1.Optimization of Extract Technology of Total Favonoids in Fruit of Polygonum viviparum L.by Orthogonal Experiment

OBJECTIVE:To optimize the extract technology of the total Favonoids from the fruits of Polygonum viviparum L.METHODS: The content of Flavonoids was computed with rutin as standard substance, and the influence of different factors and levels on the extract technology were investigated by means of simple factor experiment and orthogonal experiment. RESULTS: The optimum extraction technology was as follows: with water-bath temperature at 80℃, the ratio of liquid to solid material at 8∶1 and the concentration of ethanol at 50%, reflux extracting total Favonoids twice(1 h at each time). Under this condition, the content of total Flavonoids was 9.6%. CONCLUSION: The extraction method is appropriate, simple and feasible, and it provided theoretic basis for the production of this product.
2.The experience of diagnosis and treatment of 121 cases of laparoscopic appendectomy

Objective To explore the operation al skills of laparoscopic appendectomy(LA) and to improve its proficiency.Methods The clinical data of 121 patients under went LA during February 2013 to May 2014 were summarized retrospectively.Results All the 121 cases were completed successfully with no complication.Conclusion With the development of laparoscope and accumulation of experiences LA can be safe and complications can be minimized if the this operation skill being obtained.
3.Introduction and Enlightenment of Compound Oral Contraceptives Usage Criteria and Guideline Issued by WTO,US and Europe Union

OBJECTIVE:To provide the reference for verifying and improving the usage criteria and guidelines of compound oral contraceptives(COCs)in China. METHODS:The usage criteria and guidelines of COCs were introduced and compared issued by WHO,US and European Union(EU). RESULTS&CONCLUSIONS:WHO has established 4 standards and guidelines for COCs, i.e. Medical Eligibilitu Criteria for Contraceptive Use(MEC),Selected Practice Recommendations for Contraceptive Use(SPR), Decision-making Tool for Family Planning Clients and Providers,Family Planning:A Global Handbook for Providers. According to the specific circumstances of the user,MEC has been divided into 4 levels according to the situation of users,i.e. suitable for use,use cautiously,relative contraindication,absolute contraindication. SPR mainly describes the specific issues of how to use vari-ous contraception methods safely and effectively. The reading card mode for Decision-making Tool for Family Planning Clients and Providers can query specific methods and steps of contraceptive informed choice fast. Family Planning:A Global Handbook for Providers can provide practical guide for the family planning service providers in the first line. On the basis of WHO MEC and SPR,some projects are modified and added in the US MEC and SPR,including the classification of breast feeding,the risk grade of DVT/PE recurrence,etc. EU have formulated COCs criteria and guidelines earlier than WHO and US;the key points and specif-ic varieties of COCs are prompted and updated based on the latest evidence- based medicine,including a new assessment of the risk of thrombosis after using the COCs,the special safety tips for the use of Diane 35? and its generics. Concerning the optimal ev-idences of current COCs usage of WHO,US and EU,combined with domestic situation,our country may provide the clear and up-to-date usage criteria and guidelines of COCs for its users and medical staff.
4.Interferon/poly (lactic-co-glycolic acid) and chitosan/poly (lactic-co-glycolic acid) composite membranes for prevention of epidural scar adhesion following laminectomy

BACKGROUND: Epidural resistance materials are an approach to prevent the epidural scar adhesion after laminectomy. A composite membrane of mechanical separation and fibroblast inhibition is promising to be widely studied.OBJECTIVE: To prepare the interferon (IFN)/poly (lactic-co-glycolic acid) (PLGA) and chitosan/PLGA composite membranes, and to observe their effects on preventing epidural scar adhesion after laminectomy in rabbits.METHODS: L2 laminectomy was performed in 120 rabbits, which were randomly and evenly divided into six groups. The control group received nothing on epidurals. In the autologous free fat group, the exposed dura was covered with subcutaneous fat. In the sodium hyaluronate group, 1 mL sodium hyaluronate was dripped onto the dura. In the PLGA, chitosan/PLGA, and IFN/PLGA groups, the exposed dura was covered with one of the three membranes at a sufficient size. The animals were sacrificed at 2, 4, 6, 8 weeks postoperatively. The scar formation and adhesion to the dura in L2 laminectomy sites were observed and scored by gross observation. At 4 weeks postoperatively, fibroblast ultrastructure was observed through the use of transmission electron microscope.RESULTS AND CONCLUSION: Widespread scars formed in the control group. The scars in the autologous free fat and PLGA groups were less compared with the control group, but there was no significant difference between these two groups. In the early stage, the scars in the sodium hyaluronate group were obviously less compared with the autologous free rat and PLGA groups, while in the later period, the difference between them was not significant, but the results from the sodium hyaluronate group were still better compared with the control group. The scars and slight adhesion to the dura were observed in the chitosan/PLGA and IFN/PLGA groups. The effect of preventing scar adhesion in these two groups was remarkably superior to that in the other groups, but there was no significant difference between these two composite membrane groups. At 4 weeks postoperatively, fibroblast morphology and function in the control, autologous free rat, and PLGA groups were better than those in the chitosan/PLGA and IFN/PLGA groups. Chitosan/PLGA and IFN/PLGA composite membranes are effective materials to prevent epidural scar adhesion after laminectomy.
5.Construction and Expression of Multi-epitopes Gene of Treponema pallidum and Immunoresponse of Recombinant Protein

OBJECTIVE To develop the strong and the specific multi-epitope antigen for the exploiting diagnosis of Treponema pallidum.METHODS The immuno-dominant epitopes of Tp0453 and Tp17 were amplified by PCR respectively,and subcloned into the expression vector pQE32 to generate multi-epitopes recombinant plasmid pQE32/Tp0453-17.The recombinant protein was expressed in Escherichia coli M15.The immunoresponse of recombinant fusion protein was analyzed by Western blot.RESULTS The multi-epitopes recombinant plasmid was successfully constructed,enzyme digestion analysis and sequencing showed that the inserted target genes were Tp0453 and Tp17 gene,compared with the gene reported by GenBank,it had 100% similarity;SDS-PAGE analysis showed the recombinant plasmid could be expressed in M15,its relatively molecular mass(Mr) of expressed product was about 52.0?103.The Western blot result showed the recombinant protein could be recognized by anti-T.pallidum positive serum.CONCLUSIONS The expressed multi-epitopes recombinant antigen showed excellent immunoresponse.The results lay the foundation for research on development of quick diagnostic kit applying to detection of T.pallidum infection.

7.Main Pathogens in Urinary Tract Infections:Analysis of Distributing Characteristics and Drug Resistance

OBJECTIVE To investigate the bacterial distribution and antibiotic resistance of main pathogens from urinary tract infection (UTI) in our hospital for the guidance of rational use of antibiotics. METHODS The antibiotic resistance of clinical pathogen isolates from urinary tract infection by routine bacterium culture from Jan 2006 to Dec 2007 were studied retrospectively. The extended spectrum ?-lactamases (ESBLs) were detected out to the Gram-negative bacilli. RESULTS There were 722 strains of pathogens in the whole 2773 urinary samples with the isolating rate 26.0%. Most of the Urinary tract infections of patients were caused by Gram-negative bacilli (69.4%),then by Gram-positive cocci (16.3%) and fungi (14.3%). The most common pathogens in urinary tract infection were Escherichia coli (47.9%),Enterococcus faecais (7.9%),E. faecium (4.0%),Proteus mirabilis (3.9%),and Pseudomonas aeruginosa (3.5%). The Gram-negative bacilli were found to be sensitive to imipenem and meropenem,but highly resistant to the most other antibiotics,while P. mirabilis was susceptive to amikacin and cephalosporins. The Gram-positive cocci were more sensitive to nitrofuvantoin and vancomycin,but highly resistant to penicillin,oxacillin and SMT. CONCLUSIONS E. coli is still the primary urinary pathogen among patients,and highly resistant to a lot of antibiotics,We should carry out cultivation,isolation,and antimicrobial susceptibility testing as soon as possible to guide reasonable clinical drug therapy.

Cytochrome P450 monoxygenase converts arachionic acid to four epoxyeicosatrienoic acid regiosomes: 5,6-EET(epoxyeicosatrienoic acid);8,9-EET; 11,12-EET and 14,15-EET.Recent studies show that EETs are involved in signal transduction. EETs open Ca 2+ -sensitive K + channel and inhibit Na + channel,Ca 2+ -sensitive Cl - channel and so on. What is more ,EETs have been demonstrated to activate PP60 c-src and initiate a tyrosine kinase cascade that mediates mitogenic effects.
