1.Analysis of patients with ovarian cancer recurrence after treatment of lymph node regions alone and prognosis of survival

Objective To explore effects,survival and prognostic factors of regional lymph node recurrence of ovarian cancer patients after surgery in our hospital.Methods 1 348 cases of ovarian cancer in our hospital,regional lymph nodes filter out the postoperative recurrence alone were selected,44 cases were analyzed through a complete medical history,then the recurrence characteristics,treatment,prognosis,and prognostic factors were obtained.Results According to the disease,44 patients were treated with the second surgery,radiotherapy or chemotherapy,completed remission after treatment in 36 cases,some improvement in 7 cases,1 case,no significant improvement.44 patients 5-year cumulative survival rate was 65.32%,lymph node dissection adjuvant chemotherapy group,local radiotherapy and chemotherapy group,chemotherapy group cumulative survival difference were statistically significant (Kaplan-Meierx2 =23.685,P =0.000);may affect prognostic factors univariate Log-rank analysis showed that age,the recurrence time,the number of tumor recurrence and prognosis (x2 =11.254,9.254,15.625,all P < 0.05) ; Cox regression analysis showed that age,time to recurrence,relapse foci were independent prognostic factors (P =0.000,0.006,0.000).Conclusion The patients with ovarian cancer recurrence after surgery alone has its unique regional lymph node characteristics,treatment should consider the factors affecting the prognosis.
2.Laparoscopy Combined with Uterine Aspiration for Tubal Interstitial Pregnancy and Cornual Pregnancy

Objective To discuss the efficacy of laparoscopy combined with uterine aspiration for tubal interstitial pregnancy and cornual pregnancy. Methods From January 2004 to January 2007,laparoscopy combined with the preservation of the oviducts was performed on 56 patients with tubal interstitial pregnancy or cornual pregnancy. During the operation,the ectopic pregnancy tissues were removed,and then uterine aspiration was carried out. Results The operation was completed in all of the cases without conversion to open surgery. One of the patients showed persistent ectopic pregnancy,and was cured by muscular injection of MTX injection. In this series,the rate of oviduct patency was 33.9% (19/ 56); 18 moths after the operation,the uterine pregnancy rate was 71.4% (40/56),ectopic pregnancy rate was 16.1%(9/56),and the secondary infertility rate was 1.2% (7/56). Conclusions It is safe and effective to treat tubal interstitial pregnancy or cornual pregnancy with laparoscopic operation combined with uterine aspiration.
3.Relationship between sleep disorder and treatment strategy in elderly patients with primary hypertension

Objective To explore the relationship between sleep disorder and management strategies in the elderly patients with primary hypertension.Methods The sleep status of 122 elderly patients with primary hypertension was evaluated by Pittsburgh Sleep Quality Index(PSQI) and Coping Style Questionnaire(CSQ).Results The mean score of PSQI was 8.34±3.81,and 63.93% (78/122) of the participants experienced sleep disorder.The most commonly used management strategy wag "problem-solving" (0.74±0.18) and "rationalization" (0.56±0.19).The management strategies were different by gender and age.The lowest score was found in the "ask-for-help" group(t=2.326,P=0.022),which was negatively related with sleep disorders (r=-0.208,P=0.022).Conclusions Intervention of management strategy in elderly patients with primary hypertension may help to improve their sleep quality.
4.The progress of radiotherapy in triple negative breast cancer

The triple negative breast cancer is one of importance in clinical subtypes of breast cancer, which is easy to recur and metastasis, and its prognosis is very poor.Radiotherapy, as an effective method for breast cancer,can reduce the risk of local recurrence.This article elaborates its characteristics,the progress of ra-diotherapy in the breast conserving surgery and modified radical mastectomy in triple negative breast cancer,when is the appropriate time for radiotherapy and radiotherapy sensitization,and hope that it will be helpful to the treat-ments.
5.The DTI in the evolution of vascular cognitive impairment mechanism based leukoaraiosis

Objective To explore the integrity of white matter changes in patients with vascular mild cognitive impairment (VaMCI)based Leukoaraiosis.Methods The patients which appeared white matter hyperintensities in subcortical were performed psychological testing,then 48 VaMCI patients and 18 without cognitive impairment volunteers(NC)were choosed and performed DTI scanning,to determine the white matter’s values of FA and ADC in the following bilaterally symmetrical regions of interest(ROI):entorhinal cortex lobe,hippocampal vault,anterior of corpus callosum,posterior of corpus callosum,inferior frontal lobe,superior temporal lobe and angular gyrus of occipital lobe. Then the differences of the data of DTI between two groups and the relevance between neuropsychological score and DTI’s data were analysed.Results ADC values were significantly higher of VaMCI group in the left inferior frontal lobe,left anterior of corpus callo-sum,left hippocampus vault and right angular gyrus compared with NC group(P <0.05),and in the left hippocampus ADC values and MoCA scores was negatively correlated(r=-0.34,P =0.02).There was no significant difference of FA values between groups. Conclusion The ADC values in many cognitive related areas was reduced in VaMCI patients.
6.Studies on Optimal Process for Active Compounds of Flos Datura by Microwave-assisted Extraction

【Objective】To optimize the process conditions for active compounds of Flos Datura by microwave-assisted extraction.【Methods】Uniform design was used to observe the effects of microwave power,microwave radiation time,solvent volume,soaking time and extracting time on the extraction of dry extract,total alkaloids,scopolamine of Flos Datura.The contents of total alkaloids and scopolamine of Flos Datura were determined by acid stain colorimetry and HPLC,and the obtaining rate of dry extract was measured by weight method.【Results】 The optimal process conditions for Flos Datura were as follows: soaking the medicinal material for 2 hours with 10 times of 75% alcohol,with continuous radiation for 25 min at the power of 730 W,at 65℃.【Conclusion】Microwave-assisted extraction is superior to soaking extraction method in increasing productivity and economizing the solvent.

ObjectiveTo explore the effect of Yinaotongluo Capsule on diffuse weighting image and enenery metabolism of ischemia-reperfuing rat.MethodsMagnetic resonance diffusion-weighted imaging(DWI) and 1H and magnetic resonance spectroscopy(MRS) were performed in different brain regions in focal cerebral ischemic-reperfusion injury model rats. ResultsYinaotongluo Capsule could significantly reduce the area and the intensity of infarction of rats by DWI 3 h and 5 d after treatment compared with the model group.The result of 1H MRS examination demonstrated Lac peak was lower and NAA peak higher than that of the model group. ConclusionYinaotongluo Capsule can effectually improve enenery metabolism during cerebral ischemia and reperfuing, so that to reduce the neuronal dysfunction and death in infarcts at the late stage.
9.Comparison of experimental autoimmune encephalomyelitis models induced by two kinds of proteolipid proteins

BACKGROUND: Experimental autoimmune encephalomyelitis has become the most classical animal model for multiple sclerosis. However, the experimental autoimmune encephalomyelitis model of China presented one-way course of disease. By using proteolipid protein 139-151 and proteolipid protein 178-191, relapse remitting experimental autoimmune encephalomyelitis models may be induced in SJL/J mice which were susceptible to immune, which have similar clinical situation, course and histologicallterations to multiple sclerosis.OBJECTIVE: To establish the relapse remitting experimental autoimmune encephalomyelitis mouse model induced by proteolipid protein, which has similar clinical situation, course and histological alterations to multiple sclerosis.DESIGN: Completely randomized controlled study.SETTING: The centre of Neuro-information, and Neurological Institute,General Hospital of Chinese PLA.MATERIALS: The study was carried out at the Laboratory of Neuro-pathology, General Hospital of Chinese PLA, from February to June 2004.Sixty female SJL/J mice with 8-12 weeks old were selected and randomly divided into proteolipid protein 139-151 group and proteolipid protein-178-191 group with 30 in each.INTERVENTIONS: After injected with proteolipid protein-139-151 or proteolipid protein-178-191, the models of relapse remitting experimental autoimmune encephalomyelitis were induced, and the body weight and neurological signs of each female SJL/J mouse were viewed. The tissue morphological changes of models were observed with hematoxylin and eosin and uxol fast blue stain.MAIN OUTCOME MEASURES: The neurological symptoms and signs,features of relapse and remitting and the perivascular inflammatory cell infiltration, demyelinated lesion of the model of experimental autoimmune encephalomyelitis mouse induced by two proteolipid protein peptides.RESULTS: All 60 mice entered the final analysis. ① Significant neurological symptoms, signs and features of relapse and remitting was manifested in the model of experimental autoimmune encephalomyelitis mouse induced by two proteolipid protein peptides. Obvious phenomena of perivascular inflammatory cuffing, satellitism, predominant perivascular inflammatory cell infiltration and demyelinated lesion were found in spinal and cerebral tissue. ②Changes of body mass: Before immunity, the body mass of mice in two groups was( 17. 84 ± 2.59) g and (17. 88 ± 0.52) g respectively. Onset of relapse of the mice in proteolipid protein-178-191 group was earlier and faster, their body mass had no distinctive change after immunization and the mean body mass was(23.52 ± 2.37) g till the 60th day. Meanwhile, Onset of relapse of the mice in proteolipid protein-139-151 group was later and slower. After the immunity, the body mass of mice was little decrease, and the body mass was (16. 70 ±0.46) g on the 60th day. ③ Neural functional scores: The highest functional scores in the two groups were not different(3.86 ± 1.10vs 3.71 ±1.05, t=0.49, P=0.628).CONCLUSION: The two different antigenic peptides of proteolipid protein can all cause the autoimmune response of central nervous system. Both models have the same characters of relapse and remitting and the severity has no significant difference. But compared with proteolipid protein 139- 151 group,onset and recover of the experimental autoimmune encephalomyelitis of the mice in proteolipid protein 178-191 group were earlier, as well as weight variance was larger, which maybe due to the different structure of two peptides.
10.Pax5 expression in Non-Hodgkin's Lymphomas and Acute Leukemias.

The Pax5 gene encodes the B-cell-specific activator protein which is a key regulator in development and differentiation of B-cell. We studied the expression of Pax5 in hematologic malignancies to evaluate the diagnostic utility as a B cell marker. Materials included 70 B cell lymphomas, 26 T cell lymphomas, 53 acute leukemias, and 6 multiple myelomas (MMs). Representative areas from the paraffinembedded tissues were selected for tissue microarray, and the expressions of Pax5 was immunohistochemically evaluated. Pax5 was strongly expressed in most of the B cell lymphomas; 44 of 47 diffuse large B cell lymphomas (93.6%), 15 of 16 marginal zone B cell lymphomas (93.8%), all 3 mantle cell lymphomas, 2 follicular lymphomas, and 2 Burkitt's lymphomas (100%). However, Pax5 was expressed in only one of 26 T cell lymphomas. Among leukemias, it was expressed in 10 of the 14 B acute lymphocytic leukemias (ALLs) (72.4%), but also in 3 of the 6 T ALLs (50%), 13 of the 26 acute myelogenous leukemias (AMLs) (50%) and in all 3 ALL arising in chronic myelogenous leukemias and 4 mixed B ALL and AML. In MMs, Pax5 was negative in all cases. We concluded that Pax5 is very useful B cell marker in classification of lymphomas, but not of acute leukemias.
