1.Comprehensive analysis of long-term survival of liver neoplasms patients after interventional therapy

Objective To investigate the correlative factors influencing long-term efficacy of patients with liver neoplasms after interventional therapy. Methods A total of 495 patients underwent transcatheter arterial chemoembolization (TACE), and the data were retrospectively analyzed. The patients were divided into two groups according to the survival time after interventional therapy: ≥5 years and <5 years. Correlative factors were compared in both two groups. Results In 31 patients survived longer than 5 years, 18 patients with Lipiodol filling type Ⅰ tumor, and 13 with type Ⅱ tumor. The 5, 7, 10 years survival rate in all 495 patients was 6.26% (31/495), 1.41% (7/495) and 0.40% (2/495), respectively. Factors including tumor pattern, clinical classification, the features of angiography, with or without heptic arteriovenous fistula, the pattern of Lipiodol filling, with or without invasion and metastasis, hepatic function, patient's age, tumor diameter, AFP value before and after TACE, the variety of AFP value after TACE influenced the long-term survival rate after interventional therapies (P<0.05). Conclusion The characteristics of tumor, patient's status, the quality of TACE, whether combined with PEI, and/(or) anti-virus treatment have significant influence on long-term efficacy after interventional therapy in patients with liver neoplasms.
2.Efficacy of endoscopic transnasal dacryocystorhinostomy by suture in the treatment of acute dacryocystitis as early as possible

Objective To explore the effect of endoscopic transnasal dacryocystorhinostomy(ET-DCR) by suture in the treatment of acute dacryocystitis(AD) as early as possible.Methods The clinical data of 32 patients with unilateral AD who underwent ET-DCR by suture as early as possible were retrospectively analyzed.Results All patients presented clear anatomic structure,intraoperative hemorrhage increased in 3 cases,swelling and pain was rapidly relieving in all patients in the first day of postoperation,the mean resolution time of congestion and swelling in the middle canthus was average 3 days(range 1-6 days),no spread of infection occurred,no facial scar appeared in all patients except one case of abscess rupture.Complete complaint relief in 26 cases,slight epiphora presented in 6 patients who confirmed for lacrimalduct obstruction and cured by intubation,and all ostial patency with no AD recurrences at the mean follow-up of 2 years (range 12 months-5 years).Conclusion ET-DCR by suture as early as possible can be used to cure acute dacryocystitis and it is effective safe and economy.
3.The clinical study of endoscopic dacryocystorhinostomy by suture

Objective To investigate the clinical value of endoscopic transnasal dacryocystorhinostomy ( ET-DCR) by suture techniques.Methods 85 patients of 98 eyes with chronic dacryocystifis were randomly divided into the two groups:control group ( without suture) and observation group ( by suture) .All cases were followed up for more than 12 months.The cure rates and occurrence of granulation tissue were compared between the two groups. Results The cure rate of the observation group ( 96.00%, 48/50 ) was higher than that in the control group (81.25%,39/48) (χ2 =5.35,P<0.05). During process of following up,the occurrence of granulation tissue at the ostium margins accounted for 14.00%(7/50) in the observation group and 31.25%(15/48) in the control group (χ2 =4.19,P<0.05).Conclusion ET-DCR by suture techniques reduces the formation of granulation tissue and improves the cure rate.Especially, ET-DCR by suture techniques for the small lacrimal sac of patients may have special advantages.
4.Protective effect of purple sweet potato flavonoids on CCL4-induced acute liver injury in mice.

OBJECTIVETo investigate the protective effect of purple sweet potato flavonoids (PSPF) on CCl4-induced acute liver injury in mice.
METHODSSixty mice were randomly divided into six groups (n=10 in each): blank group, model group, PSPF groups (400 mg*kg(-1), 200 mg*kg-1 and 100 mg*kg(-1)) and positive control group (DDB 150 mg*kg(-1)). Acute liver injury was induced by administration of peanut oil with 0.1% CCl4 (10 mg*kg(-1)) in mice. The viscera index, serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and lactate dehydrogenase (LDH) were measured, and the activities of superoxide dismutase (SOD) and the contents of malondialdehyde (MDA) in hepatic tissues were also measured. The pathological changes of liver were observed with microscopy.
RESULTSPSPF significantly decreased serum ALT, AST and LDH levels (P<0.05 or P<0.01) and MDA content in hepatic tissues (P<0.01), increased the activities of SOD (P<0.01).
CONCLUSIONPurple sweet potato total flavonoids can prevent CCl4-induced acute liver injury in mice, which may be related to inhibition of lipid peroxidation and reduction of oxygen free radicals.

5. Voxel-based morphometry (VBM) MRI analysis of gray matter in patients with occupational noise-induced hearing loss

Objective:
To investigate the changes of brain gray matter volume in patients with occupational noise-induced hearing loss by voxel based morphometry (VBM) .
Methods:
16 age-and education-matched healthy controls and 42 patients with occupational noise induced hearing loss, including 27 in mild group and 15 in severe group, received MRI 3D-FSPGR sequence T1WI sagittal scan, and then underwent VBM of brain gray matter volume data analysis.

6.Analysis on point mutation of the CDKN2/p16 gene in lung cancer.

OBJECTIVETo study the status of CDKN2/p16 gene point mutation in lung cancer.
METHODSPolymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) and sequencing were used to detect the point mutation of CDKN2/p16 gene exon 2 in 89 cases of lung cancer.
RESULTSIn 69 cases of the lung cancer without deletion of CDKN2/p16 gene exon 2, 16 cases were found to have suspicious abnormality of CDKN2/p16 gene exon 2 by PCR-SSCP, and in these 16 cases, 9 were found to harbor point mutations of CDKN2/p16 gene exon 2 by automated sequencing analysis.
CONCLUSIONThe point mutation is one of the mechanisms for CDKN2/p16 gene inactivation, but it is not the chief mechanism. The inactivation of CDKN2/p16 gene aroused by point mutation plays a role to some extent in the genesis and progression of lung cancer.

7.Preparation of chitosan-collagen sponge and its application in wound dressing.

We have prepared a wound dressing made from chitosan and collagen. Its clinical curative effect was detected. Chitosan solution was put into purified collagen solution. Then, the solution became sponge by means of freeze drying, and it was subjected to a series of toxicology tests, including acute toxicity, stimulation test, allergic and hemolysis tests, as well as the clinical test of openning trauma in orthopedics. All of the results of toxicology tests were negative. The chitosan-collagen sponge could not only accelerate the speed of curing but also restrain the extravasate. Therefore, the chitosan-collagen sponge has good biocompatibility and clinical curative effect. It is a prospective security-biomaterial for medical use.

8.Mutation analysis of thyroid peroxidase gene in a Chinese family with congenital hypothyroidism and literature review

In this paper, thyroid peroxidase (TPO) gene mutation was studied in a congenital hypothyroidism (CH) family with 2 patients, to explore the mechanism of c. 2268dupT homozygous mutation leading to thyroid nodules, as well as the relationship between TPO gene mutation and thyroid cancer. The result suggested that TPO gene mutation is one of the reasons for congenital hypothyroidism, TPO gene c. 2268 dupT mutations can lead to thyroid nodules, which may be related to long-term high TSH stimulation and truncated protein accumulation. TPO gene mutations have a certain correlation with thyroid cancer, the risk may increase with the increase of age, but the mechanism is not clear at present. In the future, further research on the correlation and mechanism between the two factors is needed, as well as to determine whether it is necessary to remove the thyroid gland to prevent the accarance of cancer in CH patients with TPO gene mutation with thyroid nodules.
9.Risk factors for urinary tract infection in kidney transplantation from brain death donor and its role in graft function.

OBJECTIVES:
Urinary tract infection (UTI) is the most common infection complication after kidney transplantation, and the reports of the incidence vary greatly among different centers. This study aims to explore the risk factors for UTI after kidney transplantation with the donation from brain death (DBD) and the impact on graft function, thus to provide theoretical basis for comprehensive prevention and treatment of UTI after kidney transplantation.
METHODS:
The clinical and laboratory data of DBD kidney transplantation from January 2017 to December 2018 in Xiangya Hospital, Central South University were collected and retrospectively analyzed. Patients were assigned into an UTI group and a non-UTI group. The base line characteristics, post-transplant complications, and graft function were compared between the 2 groups. Multivariate logistic regression was used to analyze the risk factors for UTI.
RESULTS:
A total of 212 DBD kidney transplant recipients were enrolled in this study. UTI occurred in 44 (20.75%) patients after transplantation. The female, the time of indwelling catheter, and postoperative urinary fistula were independent risk factors for UTI after DBD kidney transplantation. A total of 19 strains of gram-positive bacteria, 12 strains of gram-negative bacteria , and 10 strains of fungi were isolated from the urine of 44 UTI patients. CONCLUSIONS
UTI after DBD kidney transplantation transplantation affects the renal function at 3 months and increases the patient's economic burden.
