoglycemic drugs.
5.Efficacy and safety of 125Iseed implantation for treating neuroblastoma in nude mice

Objective To examine the efficacy and safety of 125Iseed implantation for treating neuroblastoma (NB) in animal models.Methods A total of 45 nude mice models of neuroblastoma were constructed and divided into the 125Igroup.control group.and blank group at 15 mice per group.The long and short diameters of the tumor were measured every 3 days.and the tumor inhibition rate was calculated every 9 days.Apoptotic and proliferative protein expression levels in tumor tissue and peritumoral tissue.as well as endocrine markers and bone marrow of the nude mice.were analyzed.The independent sample t test was used to compare the mean scores.and ANOVA was used for comparison between multiple groups.Results Tumor volume inhibition rate was significantly higher in the 125Igroup than in the control group and blank group on days 9.18.and 27(all P<0.05).Caspase-3 expression in tumor tissues was significantly higher in the 125Igroup than in the control group and blank group (all P<0.05).whereas proliferating cell nuclear antigen (PCNA) expression was significantly lower in the 125Igroup than in the control group and blank group (all P<0.05).There was no significant difference in Caspase-3 and PCNA expression between the control group and blank group (all P>0.05).In addition.no significant difference in the expression of Caspase-3 and PCNA in peritumoral tissue was observed between the 125Igroup.control group.and blank group (all P>0.05).Cell apoptosis in tumor tissue was significantly lower in the blank group and control group than in the 125Igroup (all P<0.05).while there was no significant difference between the blank group and the control group (P>0.05).There was no significant difference in endocrine markers between the three groups (P>0.05).There was no significant bone marrow suppression in the 125Igroup.and this observation was similar to those in the control group and blank group (all P>0.05).Conclusions 125Iseeds have significant toxicity to NB.125Iseed implantation is safe in nude mice with NB within the therapeutic doses.
6.Preparation Process of Compound Asiaticoside Gel Plasters

Objective: To optimize the formula of compound asiaticoside gel plasters, and determine the optimal preparation process. Methods:Using the initial adhesion, viscosity and appearance as the indices, the gel plasters were evaluated by orthogonal design in order to study the effects of various base ratios on the preparation and screen the optimal formula and preparation process. Re-sults:The optimal formula was dihydroxyaluminium aminoacetate∶EDTA∶glycerol∶CMC-Na of 0. 25∶0. 05∶45∶1. Conclusion: Com-pound asiaticoside gel plasters with the optimal formula and preparation technique show good adhesion and uniformity, which can meet the need of large-scale production.
7.Changes of the first-phase insulin secretion in first-degree relatives of type 2 diabetic subjects with different glucose-tolerance

AIM: To investigate the role of the insulin secretion as well as insulin sensitivity in the onset and development of the type 2 diabetes mellitus (T2DM). METHODS: This study included 38 normal glucose tolerance(NGT) subjects without family history of diabetes, 32 NGT and 36 impaired glucose tolerance(IGT) subjects who were the first-degree relatives of type 2 diabetes, and 35 newly-diagnosed type 2 diabetic patients. ? cell function and insulin sensitivity were examined by using oral glucose-insulin release test (OG-IRT) with SIM reflecting insulin sensitivity and intravenous glucose tolerance test (IVGTT) with AIR3-5 reflecting the first-phase insulin secretion function. AIR3-5 was calculated as the average incremental plasma insulin concentration from the third to the fifth minute after the glucose bolus. AIR3-5 and SIM were compared among the four groups. RESULTS : From NGT, IGT to type 2 diabetes, there was a decreasing tendency of the level of AIR3-5 and SIM, with NGT the highest, IGT the medium and T2DM the lowest. The level of the AIR3-5 in the subjects of NGT who were first - degree relatives of type 2 diabetes were lower than those of NGT without family history of diabetes( P 0.05) . CONCLUSION: Insulin secretion dysfunction may be the primary factor in the onset of type 2 diabetes. Along with the developing of IGT and type 2 diabetes, the insulin secretion and sensitivity decrease.
8.Myocardial Protection Effect of Dexmedetomidine in Patients Undergoing Open-heart Surgery under CPB

Objective To observe the myocardial protective effect of dexmedetomidine in patients undergoing open-heart surgery under cardiopulmonary bypass (CPB).Methods 50 patients of open-heart surgery under CPB were randomly divided into two groups equally, namely observation group and control group.Observation group was treated with injection of dexmedetomidine at 0.5 μg/kg for 15 min, and then maintained at 0.4 μg/kgoh.The control group was given equal volume of normal saline.Concentrations of IMA and cTnI were determined before anaesthesia (t0), after 30 minutes of CPB (t1) and after surgery (t2).Results IMA and cTnI concentrations of t1 and t2 in the observation group were significantly lower than those in the control group (P<0.05).Conclusion Dexmedetomidine has obvious protective effect on myocardium, which can reduce open-heart surgery of patients with myocardial ischemia reperfusion injury (MIRI).
9.Simultaneous Determination of Eprosartan/Hydrochlorothiazide Tablets by HPLC

Objective:To establish an HPLC method for the simultaneous determination of eprosartan/hydrochlorothiazide tablets. Methods:Isocratic separation was achieved on a Phenomenex C18 column(250 mm × 4. 6 mm, 5 μm) using the mobile phase com-posed of 0. 5% formic acid-acetonitrile(60∶40, pH 2. 80). The flow rate was 1. 0 ml·min-1, the detection wavelength was 272nm, the column temperature was 30℃ and the injection volume was 20μl. Results:The linearity between peak area and concentration was observed within the range of 60. 0-1 200. 0 mg·L-1(r=0. 999 9) for eprosartan and 1. 25-25. 00 mg·L-1(r=0. 999 9) for hydro-chlorothiazide. The mean recovery of eprosartan and hydrochlorothiazide was 100. 02%(RSD=0. 35%, n=9) and 97. 93%(RSD=1. 54%, n=9), respectively. Conclusion:The method is simple, sensitive and accurate, and can be applied in the determination of eprosartan/hydrochlorothiazide tablets.
10.Preparation and Preliminary Stability of Candesartan Cilexetil Tablets

Objective:To prepare and optimize candesartan cilexetil tablets,and study the stability preliminarily. Methods:The formula was optimized by Box-Behnken experiment design,the ratio of lactose to pregelatinized starch( X1 ),the amount of disintegrant ( X2 ,%)and the amount of lubricant( X3 ,%)were selected as the independent variables,and weight difference( Y1 ,%),friability ( Y2 ,%),disintegration time( Y3 ,%)and candesartan cilexetil dissolution( Y4 ,%)were the dependent variables. The release rate of candesartan cilexetil tablets and the reference tablets were compared by similarity factor( f2 value). Preliminary stability was studied by high-temperature test,high-humidity test and illumination test. Results:The optimal formula of the tablets was as follows:the ratio of lactose to pregelatinized starch was 7:1,the amount of disintegrant was 5. 5%,and the amount of lubricant was 0. 5%. The f2 for the candesartan cilexetil tablets and the reference tablets in different dissolution meda was 60. 62,73. 34,66. 95 and 68. 60,respec-tively. Conclusion:The formula design is reasonable,the preparation process is feasible and the quality can be controlled.
