1.Respiratory Tracts Inflammation Induced by Inhalable Particles From Different Sources

Objective To study the mechanism of respiratory tracts inflammation induced by the particles with an aerodynamic diameter of less than 10 ?m (PM10) and to observe the effect of high exposed group. Methods Particles were collected at kitchen, smoking-room, roadside and lake-side (the control group). Suspension of PM10 and rat models treated with PM10, kitchen oil smoke, cigarette smoke, road dust were established with a control group. On 22th day, the counts of total leukocyte and neutrophils in bronchoalveolar lavage fluid (BALF) and superoxide dismutase (SOD), glutathion peroxidase (GSH-Px), malondiadehyde (MDA), Cytokin-induced neutrophil chemotactics (CINC) in lung homogenate were measured and histopathological examinations were conducted on lung tissues. Objective To explore the upstream signal transduction pathway of neutrophil elastase(NE)-induced mucin(MUC)5AC gene expression.Methods A549 cells were either incubated with NE alone or with DMTU,aprotinin or AG1478.The level of MUC5AC mRNA was measured with RT-PCR.The activation of epidermal growth factor receptor(EGFR) and signaling were assessed by measuring the release of epidermal growth factor(EGF) and the phosphorylation of EGFR.Results NE increased MUC5AC gene expression accompanied by an increase of EGF and phosphorylated EGFR.DMTU,aprotinin and AG1478 significantly inhibited MUC5AC gene expression.DMTU and aprotinin significantly decreased the level of EGF and phosphorylated EGFR.ConclusionNE can induce MUC5AC gene expression via EGFR signalling in A549 cells.The upper stream involves oxidants,activation of tissue kallikrein and EGF.

Small RNA (miRNA) can regulate post-transcriptional level of mRNA. miRNA is closely related to the occurrence and development of a variety of human diseases such as gastric cancer, lung cancer, breast cancer, ovarian cancer, prostate cancer, pancreatic cancer, liver cancer, etc. Many kinds of miRNAs in human cancers are significantly abnomadly expressed. miRNA can be regarded as a marker for many human cancers diagnosis.
4.Effect of combination of tripterygium glycosides and valsartan on chronic glomerulonephritis complication with hypertension and effect on inflammatory cytokines

Objective To explore the clinical curative effect of combination of Tripterygium glycosides and Valsartan on chronic Glomerulonephritis complication with hypertension and the influence on inflammatory cytokines.Methods 90 cases of patients with chronic glomerulonephritis complication with hypertension in our hospital were selected from January 2015 to January 2016,To adopt randomized single blind controlled design methods,were divided into the treatment group Tripterygium glycosides combined with Valsartan,and the control group Tripterygium glycosides,each group were 45 cases,continuous medication for 24 weeks.Before and after treatment respectively observe the changes in blood pressure,24h urinary protein,endogenous creatinine clearance rate(Ccr),blood urea nitrogen(BUN),serum creatinine(Scr),glomerular filtration rate(GFR),inflammatory cytokines such as tumor necrosis factor-α,hypersensitive C-reactive protein,interleukin-6 as reference indicators.Results All the patients were observed for 24 weeks,no adverse effects were found.The effective rate of the treatment group was 95.55%,significantly higher than 75.55%of the control group(P<0.05).After treatment,systolic blood pressure(SBP),diastole blood pressure(DBP),24h urinary protein,SCr and BUN were all decrease,while Ccr and GFR were improved,and inflammatory cytokines such as tumor necrosis factor-α,hypersensitive C-reactive protein,interleukin-6 were reduced in the treatment group and the control group,the difference was statistically significant(P<0.05).Compared with the control group,blood pressure,24h urinary protein,Ccr, Scr,GFR,TNF-α、hs-CRP were improved in the treatment group(P<0.05).Conclusion Tripterygium glycosides combined with Valsartan treatment has good clinical curative effect,It can effectively relieve the symptom of chronic Glomerulonephritis complication with hypertension.
5.Risk factor analysis on failure of internal fixation of unstable femoral intertrochanteric fractures treated with PFNA in elderly patients

Objective To analyze the risk factors of failure of internal fixation of unstable femoral intertrochanteric fractures treated with proximal femoral nail anti-rotation (PFNA) in elderly patients,and to provide reference for clinical treatment.Methods Collected the clinical data of 251 aged and unstable femoral intertrochanteric fracture patients who have recieved PFNA treatment from January 2012 to December 2016.Preoperative clinical general data and intraoperative clinical factors were used as analysis variable to analyze the high risk factors and risk factors of internal fixation failure in PFNA treatment by χ2 test ,Wilcoxon rank sum test and Logistic regression.Results Through the analysis of 78 aged patients with internal fixation failure after PFNA treatment,the χ2 test and Wilcoxon rank sum test showed that,type of fractures,osteoporosis Singh index grade, tip-apex distance,application of hormone,basic diseases were risk factors for internal fixation failure after PFNA treatment (P<0.05).Among thses factors,type of fractures,osteoporosis Singh index grade, tip-apex distance were independent high risk factors for internal fixation failure after PFNA treatment(P<0.05).Conclusion Severe type of fractures (type Ⅲ and Ⅳ),osteoporosis of grade 4 to 6,and tip-apex distance greater than 25 mm are high risk factors of failure of internal fixation of unstable femoral intertrochanteric fractures treated with PFNA,and it is better to give more attention and effective intervention or other treatment.

7.Arterial Switch Operation in Older Infants With Severe Pulmonary Hypertension

Objective To introduce experience of switch operation applied to treat the transposition of the great arteries(TGA) and Taussing-Bing deformity.Methods Between June,2000 and Aug,2002, 27 consecutive patients underwent an arterial switch operation at our institution. The patients including TGA with intact ventricular septum in 5, TGA with ventricular septal defect in 18, Taussing-Bing deformity in 3, and corrected TGA in 1.Age ranged from 3 days to 6 years at operation (mean, 10 1?5 7 months), and the mean body weight was 6 3?2 81kg. Twenty patients were older than age 1 month. Ninteen patients had pre-operative catheterization. Seventy-four percents had severe pulmonary hypertention. Two patients had left ventricular outlet stenosis. Coronary type A distribution was recognized in 23 cases,type D in 4, and one of them had the origin of the left descending artery tunneled in the aortic wall. The great arteries were side by side in 3 cases. One patient underwent balloon atrial septostomy and another one underwent pulmonary banding and systemic to pulmonary shunt preoperatively. The great arteries were transected above the valvular commisures,the coronary ostia with all the adjacent sinus of Valsalva were excised and re-implanted to the proximal neo-aorta,then aortic anastomosis was carried out.The proximal neo-pulmonary trunk was reconstructed with a large autologous native pericardium as a posterior patch.The pulmonary anastomosis was completed,after the aortic cross clamp was released.The VSDs were repaired through the atrium or proximal aorta with Dacron patches.Results The hospital mortality was 7 4% (2 cases), and no death cases were directly related to any coronary artery problem. One perioperative death was a 5 day-old neonate with TGA and an intact septum who had refractory hypotension, hypoxemia, and acidosis preoperatively who underwent an emergency operation. The patient had a refractory low cardiac output syndrome postoperatively, and died after 20 hours. Another patient had a chylothorax and died of allergy from iodophor 22 days postoperation. The pulmonary pressure had gone down significantly in 20 patients who had severe pulmonary hypertension preoperatively (the mean pressure 46.7mmHg preoperation, and 31.3mmHg postoperation). Follow-up of 1 to 26 months was achieved in all survivors, with no late complications and death. Conclusions The arterial switch procedure for age over 1 month infants with severe pulmonary hypertention still has satisfactory efficacy.
8.Treatment of Unstable Intertrochanteric Fractures in the Eldly with the Prosthetic Artificial Fernord Head

Objective To explore the effect and methods of hip prosthesic replacement of comminuted interochanteric fracture of femur in the eldly.Methods We adopted bone cement hip prosthesic replacements to treat interochanteric fracture of femur in the the eldly.There were 34 cases,with male 11 cases and female 23 cases,aged 74 ~90years old.The fracture was classified according to Evans,with Ⅰa 4 cases,EvansⅠb 12 cases,EvansⅠc 7 cases,EvansⅠd 8cases and EvansⅡ 3 cases.Results All 34 cases got operation successfuly,with the follow-up time of 7 to 48 months postoperatively for all 34 cases.The average hospital stay of the patients was 21 days.The average time to ambulation was 13 days.The incidence rate of the hip joint was 10 percent.Conclusion Bone cement twin exceedingly prosthesic replacement for interochanteric fracture in the the eldly has advmtages of early full weight-bearing and rapid rehabilitation.
9.Effect of ERCP and endoscopic treatment on the prognosis of acute pancreatitis

10.Ultrasound-guided percutaneous microwave ablation of lung cancer

Objective To evaluate the efficacy and feasibility of ultrasound-guided percutaneous microwave ablation for peripheral lung cancer. Methods Seventeen patients with peripheral lung cancer (23 nodules) were treated with ultrasound-guided microwave ablation. The microwave energy was initially set at 60 W for 100s, and then at 30~40 W for 300~600 s. The treatment was conducted by using multiple simultaneously-powered antennas in lesions ≥3 cm in diameter. Results After the treatment, all the nodules were decreased in size and the blood flow signals inside the nodule disappeared (14 nodules) or weakened (9 nodules). Enhanced CT scanning revealed no enhancement in 16 nodules and partial enhancement in 7. Repeated biopsy in 4 patients showed a complete tumor necrosis. Clinical symptoms disappeared in 8 patients and subsided in 9 patients 1 month after treatment. Follow-up observations in 15 patients for 6~47 months (mean, 23 months) showed 9 survivors, without serious complications. Conclusions Ultrasound-guided percutaneous microwave ablation is an effective, safe, and feasible method for treating peripheral lung cancer.
