1.Diagnosis, stages and epidemiologic studies of chronic kidney disease in elderly adults

Since the chronic kidney disease (CKD) and end-stage renal disease (ESRD) can cause serious disease burden and consume huge amounts of health resources to human life,research on CKD received increasing attention.For the particularity of the elderly,whether the current definition,stages and renal function assessment of CKD should be distinguished from the adults has been controversial.We conducted a comprehensive review on these issues.Furthermore,the studies on the epidemiologic status,trends,and characteristics of CKD for the elderly were summarized,and possible reasons for the differences between the various studies were analyzed.
2.Role of evidence-based medicine in the cultivation of clinical medicine undergraduates' scientific research ability

The paper described the concept of evidence-based medicine and its guiding role in clini-cal practice and scientific research. It emphasized the importance of teaching evidence-based medicine in clinical medicine teaching of medical college students, and put forward some suggestions on how to carry out evidence-based medicine teaching among undergraduate students. In the course of undergraduate study, students should have preliminary grasp of how to apply the theory of clinical epidemiology to evaluate the evidence of clinical research so that they could evaluate the authenticity, importance and applicability of the research evidence . Besides , they could correctly understand and apply the evidence of evidence-based medicine in clinical practice in the future, and at the same time cultivate their scientific research thinking and innovative ability.
3.Approaches of Etiquette Education for College Students under Harmonious Language Atmosphere

The absence of etiquette of college students,in its nature,is an issue of college students' development.Higher institutes should aim to cultivate college students into harmonious figures,carry out etiquette education,and promote the all-round development of college students.Based on a questionnaire analysis,this paper explores the causes of absence of etiquette of college students,and comes up with approaches to promoting etiquette education for college students.
4.The Management of Community-based Medical Centers on Ischemic Stroke Patients in Urban Areas of Beijing

Objective To evaluate how well the risk factors associated with stroke recurrence been controlled by community-based medical centers in urban areas of Beijing.Methods A cross-sectional study was performed in 5 community-based medical centers in urban areas of Beijing.A total of 999 patients with the experience of ischemic stroke were evaluated from Dec.2004 to Nov.2005.Results Ninety-six percent of patients suffered from at least one of the most common diseases(risk factors) associated with stroke recurrence,i.e.hypertension(HT),heart disease,diabetes,hyperlipidemia and obesity,and 80% of patients were with more than 1 disease.The proportion of HT was 79.1%(790/999) and the awareness,treatment and control rates of HT patients were 93.3%(737/790),84.3%(666/790) and 40.3%(318/790),respectively.The proportion of heart diseases and obesity among 999 stroke patients were 34.4% and 19.1%%,respectively.31.9%(319/999) of patients had diabetes and 84.0%(268/319) of the patients knew their history of diabetes.Of 319 diabetes patients,68.3%(218/319) were under active treatment and 45.4%(99/218) of them had their fasting blood glucose well controlled.The proportion of hyperlipidemia was 72.9% and the treatment rate for hyperlipidemia was as low as 29.5%(215/728).There were 43.7% and 50.0% of patients quit smoking and alcohol drinking after their acute stroke.72.7%(726/999) of patients were receiving aspirin regularly.Conclusion The proportion of cardiovascular diseases were high among the patients with ischemic stroke who were under the management of community-based medical centers.The community-based medical centers have played an important role in the secondary prevention of stroke.However,the management for ischemic stroke patients with diabetes and hyperlipidemia needs to be improved.
5.Research on the health care needs of the elderly in Beijing

Objective To find out about the current health situation of the elderly in the Beijing area and their health care needs so as to provide basis for the formulation of health care policies for the elderly by the government and relevant departments and for the development of health care programs for the elderly in accordance with their needs. Methods A sample survey, covering such topics as the health situation, medical treatment, nursing, rehabilitation and health care needs of the elderly and community health service, was made of 2 487 elderly people aged 60 and above in different parts of Beijing, including the city proper, the part linking city and country, and the suburbs. An analysis was then made of the difference in the health situation and health care needs of the elderly between various parts and populations. Results Chronic diseases constituted the major health problem for the elderly, with hypertension, coronary heart disease, osteoarthrosis, and cerebrovascular disease ranking in the first four places and the incidence of diabetes increasing rapidly. About 10 percent of the elderly, who suffered from somatic dysfunction, had reduced daily life skills and needed assistance from others. An increased number of elderly people also meant a greater demand for medical and nursing services. Community health service, the advantages of which bacame increasingly manifest, was thus popular with the elderly. Financial difficulty was the chief factor affecting the elderly in their seeking medical service. Conclusion The elderly is a weak population in health. With the aging of the general population, the health care needs of the elderly are on the rise. As health is the major factor affecting the life quality of the elderly, enhanced efforts in the prevention and treatment of chronic diseases ought to be taken as the central task in the health care of the elderly. They are also the key to improving the life quality of the elderly.
6.A study on the value of the serial position effect combined with delayed recall of auditory verbal learning test in distinguishing elderly patients with different levels of cognitive impairment

Objective:To explore the diagnostic value of the serial position effect combined with delayed recall of auditory verbal learning test in diagnosing the elderly patients with different levels of cognitive impairment.Methods:A total of 310 subjects were enrolled and divided into three groups: the normal control group(NC group, n=128), the mild cognitive impairment group(MCI group, n=133)and the mild Alzheimer's disease group(mild AD group, n=149)matched for gender, age and education level.Comprehensive neuro-psychological tests were performed on all subjects, and the Auditory Verbal Learning Test-Huashan version(AVLT-H)was used to analyze the serial position effect.Results:There were significant differences in the primacy effect of the second and third time of immediate recall, short-term and long-term delayed recall among the three groups( P<0.05), but no difference was found in the recency effect between the MCI and mild AD groups( P>0.05). The long-term delayed primacy effect combined with the short-term delayed recall showed a better validity to distinguish those elderly subjects with different levels of cognitive impairment than other memorial indexes, and the correlative sensitivity and specificity were 83.21% and 71.43%(NC group vs.MCI group), 85.71% and 92.86%(NC group vs.mild AD group), 66.67% and 75.57%(MCI group vs.mild AD group), respectively. Conclusions:The long-term delayed primacy effect combined with the short-term delayed recall can assist in distinguishing elderly patients with different levels of cognitive impairment.

8.Relationship between serum uric acid and metabolism syndrome in community-based elderly population in Beijing: a cross-sectional survey

Objective To investigate the relationship between serum uric acid (SUA) and metabolism syndrome (MS) in community-based elderly population in Beijing.Methods A community-based and cross-sectional survey on 1821 elderly person was performed in Beijing in 2000.Questionnaire survey,physical examination and blood biochemical tests were conducted.Relationship of MS and its components with SUA was analyzed.Results Among 1821 subjects,there were 365 cases with MS (20.0%) and 1456 cases without MS (80.0%).SUA level was higher in MS group than in non-MS group [(5.20±2.27) μmol/L vs.(4.50±1.44) μmol/L,t=-7.34,P=0.000].All subjects were divided into 4 groups according SUA level:goup 1 (≤3.5μmol/L,n=443),group 2 (3.6 μmol/L-4.4 μmol/L,n=465),group 3 (4.5 μmol/L-5.4 μmol/L,n=446) and group 4 (≥5.5 μmol/L,n=467).SUA level was increased along with the increased BMI,TG,TC,LDL-C levels,and decreased along with the decreased HDL-C level,which had significant differences (F=43.07,9.32,22.36,8.19,9.39,all P<0.001).There were significant distribution differences in MS components numbers in the 4 groups (x2=73.62,P=0.000).After adjusting for age,sex,residential,education level,marriage,smoking,drinking and exercise habits,logistic regression analysis showed that MS was related with SUA level,and the risk of MS was increased along with the increased SUA level.Conclusions MS is correlated with SUA level in elderly people.SUA level should be observed and controlled effectively in the elderly.
9.Inhibitory effect of siRNA-mediated MSK1-silencing on the proliferation of CNE2 cells and its mechanisms

Objective Abnormal activation of mitogen-and stress-activated kinase (MSK1) plays an important role in the development of various cancers.This study was to explore the effect of small interfering RNA (siRNA)-mediated MSK1-silencing on the proliferation of human nasopharyngeal carcinoma (NPC) cells and its underlying mechanism.Methods The siRNA vector targeting MSK1 was constructed and transfected into CNE2 cells, and the NPC cell line stably expressing MSK1 was established.Then the cells were divided into a blank control (without transfection of the plasmid), a negative control (with stable transfection of the negative control plasmid), and an experimental group (with stable transfection of the positive recombinant plasmid).The expressions of MSK1 mRNA and protein were detected by real-time quantitative PCR and Western blot, respectively, the proliferation of the cells determined by CCK-8 and colony formation assays, the cell cycles analyzed by flow cytometry, the level of histone H3 phosphorylation at Ser10 examined by Western blot, and The transcriptional activity and expression of the c-jun protein measured by dual-luciferase reporter gene assay and Western blot.Results Compared with the blank control, the inhibition rates of cell proliferation at 48, 72 and 96 hours were significantly reduced in the experimental group (P<0.05), and so were the colony formation ability of the cells (P<0.01) and the expression and transcriptional activity of the c-jun protein (P<0.05).In comparison with the negative control, the experimental group showed significant decreases in the rate of cell growth after 24 hours, the inhibition rates of cell proliferation at 48, 72 and 96 hours (P<0.05), the number of formed colonies ([221.00±20.08] vs [99.67±15.57] / 300 cells, P<0.01), the proportion of S-phase cells (P<0.01), and the expression of the c-jun protein in the CNE2 cells ([100.00±0.00] vs [48.77±10.71] %, P<0.05), but a remarkable increase in the percentage of G0/G1-phase cells (P<0.01).Furthermore, histone H3 phosphorylation at Ser10 was markedly reduced (P<0.01) but no significant change was observed in the expression of the total c-jun protein in the experimental group.Conclusion Knockdown of MSK1 using siRNA can significantly inhibit the growth and proliferation of CNE2 cells, which may be closely related to the decreased phosphorylation of histone H3 and subsequently down-regulated transcriptional activity of c-jun.
10.The effect of intervention on blood pressure and incidence of cerebrovascular diseases

Objective To study the effect of intervention on blood pressure and incidence of cerebrovascular diseases. Methods In 1987, two cohorts population were selected in urban areas of Changsha . One was intervention group , another was control group. Baseline blood pressure levels were investigated and the events of stroke were collected. Results After 14 years, the cumulative stroke events were 89 in the intervention group and 128 in the control group; the mean blood pressure increased with statistical significance in each group except diastolic pressure in intervention group, but the control group increased more significantly; the analysis of Kaplan-Meier displayed that the rate of non-stroke events were higher in intervention group than that in control group and the analysis of COX regression indicated that the risk for stroke-events were 1.4 times higher in control group. Conclusions The intervention of risk factors can delay the increase of blood pressure by aging and reduce the risk of stroke-events.
