1.Comparative Analysis between Diatom Nitric Acid Digestion Method and Plankton 16S rDNA PCR Method

Objective To com pare and explore the application value of diatom nitric acid digestion method and plankton 16S rDNA PCR method for drow ning identification. Methods Forty drow ning cases from 2010 to 2011 were collected from Department of Forensic Medicine of Wenzhou Medical University. Sam ples including lung, kidney, liver and field water fromeach case were tested with diatom nitric acid digestion method and plankton 16S rDNAPCR method, respectively. The Diatom nitric acid digestion method and plankton 16S rDNAPCR method required 20 gand 2g of each organ,and 15 mL and 1.5 mL of field water, respectively. The inspection time and detection rate were com pared between the two methods. Results Diatom nitric acid digestion method m ainly detected two species of diatom s, Centriae and Pennatae, while plankton 16S rDNA PCR method am plified a length of 162 bp band. The average inspection time of each case of the Diatom nitric acid digestion method was (95.30±2.78) min less than (325.33±14.18)min of plankton 16S rDNA PCR method (P<0.05).The detection rates of two methods for field water and lung were both 100% . For liver and kidney, the detection rate of plankton 16S rDNA PCR method was both 80% , higher than 40% and 30% of diatom nitric acid digestion method (P<0.05), respectively. Conclusion The laboratory testing method needs to be appropriately selected according to the specific circum stances in the forensic appraisal of drow ning. Com pared with diatom nitric acid digestion method, plankton 16S rDNA PCR method has practice values with such advantages as less quantity of sam ples, huge inform ation and high specificity.

3.Apoptosis of HepG2 cells after transfection with LIGHT gene and interferon-γ

Objective To investigate apoptosis of HepG2 ceils after transfecfion with LIGHT gene and interferon-γ. Methods LIGHT gene and interferon-γ were transfected into HepG2 cells by liposome mediated method. The HepG2 cells were divided into group A (transfected with LIGHT gene or interferon-γ), group B (transfeeted with LIGHT gene and interferon-γ) and group C (non-transfection group). The apoptosis rate of the HepG2 cells and the expression of Bcl-2 and Caspase-8 were detected 12, 24, 48 hours after transfeetion. Results (1) The apoptosis rates of HepG2 cells at hour 12, 24 and 48 after transfeetion were 18.8% ± 3.5%, 25.7%± 2.8% and 36.4% ±3.6% in group A, 23.8% ±2.4%, 31.1% ±2.1% and42.5% ±4.5% in group B, and 8.7% ± 2.1%, 9.3% ± 1.6% and 10.9% ± 1.2% in group C. There was a significant difference in apoptosis rate among the 3 groups (F = 15.69, 53.33, 48.28, P < 0.01). (2) The expression of Bcl-2 in HepG2 cells at hour 12, 24 and 48 after transfection was 16.4% ± 5.0%, 13.4% ± 3.5% and 8.6% ± 2.3% in group A, 14.7%±3.8%, 9.1% ±2.0% and 4.6% ±2.0% in group B, and 25.3% ±6. 3%, 19.8% ±4.4% and 10.1% ±3.8% in group C. There was a significant difference in the expression of Bcl-2 among the 3 groups (F = 6.19, 12.29, 5.81, P <0.05). (3) The expression of Caspase-8 at hour 12, 24 and48 after transfection were 19.3% ±2.4%, 27.2% ±1.9% and 33.7% ±3.0% in group A, 22.7% ±2.2%, 30.9% ±3.1% and 38.2% ±3.2% in group B, and 1.2% ±0.8%, 1.8% ±0.6% and 3.2% ±1.5% in group C. There was a significant difference in the expression of Caspase-8 among the 3 groups (F =71.54, 112. 78, I01.61, P < 0.01). Condusions LIGHT gene can signiticanfly promote cell apoptosis through regulating the expression of Bcl-2 and Caspase-8. Interferon-γ enhanced the effect of LIGHT gene on the apoptosis of HepG2 cells.
4.Clinical application of heterogeneous acellular dermal matrix with autologous bone meal in open tympanoplasty.

OBJECTIVE:
To evaluate the clinical value of heterogeneous acellular dermalmatrix with autologous bone meal in open tympanoplasty.
METHOD:
Twenty-eight cases (30 ears) with middle ear cholesteatoma were trea- ted by open tympanoplasty and repaired by heterogeneous acellular dermalmatrix autologous bone meal on study team. Twenty-two cases (22 ears) with middle ear cholesteatoma were treated by open tympanoplasty on control team. All patients were followed up for 12 to 18 months and assessed the fuction postoperatively.
RESULT:
The re- construction of external auditory canal structure is close to normal, and no narrow happens on study team. The rate of dry ear was about 90%. All cases had no recurrence of cholesteatoma.
CONCLUSION
Application of decellu- larized dermal matrix with autologous bone meal can rise early to cover the wound, promote wound healing and to reduce the external auditory canal, reduce the effect of granulation and scar formation. It is a kind of method of repair to be promoted.

5.Exploration in the teaching of forensic medicine for international students

Basic knowledge of forensic science is theoretical and abstract,therefore,confir-matory experiment teaching is often adopted in the experiment teaching. International education has become an important topic in medical colleges and universities. Forensic medicine is a compulsory subject for medical international students. This article discussed the exploration and practice in teach-ing forensic medicine for international students at Wenzhou Medical University,which included faculty constructing and training,encouraging whole English teaching by young teachers,applying heuristic interactive teaching and case teaching,establishing strict attendance and appraisal system and con-ducting comprehensive evaluation on medical teaching quality and teaching management.
6.Construction and practice of exercise rehabilitation program for COPD patients based on health action process approach

Objective:To explore the effects of exercise rehabilitation program for chronic obstructive pulmonary disease (COPD) patients based on health action process approach (HAPA) on exercise self-efficacy and psychological resilience in COPD patients.Methods:From January to August 2019, we selected 100 COPD inpatients from Zhumadian Central Hospital as the research objects by convenience sampling. A total of 50 patients hospitalized from January to April 2019 were in control group, and 50 patients hospitalized from May to August 2019 were in observation group. Control group carried out the COPD routine nursing, while observation group implemented the exercise rehabilitation program for COPD patients based on HAPA. The effect of intervention was evaluated with the Exercise Self-Efficacy Scale and the Connor-Davidson Resilience Scale (CD-RISC) .Results:In the end, there were 46 valid questionnaires in control group, and 47 valid questionnaires in observation group. The scores of exercise self-efficacy of patients in observation group and control group three months after discharge were (8.89±2.70) and (7.73±1.67) respectively with a statistical difference ( t=2.485, P=0.015) . Three months after discharge, scores of three dimensions of CD-RISC, namely hardiness, strength and optimism, of observation group were (35.55±6.16) , (23.79±4.88) and (11.25±2.72) respectively, and those of control group were (32.33±6.79) , (21.22±4.17) and (9.80±2.56) , and differences were statistically significant ( t=2.396, 2.728, 2.646; P<0.05) . Conclusions:HAPA-based exercise rehabilitation program for COPD patients has a positive significance for improving the patients' exercise self-efficacy and psychological resilience.
7.Analysis of Peripheral B Cell Subsets in Patients With Allergic Rhinitis.

PURPOSE: Recent evidence suggests that B cells can both promote and inhibit the development and progression of allergic disease. However, the characteristics of B cell subsets in patients with allergic rhinitis (AR) have not been well documented. This study aimed to analyze the characteristics of B cell subsets in the peripheral blood of AR patients. METHODS: Forty-seven AR patients and 54 healthy controls were enrolled in this study, and the B cell subsets in peripheral blood of all subjects were analyzed by flow cytometry. Moreover, the serum total immunoglobulin E (IgE) and IgE concentrations secreted into the cultured peripheral blood mononuclear cells (PBMCs) were measured by using enzyme-linked immunosorbent assay. RESULTS: We found the peripheral blood of AR patients contained higher percentages of memory B cells, plasma cells, and CD19+CD24hiCD27+ regulatory B cells (Bregs) than those of age-matched healthy controls (P < 0.05), while the percentages of naïve B cells and CD19+CD24hiCD38hi Bregs were significantly lower in AR patients than in healthy individuals (P < 0.05). In addition, the serum total IgE and IgE concentrations secreted into the cultured PBMCs were elevated in AR patients than in the healthy controls (P < 0.05). CONCLUSIONS: Our findings indicate that AR patients were characterized by increase in terminally differentiated memory B cells or plasma cells and decreases in CD19+CD24hiCD38hi Breg cells in the peripheral blood.

8.Effects of intervention based on self-regulation theory on self-management behavior and coping styles in COPD patients

Objective:To explore the effect of intervention based on self-regulation theory on self-management behavior and coping styles of chronic obstructive pulmonary disease (COPD) patients.Methods:From January to June 2019, convenience sampling was used to select COPD patients hospitalized in Zhumadian Central Hospital as the research object. Patients were randomly divided into control group ( n=45) and experimental group ( n=48) . Control group conducted routine care and health education for COPD patients, and experimental group carried out an intervention program based on self-regulation theory on the basis of routine care. The intervention effect was evaluated with the Self-Management Scale for Chinese Patients with Chronic Obstructive Pulmonary Disease and the Simple Coping Style Scale. Results:After three months of intervention, the scores of symptom management, daily life management, emotion management, information management and self-efficacy of patients in experimental group were (31.74±3.62) , (46.17±5.48) , (39.55±4.49) , (23.65±4.21) , and (30.00±4.37) respectively, all higher than those in control group, the differencs were statistically significant ( t=5.829, 3.613, 3.208, 3.002, 2.762; P<0.01) . After intervention, the scores of positive coping styles of patients in experimental group and control group were (1.89±0.52) and (1.66±0.49) respectively with a statistical difference ( t=2.192, P<0.05) . Conclusions:Intervention program based on self-regulation theory can improve the self-management behavior of COPD patients, and encourage patients to adopt active coping styles to deal with the disease.

Objective:
To discuss the relationship between lingual tonsil hypertrophy and laryngopharyngeal reflux.
Methods:
Ninety-two patients who received throat surgery in Nanfang Hospital between October 2015 and October 2016 were enrolled. Twenty-six healthy volunteers were recruited as normal controls. All participants were assessed with the reflux finding score(RFS) and the size of lingual tonsils were evaluated using a clinical grading system proposed by Friedman under electronic laryngoscope. The score of reflux symptom index(RSI), personal history and medical history were gathered. Biopsy specimens of lingual tonsils were taken from all participants for the immunohistochemical stain of pepsin.SPSS 19.0 software was used for statistical analysis.
Results:
There were 46.2% (12/26) pepsin-positive and 53.8% (14/26) pepsin-negative volunteers in normal controls. Objective:
To assess the current evidence regarding the efficacy, safety, and potential advantages of endoscopic compared with open salvage surgery for patients with local recurrent nasopharyngeal carcinoma.
Methods:
A systematic search of Pubmed/Medline, Embase, and Cochrane databases ranged between 2000 and 2017 was conducted. Included studies reported specific residual or local recurrent nasopharyngeal cancer survival data.
