1.Randomized Controlled Trail of Ganlu Qingwen Prescription for Treatment of Community-acquired Pneumonia

ObjectiveTo explore the regulatory effect of Ganluqingwen prescription on inflammation and immunity by observing the clinical efficacy of Ganluqingwen prescription in the treatment of community-acquired pneumonia (CAP), so as to provide a clinical basis for the treatment of CAP by traditional Chinese medicine (TCM). MethodsA randomized controlled trial was conducted by selecting patients who were diagnosed with CAP and identified as wind-heat attacking lungs in Xinjiang Uygur Autonomous Region Hospital of TCM from January 2024 to May 2024 and assigning the patients to a control group (treated by western medicine treatment) or an experimental group (treated by Ganluqingwen prescription combined with western medicine). The data of the enrolled patients before treatment, for three-day treatment, for seven-day treatment, and for 14-day treatment were collected, including basic information, medical history, pneumonia severity index (PSI) classification, and distribution and difference of laboratory and imaging information indexes. The peripheral blood specimens were collected from the patients. and the changes of inflammatory factors in peripheral blood were detected by using enzyme-linked immunosorbent assay (ELISA) reagent kits and flow-type multifactor microarrays to evaluate the clinical safety and efficacy of Ganluqingwen prescription in CAP. ResultsCompared with those in the groups before treatment, the total scores of TCM syndromes significantly decreased in both groups (P<0.05). Compared with those in the control group after treatment, the total scores of TCM syndromes decreased more significantly in the experimental group (P<0.05). Compared with the control group after treatment, the experimental group displayed a significantly reduced number of days of fever in patients (P<0.05). Compared with those in the groups before treatment, the leukocyte, neutrophil counts, C-reactive protein (CRP), procalcitonin (PCT), interleukin (IL)-6, alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (Cr), creatine kinase (CK), and creatine kinase isoenzymes (CK-MB) in both groups decreased (P<0.05) after treatment. Compared with that in the control group after treatment, the decrease of leukocyte, neutrophil counts, CRP, PCT, IL-6, ALT, AST, Cr, CK, and CK-MB was more pronounced in the experimental group (P<0.05). Compared with those in the group before treatment, the partial pressure of carbon dioxide increased in the experimental group for 3 d of treatment (P<0.05), and the standard alkali residual, actual alkali residual, standard bicarbonate concentration, and actual bicarbonate concentration increased in the experimental group for 7 d of treatment (P<0.05). Compared with that in the group before treatment, D-dimer decreased in the control group for 7 d of treatment (P<0.05). D-dimer and activated partial thromboplastin time (APTT) decreased in the experimental group for 3 d of treatment (P<0.05), and D-dimer, fibrinogen (FIB), and APTI significantly decreased in the group for 7 d of treatment (P<0.05). Compared with the group for 3 d of treatment, the experimental group for 7 d of treatment showed decreased FIB (P<0.05). Compared with those in the groups before treatment, the levels of inflammatory factors IL-4, IL-10, and IL-13 were elevated in the peripheral blood of the two groups after treatment, and the levels of B lymphocyte chemoattractant (BLC), interferon gamma-induced protein 10 (IP-10), tumor necrosis factor-alpha (TNF-α), interferon-gamma (IFN-γ), monocyte chemoattractant protein-1 (MCP-1), CRP, IL-2, IL-6, IL-8, IL-17, IL-22, and IL-23p19 were significantly reduced (P<0.01). Compared with the control group after treatment, the experimental group exhibited more significant improvement in indexes above (P<0.01). ConclusionThe group treated by Ganluqingwen prescription combined with western medicine shows more significant effects on reducing total scores of TCM syndromes, lowering the ability of leukocyte and neutrophil counts, decreasing BLC, IP-10, TNF-α, IFN-γ, MCP-1, CRP, IL-2, IL-6, IL-8, IL-17, IL-22, and IL-23p19 in the peripheral blood of the patients, and elevating levels of IL-4, IL-10, and IL-13 than the group treated by western drugs alone.

3.Analysis of the epidemic characteristics and disease burden of hospitalized children with viral myocarditis in China from 2016 to 2021

Objective:This study aimed to provide basic data for the prevention, diagnosis and treatment of pediatric viral myocarditis (VMC) in China through analyzing the epidemic characteristics and disease burden of pediatric inpatients with VMC from 2016 to 2021.Methods:We performed a descriptive statistical analysis to the age, genders, seasons, regions and hospitalization cost and days of pediatric VMC inpatients and the death. All of the information was obtained from 27 Children′s hospitals or Maternal and Child Health hospitals of 23 provinces of China from 2016 to 2021.Results:A total of 7 647 599 cases including 1 646 VMC inpatients were admitted into our study. The annual numbers of hospitalizations were 173, 227, 313, 301, 295 and 337, with the hospitalized constituent ratios being 14.9/100 000, 17.9/100 000, 23.0/100 000, 20.5/100 000, 26.5/100 000 and 26.4/100 000 from 2016 to 2021. In recent 6 years, the proportion of VMC hospitalizations had increased yearly ( P<0.001), and had associated with the onset age ( P<0.001). Aged 12-≤18 years owned the highest hospitalized constituent ratio. The Northeast of China owned the largest number of VMC inpatients, and the East second to it. Among the 1 646 VMC children, there were 68 deaths, with the hospitalized case fatality rate of 4.13%. There were no significant differences between genders, age, seasons, years and fatality rate of VMC inpatients. For the diseases burden, the median of hospitalization days of all VMC inpatients was 10 days (IQR 6, 21), and the median of hospitalization cost was 1 1 842.3 RMB (IQR 6 969.22, 19 714.78). The median of hospitalization days of deceased VMC children was only 1 day (IQR 1, 3), the median cost could be 8 874.03 RMB (IQR 5 277.94, 5 6 151.59). Conclusions:In this study, we found that proportion of hospitalization of VMC children increased year by year, adolescence might be a risk factor of VMC. The fatality of VMC inpatients could be up to 4.13%, and the death led to a huge economic burden of society, family and individuals.
4.Pathogenic analysis of severe community-acquired pneumonia in children in Shijiazhuang, Hebei province, 2014-2016

Objective:To investigate the etiological distribution of severe community-acquired pneumonia (CAP) in children in Hebei Province.Methods:The nasopharyngeal swab samples and clinical data from 314 children with severe pneumonia were retrospectively analyzed between January 2014 and January 2016.Results:Among the 314 children, 298 (94.94%) showed positive result for pathogens in their nasopharyngeal swab samples, with 246 cases (78.34%) of multiple pathogens, predominantly mixed viruses and bacteria (206 cases, 65.61%). A total of 848 strains of pathogens were detected, including 483 strains (56.96%) of viruses, predominantly respiratory syncytial virus with 97 strains of subtype A and 86 strains of B. Bacteria and atypical pathogens ( Mycoplasma pneumoniae) had 365 strains (43.04%), mainly Streptococcus pneumoniae (220 strains) and Haemophilus influenzae (119 strains). The detection rate of pathogens was higher in children under one year of age ( χ2=21.389, P<0.001). There were no significant differences in different seasons, but the detection rates of respiratory syncytial virus A, respiratory syncytial virus B, and Streptococcus pneumoniae were higher in autumn, winter, and spring, respectively, with statistically significant differences ( χ2=22.205, P<0.001; χ2=37.874, P<0.001; χ2=11.380, P=0.009). Conclusions:Nasopharyngeal swab sample testing in children with severe pneumonia typically shows a coexistence of viral and bacterial pathogens, with detection rates varying among different age groups and seasons. Nasopharyngeal swab sample testing for pathogens provides valuable references for the identification of clinical pathogens.
5.Practice and effect evaluation of refined setting of evidence-based reference prescription pre-audit rules in anes-thesiology department

OBJECTIVE To optimize the pre-audit rules for anesthesia prescriptions, improve the audit quality and rational drug use. METHODS The inpatient medical orders of anesthesia department from prescription pre-audit system of the Affiliated Hospital of Qingdao University (hereinafter referred to as “our hospital”) were analyzed from April 1 to 30, 2023. The classification statistics and evidence-based inquiry were carried out for irrational drug use issues; combined with our hospital’s current implementation of the Anesthesiology Clinical Pathway Medication Standards, the audit rules were set in details, and audit management and communication feedback processes were established. The total number of monthly audited orders, the number of pre-audit pop ups, system interception rate, physician modification rate after system audit, pharmacist audit rate, and reasonable rate of medical orders after refined setting of rules (May-December in 2023) were compared with before setting (April in 2023); the average medication cost per anesthesia session after refined setting of rules was also compared with before setting (May- December in 2022). RESULTS Irrational drug use in the anesthesia department mainly included inappropriate indications, inappropriate administration routes, inappropriate usage and dosage, inappropriate compatibility, medication problems in special populations, and improper medication during the perioperative period. After the refinement of the rules, the number of pre-audit pop ups in the anesthesia department significantly decreased over time, and gradually reached a stable state after continuous improvement. Compared with before setting, the system interception rate after the refinement of rules (P<0.001), physician modification rate after system audit (P<0.001) both increased significantly, while the pharmacist audit rate significantly decreased (P<0.001). There was a linear trend between the reasonable rate of medical orders and the month from May to December in 2023 (P<0.05). Compared with before the setting, the average medication cost of anesthesia per session decreased from 720.72 yuan to 528.21 yuan, with a decrease of 26.71%. CONCLUSIONS Based on evidence-based reference, refining pre-audit rules for anesthesia prescriptions can significantly improve the quality of prescription examination, promote rational drug use, and save patient’s medical expenses.
6.Analysis of 9 patients with adolescence-onset methylenetetrahydrofolate reductase deficiency

Objective:To explore the diagnosis and treatment of adolescence-onset methylenetetrahydrofolate reductase (MTHFR) deficiency.Methods:This was a retrospective case study. Nine patients with adolescence-onset MTHFR deficiency were diagnosed at Peking University First Hospital from January 2016 to December 2022, and followed up for more than 1 year. Their general information, clinical manifestations, laboratory tests, cranial images, MTHFR gene variants, diagnosis, treatment, and outcome were analyzed retrospectively.Results:The 9 patients came from 8 families. They had symptoms at age of 8.0 years to 17.0 years and diagnosed at 9.0 years to 17.5 years. Eight were male and 1 was female. Two patients were brothers, the elder brother developed abnormal gait at 17.0 years; and the younger brother was then diagnosed at 15.0 years of age and treated at the asymptomatic stage, who was 18.0 years old with normal condition during this study. The main manifestations of the 8 symptomatic patients included progressive dyskinesia and spastic paralysis of the lower limbs, with or without intellectual decline, cognitive impairment and behavioral abnormalities. Totally, 15 variants of MTHFR gene were identified in the 9 patients, including 8 novel variants. Five patients had brain image abnormalities. Increased plasma total homocysteine level (65-221 μmol/L) was found in all patients, and decreased to 20-70 μmol/L after treatment with betaine and calcium folinate. Besides, the 8 symptomatic patients had their behavior and cognitive problems significantly improved, with a legacy of lower limb motor disorders.Conclusions:Late-onset MTHFR deficiency can occur in adolescence. The diagnosis is usually delayed because of non-specific clinical symptoms. The test of blood total homocysteine could be used as a selective screening test. Eight novel varients of MTHFR gene were identified. Timely treatment can improve clinical condition significantly, and pre-symptomatic treatment may prevent brain damage.
7.Clinical features and follow-up study on 55 patients with adolescence-onset methylmalonic acidemia

Objective:To investigate the clinical features and outcomes of adolescence-onset methylmalonic acidemia (MMA) and explore preventive strategies.Methods:This was a retrospective case analysis of the phenotypes, genotypes and prognoses of adolescence-onset MMA patients. There were 55 patients diagnosed in Peking University First Hospital from January 2002 to June 2023, the data of symptoms, signs, laboratory results, gene variations, and outcomes was collected. The follow-ups were done through WeChat, telephone, or clinic visits every 3 to 6 months.Results:Among the 55 patients, 31 were males and 24 were females. The age of onset was 12 years old (range 10-18 years old). They visited clinics at Tanner stages 2 to 5 with typical secondary sexual characteristics. Nine cases (16%) were trigged by infection and 5 cases (9%) were triggered by insidious exercises. The period from onset to diagnosis was between 2 months and 6 years. Forty-five cases (82%) had neuropsychiatric symptoms as the main symptoms, followed by cardiovascular symptoms in 12 cases (22%), kidney damage in 7 cases (13%), and eye disease in 12 cases (22%). Fifty-four cases (98%) had the biochemical characteristics of methylmalonic acidemia combined with homocysteinemia, and 1 case (2%) had the isolated methylmalonic acidemia. Genetic diagnosis was obtained in 54 cases, with 20 variants identified in MMACHC gene and 2 in MMUT gene. In 53 children with MMACHC gene mutation,1 case had dual gene variants of PRDX1 and MMACHC, with 105 alleles. The top 5 frequent variants in MMACHC were c.482G>A in 39 alleles (37%), c.609G>A in 17 alleles (16%), c.658_660delAAG in 11 alleles (10%), c.80A>G in 10 alleles (10%), c.567dupT and c.394C>T both are 4 alleles (4%). All patients recovered using cobalamin, L-carnitine, betaine, and symptomatic therapy, and 54 patients (98%) returned to school or work.Conclusions:Patients with adolescence-onset MMA may triggered by fatigue or infection. The diagnosis is often delayed due to non-specific symptoms. Metabolic and genetic tests are crucial for a definite diagnosis. Treatment with cobalamin, L-carnitine, and betaine can effectively reverse the prognosis of MMA in adolescence-onset patients.
8.Optimization of processing technology and quality characterization of wine-steamed Taxillus chinensis

OBJECTIVE To optimize the steaming and processing technology of wine-steamed Taxillus chinensis, and to characterize its quality. METHODS Using the content of avicularin, quercitrin, quercetin and appearance traits as evaluation indicators, the analytic hierarchy process (AHP)-entropy weight method was used to determine the weights of each indicator, and the comprehensive scores of those indicators were used as response values. Box-Behnken response surface method was used to investigate the effects of solid-liquid ratio (g/mL), soaking time, and steaming time on the processing technology of wine-steamed T. chinensis, optimize the best processing technology, and verify it. Fifteen batches of T. chinensis decoction pieces from different origins were used to prepare wine-steamed T. chinensis using the best processing technology, and their qualities were characterized. RESULTS The optimal processing technology for wine-steamed T. chinensis was to take 100 g of T. chinensis decoction pieces, add 20 mL of yellow wine, seal and moisten for 2 h, steam at normal pressure for 1 h, take out and dry at 50 ℃. The surface of wine-steamed T. chinensis prepared by the optimal processing technology was reddish brown or brownish, and its powder was dark brown, with a hard or brittle texture that was easy to break, and had a slight aroma of alcohol, and an astringent taste. Results of microscopic and thin-layer identification for the stem cross-section of wine-steamed T. chinensis were the same as those of raw T. chinensis. The contents of moisture, total ash and acid-insoluble ash were 3.92%-8.75%, 2.27%-5.08%, and 0.19%-0.82%, respectively; the contents of water-soluble extract were 11.28%-18.56%, and the contents of alcohol-soluble extract were 3.36%-8.58%; the contents of avicularin, quercitrin, and quercetin were 0.22-1.64, 0.26-2.45, and 0.01-0.38 mg/g, respectively. CONCLUSIONS This study successfully optimized the processing technology of wine-steamed T. chinensis and preliminarily characterized its quality, which can provide reference for the standardized processing and establishment of quality standards for wine-steamed T. chinensis decoction mail:wyl@sxtcm.edu.cn pieces.

Objective To optimize the formula of Dahuang Xiaoshi decoction components based on its hepatoprotective activity and evaluate their efficacy.Methods The Wistar rats were randomly divided into blank group,model group,ursodeoxycholic acid group(positive group),and orthogonal groups of Dahuang Xiaoshi decoction components.The acute liver injury model induced by alpha-napthyl-i sothiocyanate(ANIT)was used to optimize the allocation ratio of Dahuang Xiaoshi decoction components by taking liver function indicators as an index and combining with multiple statistical methods.The additional Wistar rats were taken to induce liver injury and optimize the compatible dosage of Natrii Sulfas(0,1,2,4 g)in Dahuang Xiaoshi decoction components based on the biological signs and liver function biochemical indicators.On this basis,the Wistar rats were randomly divided into blank group,model group,ursodeoxycholic acid group and different dosages(low,medium and high)of the Zhibaihuang components group.The liver protective effect of Zhibaihuang components was systematically evaluated through the general biological signs,liver function biochemical indicators,lipid peroxide indicators,liver pathological examination and bile transport-related indicators.Results Dahuang Xiaoshi decoction components optimized by orthogonal and multiple statistics could improve the biological signs and ameliorate the biochemical abnormalities in rats with liver injury.Dahuang Xiaoshi decoction components combined with different dosages of Natrii Sulfas could slow down the mass loss of ANIT-induced acute liver injury rats(P<0.01 or P<0.05)and recall the abnormally elevated serum liver function enzyme activities(P<0.01 or P<0.05).Except for alkaline phosphatase(ALP),there was no statistical difference in regulating other liver function enzyme activity between different allocations of Natrii Sulfas.After comprehensive consideration,its composition was optimized to the Zhibaihuang components without Natrii Sulfas.Further pharmacodynamic evaluation results showed that the optimized Zhibaihuang components could improve their abnormal biological signs(P<0.01 or P<0.05),decrease the serum liver function enzyme activity(P<0.01 or P<0.05)and the levels of T-BiL and TG(P<0.05),and restore the levels of hepatic lipid peroxide(P<0.01 or P<0.05),repair liver pathological injury,adjust the expression of bile transport proteins(P<0.05),and thus exert good liver protective activity.Conclusion The optimized Dahuang Xiaoshi decoction components,Zhibaihuang components,was obtained through orthogonal,multiple statistics and univariate investigation.It could improve the abnormal biological signs of animals,protect the liver and reduce enzymes,resist lipid peroxidation,restore abnormal metabolic indicators,and repair liver pathological injury,which provides a reference for its further clinical application and development.
