1.Endotoxin Inhibits Orphan Opioid Receptor Gene Expression in Peripheral Blood Lymphocyte

Objective: To investigate the effects of endotoxin stimulation on the expression of orphan opioid receptor (ORL-1) gene in peripheral blood lymphocyte in vivo. Methods: Total RNA was extracted from peripheral blood lymphocytes, which have been incubated with variable lipopolysaccharide concentrations(0, 0.1, 10,100ng/ml) for different periods of time(0, 3, 6, 12, 24h). The level of mRNA was measured with the method of semi-quantitative reverse transcription polymerase chain reaction(RT-PCR) using GAPDH as an internal standard. The specificity of the amplified DNA fragments was verified by Southern blot. Results: The level of ORL- 1 mRNA in peripheral blood lymphocyte was 1.060±0. 390 under the basal condition. An endotoxin dose-and time-dependent inhibition of ex vivo ORL-1 gene expression in lymphocyte was observed. A significant descrease of the ORL-1 mRNA was detected in lymphocyte at 3 hrs with the stimulation of 100ng/ml of endotoxin, at 6 hrs with the stimulation of 10ng/ml of endotoxin, at 12 hrs with the stimulation of 0.1ng/ml of endotoxin.No significant change of ORL-1 mRNA level was found without the stimulation of endotoxin. Conclusions: Endotoxin inhibits the expression of ORL-1 gene in normal human peripheral blood lymphocyte. The level of ORL-1 mRNA is related to the functional state of lymphocyte. Nociceptin(OFQ) may have an important immunological function.

Objective To observe the effect of Pingchuanning on the levels of LTB4 and sICAM-1 of guinea pigs with asthma. Methods The model guinea pigs of asthma were sensitized with ovalbumin (OVA),the changes of the delitescence of asthma was observed. The levels of LTB4 in serum and sICAM-1 in bronchoalveolar lavage fluid (BALF) were also observed by means of enzyme-linked immunosorbent (ELISA). Result Comparing with the model group,the delitescence of asthma was obviously prolonged in guinea pigs treated by Pingchuanning. 3.Therapentic effect of percutaneous balloon compression versus percutaneous radiofrequency thermocoagulation on idiopathic trigeminal neuralgia in aged patients

Objective To compare the effect of percutaneous balloon compression (PBC) versus percutaneous radiofrequency thermocoagulation treatment (PRFT) on idiopathic trigeminal neuralgia in elderly patients. Methods The 87 patients with idiopathic trigeminal neuralgia were enrolled, 42 patients were treated by PBC (PBC group), and the other 45 patients were treated by PRFT (PRFT group). The effects of two therapies were evaluated after treatment. Results The early response rate (92.9% vs. 95.6%) and cure rate (88.1% vs. 88.9%) of PBC group versus PRFT group had no significant differences (both P>0.05). The inhibitory response rate of trigeminal nerve was significantly greater in PBC group than in PRFT group after treatment (69.0% vs. 35.6%, P<0.05), and 1 patient had cardiac arrest in PBC group. The two groups had different degrees of facial numbness and sensory dysfunction. The 39 patients treated by PBC had numbness in three divisions of trigeminal nerve, while 43 patients treated by PRFT had the high selection and showed the main numbness in the ill division of trigeminal nerve. The chewing gravis rate was higher in PBC group than in PRFT group (71.8% vs. 20.9%, P<0.05). There was no neuralgia recurrence in both groups during 6 monthe′ follow-up. Conclusions The effect of treating the elderly patients with idiopathic trigeminal neuralgia is similar by PBC and PRFT. The PBC therapy under general anesthesia is more convenient for patients who can not cooperate.The PRFT has much higher selectivity in trigeminal nerve damage, lower inhibition rate of the trigeminal nerve, and less influence on the masticatory muscle function.

Objective The purpose of this study was to investigate the effects of propofol on the platelet function.Methods Thirty ASA Ⅰ-Ⅱ patients undergoing elective minor surgery were allocated into two groups: propofol group (n=20) and control group (n=10). The mean age of the patients was (37?8)yr. Patients who had blood disease or had been exposed to any medication with known platelet effects were excluded. The patients were premedicated with phenobarbital sodium 2mg?kg -1 and atropine 0.01 mg?kg -1. In propofol group anesthesia was induced with propofol 2 mg?kg -1, fentanyl 4?g?kg -1 and vecuronium 0.1 mg?kg -1 and maintained with 0.8%-1.2% isoflurane inhalation supplemented with fentanyl and vecuronium. The duration of operation averaged (85?15)min. Blood samples were taken from peripheral vein before induction of anesthesia, 5 and 30 min after induction and 1h after termination of propofol infusion for the assessment of CD62P,CD63 and CD41/CD61 on the platelet membrane surface by flow cytometry. Platelet count, bleeding time and ACT were also determined at the same time.Results Following the administration of propofol the expressions of CD62P and CD63 on the platelet membrane surface were significantly decreased, whereas the expression of CD41/CD61, platelet count, bleeding time and ACT did not change significantly.Conclusions Propofol inhibits the expression of platelet membrane glycoproteins CD62P and CD63 and may contribute to the impairment of platelet function.
7.DIAGNOSIS AND TREATMENT OF DIFFUSE AXONAL INJURY

Objective To provide a description of clinical manifestations, special imaging features and treatment of diffuse axonal injury(DAI).Methods Retrospective analysis of 32 cases of diffuse axonal injury.Result 87.5% patients were involved in traffic accident. All patients came into persistent comatose state immediately. 75% patients had abnormal CT scan findings. MRI showed more sensitive. It could reveal injury appearance of corpus callosum and brain stem clearly. Hypothermic therapy and calcium ion antagonist were used. Conclusions combination of clinical manifestations and special imaging features helped to diagnosis of DAI in early stage. Intensive care and preventing secondary brain injury were the important treatment points.

Objective The purpose of this study was to investigate the effects of cardiopulmonary bypass(CPB) on monocyte HLA DR expression and immune function Methods Sixteen ASA Ⅱ Ⅳ patients [mean age (38 5?5 1)yr,mean body weight (42 9?10 2kg)] of both sexes (male 7,female 9) with rheumatic value disease undergoing mitral value replacement (11 patients) and mitral value and aortic value replacement (5 patients) with moderate hypothermic CPB were enrolled in this study Another ten patients undergoing lung lobectomy were used as control group Patients suffering from infection, immuno deficiency or receiving corticosteroid or immunoregulatory drugs were excluded Premedication included intramusallar midazolam 0 1mg?kg -1 and morphine 0 1mg?kg -1 Anesthesia was induced with intravenous midazolam 0 15mg?kg -1 , fentanyl 8?g/kg and vecuronium 0 1mg?kg -1 and maintained with isoflurane inhalation (0 8%~1 2%) and intermittent boluses of fentanyl and vecuronium The total dose of fentanyl was (18 3?3 2)?g/kg in CPB group and (9 6?1 3)?g/kg in control group CPB time was (74 2?22 5) min and aortic clamping time (57 5?19 2) min Blood samples were taken from CVP line before and 5 min after induction of anesthesia, before and after CPB and on the 1st, 3rd and 5th postoperative day for determination monocyte count and percentage of HLA DR + monocytes Results The number of monocytes decreased at the end of surgery but greatly increased on the postoperative days The percentage of HLA DR + monocytes also decreased at the end of surgery and decreased further on the 1st postoperative day but started increasing on the 3rd postoperative day In two patients who developed infection the percentage of HLA DR + monoeytes was lower than the average percentage of HLA DR + monocytes of the other 14 CPB patients In control group the percentage of HLA DR + monocytes also decreased significantly after operation but there was no significant change in the number of monocytes However there was a significant difference in monoeyte HLA DR expression between the two groups Conclusions The changes in monocyte HLA DR expression and monocyte count may be induced by impaired immune response after CPB and dynamic monitoring of monocyte HLA DR expression may help to predict complication of infection after CPB
10.Effects of different modes of mechanical ventilation on expression of ?-defensin-2 gene and protein in ventilator-associated pneumonia

Objective To evaluate the effects of conventional mechanical ventilation (CMV) and mechanical ventilation with ideal PEEP and permissive hypercapnia (PHY + PEEPi) on expression of ?-defensin-2 (HBD-2) gene and protein in ventilator-associated pneumonia (VAP). Methods Forty-eight male SD rats weighing 280-320 g were randomly divided into 2 groups: (1) CMV group (VT = 12 ml? kg-1 , RR = 70bpm, FiO2 = 1.0) (n = 24) and (2) PHY + PEEPi group (VT =6 ml?kg-1 , RR = 90bpm, PEEP = 0.2 kPa, FiO2 = 1,0) (n = 24) . The animals were anesthetized with intraperitoneal 20% urethane 1 ml?100 g-1 , tracheotomized and mechanically ventilated. After being ventilated for 24 h P. aeruginosa (3 ? 108 CFU/ml) 0.2 ml was introduced into trachea to induce pulmonary infection. Three animals were sacrificed at following intervals: before and 1.5 h, 3h,6h,12h,3d and 5 d after introduction of P. aeruginosa. Lung tissaes were obtained from middle and lower lobes of left lung for microscopic examination and determination of expression of mRNA of HBD-2 by RT-PCR and HBD-2 protein level by Western blot analysis. Right lung was lavaged and broncho-alveolar lavage fluid (BALF) was collected for bacteriological examination. Results There were significantly more severe pathological changes in the lung in CMV group as compared with PHY + PEEPi group. In CMV group the levels of up-regulation of HBD-2 mRNA and protein expression were significantly lower after 3h than those in PHY + PEEPi group. The positive rate of blood and BALF bacterial culture was also higher in CMV group. The survival rate of PHY + PEEPi group was 76% , significantly higher than that of CMV group (40% ) ( P
