1.Clinical observation of Zhisou Huatan capsules combined with levofloxacin in the treatment of acute exacerbations of chronic bronchitis

Objective To investigate zhisouhuatan capsule combined with levofloxacin in the treatment of patients with acute exacerbation of chronic bronchitis.MethodsRetrospective analysis was used to analysis the clinical data of patients with acute exacerbations of chronic bronchitis treated in our hospital from February 2015 to December 2016,according to the treatment methods, which were divided into levofloxacin treatment group and zhisouhuatan capsule combined with levofloxacin in the treatment group.After 4 weeks treatment, the symptom scores, lung function, inflammatory cytokine levels and side effects of the two groups were observed before and after treatment.ResultsThe symptom score of the two groups of patients before treatment had no difference, after treatment, the cough, expectoration and lung auscultation score of combined treatment group were lower than levofloxacin group (t=9.039, 3.574, 7.962, P<0.001);There was no difference between the two groups before treatment.After treatment, the levels of FEV1, FEV1/FVC, MMEF, PEF and FVC in the combined treatment group were higher than that in the levofloxacin group;There was no difference in serum inflammatory cytokine levels between the two groups before treatment.After treatment, the levels of IL-8, hs-CRP and TNF-in the combined treatment group were lower than that in the levofloxacin group (t=25.301, 3.615, 5.733, P<0.001);After 4 weeks of treatment, the blood and urine test results showed that the two groups of patients were not out of liver and kidney dysfunction, with good safety.ConclusionZhisouhuatan capsule combined with levofloxacin on acute exacerbation of chronic bronchitis has good curative effect, can significantly improve symptoms and pulmonary function of patients, with clinical application value.
2.Evaluation of Resectability for Carcinoma of the Gallbladder with Dual-phase Multislice Spiral CT

Objective To evaluate the value of dual-phase MSCT with 3D reconstruction in the staging and resectability of gallbladder carcinoma.Methods Forty patients with suspected gallbladder carcinoma by clinical examination and routine sonography were prospectively studied with dual-phase MSCT. Of these patients, only 35 who underwent a laparotomy for extended cholecystectomy or a palliative surgery were included in the study. Three-dimensional volume rendering and multiplanar reformation was used for evaluation of the vascular invasion and anatomy. The staging and resectability of lesions determined by CT and the results were compared with operative findings.Results On the basis of the CT findings, 13 tumors were resectable and 22 were unresectable. On surgery, 17 tumors were found to be resectable and the remaining was unresectable. Overstaging by CT occurred in 4 patients due to overassessment of duodenal infiltration, pancreatic infiltration and colonal infiltration. CT had a sensitivity of 76.5, a specificity of 100, an accuracy of 88.6, a positive predictive value of 100 and negative predictive value of 81.8 in determining resectability of gallbladder carcinoma. For the diagnosis of hepatic and vascular invasion, and involvement of bile duct by the tumor, there was 100 correlation between CT and surgery. Vascular variations were found in six of 17 patients who underwent radical cholecystectomy.Conclusion Dual-phase MDCT with 3D reconstruction can predict resectability of gallbladder carcinoma and determine the vascular road map before surgery.

4.Study on purification and immunologic effect of 42KD protein from group B Streptococcus

Objective To look for a vaccine on the base of surface protein which can elicit protective immunity.Methods Purification of 42KD protein from Streptococcus serotype Ⅲ was used one-step chromatography.Antibody response was evaluated in mice immunized by 42KD protein with different immunization dose and by the different immunization procedure.After immunizing the mice in the optimum procedure,we challenge the mice in intradermal with Streptococcus to observe the active and passive protection.Results The molecular weight(Mr)of the protein was 42.33×103 and its point isoelectric(pI)was 6.6.Its property might be a glycoprotein which could be released to supernatant.The highest titer of antibody against 42KD protein Was 1:24576.Which indicated that 42KD protein had good immunogenicity.The immunity exponent of NIH mice was 100 in test group and infant mice was 40.Conclusion The data suggests that 42KD protein may elicit protective immunity to adult and infant mice.Through protecting test with different immunizing doses,the findings indicate that protecting capability is relative to the titer of IgG specificantibody.
5.The effect of saccharomyces boulardii combined with chiropractics in asphyxial newborn with ;feeding intolerance

Objective To observe the effect of saccharomyces boulardii combined with chiropractics in asphyxial newborn with feeding intolerance. Methods One hundred and twenty asphyxial newborn with feeding intolerance were allocated into observation group (68 patients) and control group (52 patients) by random digits table. The newborn in control group accepted trace feeding and parenteral nutrition treatment, and the newborn in observation group accepted additional oral saccharomyces boulardii and chiropractic therapy on the base of control group. The time of reaching to full enteral feeding, the time of restoring to birth weight, the incidence of hyperbilirubinemia and the total efficiency of treatment were observed. Results The time of reaching to full enteral feeding, the time of restoring to birth weight in either mild asphyxial newborn or severe asphxial newborn in observation group were significantly lower than those in control group (P<0.01 or<0.05). The incidence of hyperbilirubinemia in mild asphyxial newborn or severe asphxial newborn in observation group was 16.7%(8/48) and 5/20, in control group was 36.8%(14/38) and 9/14, and there were significant differences (P<0.05). The total efficiency of treatment in observation group was significantly higher than that in control group:91.2%(62/68) vs. 69.2%(36/52), P<0.05. Conclusions Saccharomyces boulardii combined with chiropractics in asphyxial newborn with feeding intolerance can reduce the time of reaching to full enteral feeding and the time of restoring to birth weight. It can decrease the incidence of hyperbilirubinemia, and the treatment effect is obvious.
6.Clinical research of echocardiography in the assessmet of persistent pulmonary hypertension of the newborns

Objective To observe the parameters of echocardiography in persistent pulmonary hypertension of the newborn(PPHN),and to discuss the influence of persistent pulmonary hypertension on cardiac geometry and left ventricul ar function.Methods 25 cases with PPHN were selected,including 11 mild,9 moderate,and 5 severe cases.Another 23 cases of neonatal jaundice were selected as control group.The left atrium diameter(LAD),left ventricular end diastolic diameter (LVDd),right ventricular end diastolic diameter (RVDd),right atrium diameter (RAD),left ventricular posterior wall thickness (LVPW),interventricular septum thickness (IVS) were measured by echocardiography.The results of echocardiography were analyzed.Results Compared with the control group,the RVDd and RAD of PPHN group were significantly increased,RVDd and RAD dilated with the increasing of pulmonary artery systolic pressure in PPHN group (t =2.53,7.09,2.14,4.77,2.88,2.04,4.59,7.12,2.34,5.12,3.07,all P <0.05) ;LVDd decreased in severe group (14.80 ±2.16) mm compared with that in the control group (15.26 ±1.83) mm,mild (16.55 ± 1.50) mm and moderate group (15.67 ± 1.87) mm(t =3.76,4.80,3.74,all P < 0.05).Conclusion Echocardiography is one of the most important method to diagnose PPHN,and can be observed in newborns with non-invasive cardiac changes directly.lt has important value in judgement of severity degree,and provide a reliable basis for clinical diagnosis and treatment.

Objective To observe the protective effect of acceleration neutruphils apoptosis on lung injury after cardiopulmonary bypass(CPB) . Methods The neutrophils were separated using discontinuous Percoll gradients in vitro and cultured for 48 hours with or without clarithromycin. Neutrophil survival was determined by typan blue dye exclusion method. Apoptotic neutrophils were detected with flow cytometry. Immune histochemistry was used to examine the expression of protein of Fas and bcl-2 which interrelated with neutrophil apoptosis. In vivo study twelve adult sheeps were randomly divided into two groups. Pulmonary artery was clamped after CPB was established and the lungs were perfused with 4℃ low molecular weight dextran solution in control group (n = 6) and 4℃ low molecular weight dextran solution with clarithromycin in protective group (n - 6), The bypass was withdrawn after 90 minutes. Respiratory function were recorded during CPB and cytokines in the lung were measured. Lung biopsies and the apoptosis of the neutrophil were also performed after operation. Results Clarithromycin significantly shortened neutrophil survival in a dose-dependent fashion. The result of flow cytometry showed that the apoptosis rates at 24h of clarithromycin group were (33.7?4.9)%, (48.0?4.9)%, (52.0?5.4)% and (53.0?7.1)% at 1, 5, 10 and 20 ?g/ml of clarithromycin respectively while the rate in control group was (31.5?3.5)% (P
9.New primary mutation of mtDNA in Leber′s hereditary optic neuropathy

Objective To analyze the new primary mutation in Chinese people with Leber′s hereditary optic neuropathy (LHON). Methods Genomic DNA was collected from 260 suspected LHON patients and 100 normal healthy persons. The mitochondria DNA mutation at nucleotide position (NP) 15257 and the hot spot (14452-14601 bp) of ND6 gene which include the mutations at NP (14482, 14498, 14568, 14596, 14495, and 14459) were screened by using polymerase chain reaction (PCR), heteroduplex-single strand conformation polymorphism (HA-SSCP) and restriction fragment length polymorphism (RFLP) analysis and sequencing. Primary mutation spectrum of Chinese race was analyzed. Results Eight kinds of polymorphism of mitochondria DNA were found in 260 suspected LHON patients and 100 normal healthy persons, including NP 14488C, 14518G, and 14617G which hadn't been reported (http://www.mitomap.org/). No mutation at NP 15257, 14482, 14498, 14568, 14596, 14495, and 14459 was found. Conclusion The NP 15257A may not be the primary mutation in Chinese. Because of the race difference, 14452-14601 bp in ND6 gene may not be the hot spot in Chinese patients with LHON, and other hot spots may exist.
10.Influence of rehabilitation for patients with dysphagia following stroke: observation of patients' nutriture and analysis of medical costs

Objective To explore the effects of comprehensive rehabilitation on nutriture and medical costs in the treatment of patients with dysphagia following stroke. Methods Eighty-seven patients with dysphagia were divided into an intervention group (IG) and a control group (CG). Comprehensive rehabilitation therapy including deglutition training, electric neuromuscular stimulation and acupuncture were used with the IG patients, but not with those in the CG. Nutriture parameters included a biochemistry index and a body measurement index. Medical costs were observed before and after one month of treatment for both groups. Results After one month of treatment, average deglutition scores and nutriture indices were significantly higher in the IG than in the CG. Medical costs for the IG had decreased significantly compared with the CG. Conclusions Comprehensive rehabilitation therapy improved deglutition function and slowed nutriture deterioration, while decreasing medical costs in the treatment of patients with dysphagia following stroke.
