1.Influence of different intervention methods on the risk factors of cardiovascular disease for women with severe pre-eclampsia history

Objective To explore different intervention methods on the risk factors of cardiovascular disease for women with severe pre-eclampsia history.Methods 78 patients with a history of severe preeclampsia in the hospital from November 2006 to November 2008 were chosen as the research subject.They were divided into the observation group and the control group according to the adopted different methods of intervention with 39 patients in each group.The control group was taken with conventional interventions for obesity intervention,and the observation group was treated with standard behavior therapy (SBT).The improving situations of the related risk factors for cardiovascular disease after three months for the two groups were compared.The effect of different intervention methods on the risk factors of cardiovascular disease was evaluated for women with severe pre-eclampsia history.Results After the intervention,the cardiovascular risk factors (systolic blood pressure,body mass,waist circumference,fasting glucose,total cholesterol/high density lipoprotein) of the two groups improved significantly than before the intervention.But the improvement effect of the observation group was better than the control group,the differences between the two groups were significant.Conclusions The intervention effect on risk factors of cardiovascular disease by standard behavior therapy for women with severe pre-eclampsia history is better than conventional way,which is worthy of clinical application.
2.Preliminary study on ventricular-arterial coupling of patients with hypertension by ultrasonography

Objective To evaluate the cardiovascular stiffness and its coupling of patients with hypertension by ultrasound.Methods Fifty patients with essential hypertension and 30 age- and gendermatched subjects without hypertension,diabetes and other cardiovascular diseases as control group were enrolled in this study.The parameters of structure and function of left ventricle,blood flow were measured by echocardiography.The blood pressure and carotid-femoral pulse wave velocity (CFPWV) were also measured.The derived corresponding parameter:end-systolic pressure(ESP),effective arterial elastance (Ea),end-diastolic (Ed),end-systolic ventricular elastance (Ees) and Ea/ Ees were calculated respectively.Results Ees was correlated positively with ejection fraction (r =0.378,P =0.005),while Ea was correlated positively with CFPWV( r =0.289,P <0.001).Ed was correlated negatively with e/a ( r =- 0.333,P =0.027).Posterior wall of left ventricle was correlated positively with Ed and Ea( r =0.388,P =0.016; r =0.336,P =0.026).Ea and Ed in patients with hypertension were significantly higher than those in control group( P <0.05),but there was no significant difference of Ees and Ea/ Ees between two groups( P > 0.05).Conclusions Arterial stiffness is associated with ventricular stiffness,and their matching relation can be applied to evaluate ventricular-arterial coupling.
3.Investigation and management of the psychological state of the assistant nurses

ObjectiveTo investigate the personnel structure,academic composition,psychological status,nursing behavior and management of the assistant nurses. MethodsThe age,educational structure,mental state and management of 110 assistant nurses were surveyed in our hospital from August 2008 to July 2011,and the results were analyzed. ResultsAmong 110 assistant nurses,24~26 year- old accounted for 60%,tertiary education accounted for 20.9%,working age mostly between 2~3 years.Depression,fear,worry,anxiety,regret,etc.were the most prominent state of mind of nurses. ConclusionsThe old and new with mentoring,early implementation of psychological interventions,maintenance of their self- esteem and strengthening the humanistic quality training can effectively stabilize psychological state of assistant nurses.The implementation of people- centered care management model not only can improve the overall professional quality of the nurses,but also can reach the ultimate goal of nurses,patients,hospitals tripartite benefit.
4.Preliminary study on new parameter of myocardial elasticity

Objective To discuss the value of the new parameter of myocardial elasticity derived from circumferential strain.Methods Right ventrieular apical pacing was established on the beagle dogs.Before and after pacing,the peak of early diastolic flow velocity (E peak),peak of atrial flow velocity (A peak),and the ratio of A/E were measured.The peak pressure-rise rate (+ dp/dtmax),peak pressure-fall rate (-dp/dt max),left ventricular systolic pressure(LVSP),LV diastolic pressure(LVDP)were measured by cardiac catheterization.The circumferential strain at the levels of base (CSRA),papillary (CSPM) and cardiac apex (CSAP) were measured,and the parameter of myocardial clasticity at the levels of base (EBA),papillary (EPM) and cardiac apex (EAP) were measured and calculated,respectively.Results ①After pacing,the + dp/dt and LVDP were increased,but the-dp/dtmax was decreased (P <0.05).② After pacing,the CSPM and CSAP were all decreased,but EBA,EPM,EA were all increased(P <0.05).③ The areas under the receiver operating characteristic curves (ROC) of CSAP,CSPM,EAP,EPM and EBA were all 1,and the area under the curve of CSBA was 0.687.EAB,EPM and EAP were significant negative correlation with CSPM and CSAP,but are positive correlated with A/E and LVDP (P < 0.05).Conclusions The parameter of myocardial elasticity derived from circumferential strain includes useful information about myocardial deformation and function.The new parameter can be applied to assess the diastolic function of left ventricle.
5.Research of the course system construction of pharmacy administration for the undergraduate

6.Significance of new parameters in evaluation the function of carotid artery

Objective To evaluate the parameters of inner diameter different rate (D%), mean wave intensity(MWl) and diameter-adjusted mean wave intension(DAMWI) in assessing the function of common carotid artery(CCA) by quality intima-media thickness(QIMT) technique. Methods Twenty-four healthy subjects were performed 2D and CDFI examination under the normal quiescent and cold pressor test(CPT) condition respectively. The diastolic and systolic diameters of left-CCA were measured by QIMT respectively. Corresponding the inner diameter different range,rate,MWI and DAMWI were calculated respectively. The curves of their ROC were drawn. Results The different range and rate of diameters were decreased when subjects suffered CPT (P< 0.05) ,but the MWI and DAMWI were increased (P<0. 05). The areas under the ROC curves were 0. 827,0. 821 and 0.728 corresponding DAMWI, MWl and different rate of diameter. There was no significant difference among them(P>0. 05). Conclusions Conclusions The changed rate of diameter can be measured exactly by QIMT. The function of CCA can Be evaluated by MW1 and DAMWI which were calculated by conventional parameters.
7.Study of effects of the thoracic pressure on the velocity of tricuspid regurgitation and estimation of pulmonary artery systolic pressure

Objective To observe the effects and the mechanism of the thoracic pressure variation on the velocity of tricuspid regurgitation in quiet respiration, to accurately estimate the pulmonary artery systolic pressure,and to verify the new proposed mechanism of respiration effects on hemodynamics using echocardiography. Methods Continuous-wave Doppler spectra of tricuspid regurgitation were recorded with Sequoia 512 in 50 patients who had stable tricuspid regurgitation. Electrocardiogram and respiratory tracing were recorded simultaneously. The velocities and the pressure gradients were recorded during different respiration phases for analyzing. Results Respiration reliably affected on tricuspid regurgitant velocity. There were three patterns of manifestation:①The velocity increased in the inspiratory phase. ②The velocity decreased in the inspiratory phase. ③The velocity changed randomly. Conclusions Thoracic pressure variations had definite effects on the velocity of tricuspid regurgitation and tricuspid gradient. This research indicated that to obtain accurate and reliable Doppler echocardiographic measurement of the pulmonary artery systolic pressure, the measurement should be at the time between inspiration and expiration.
8.Research in deformation and motion of interventricular septum by velocity vector imaging combining with radius of curvature and length of arc

Objective Radius of curvature (RC) and velocity vector imaging (VVI) were joined to evaluate the deformation and motion of interventricular septum(IVS). Methods VVI was performed in fifteen healthy volunteers and obtained dynamic images at chordae tendineae of mitral valve in short axis view. The images were stored and analyzed to obtain circumferential peak stain of IVS. In the same short axis view, the static images were captured at end-systole and end-diastole respectively before they were analyzed by self-made software. The length of arc and RC were measured automatically by the software, so the shortening rates of them could were calculated. Results The length of arc and RC were longer at end-diastole than those at end-systole(P<0.0001). The shortening rate of length of arc was compared with circumferential peak stain of IVS (P=0.123) and they were correlation (r = 0. 840, P = 0. 0001 ). Blant-Altman analysis indicated that the concordance was well. Conclusions The length of arc and RC can evaluate the deformation and motion of IVS.
9.A new way to study the motion of interventricular septum by echocardiography

Objective To evaluate the effect of normal respiration on motion and disfiguration of interventricular septum (IVS) by observing the change of interventricular septum radius of curvature (IVSRC). Methods The 2D short-axis images at chordae tendineae of mitral valve were recorded during inspiration and expiration respectively by Acuson Sequoia 512 in 21 normal subjects. Speed of blood flow of each valve, electrocardiogram and respiratory tracing were recorded simultaneously. Two doctors who were double-blind measured the IVSRC using self-designed software. Results During normal inspiration, the peak E velocity of mitral valve and the flow velocity of aortic valve decreased,while the peak E velocity of tricuspid valve and the flow velocity of pulmonary valve increased. The IVSRC curvatures were (25.914± 6.082)mm during inspiration, and (22.296±4.398)mm during expiration, there was difference in two respiration phases(P <0.0001). Conclusions IVSRC can be used to evaluate motion and disfiguration of IVS. It has a good repeatability and is an effective way to observe the change of pleural pressure which is produced by respiration.
