1.Research on levels of IL-17,IL-4 and IFN-γ in induced sputum from pediatric patients with asthma

Objective To investigate the production of IL-17,IL-4 and IFN-γ in sputum of pediatric patients with asthma.Methods 61 pediatric patients with asthma in stage of exacerbation were enrolled as exacerbation group.and these patients in remission stage after therapy were enrolled as remission group, and 45 healthy children were chose as control group.The percentage of neutrophils and eosinophils were counted in regular way with sputum pigmentation,and the level of IL-17,IL-4 and IFN-γ were detected by enzyme-linked immunosorbent assay.Results IL-17 in exacerbation group Was highest among three groups, and IL-17 in remission group was higher than that in control group.In exacerbation group,IL-4 was higher and IFN-γ was lower than those in other two groups,and no significant difference Was found between remission group and control group.While in exacerbation stage,IL-17 was positive correlated with IL-4,and IFN-γ was negative correlated with IL-17 and IL-4.Conclusions IL-17 and IL-4 are related to the occurrence and exasperating of asthma,and IL-17 is suit for indicating the inflammation of asthma.IFN-γ is a protective factor of asthma.
2.The effects of shade and light irradiation on the polymerization of composite resin core materials

Objective:To evaluate the effects of shade and light irradiation condition on the polymerization of composite resin core material.Methods:The light-proof silicon rubber cuboid mold with one end open were stringed and filled by dual-cured flowable composite resin core material with the shade of dentine and white(Paracore) respectively,then the open end of mold was irradiated directly by a light unit at 1 000 mW/cm2 ×20 s and 3 200 mW/cm2 ×6 s respectively.And all specimens of 4 groups(n =5) were stored in the light-proof box.1 h and 24 h after irradiation,Knoop microhardness was measured along the vertical surfaces of the specimens at the depth of 1,2 and 3 mm respectively.Data were analyzed by One-way ANOVA.Results:At all light irradiation conditions,the hardness of all specimens decreased with the increase of measurement depth.24 h after irradiation,the hardness of dentine shade specimens at each measurement depth showed no difference between 1 000 mW/cm2 × 20 s and 3 200 mW/cm2 × 6 s irradiation,while the white shade specimens irradiated by 1 000 mW/cm2 × 20 s showed higher hardness than those by 3 200 mW/cm2 × 6 s.Conclusion:To obtain sufficient polymerization of composite core material with different shade optimal light irradiation condition should be selected.
3.Effect of FeixianFang on MMP-1,MMP-2 and TIMP-1,TIMP-2 in rats model of pulmonary fibrosis induced by bleocin

Objective: To observe the effect of FeixianFang on matrix rebuilding of pulmonary fibrosis in rats.Methods: Rats were randomly divided into the model group,the sham operation group,the FeixianFang group and the prednisone group.Pulmonary fibrosis model was set up by intratracheal injection of bleocin.On the 14th,28th and 45th day,MMP-1,MMP-2 and TIMP-1,TIMP-2 in the serum were detected.Results: FeixianFang can significantly reduce the level of MMP-1,MMP-2 and TIMP-1,TIMP-2 in the serum of rats with pulmonary fibrosis.Conculusion: FeixianFang can inhibit matrix rebuilding in pulmonary fibrosis rats.
4.Clinical research on unrelated cord blood transplantation treatment for mucopolysaccharidosis Ⅰ

Objective To explore the efficacy of unrelated cord blood transplantation treatment of mucopolysaccharidosis Ⅰ (MPS Ⅰ).Methods A 4-year-and-2-month-old boy with MPS Ⅰ who received treatment of human leucocyte antigen-mismatched unrelated cord blood stem cell transplantation after diagnosis was identified.The pre-treatment regimen was Busulfan + Cyclophosphamide + Fludarabine (Bu/Cy4 + Flud).Bu with the dosage of 1.2 mg/kg,once every 6 hours,4 days;Cy with the dosage of 50 mg/(kg · d) for 4 days and Flud with the dosage of 30 mg/(m2 · d) lasted for 4 days,respectively.The day that the graft was transplanted was defined as 0 day,days betore transplantation as negative days,days after transplantation as positive days.After pre-treatment,4.60 × 107/kg of cord blood nucleated cells and 3.05 × 105/kg CD34 positive cells were transplanted into the child.The combination of Antihuman thymocyte globulin,Cyclosporin A and Mycophenolate mofetil was administrated for prophylaxis of graft versus host disease(GVHD).After transplantation,the patient was given granulocyte colony stimulating factor to promote reconstitution of hematopoiesis.Results The myeloid and platelet engraftment time was respectively 15 days and 24 days after transplantation.Short tandem repeat (STR) DNA fingerprinting showed a full donor chimerism on day 21 after transplantation,and the full donor chimerism was stable afterwards.The peripheral-blood α-L-iduronidase (IDUA) activity returned to the normal value,and the IDUA gene sequencing did not demonstrate any mutation in 83 days after transplantation.On day 12 after transplantation,pulmonary infection with pulmonary hypertension occurred.Grade-Ⅱ acute intestinal GVHD occurred on day 15,Grade-Ⅱ acute cutaneous GVHD on day 51,and chronic GVHD (cutaneous,localized) on day 180.Otherwise,the patient complicated with hemorrhagic cystitis on day 35.These complications was cured favourably.In an 18-month-follow-up,the height of the boy increased by 3 cm,and his body weight had increased by 2.4 kg.His corneas regained clear,and his hepatosplenomegaly disappeared.The glycosaminoglycan of urine was negative.The neurocognitive performance of the boy had a little improvement.The abnormalities of fingers and other skeletons had no marked change.Conclusions Unrelated cord blood transplantation for MPS Ⅰ have definited effect.It is the first case report in China on treatment of MPS Ⅰ by unrelated cord blood transplantation.The researchers have accumulated some preliminary experience for future treatment of MPS Ⅰ by unrelated cord blood transplantation.
5.Determination of sildenafil citrate content by capillary zone electrophoresis

Objective: To establish capillary zone electrophoresis method for determination of sildenafil citrate (Viagra) content in its troche. Methods: Using tetrandrine as internal standard(IS), the electrophoretic separation was achieved with 25 mmol/L borate (pH=7.89) running buffer. And a voltage of 14 kV was applied to the 40 cm×75 μm(i.d) capillary. The analytes were introduced into capillary by siphon (1 s) and determined with on-column monitoring at 214 nm. Results:The determination could be completed within 4 min and the minimum concentration of detection was 5 μg/ml.The analytical results of sildenafil citrate samples demonstrated a good linear relationship within the range of 24-480 μg/ml.The relative standard deviations (RSD) of intra-day was less than 1.58% and that of inter-day was less than 2.46%.The present recoveries were between 95%-105%. Conclusion:The CZE method is accurate, simple, rapid and reliable, so it can be applied to the determination of sildenafil citrate content.
6.A rat model of pulmonary fibrosis induced by infusing bleomycin quickly through tracheal intubation

OBJECTIVE: To study the approach for developing a rat model of pulmonary fibrosis induced by bleomycin (BLM). METHOD: Different doses (7, 6, 5, 3.4, 2, 1 mg/kg) of bleomycin A5-saline were infused into the rats' lung in bleomycin-treated group through tracheal intubation, and rats in sham-operated group were infused with same volume of saline. The living state and lung pathology of the rats were observed. The author deeply studied the condition of the rats in 1 mg/kg bleomycin-treated group, and the changes of body weight and lung pathology were observed. Lung quotient, the content of transforming growth factor beta1(TGF-beta1) and platelet-derived growth factor (PDGF) in serum were measured on the 14th, 28th and 45th day of the experiment. RESULTS: The study demonstrated that infusing large doses of bleomycin A5 quickly through tracheal intubation had a high mortality, and infusing 1 mg/kg quickly could successfully develop an animal model of pulmonary fibrosis. Compared with the sham-operated group, fibrosis was appeared obviously in the rats' lung in 1 mg/kg bleomycin A5-treated group after 14 days of experiment, diffuse fibrosis was appeared after 28 days of experiment, and the fibrosis became more severe after 45 days of experiment. The body weight of the rats in bleomycin-treated group was declined after 3, 7 and 14 days of experiment as compared with the sham-operated group (P<0.01). Twenty-one days after the experiment, the body weight was declined too, but there was no significant difference between the bleomycin-treated group and the sham-operated group (P>0.05). Lung quotient was increased 14, 28 and 45 days after the experiment (P<0.01), the level of serum TGF-beta1 began to increase since 28 days after the experiment (P<0.05, P<0.01), and the level of serum PDGF also increased gradually 45 days after the experiment (P<0.05). And the mortality rate of 1 mg/kg bleomycin A5-treated group was lower than those of the other doses of bleomycin A5-treated groups. CONCLUSION: A rat model of pulmonary fibrosis can be duplicated successfully by infusing 1 mg/kg bleomycin A5 quickly through tracheal intubation.
7.Multi-slice Spiral CT Findings of Small Cholangiocarcinoma of Common Bile Duct

Objective To analyse the multi-slice spiral CT(MSCT) findings of small cholangiocarcinoma of common bile duct.Methods 15 cases with pathologically verified small cholangiocarcinoma of common bile duct were undergone unenhanced and three-phase contrast-enhanced MSCT scan.The entire morphologic changes of common bile duct were analysed with curved planar reformation(CPR).Results The attenuation of tumor relative to pancreas was iso-density in all cases at plain CT scan,hypo-density in 10 cases,iso-density in 3 cases and hyper-density in 2 cases at arterial phase,hypo-density in 1 case,iso-density in 3 cases and hyper-density in 11 cases at portal phase,iso-density in 5 caaes and hyper-density in 10 cases at delayed phase.The focal wall thickening of common bile duct appeared as circular or eccentric in 13 cases,intraluminal nodule in 2 cases,common bile duct was narrowing sharply in 11 cases and ending abruptly in 4 cases at obstructive level.Conclusion The small cholangiocarcinoma of common bile duct is of certain characteristics at unenhanced and three-phase contrast-enhanced CT scan.
8.Quantitative Analysis of Diosgenin in Jingangteng Soft Capsule by RP-HPLC

Objective To determine diosgenin content in Jingangteng soft capsule by RP-HPLC.Methods A RP-HPLC was performed on a Shim-Pak VP-ODS C18column(5?m,4.6 mm? 250 mm),the mobile phase consisting of acetonitri le-H2O(87∶13) and the detected wavelength at 209 nm.Results A good line arity was obtained in the range 0.0595~ 14.875 ? g(r=0.99999) for diosgenin. The average recovery and RSD were 99.4 % and 2.0 % respectively(n=5).Concl usion The method is simple,rapid,stable and reliable,and can be used to determine diosgenin content in Jingangteng soft capsule.
9.A study of the mobilization, collection and selection of autologous peripheral blood stem cells in patients with autoimmune diseases undergoing autologous hematopoietic stem cell transplantation in juvenile severe autoimmune disease

Objective To explore the safety of mobilization and collection as well as the feasibility of selection of autologous peripheral blood stem cells (auto-PBSC) from patients with juvenile severe autoimmune diseases (AID) for autologous hematopoietic stem cell transplantation (auto-HSCT). The clinical significance of these procedure is evaluated. Methods Eight patients with AID, including four patients with systemic lupus erythematosus(SLE),two patients with dermatomysoitis, one patient with juvenile rheumatoid arthritis (JRA), one patient with multiple sclerosis(MS),underwent auto-HSCT. Auto-PBSCs were mobilized in 8 patients using cyclophosphamide(CTX) and granulocyte colony-stimulating factor (G-CSF), and their PBSCs were collected by CS-3000 Blood Cell Separator, then the CD34+cells were selected and purified by CliniMACS CD34+cell selection device. The CD34+ cells were frozenand preserved under -80 ℃ ALL patients received non-myeloablative or lymphoablative conditioning regimens which consisted of CTX/Mel/ATG or CTX/ATG or BEAM/ATG. All patient received CD34+ cells transplantation. The safety of mobilization and collection process of auto-PBSC as well asthe feasibility of selection and purification of CD34+cells were recorded and hematopoietic reconstruction were evaluated. Results All patients tolerated the collection process well, and there was no mobilization-related mortality. The number of collected MNCs and CD34+ cells were 8.35×108/kg and 7.92×106/kg respectively. The number of CD34+ and CD3+ cells after purification was 6.28×106/kg and0.71 ×105/kg respectively. The mean granulocytes and platelet engraftment occurred on days 11 and 15 after G-CSF regimen, and they can be collected using CS-3000 instrument. PBSC mobilization and collection from patients with juvenile severe AID is safe. The CD34+ cell can be highly purified. The auto-PBSC CD34+cell transplantation is an alternative therapy for severe AIDs that do not respond to conventional treatments.
10.Observation in effect of different follow-up types on discharged patients with indwelling D-J stents

Objective To explore the effect of health education through different postoperative follow-up method, including telephone、Email and QQ, on patients with indwelling D-J stents. Methods 319patients with indwelling D-J stents were divided into the control group(88 cases), the telephone group(89 cases), the Email group (70 cases) and the QQ group (72 cases). M1 patients received rourine health education during hospitalization and before discharge, the latter three groups received follow-up by telephone、Email and QQ after discharge respectively. The rehabilitation effect was observed in the four groups. Results The complication rates of the telephone group, the Email group and the QQ group was significantly less and the mastering of knowledge about prevention and handling of complication was better than the control group during the follow-up. Conclusions Different types of follow-up can be selected by patients according to their actual status. Decreasing complication rate, favorable social benefit and approval of the patients family members will be seen due to involvement of the patients family in health education.
