1.Clinical Observation on Modified Bushen Huoxie Decoction for the Treatment of Vascular Dementia

[ Objective ] To observe the therapeutic effect of Bushen Huoxie Decoction (mainly composed of Radix Rehmanniae Preparata, Fructus Corni, Rhizoma Dioscoreae, Carapax et Plastrum Testudinis, Radix Polygoni Multiflori, Fructus Lycii, Radix Morindae Officinalis, Herba Cistanches, Rhizoma Acori Tatarinowii, Radix Polygalae, etc. ) for the treatment of vascular dementia (VD) . [Methods] Eighty cases of VD were randomized into two groups by using random number table: group A ( n = 40) was treated with modified Bushen Huoxie Decoction and group B (n = 40) with duxil and nimodipine. After a 3-month treatment, the therapeutic effect of the two groups was evaluated, and mini-mental state examination (MMSE), activities of daily life (ADL) and blood rheology before and after treatment were observed. 2.Clinical effect analysis in the treatment of acute limb arterial critical ischemia

Objective To discuss the therapeutic effect in the treatment of the acute limb arterial critical ischemia.Methods Collect thirty-nine cases of acute limb arterial critical ischemia in Renji Hospital from Janary 2014 to July 2016.According to the patients' manifestation,these operations were porfermed including thrombectomy,cathetery-directed thrombolysis,mechanical suction bolt,percutaneous angioplasty and stenting.The effect and complications were observed.Results The eighteen patients in 39 cases (46.2%) were dead,including 5 cases without operation,13 operation.The eight cases were amputated during 34 cases of operations.In the 21 out-patients safely,2 cases were not followed up.The time of follow-up was from 3 to 27 months,on average 14.3 months.During the 21 patients,5 cases died from heart cerebrovascular or tumor diseases,3 cases with footdrop,2 cases with toe amputations,3 cases with distal leg and foot anesthesias.Conclusions The patiens with acute limb arteries critical ischemia must be treated as early as,and reinforced the management of multiple organ function,which may improve the diseases' therapeutic effect.
3.Management of major blood vessel injury during urological laparoscopic surgery

Objective To explore the causes,management and prevention of major blood vessel injury during urological laparoscopic surgery.Methods Six cases of major blood vessel injuries happened in 1700 laparoscopic surgeries from January 2007 to July 2011.All of the cases were males.Patient age was (53 ± 14) years.There were 4 extraperitoneal and 2 transperitoneal procedures including 3 adrenalectomies,1 radical cystectomy,1 radical prostatectomy and 1 radical nephrectomy.There were lacerations in 3 cases of vena cava,2 cases of external iliac vein and one case of renal vein.The length of laceration was (0.68 ±0.29) cm and blood lost was (114 ++ 79) ml.Results Five of the patients were managed with laparoscopic techniques by suction compressing bleeding sites,dissecting related vessels,adding extra trocar and repairing laceration by suturing in four cases and clipping bleeding site in one case.The bleeding control management lasted (21.0 ± 5.6) min.One laparoscopic adrenalectomty for the treatment of pheochromocytoma converted to open surgery because of increasing blood pressure.All the patients were followed up for (4 ± 2) months.No more related complication occurred.Conclusions Lymph node dissection,local adhesion and energy source are the main causes for blood vascular injuries.This kind of injuries may occur at any stages during a laparoscopic surgery and laparoscopic repairing is safe and feasible.
4.Analysis of clinical efficacy and safety of endovascular technique in treatmenting complex renal artery aneurysms

Objective:To analyze the clinical efficacy and safety of endoluminal treatment of complex renal artery aneurysm (RAA).Methods:The clinical data and follow-up results of 19 patients with complex RAA admitted to Renji Hospital, Shanghai Jiaotong University School of Medicine from November 2014 to September 2021 were retrospectively analyzed. Two patients were treated with simple spring coil embolization into the aneurysmal artery, 14 patients were treated with simple spring coil embolization of the aneurysmal cavity, and 3 patients were treated with stent-assisted + spring coil embolization technique. Based on the location of the aneurysm, RAA were classified into type Ⅰ, Ⅱ, and Ⅲ. 7 patients with type Ⅰ, 10 patients with type Ⅱ, and 2 patients with type Ⅲ were studied. Variance analysis was used to compare the differences in glomerular filtration rate(GFR) of the affected side when the tumor was located at different locations, and Pearson was used to analyze the correlation between the number of coils implanted and the size of the tumor and GFR.Results:Ten of the 19 patients who were underwent successful endoluminal intervention. The average size of the patients′ aneurysms was (20.89±6.65) mm, and the average number of spring coils implanted was 8.22±3.08. The preoperative and postoperative serum creatinine were in the normal range, and no RAA tumor enlargement or recurrence was found during the follow-up period. The postoperative GFR was abnormal in patients with type Ⅰ, type Ⅱ, and type Ⅲ renal aneurysms, and the mean GFR value differed among the three types of patients( P=0.003). There was a negative correlation between the postoperative GFR values of the affected kidney and the number of spring coils implanted ( P=0.047), and no significant relationship between GFR and aneurysm size. Conclusion:The endovascular technique is an effective and safe means of treating complex RAA.
5.Rehabilitation of chronic respiratory diseases domestic and overseas in recent ten years: a visualization analysis

ObjectiveTo visually analyze the research hotspots and future development trends in the field of rehabilitation of chronic respiratory diseases domestic and overseas in the last decade. MethodsLiteratures about rehabilitation of chronic respiratory disease from 2011 to 2021 were retrieved. The Physiotherapy Evidence Database (PEDro) was used to access the quality of the literatures, and CiteSpace 5.8.R3 was used to analyze the number of publications and keywords. ResultsA total of 2 477 English articles and 3 308 Chinese articles were included. The overall volume of literature published in the field of rehabilitation of chronic respiratory diseases domestic and overseas showed a fluctuating increase. A total of 63 English articles met the requirement of medium-high quality with a score of four to nine on the PEDro scale, and the overall average score was 5.44. A total of 78 domestic articles scored five to eight, and the overall average score was 5.21. Domestic research in this field mainly focused on the pulmonary rehabilitation methods for patients with chronic obstructive pulmonary disease (COPD) and their effects on patients' quality of life and lung function, the process of development of heart-related diseases complicated by COPD, and the effects of nursing interventions, community rehabilitation and health education for chronic respiratory diseases, and exploration of precise pulmonary rehabilitation assessment, etc. In contrast, foreign countries mainly studied the physical activity and exercise capacity of patients with chronic respiratory diseases, emphasizing the self-management of patients, and focused on the cross-sectional research of other diseases such as combined aortic stenosis, and made "physical activity" and "exercise" as the frontier of research. ConclusionOverseas pulmonary rehabilitation programmes involve a wide range of chronic respiratory diseases, requiring multidisciplinary teamwork, strengthening patient self-management at home and focusing on changes in physical activity, motor function and quality of life after exercise, while the focus is on pulmonary rehabilitation in hospitalized COPD patients, also including the role of traditional Chinese exercise in China.
6.Improvement Effects of Sanggenone C on Lipid Accumulation in FFA-induced Human Liver Cancer HepG 2 Cells

OBJECTIVE:To study the improvement ef fects of sanggenone C on lipid accumulation in human liver cancer HepG2 cells induced by free fatty acid (FFA). METHODS :HepG2 cells were divided into control group ,model group , fenofibrate group (10 μmol/L),sangerone C low ,medium and high concentration groups (2,4,8 μmol/L). Except for control group,other groups were treated with 1 mmol/L FFA to induce lipid accumulation model ,and administration groups were cultured with relevant medium containing drugs. The lipid accumulation was observed by oil red O staining ,and lipid level and triglyceride (TG) content were also determined. Real-time PCR and Western blot assay were adopted to detect the mRNA and protein expression of PPARα,CPT-1,SREBP-1c,FAS,SIRT1 and PGC- 1α in HepG2 cells. RESULTS :Compared with control group , the nucleus was atrophied significantly and the volume became smaller ,and the number of lipid droplets was significantly increased;the level of lipid ,TG content ,mRNA and protein expression of SREBP- 1c and FAS were significantly increased (P< 0.05 or P<0.01),mRNA and protein expression of PPARα,CPT-1,SIRT1 and PGC- 1α were decreased significantly(P<0.01). Compared with model group ,no obvious nucleus atrophy and normal volume were observed in sangerone C groups ,and the number of lipid droplets was significantly reduced ;the levels of lipid ,TG content ,mRNA and protein expression of PPARα pathway related genes (except for SREBP- 1c protein in saggenone C low concentration group )were significantly reversed (P< 0.05 or P<0.01). CONCLUSIONS :Sangenone C can significantly improve the lipid accumulation of HepG 2 cells,and its mechanism may associated with regulating PPAR α signaling pathway,improving cell lipid oxidation ability and inhibiting lipid synthesis.
