1.Analysis on Status Quo and Tendency of Chinese Materia Medica Injection Used in Our Hospital during 2002~2004

OBJECTIVE:To evaluate the drug use status of Chinese materia medica injection(CMMI)in our hospital.METHODS:Chinese materia medica injection(CMMI)used in our hospital from2002to2004was analyzed statistically in terms of consumption sum,defined daily dose(DDDs)and the average daily consumption sum(DDDc).RESULTS:The consumption sum of CMMI in3years increased year by year with annual increase rates of29.7%and32.3%,respectively.The cardio-cerebral vascular CMMI steadily dominated the first place in terms of consumption sum and DDDs in3years with a steady increase year by year from a high starting point.The DDDc of antitumor CMMI ranked the toppest.As for the consumption sum for single variety,xuesaitiong steadily ranked the first place in3years.CONCLUSION:The fast increase of the consumption of CMMI shows that it has a great market demand.
2.Determination of the Content of Stilbene Glycoside in Zhipingkang Misture by HPLC

OBJECTIVE:To establish an HPLC method for the determination of Stillbene glycoside (THPG) in Zhipingk-ang misture.METHODS:The chromatographic column was Hypersil C18(250 mm?4.6 mm,5 ?m).The mobile phase consisted of acetonitribl-water(25∶75) at a flow rate of 1.0 mL?min-1 with detection wavelength set at 320 nm. RESULTS: There was a good linear relationship within the concentration range of 0.5~8.0 ?g for THPG. The average recovery of THPG was 100.03%(RSD=1.07, n=6).CONCLUSION: The adoptd method is accurate, reproducible and suitable for the quality con-trol of Zhipingkang misture.
3.The relationship between levels of plasma thrombopoietin and thrombocytopenia

Objective:To investigate the relationship between levels of plasma thrombopoietin(TPO) and thrombocytopenia.Methods:Sixty-eight thrombocytopenia with different causes were injected with rhIL-11 domestically made, by 25 ?g/(kg?d) for 10 days continuously. The plasma TPO levels were determined by quantitative sandwich enzyme immunoassay technique in thrombocytopenic patients between before and after treatment.Results:(1)The serum TPO controls of patients with thrombocytopenia after acute leukemia(AL) chemotherapy were lower than normal controls. The plasma TPO levels in aplastic anemia(AA) and myelodysplastic syndrome(MDS) patients were all elevated compared to those in normal controls.The serum TPO levels in patients with idiopathic thrombocytopenic purpura(ITP) and liver cirrhosis had no difference with the normals. Bone marrow megakaryocytes(MK) count in after chemotherapy AL and AA patients were lower than those with normal serum TPO levels controls. (2)In all above patients used interleukin-11(rhIL-11), their TPO levels were close to normal in efficiency patients. On the contrary, their TPO did not change dramatic. Those react to rhIL-11, their TPO levels were inversely correlated with platelet count.Conclusion:Testing serum TPO levels would be helpful for distinguish the thrombocytopenia with different causes, and provides theorical support to patients to use rhIL-11 reasonably.
4.Clinical Features and Influential Factors of Therapeutic Efficacy of Reflux Esophagitis in Elderly Patients with Type 2 Diabetes Mellitus

Background:The prevalence rate of diabetes mellitus ( DM ) associated with gastroesophageal reflux disease ( GERD)is increasing recently,and the relationship between DM and GERD has become a research hotspot. Aims:To study clinical features and influential factors of therapeutic efficacy of reflux esophagitis( RE)in elderly patients with type 2 DM(T2DM). Methods:Two hundred elderly patients with RE diagnosed by gastroscopy from March 2011 to October 2013 were enrolled,and divided into T2DM associated with RE( T2DM)group and RE group. The clinical features, endoscopic findings and therapeutic efficacy of the two groups were analyzed,and the influences of DM course and control of blood glucose on efficacy of RE were also analyzed. Results:The main manifestations of RE were extraesophageal symptoms,typical esophageal symptoms were less common. The overall efficacy rate decreased with the increase of endoscopic grade of RE,however,extraesophageal symptoms improved significantly. With the prolonging of DM course,the endoscopic grade of RE was increased,and the control of blood glucose was worse. The overall therapeutic efficacy was significantly reduced with the prolonging of DM course in patients with well controlled blood glucose. Conclusions:Extraesophageal symptoms are common,endoscopic grade of RE is worse and blood glucose is difficult to control in elderly T2DM patients associated with RE. DM course and status of blood glucose control have influences on the therapeutic efficacy of RE.

As the first domestically originated cell cycle protein-dependent kinase 4/6 inhibitor,dalpiciclib has been approved by the State Drug Ad-ministration for the treatment of hormone recep-tor-positive/human epidermal growth factor recep-tor 2-negative advanced or metastatic breast can-cer in combination with fulvestrant or aromatase inhibitors.This article focuses on the progress of dalpiciclib research in breast cancer,summarizing the drug's mechanism of action,phase Ⅰ-Ⅲ clinical trials,and drug safety issues.
6.Relationship of the Function and Gene Polymorphism of Insulin Receptor Substrate with Type 2 Diabetes Mellitus

OBJECTIVE: To investigate the relationship of the function and gene polymorphism of insulin receptor substrate (IRS) with type 2 diabetes mellitus (T2DM), and to provide a new perspective for T2DM drug development. METHODS: Relevant literatures included in CNKI, Wanfang, VIP, PubMed, SpringerLink and other databases from Jan. 1991 to Nov. 2017 were retrieved by using "Insulin receptor substrate" "Type 2 diabetes" "Insulin resistance" "Polymorphism" as Chinese keywords, and "Insulin receptor substrate" "IRS" "Type 2 diabetes" "Insulin resistance" "Polymorphism" as English keywords. The relationship of the function and gene polymorphism of IRS family with T2DM was reviewed. RESULTS & CONCLUSIONS: A total of 328 literatures were retrieved, of which there were 38 valid literatures. At present, IRS family has six members (IRS-1 to IRS-6). The dysfunction of IRS-1 and IRS-2 will lead to insulin resistance and induce T2DM. The relationship of IRS-3 and IRS-4 with T2DM remains controversial. IRS-5 and IRS-6 were newly found and their functions are not clear. The Gly972Arg mutation of IRS-1 is positively correlated with the pathogenesis of T2DM. Gly1057Asp mutation of IRS-2 combined with obesity can induce insulin resistance, but there is controversy. The mutation types of IRS family other members include Ala94Thr, Ala512Pro and Ser892Gly mutation of IRS-1, ACC, Ala157Thr and Leu647Val mutation of IRS-2. The relationship between these types of mutation and T2DM has not yet been fully supported. Multiracial and large-scale studies are required. Some achievements have been made in the present study, but the study is not yet comprehensive. Relationship of IRS family members and their mutation sites with T2DM still needs to be further tested in the expanded population.
7.Literature analysis of semaglutide-induced adverse drug reaction

OBJECTIVE To explore the characteristics of adverse drug reaction (ADR) induced by semaglutide and provide a reference for clinically safe medication. METHODS Using search terms such as “semaglutide” and “adverse reactions” in both Chinese and English, the case reports about ADRs caused by semaglutide were searched and analyzed from PubMed, Web of Science, SpringerLink, CNKI, Chinese Medical Journal Full-text Database, Wanfang Medical Network and VMIS. RESULTS Overall 14 literature were included, involving 17 patients. Among 17 patients, 9 were female and 8 were male, with the age ranging from 25 to 80 years. Eight patients were given two or more drugs, and eight patients took 0.25 mg as the initial dose; the ADR occurred most frequently within 6 months (94.12%). Sixteen patients’ symptoms improved or recovered after drug withdrawal and symptomatic treatment. Eleven patients did not mention whether to continue to use semaglutide in the future. Nine patients underwent ADR correlation evaluation, and 1, 3, 1 and 4 cases were determined to be “definite”,“ probable”,“ possible”, and “doubtful” respectively. Semaglutide-induced ADRs involved multiple organs or systems, most of which were the digestive system (35.29%), followed by skin tissue (29.41%). Among them, acute gastric dilation, severe liver injury, calculous cholecystitis, bullous pemphigoid, eosinophilic fasciitis, acute kidney injury, acute interstitial nephritis, depression and acute hemolytic anemia were not mentioned in the instruction. CONCLUSIONS ADRs caused by semaglutide can occur in all ages, mainly within 6 months after medication, and mainly involve the digestive system, skin tissue, etc. Great attention should be paid to pharmaceutical care for those patients with liver and kidney dysfunction, neuropsychiatric diseases, glucose-6-phosphate dehydrogenase deficiency, etc. When ADR occurs, drug withdrawal and symptomatic treatment should be performed promptly to ensure patients’ medication safety.
8.Treatment for abdominal infection caused by metallo- β-lactamase-producing Klebsiella pneumoniae in an infant after liver transplantation :a case report and literature review

OBJECTIVE To investigate the treatment plan for az treonam-resistant metallo- β-lactamase(MBL)-producing Enterobacteriaceae infection in pediatric solid organ transplant recipients. METHODS The clinical data of aztreonam-resistant MBL-producing Klebsiella pneumoniae caused intra-abdominal infection of an infant after liver transplantation were retrospectively analyzed. Abdominal infection occurred after operation. The pathogenic bacterium was MBL-producing K. pneumoniae . The drug sensitivity results showed that the infant was resistant to aztreonam. Based on the results of sensitivity test ,polymyxin B combined with tigecycline were selected as initial regimen. The treatment effect was poor ,with recurrent disease and shock spots. The clinical pharmacist assisted the clinician to formulate treatment regimen of ceftazidime avibactam 0.5 g,q8 h combined with aztreonam 0.18 g,q6 h. Relevant domestic and foreign literature were reviewed ,and the treatment plan of MBL-producing Enterobacteriaceae infection after solid organ transplantation was summarized. RESULTS & CONCLUSIONS The infant was finally cured and discharged with ceftazidime avibatan combined and aztreonam. Several foreign literature reported that ceftazidime avibactam combined with aztreonam could effectively treat the infection caused by aztreonam-resistant MBL-producing Enterobacteriaceae infection in patients with organ transplantation. It is expected to be an effective treatment for aztreonam-resistant MBL-producing Enterobacteriaceae infection in pediatric solid organ transplant recipients.
9.Vedolizumab-associated lung toxicity:a case report and literature analysis

OBJECTIVE To introduce a case of interstitial pneumonia induced by vedolizumab (VDZ), summarize and analyze the characteristics of the occurrence of VDZ-associated lung toxicity, and provide a reference for the clinically safe use of drugs. METHODS From the perspective of clinical pharmacists, the diagnosis and treatment of a case of VDZ-induced interstitial pneumonia were retrospectively analyzed, and the association of adverse drug reactions (ADR) was analyzed. Retrieving from Chinese and English databases such as CNKI, VIP, PubMed and Web of Science, case reports of VDZ-associated lung toxicity were summarized and analyzed. RESULTS The patient developed interstitial pneumonia during the use of VDZ and empirical anti- infective therapy with antibiotics was ineffective. After withdrawal of VDZ and treatment with methylprednisolone, symptoms and imaging examinations improved, but interstitial pneumonia still existed. According to Naranjo’s ADR evaluation scale and based on China Adverse Drug Reaction Reporting and Monitoring Manual, the association between VDZ and interstitial pneumonia was “very likely”. Results of the literature analysis showed that among 29 involved patients (including the patient reported in this article), there were 19 males and 10 females, with an average age of (49.24±17.06) years. Lung toxicity mainly included VDZ- associated pneumonia, eosinophilic pneumonia, pulmonary granulomas or necrobiotic nodules, interstitial lung injury, etc., which mostly occurred ≤24 weeks after medication (58.62%). The main clinical manifestations included cough, dyspnea and fever. The vast majority of patients improved or recovered after drug withdrawal and/or glucocorticoid treatment, but one patient died of respiratory failure. CONCLUSIONS Lung toxicity is a rare ADR of VDZ with insidious onset and nonspecific symptoms. Once patients experience symptoms such as cough and dyspnea, early diagnosis,timely drug withdrawal,and symptomatic treatment measures such as glucocorticoid should be taken to ensure medication safety.
