1.Effect of early rehabilitative intervention on prognosis of patients with hypertensive cerebral hemorrhage

Objective To explore the effect of early rehabilitative intervention on prognosis of patients with hypertensive cerebral hemorrhage.Methods 60 patients with hypertensive cerebral hemorrhage were randomly divided into the observation group and the control group,each group 30 cases.Both two groups received common therapy,while the observation group was given early rehabilitative intervention.Results After treatment for one month,the total effective rate of the observation group was obviously higher than that of the control group (90.00% vs 73.33%,x2 =2.81,P < 0.05).After treatment for three months,the living ability of experiment group was obviously better than control group (66.67 % vs 50.00%,x2 =2.69,P < 0.05).Conclusion Early rehabilitative intervention in patients with hypertensive cerebral hemorrhage can increase clinical effect and improve prognosis.
2.Study on conformance between two kinds of detection method of anti-HBs antibody

Objective To analyze the conformance between the quantitative and qualitative tests of hepatitis B surface antibody (anti-HBs).Methods The chemiluminecence microparticle enzyme immunoassay(CMIA)was adopted to quantitatively detect anti-HBs and the enzyme linked immunosorbent assay(ELISA)was adopted to qualitatively detect anti-HBs.Results With the CMIA as the reference experiment,Se ,Sp ,J ,PV+ and PV-of anti-HBs detected by ELISA were 0.95,0.53,0.48,0.74 and 0.88 respec-tively,k=0.51;when the absorbance was 0.400 9,Se ,Sp ,J ,PV+ and PV-were 0.50,0.95,0.45,0.93 and 0.43 respectively;for the samples exceeding the absorbance range of 0.104 3 -0.400 9,Se ,Sp ,J ,PV+ and PV-qualitatively detected by ELISA were 0.90,0.91,0.81,0.93 and 0.88 respectively,k =0.81.Conclusion Determining cutoff value with the absorbance value 0.105 as the ELISA detection result has the good detection sensitivity(Se =0.95)and the better negative prediction value(PV-=0.88),the negative anti-HBs detected by ELISA may be considered that the anti-HBs concentration was less than 10 mIU/mL without the conservation value;when the sample absorbance ≥0.400 9,the anti-HBs concentration is ≥10 mIU/mL,which may be considered to have the conservation value;the gray area range of anti-HBs detected by ELISA is mainly the absorbance of 0.105-0.400 9,the true anti-HBs level should be quantitatively detected.
3.Effects of dexamethasone on toll-like receptor 4 mediated signaling pathways in mice dendritic cells

AIM: To observe the effects of dexamethasone on the expression of toll-like receptor 4(TLR4)and the activation of NF-?B of dendritic cells(DC),and investigate the mechanism of dexamethasone suppressing differentiation,maturation,and function of DC.METHODS: DC generated from C57BL/6 murine bone marrow cells were induced by GM-CSF and IL-4.Dexamethasone was added in the DC's culture system,the cells were analyzed by flow cytometry(FCM) to determine the expression of TLR4 & CD11c,and by electrophoretic mobility shift assay(EMSA) to verify the activation of transcription factor NF-?B of DC.RESULTS: Dexamethasone could suppress the expression of CD11c and the activation of NF-?B with a dose-effect relationship in DC,but it could not suppress the expression of TLR4 of DC.CONCLUSION: The mechanism of dexamethasone involved in inhibition of differentiation,maturation,and function of DC is perhaps related to suppression of NF-?B activation.
4.Risk factors for acute kidney injury in patients with acute ischemic stroke

ObjectiveTo investigate the risk factors for acute renal injury (AKI) in patients with ischemic stroke.MethodsPatients with ischemic stroke were enrolled retrospectively.The general clinical data, vascular risk factors, drug use, stroke etiological typing, stroke severity, and baseline biochemical indices were collected.They were divided into either an AKI group or a control group according to whether AKI occurred or not.Multivariable logistic regression analysis was used to analyze the independent risk factors for occurring AKI in patients with ischemic stroke.ResultsA total of 214 patients with ischemic stroke were enrolled, including 32 (14.95%) had AKI and 182 (85.05%) did not have AKI.The proportions of patients in heart failure (62.50% vs.41.21%;χ2=4.998, P=0.025), mannitol use (87.50% vs.43.96%;χ2=20.643, P<0.001), furosemide use (87.50% vs.43.96%;χ2=20.643, P<0.001), contrast agent use (37.50% vs.19.23%;χ2=5.300, P=0.021), and contrast dosage >200 ml (28.13% vs.9.89%;χ2=6.637, P=0.010), as well as NIHSS score (18.0±4.5)vs.8.0±3.2;t=15.249, P<0.001), diastolic blood pressure (89.98±9.1 mmHg vs.80.56±8.19 mmHg, 1 mmHg=0.133 kPa;t=5.898, P<0.001), fasting blood-glucose (10.54±4.31 mmol/L vs.6.32±1.32 mmol/L;t=5.898, P<0.001), blood urea nitrogen level (11.21±2.13 mmol/L vs.7.98±2.34 mmol/L;t=7.293, P<0.001), and arterial lactate concentration (3.98±0.12 mmol/L vs.0.91±0.25 mmol/L;t=68.003, P<0.001) in the AKI group were significantly higher than those in the non-AKI group.Multivariate logistic regression analysis showed that after adjusting various confounding factors, higher NIHSS score (odds ratio [OR] 1.910,95% confidence interval[CI] 1.517-6.012;P=0.024), higher diastolic pressure (OR 1.816, 95% CI 1.652-3.876;P=0.018), arterial lactate concentration (OR 1.553, 95% CI 1.256-1.763;P=0.019), mannitol use (OR 3.765, 95% CI 2.081-9.658;P=0.017), furosemide use (OR 5.329, 95% CI 3.085-8.763 P=0.010), contrast agent use (OR 2.097, 95% CI 1.364-2.456;P=0.031), and contrast dosage >200 ml (OR 3.294, 95% CI 1.464-2.786;P=0.021) were the independent risk factors for AKI in patients with acute ischemic stroke.ConclusionsThe NIHSS score, diastolic blood pressure, arterial lactate concentration,mannitol use, furosemide use, contrast agent use and contrast dosage >200 ml were associated with AKI in patients with ischemic stroke.
5.Analysis on CTPA Image Quality by Using High and Low Concentration Contrast Agent with Various Injected Flow Velocities

Purpose To compare the CTPA image quality by using contrast agent with different concentration at different injection rate so as to provide suitable contrast agent injection for patients.Materials and Methods A total of 346 patients with suspected acute pulmonary embolism who required to undergo CTPA examination were randomly assigned to high (370 mgI/ml) and low (320 mgI/ml) concentration groups,and each group was further divided into six subgroups with different velocity (3.0,3.2,3.4,3.6,3.8 and 4.0 ml/s).The CT value of the main pulmonary artery,right pulmonary upper lobe artery and right lung under leaf posterior basal segmental artery was measured.Results In the high concentration group,there were no significant differences in pulmonary artery average CT value,noise,single to noise ratio (SNR) and contrast to noise ratio (CNR) among the subgroups with different velocity (P>0.05).In the low concentration group,the difference was not statistically significant in pulmonary artery average CT value (P>0.05) among the subgroups with different velocity;however,the noise,SNR and CNR of 3.0 ml/s subgroup had significant differences compared with other subgroups (P<0.05).There was no significant difference in average CT value of pulmonary artery between the subgroups with the same velocity in the two concentration groups (P>0.05).In addition,except that the noise,SNR and CNR of 3.0 ml/s subgroup showed significant differences with other subgroups either in high concentration group or in low concentration group (P<0.05),there were no significant differences in the above-mentioned parameters among other subgroups with the same velocity in both groups (P>0.05).Conclusion Compared with high concentration contrast agent,the image obtained by using low concentration contrast agent shows no difference in pulmonary artery average CT value but with low iodine flow and iodine flow rate,which can reduce the risks of contrast media induced nephropathy (CIN) and contrast agent extravasation.
6.Reality of Negative anti-HBc and Positive anti-HBe Results of Testing HBV-M with ELISA

Objective To research the reality of ELISA testing results with negative anti-HBc and positive antiHBe.Methods CMIA was carried out to retest antiHBc and antiHBe of 105 samples which were initially tested to have negative antiHBc and positive antiHBe.Results Among the 105 samples retested by CMIA,there were three different results,positive antiH-Bc with positive antiHBe,negative antiHBc with negative antiHBe and positive antiHBc with negative antiHBe,whose pro-portions were 72.38%,21.91% and 5.71% respectively;the fasle negative rates of antiHBc were 78.10% in total and 93.33%,96.15% and 47.37% in 3 subgroup with S/CO 1.00~ 1.20,1.20~2.00 and 2.00~ 3.00,respectively;the true positive rates of antiHBe were 72.38% in total and 42.86%,88.14% and 56.25% in 3 subgroups with S/CO 0.00~0.10, 0.10~0.80 and 0.80 ~ 1.00,respectively.Conclusion HBV-M results with negative antiHBc and positive antiHBe by ELISA could give suggestive value and not reflect true information about antiHBc and antiHBe with three alternatives which would be obtained through retesting by a second assay.
7.Expression and adjuvant effects of the fusion peptide TBP5.

Thymopentin (TP5) and bursopentin (BP5) are both immunopotentiators. To explore whether the TP5-BP5 fusion peptide (TBP5) has adjuvant activity or not, we cloned the TBP5 gene and confirmed that the TBP5 gene in a recombinant prokaryotic expression plasmid was successfully expressed in Escherichia coli BL21. TBP5 significantly promoted the proliferation of thymic and splenic lymphocytes of mice. The potential adjuvant activity of the TBP5 was examined in mice by coinjecting TBP5 and H9N2 avian influenza virus (AIV) inactivated vaccine. HI antibody titers, HA antibodies and cytokines levels (IL-4 and IFN-γ) were determined. We found that TBP5 markedly elevated serum HI titers and HA antibody levels, induced the secretion of both IL-4 and IFN-γ cytokines. Furthermore, virus challenge experiments confirmed that TBP5 contributed to inhibition replication of the virus [H9N2 AIV (A/chicken/Jiangsu/NJ07/05)] from mouse lungs. Altogether, these findings suggest that TBP5 may be an effective adjuvant for avian vaccine and that this study provides a reference for further research on new vaccine adjuvants.

8.Cognitive function and quality of life and nutritional status in chronic renal failure patients of different dialysis modalities

ObjectiveTo assess the effect of hemodialysis(HD),hemodiafiltration(HDF) and hemodialysis(HD) and hemoperfusion(HDP) on the quality of life,cognitive function and nutritional status in chronic renal failure(CRF) patients.MethodsAll patients were randomly divided into HD,HDF and HDP group.Mini-mental state examination(MMSE),Wisconsin card sorting test (WCST),SF-36 were measured for cognitive function,quality of life and nutrition indicators test.ResultsAfter 3,6 months treatment,MMSE ( respectively ( 22.62 ±3.85),(25.10 ±2.26),(25.35 ±2.44)and (23.87 ±4.19),(25.10 ±2.26),(27.19 ±2.23)),WCST indicators were statistically significant in the three groups (P < 0.05 ) ;after 1,3,6 months treatment,the PF,RP,BP,GH,SF and nutrition indicators were significant difference too(P<0.05 ) ; HDP and HDF,HD groups were statistically significant(P< 0.05 ).ConclusionHDF and HDP obviously influence on long-term cognitive function,physical health domains and nutritional status in CRF patients,and superior to HD.
9.Surgical treatment of primary retroperitoneal tumors:an analysis of 62 cases

Objective To explore the experience in treatment of primary retroperitoneal tumors (PRPT).Methods The clinical data of 62 cases with primary relroperitoneal tumors from 2001 to 2007were studied retrospectively.Results The abdominal mass was the main symptom in PRPT.There were 29 cases of benign tumors and 33 cases of malignanat tunlors respectively.Complete resections were performed in 47 patients(76%),partial resection was conducted for 10 patients.Resection could not be performed in 5 patients so only biopsy was conducted,12 patients accepted combined organ resections,2 patients died.Conclusion Adequate preoperative preparation and excellent surgical technique are the keys to compele tumour resection.
10.Comparison of Flavonoids Contents in Scutellaria Barbata D.Don from Different Harvest Time

Objective To determine the optimum harvest time of Scutellaria barbata D.Don with flavonoids contents as the index. Methods The content of total flavonoids in Scutellaria barbata D. Don was determined by ultraviolet-visible spectrophotometry with scutellarin as standard control, and scutellarin and cosmosiin in Scutellaria barbata D. Don were determined by HPLC. Results The contents of total flavonoids and scutellarin in Scutellaria barbata D.Don of different harvest time were complied with the quality standards in the China Pharma Copoeia, and reached the peakin blossom . The contents of total flavonoids, scutellarin and cosmosiin were (45.74±0.95) ,(5.58±0.16 ) and (17.39±0.42) mg?g-1 , respectively. Conclusion The Scutellaria Barbata D.Don may be collected at the flowering time with luxuriant foliage.
