1.Prolongation of cardiac allograft survival in mice by Decitabine-induced upregulation of Foxp3 expression

Objective To investigate the effect of Foxp3 gene expression induced with the DNA methyltransferase inhibitor 5-aza-2'-deoxycytidine (Aza) in recipients on ameliorating allograft rejection and prolonging allograft survival and the possible mechanisms.Method BALB/c and C57BL/6 splenocytes treated with mitomycin (MMC) served as the stimulators,those not treated with MMC as responders,and they were subjected to one-way mixed lymphocyte culture (MLC).In the presence or absence of Aza,the percentage of CD4+ CD25+ Tregs was detected by flow cytometry.Abdominal heterotopic heart transplantation model was established by using inbred male Balb/c mice as donors and C57BL/6 as recipients respectively.In experimental group,recipients were intravenously administrated with decitabine (1.5 mg/kg/day) on the day 1 to day 3 post-transplantation.The control mice were treated with normal saline.CD4+ CD25+ Tregs proportion in the blood and Foxp3 mRNA in cardiac grafts and spleen of recipients were measured respectively,and the survival and the histopathologic changes of the cardiac grafts were also observed.Result Decitabine enhanced the proliferation CD4 + CD25 + Tregs of MLC in vitro.The median survival time of the cardiac grafts in experimental group (11 days) was longer than that in control group (7 days) (P<0.01).As compared with control group,Foxp3 mRNA expression in the cardiac grafts and spleen in recipient mice was significantly up-regulated,and CD4+ CD25 + Tregs proportion was increased (P<0.01) and the proliferation of lymphocytes and monocytes infiltration was inhibited.Conclusion Up-regulation of Foxp3 gene expression induced with decitabine in recipients could alleviate cardiac allograft rejection and prolong cardiac allograft survival via the induction of proliferation and differentiation of CD4+ CD25 + Foxp3 + Tregs.
2.Inhibitive effect of IL-35 on cardiac allograft rejection in mice

Objective To investigate the action mechanism of IL-35 gene transfection ameliorating cardiac allograft rejection and prolonging allograft survival.Methods pEBI3-L-p35-Fc plasmid was amplified by polymerase chain reaction.In vitro plasmid DNA pEBI3-L-p35-Fc or pSec-L-Fc was,respectively,transfected into HEK293 cells using Lipofectamine 3000.At 48 and 72 h after transfection,IL-35 concentration in culture supernatant of transfected HEK293 cells was detected by ELISA.Balb/c and C57BL/6 splenocytes treated with mitomycin (MMC) served as the stimulators,those not treated with MMC as responders,and they were subjected to one-way mixed lymphocyte culture (MLC).In the presence or absence of IL-35,the percentage of CD4+ CD25+ Tregs was detected by flow cytometry.Abdominal heterotopic heart transplantation model was established by using inbred male Balb/c mice as donors and C57BL/6 as recipients respectively.In experimental group,recipients were intravenously administrated with IL-35 plasmid (50μg) on the day 1 to day 3 post-transplantation.The control mice were treated with normal saline.The IL-35 expression in the blood,CD4+ CD25+ Tregs proportion in the blood and spleen,and the survival and the histopathologic changes of the cardiac grafts were also observed.Results In vitro the transfected HEK293 cells expressed IL-35.IL-35 enhanced the proliferation of CD4+ CD25+ Tregs of MLC in vitro.The median survival time of the cardiac grafts in experimental group (16 days) was significantly longer than in control group (7 days) (P<0.01).As compared with control group,CD4+ CD25+ Tregs proportion was significantly increased (P<0.01),CD8+ T cells proportion was decreased (P<0.01) and the proliferation of lymphocytes and monocytes infiltration was inhibited in the experimental group.Conclusion IL-35 could alleviate cardiac allograft rejection and prolong cardiac allograft survival via the induction of proliferation and differentiation of CD4+ CD25+ Tregs and inhibition of proliferation of CD8 + effector T cells.
3.Clinical evaluation of total knee arthroplasty in treatment of severe adult Kashin-Beck disease

Objective To evaluate the clinical effects of total knee arthroplasty (TKA) in treatment of severe adults Kashin-Beck disease (KBD). Methods Sixteen cases of KBD patients underwent TKA in Shaanxi Provincial People's Hospital, including 2 males (2 knees) and 14 females (17 knees), aged 41 to 56 years, mean (56.38 ± 6.40) years, left knee in 8 cases and right knee in 11 cases, knee varus in 15 cases and valgus knees in 4 cases. Visual Analogue Scale/Score (VAS), Hospital for Special Surgery (HSS) scores, knee range of motion, varus deformity and postoperative complications were observed before and after TKA. Results In this group of TKA patients, the levels of VAS scores in pre-total knee arthroplasty (pre-TKA), 2 weeks post-total knee arthroplasty (post-TKA), 3 months post-TKA, and at the end of the follow-up were 7.51 ± 1.00, 3.56 ± 1.29, 1.83 ± 1.40 and 1.10 ± 0.87, respectively. The level of VAS scores in 2 weeks post-TKA was significantly lower than that in pre-TKA (P<0.01), and the VAS levels were continued to decrease in post-TKA (all P< 0.01). Total HSS score at the end of the follow-up post-TKA was 78.60 ± 5.30, which was significantly higher than that in pre-TKA (43.59 ± 10.08, t=19.21, P< 0.01). At the end of the follow-up post-TKA, in addition to the muscle strength, the levels of pain, knee function, activity, flexion deformity and stability (25.94 ± 4.17, 15.88 ± 3.70, 14.09 ± 1.03, 6.79 ± 2.25, 8.58 ± 1.30) were significantly higher than those in pre-TKA (11.56 ± 5.39, 7.56 ± 1.75, 9.86 ± 3.85, 3.05 ± 3.22, 5.00 ± 3.07, t= 16.00, 8.32, 6.43, 7.07, 6.95, all P< 0.01). At the end of follow-up post-TKA, the knee degree of extension [(3.05 ± 2.71)°] was significantly lower than that in pre-TKA [(15.11 ± 11.30)°, t= -5.40, P< 0.01], the knee degree of flexion [(115.79 ± 9.65)°] was significantly higher than that in pre-TKA [(93.95 ± 22.40)°, t=6.02, P< 0.01), the degree of varus [(2.40 ± 2.40)° ] and valgus [(3.75 ± 2.50)° ] deformity was significantly lower than those in pre-TKA [(11.33 ± 10.43)°, (18.00 ± 5.72)°, t = - 4.15, - 3.61, all P< 0.05]. One patient was diagnosed as knee tuberculosis in 6 months post-TKA. There was no complication in this group of patients. Conclusion The TKA in severe adults knee of KBD can significantly reduce knee pain, improve knee function, correct joint deformities and improve quality of life in patients, and shows good clinical results.
4.Comparative study of 18F-FDG PET/CT and 99Tcm-MDP bone scintigraphy in detecting multiple myeloma bone destruction

Objective To explore the application value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18 F-FDG PET/CT) and 99Tcm-methylene diphosphonate (99 Tcm-MDP) bone scintigraphy for detecting bone destruction in multiple myeloma (MM).Methods 18 F-FDG PET/CT and 99Tcm-MDP bone scintigraphy results of 27 MM patients were analyzed retrospectively.Inspection areas checked by magnetic resonance imaging (MRI) and X-ray were the limited scopes.The location and number of bone destruction were recorded,and the maximal standardized uptake value (SUVmax) was measured simultaneously.The results were comparatively analyzed.Diagnostic certainty regarding the presence or absence of bone destruction was evaluated according to the reference standard consisting of MRI and X-ray.Results A total of 235 lesions were found according to the reference standard.Of these,227 lesions (97%) were identified by 18F-FDG PET/CT,whereas 187 lesions (80%) were identified by bone scintigraphy,with a significant statistical difference (x2 =32.43,P < 0.05).SUVmax was 8.3 ± 1.7 (4.3 to 18.9).The discovery rates of bone fracture of 18F-FDG PET/CT and bone scintigraphy were 100% (97/97) and 90% (87/97),and there was a significant statistical difference between them (x2 =78.09,P < 0.05).Conclusion 18 F-FDG PET/CT is a possible method to detect bone lesions in patients with MM,and is better than 99Tcm-MDP bone scintigraphy.

7.Effects of Sevelamer Carbonate Combined with Routine Treatment on Serum Inflammatory Factors, HO-1 and iPTH Levels of Chronic Renal Failure Patients with Hyperphosphatemia

OBJECTIVE: To investigate the effects of sevelamer carbonate combined with routine treatment on serum inflammatory factors, heme oxygenase-1 (HO-1) and intact parathyroid hormone (iPTH) levels of chronic renal failure (CRF) patients with hyperphosphatemia.METHODS: Totally 60 chronic renal failure patients with hyperphosphatemia in department of renal internal medicine, Tangshan Workers' Hospital during Jan. 2014 to Jan. 2015 were divided into control group and observation group according to random number table, with 30 cases in each group. Control group received routine treatment as reducing blood glucose, lowering blood lipid, lowering blood pressure, reducing phosphorus, protecting kidney. Observation group was additionally given sevelamer carbonate (dose was 0. 8 g, 3 times a day, during meal) on the basis of control group, for 4 weeks. The levels of IL-6, CRP, HO-1, iPTH, Ca and P were compared between 2 groups. The occurrence of ADR was compared between 2 groups. RESULTS: Two cases were withdrewn from the study in each group, finally 28 cases were included in the study in each group. Before treatment, there was no statistical significance in each index between 2 groups (P>0. 05). Compared with before treatment, the serum levels of IL-6, CRP, iPTH and P in 2 groups were decreased significantly after treatment (P<0. 05), while serum level of Ca was increased significantly (P<0. 05); the improvement of above indexes in observation group were more dovious than control group (P<0. 05). There was no statistical significance in the incidence of ADR between 2 groups (P>0. 05). CONCLUSIONS: Sevelamer carbonate combined with routine treatment can effectively reduce the serum levels of IL-6, CRP, HO-1 and iPTH and regulate Ca and P metabolism of CRF patients with hyperphosphatemia with good safety.
8. Protetive effects of anemoside B4 on kidney tissue of rats with chronic renal failure and its mechanism

Objective: To investigate the effects of anemoside B4 on the blood oxygen stress, inflanmation state and the expressions of apoptosis-related proteins in kidney tissue of the rats with chronic renal failure (CRF), and to illuminate the mechanism of anemoside B4 in protecting the kidney tissue of the CRF rats. Methods: A total of 75 SD male rats were divided into control group, model group, positive control group, low dose of anemoside B4 group and high dose of anemoside B4 group. The CRF rat models were established by continuous administration of adenine for 21 d. After successful modeling, the rats in low and high doses of anemoside B4 groups were given 1 and 2 mg · kg-1 anemoside B4 by gavage and the rats in positive control group were given 0.5 mg · kg-1 dexamethasone, lasted for 28 d; the rats in control group and model group were given the same amount of normal saline. Biochemical analyzer was used to detect serum urine creatinine (Scr) and urea nitrogen (BUN) levels. The kit method was used to detect the activity of superoxide dismutase (SOD) and the levels of malondialdehyde (MDA) in kidney tissue of the rats. ELISA was used to detect the levels of interleukin-1β (IL-1β), interleukin-6 (IL-6) and tumor necrosis factro-α (TNF-α) in kidney tissue of the rats. Western blotting method was used to detect the activation of PI3K/Akt pathway and the expression levels of apopotosis-related proteins Bcl-2, Bax and Caspase-3 in kindey tissue. Results: Compared with control group, the serum Scr and BUN levels of the rats in model group were increased (P<0.01), the number of normal glomerulus and renal corpuscles were decreased, the activity of SOD in kidney tissue was decreased (P<0.01), the levels of MDA, IL-1β, IL-6 and TNF-α were increased (P< 0.01), the expression levels of Bax and Caspase-3 proteins were increased (P<0.01), and the expression levels of p-Akt and Bcl-2 proteins were decreased (P<0.01). Compared with model group, the serum Scr and BUN levels of the rats in positive control group, low and high doses of anemoside B4 groups were significantly decreased (P< 0.01), the number of normal glomerulus and renal corpuscles were increased significantly, the activities of SOD in kidney tissue were increased significantly (P<0.01), the levels of MDA, IL-lβ, IL-6, and TNF-α were significantly decreased (P<0.01), the protein expression levels of Bax and Caspase-3 were decreased significantly (P<0.01), and the expression levels of p-Akt and Bcl-2 proteins were increased significantly (P<0.01). Conclusion: Anemoside B4 can protect the renal tissue injury by reducing the levels of Scr and BUN in serum, reducing oxidative stress response, reducing the release of inflammatory factors, and activating the PI3K/Akt pathway of renal tissue.
9.Clinical analysis of otitis media with effusion in adults and children.

OBJECTIVE:
To explore the different and common features between adults and children with otitis media with effusion (OME) on course of disease, clinical characteristics and audiometric changes.
METHOD:
Pure tone audiometry, tympanogram and clinical information were analyzed and compared between 34 cases of adults and 42 cases of children with OME.
RESULT:
The course of disease was relatively short in children. The adenoid vegetation inflammation was more frequently found in children, while the OME occurred more often in adults. The air conduction and bone conduction threshold increased both in children and adults, particularly at high frequencies of 2 and 4 kHz. The bone conduction threshold at 4 kHz in adults was significantly higher than that in children (P < 0.05). In the cases with C type tympanogram, the negative pressure in adults was even lower than that in children (P < 0.05).
CONCLUSION
The OME in children is often relatively shorter in course of disease, which is mainly caused by adenoid vegetation. Sensorineural deafness can occur both in children and adults, especially at high frequencies. OME in children is less severe, however, it may lead to negative effects in learning and cognition, which should be diagnosed and managed in time.

10.Assessment of quality of life after total knee arthroplasty in patients with severe knee osteoarthritis of Kashin-Beck disease

Objective To evaluate the quality of life of patients with Kaschin-Beck disease (KBD)receiving total knee arthroplasty (TKA) before and after the operation.Methods Clinical efficacy of 25 patients with KBD who underwent TKA in Department of Orthopaedics,Shaanxi Provincial People's Hospital from January 2015 to January 2017 was prospectively analyzed.A questionnaire survey on KBD quality of life (KBDQOL)was carried out to evaluate the quality of life of the patients before,6 months and 1 year after the surgery.Results The scores of physical function,activity limit,social support,mental status and total health scores of KBDQOL in 6 months after the surgery (30.60 ± 3.73,23.24 ± 2.03,15.16 ± 1.62,18.92 ± 2.89,12.80 ± 2.57) and 1 year after the surgery (32.00 ± 3.19,23.76 ± 1.59,15.60 ± 1.29,20.16 ± 2.67,17.28 ± 3.88) were significantly higher than those of before the surgery (18.84 ± 4.94,21.04 ± 2.72,12.80 ± 2.06,13.68 ± 3.42,7.92 ± 1.93,P < 0.05).However,there was no significant difference in economic scores before,6 months and 1 year after the surgery (10.68 ± 2.98,10.60 ± 2.78,10.48 ± 2.80,P > 0.05).Conclusions The quality of life after TKA in patients with KBD severe knee osteoarthritis is significantly better than that before the surgery.The KBDQOL questionnaire is an appropriate tool for evaluating the quality of life in KBD patients.
