Objective To build clinical nursing pathway for artificial abortion operation and explore the effect of this method. Methods The nursing pathway was designed and carried out. The satisfaction degree with nursing, status of patients' awareness about contraception and artificial abortion operation was compared before and after the nursing pathway was implemented. Results Before and after the nursing pathway was implemented,the proportion of missing item of operation record was 0.636 and 0.013, the satisfaction degree with nursing was 77.0% and 92.3%, rate of patients' awareness about contraception and artificial abortion operation was 47.6% and 96.1%. Conclusions The clinical nursing pathway for artificial abortion operation may improve the degree of satisfaction- and the status of patients' awareness about contraception and artificial abortion operation, and decline the proportion of missing item of operation note, it ensures high nursing quality and safety surgery, which is worthy of popularizing.

Objective To compare the difference in constituents from different parts of wild and cultivated Ephedra sinica. Methods Using the solvent of 50% methyl alcohol to extract from the stems and roots of wild and cultivated E. sinica under heating reflux, respectively and then determine the content of ephedrine in each extractions by HPLC. The volatile oil was extracted by stream-distillation and analyzed by GC-MS method. Results The content of the ephedrine in extraction of stems and roots of wild E. sinica was 0.55% and 0.000 57% while the content of the cultivated was 0.26% and 0.001 7%. Forty-five compounds in the volatile oil were identified by GC-MS method, among them 13 compounds existed in both wild and cultivated E. sinica. Conclusion The content of the ephedrine is about two times of the cultivated in wild E. sinica′s stems, while it is very low in the roots of two kinds of herbs. And ephedrine is not found in the volatile oil.

AIM:To characterize the effect of estradiol on proliferation,differentiation and extracellular matrix(ECM) accumulation in stromal cells through regulation of BPH-1 paracrine.METHODS:BPH-1 cells were stimulated with different concentrations of estradiol.Conditioned media(CM) were harvested and their effects on stromal cell cultures were tested.Cell proliferation was determined by MTT assay.mRNA of smoothelin,fibronectin,collagen Ⅳ and transforming growth factor ?1(TGF-?1) were analyzed by real-time RT-PCR.Western blotting was used to determine smooth muscle myosin heavy chain(SMMHC).ELISA and radioimmunoassay were respectively used to measure fibronectin,TGF-?1 and collagen Ⅳ protein expressions.RESULTS:Estrodiol stimulated the expression and secretion of TGF-?1 in BPH-1 cells.The proliferation of stromal cells increased when they were cultured with CM harvested from estrogen treated BPH-1 cells.The mRNA levels of collagen Ⅳ and smoothelin increased in stromal cells treated with CM from BPH-1 cells.The results of radioimmunoassay also showed that the collagen Ⅳ protein level up-regulated in the supernatants and cell extracts of CM-treated stromal cells.A neutralizing antibody to TGF-?1 inhibited the stimulation of collagen Ⅳ and SMMHC by BPH-1 CM.The expression of fibronectin was only marginally changed in stromal cells cultured in the presence of BPH-1 CM.CONCLUSION:The BPH-1 cells increase ECM accumulation and differentiation of stromal cells through TGF-?1.Estradiol stimulate differentiation of stromal cells by induction of TGF-?1 expression.Estradiol stimulate proliferation by influencing the factors secreted from prostatic epithelial cells.

We reviewed the clinical data of 24 patients with postpartum hemorrhage undergoing transcatheter arterial embolism from March 2009 to September 2011.Twenty out of 24 patients underwent uterine artery embolism,while other 4 underwent internal iliac artery embolism.The hemorrhage reduced markedly 2 to 9 min after arterial embolism in all patients,except 2 who underwent secondary embolism due to the intermittent postoperative hemorrhage. The postoperative complications included low fever and discomfort in lower abdomen.Nineteen patients were followed up for a mean period of ( 17.3 ± 1.1 ) months,no recurrent hemorrhage was observed after discharge.The normal menstrual cycle recovered 4 to 6 months after operation.Three patients underwent pregnancy successfully after discharge.The results indicate that transcatheter arterial embolism is safe and effective in the treatment of postpartum hemorrhage and it should be recommended as a feasible choice in clinical practice.

Objective To investigate the biological characteristics and management of multifocal papillary thyroid carcinoma. Methods Clinical data of 86 patients who underwent thyroidectomy and were pathologically diagnosed as progressive multifocal papillary thyroid carcinoma from 1995 to 1997 in Tianjin Cancer Hospital were retrospectively analyzed. Results There were 86 cases of multifocal papillary thyroid carcinoma, accounting for 23.4% of all papillary thyroid carcinoma of the same period. In the 86 cases of multifocal papillary thyroid carcinoma, there was unilateral in 12 cases and bilateral in 74 cases; cervical lymph node metastasis in 51 cases ( 59. 3% ); combined microcarcinoma of 46 cases ( 53.5% );Hashimoto's thyroiditis was concomitant in 23 cases (26.7%); local invasion was found in 19 cases (22. 1% ); distant metastasis occurred in 1 case ( 1.2% ); 10-year survival rate was 95.3%. American Joint Committee on Cancer ( AJCC ) stage was associated with prognosis significantly ( x2 = 63. 395, P =0.000). Conclusions Multifocal papillary thyroid carcinoma often occurs bilaterally, concomitant microcarcinoma and Hashimoto's thyroiditis is common with a comparatively favorable prognosis. AJCC stage is still the best prognostic factor.

Objective To investigate a reliable and practicable model of intraoperative neurophysiological monitoring (IONM) in oblique lateral interbody fusion (OLIF) combined with posterior transpedicle internal fixation.Methods A total of 34 consecutive patients who underwent OLIF combined with posterior transpedicle internal fixation with IONM at the SRRSH from November 2014 to July 2016 were collected and follow-up at least for 3 months postoperative.The intraoperative somatosensory evoked potentials (SEP),motor evoked potentials (MEP),dermatomal somatosensory evoked potential (DSEP) and spontaneous electromyography (sEMG) were used and evaluated,besides,triggered electromyography (tEMG) was used to monitor the implantation of pedicle screws,the possible reasons and treating procedures of monitoring alerts were summarized.Results A total of 34 cases of patients were effectively monitored during OLIF combined with posterior transpedicle internal fixation.SEP monitoring was successful in all patients,and no abnormal SEPs were observed in all cases.DSEP monitoring was successful in 20 cases (58.8％),and no abnormal DSEPs were observed in these patients.MEP was successfully monitored in all cases,eliminating the anesthesia factor,no abnormal MEPs were observed in all cases.The sensitivity and positive predictive value of SEP,MEP,DSEP were 0％.3 cases had abnormal sEMG reactions during the channel establishment which disappeared after adjustment of channel,and 2 cases which appeared abnormal wave recovered after brief observation.Among the 5 patients with abnormal wave,one case got numbness,one patient felt pain in front of the thigh postoperative.Among the other 29 patients without abnormal wave during channel establishment,one case appeared numbness.These 3 patients with neurologic symptoms recovered in 3 months after conservative treatment.The sensitivity and specificity of sEMG was 66.7％ and 90.3％ respectively,negative predictive value and positive predictive value were 96.6％ and 40％ respectively.6 screws got tEMG response after tapping even the intensity was less than 10mA,which showed broken codex of pedicle according to postoperative CT scan.Cornclusion Intraoperative application of sEMG monitoring in OLIF can obviously reduce the incidence of the neurological deficit,which was an effective method,tEMG monitoring during lumbar pedicle screw fixation can detect the mal-positioned screws and protect the nerve roots.SEP,MEP and DSEP did not seem well suited for OLIF.

Objective To reestablish neuroimmunoregulation network and its function.Methods Whole-some gene array was used to analyze the gene expression involved in neuroimmunoregulation function of the lateral hypothalamic(LH) in rats at different immunizational time.The function trees of different genes were analysed with the PathWay Miner public database.Results 632 genes were differentially expressed: including 374 of 2-day immunized group,62 of 4-day immunized group and 196 of 6-day immunized group.Function signal pathway analysis for 398 up-regulated genes showed 27 genes were involved in 31 cell functional signal conduction pathways,including the well-known signal conduction pathways of synaptic reconstruction.Conclusion Synapses reconstruction appears to be the important pattern of functional reestablishment of neuroimmunoregulation network.

Objective To observe the outcomes of one-stage posterior short-level pedicle screw fixation combined with anterior fixation of severe thoracolumbar fractures.Methods A retrospective case series study was performed on 21 patients with severe thoracolumbar fractures stabilized by posterior short-level pedicle fixation combined with anterior internal fixation at one stage from January 2012 to December 2014.There were 16 males and 5 females,at age of 17 and 64 years [(38.7 ± 11.4) years].The involved segments included T11 in 2 patients,T12 in 5,Lt in 6 and L2 in 8.For AO fracture classification,type A fractures were seen in 4 patients,type B in 7 and type C in 10.Thoracolumbar injury classification and severity score (TLICS) was (8.12 ± 0.87) points (range,7-10 points).Frankel neurological performance scale was Grade B in 8 patients,Grade C in 11 and Grade D in 2.Operation time,blood loss,nerve function,kyphosis correction and complications were reported.Results Operation time was (234.5 ±57.3)min (range,180-360 min),and blood loss was (387.4 ± 124.4) ml (range,260-950 ml).Time of follow-up was (19.8 ± 3.5)months (range,14-25 months).Nerve function of 18 patients was improved by at least one Frankel scale.Cobb angle was (4.1 ±5.3)° at postoperative 3 days and (4.0 ± 4.9)°at the final follow-up,showing significant differences from that before operation [(-9.3 ± 4.2) °] (P ＜ 0.05).While the difference of Cobb angle did not differ significantly at postoperative 3 days and at final follow-up.No cerebrospinal fluid leakage,vascular injury,incision infection or nerve function deterioration occurred.Conclusion One-stage posterior short-level pediele screw fixation combined with anterior decompression and bone graft fixation is characterized by short operation time,few blood loss,good correction of traumatic kyphosis and good neurological recovery,indicating a good surgical choice for severe thoracolumbar fractures.
