1.Ankle Plantar and Dorsal Flexor Muscles Strength in Healthy Young Adults

The purposes of this study were: (1) to test healthy young adults' ankle plantar and dorsal flexor muscles strength, providing integrate data and scientific basis for sports training, sports injury and recovery. (2) to investigate the biomechanical rule of healthy adults' ankle plantar and dorsal flexor muscles. Methods: 70 college students (35males and 35 females) volunteerily took part in the test. An advanced Cybex-6000 dynamometer was employed. This test consisted of two sections, isokinetic concentric (CON) test and isometric test. Results: males and females adults' ankle dorsal flexor isometric peak torque (PT) increased and plantar flexor isometric PT decreased with the increasing of testing angle. CON PT decreased while the test angle velocities increased. Plantar flexor PT were higher than corresponding dorsal flexor in two tests ( P
2.Comparison of sedative effects of different fractions from patrinia scabiosaefolia fisch in mice

Objective To investigate the sedative effects of different fractions of patrinia scabiosaefolia fisch. in mice and provide some valuable information for isolating components with sedative effect.Methods Petroleum, acetic ether and n-butanol fraction was isolated from the patrinia scabiosaefolia fisch.. The sedative effects were determined with locomotor activity assay, hole board test and sodium thiopental-induced sleeping time assay, and it was compared with the effects of total 70% ethanol extract. Results N-butanol fraction showed better sedative effect compared with others, and it was in a dose-dependent manner. Conclusion The results showed that the main active principles of patrinia scabiosaefolia fisch. were the n-butanol fraction, and the promising active chemical might be obtained from this fraction.
3.Contrast research of MRI,CT signals of pathological changes of ligamentum flavum

[Objective]To analyze the MRI and CT characteristics of the pathological changes of ligamentum flavum.[Method]Clinical data of MRI and CT characteristics of 48 cases of lagamentum flavum pathological changes were analyzed and compared retrospectively.[Result]MRI showed"\"signal in sagital position and"V"signal from the vertebral canal in stalk postition.CT showed two sides of soft tissue signal of vertebral canal and some part presented bone substance density.[Conclusion]The MRI,CT images of the ligamentum flavum pathological changes have both advantages and disadvantages.It would benefit the clinical therapy only with the combination of them.
4.Primary clinical application of new pterygo-shape titanium mesh cage in surgical procedure of cervical spondylosis

[Objective]To apply interal fixation with new pterygo-shape titanium mesh cage in the treatment of degenarative cervical spondylosis.[Method]Twenty-seven patients of degenarative cervical spondylosis were reconstructed after vertecotomy with the cage including 19 cases of CDH,8 cases of OPLL,18 cases in male and 9 cases in female;single segment degenaration were 21 cases,double segment degenaration were 6 cases.All cases were excluded fracture,dislocate by X-ray and MRI preoperatly.[Result]All 27 patients were followed up for an average time of 10 months(from 6 to 14 months).The anverage time of bone graft fusion was 10 weeks.[Conclusion]The pterygo_shape titanium mesh cage can provide immediate biomechanical stability and prevent the mesh cage displace,subsidence,exodus after operation.The surgical procedure is simple,safety and without illic-bone graft,meanwhile can greatly relieve patients' economy burden.Then the internal fixation is elastic fixation,the bone fusion shows a satisfactory result.
5.Stent implantation through butt joint of PTBD and ERCP for the treatment of obstructive jaundice

Objective To study the technique and to evaluate the clinical value of stent implantation by butt joint of percutaneous transhepatic biliary drainage (PTBD) and endoscopic retrograde cholangio-pancreatography (ERCP) for the treatment of obstructive jaundice. Methods 24 patients with obstructive jaundice were treated by butt joint of PTBD and ERCP after the ERCP failure, and the clinical results were evaluated. Results A total of 24 cases with obstructive jaundice were successfully operated by butt joint of PTBD and ERCP. In PTBD, 10 cases were punctured from right side, and 14 cases were from left side. PTBD, ERCP and butt joint were done simultaneously in 16 cases, and separately in 8 cases. The total serum bilirubin decreased by 47.07 in 4 days, and the direct bilirubin decreased by 45.43. Infection of biliary tract was the main complication. Conclusion Stent implantation by butt joint of PTBD and ERCP for the treatment of obstructive jaundice may be chosen to treat the patients who failed in ERCP alone.
6.Role of endoscopic color Doppler ultrasonography in the preoperative localization of insulinomas

Objective To investigate the role of endoscopic color Doppler ultrasonography (ECDUS) in the preoperative localization of insulinomas. Methods Seven patientsd woman) with biochemically diagnosed insulinoma underwent abdominal US, spiral CT and ECDUS in an attempt to precisely localize the tumor before surgery. Surgical and pathological results were considered as the definite localization of tumor. The role of ECDUS in the preoperative localization of insulinomas was compared with that of abdominal US and spiral CT. Results Ten tumors were detected by surgery and pathology. The sensitivity of ECDUS in identifying insulinomas was 8/10 compared with 1/10 for CT and 0/10 for abdominal US. Two tumors not detected by ECDUS had a size below 0. 5cm. Conclusions ECDUS is superior to spiral CT or abdominal US, and should be served as the first choice in the detection of pancreatic isulinomas. ECDUS identification depends on the size of the tumor and 'is hard to detect the tumors smaller than 0. 5cm.
7.The Current Situation of Hospice Care and Influencing Factors in 235 Cases

Objective:To survey the level of hospice care and the influencing factors in Shaanxi province. Methods:Interview and return visit method was used to investigate. Results:In total 235 patients, 37. 02% patients received hos-pice care. There is not significant difference between hospice users and no hospice users in sex and geographic differ-ences. There was significant difference between hospice users and no hospice users in personal ages, and in gap between urban and rural areas. and in medical payment, and in basic lesions. Malignant tumor patients receive hospice care most-ly. No patient received hospice care patients with acute trauma. The effect of ages on hospice is maximum ( OR =2. 877). Basic lesions is an important cause of influence whether patients receiving hospice care services(OR=1. 569). Conclusion:The level of hospice care is low in Shaanxi. The age and basic lesions is influencing factors of hospice care. The geographic differences has no effect on hospice care.
8.Clinical Efficacy of Acupuncture for Insomnia and Its Effect on 5-HT

ObjectiveTo explore the value of acupuncture in treating insomnia due to stagnated liverqi transforming into fire and its theoretical foundation, by observing the treatment effect of acupuncture in treating insomnia due to stagnated liverqi transforming into fire and its influence on 5-hydroxy tryptamine (5-HT) content.MethodTotally 187patients with insomnia due to stagnated liver qi transforming into fire were selected and randomized into two groups. The 93 cases in the control group were intervened by orally taking Trazodone, 100 mg for each dose, twice a day, 10 d as a course, totallyfor 2 courses; the 94 cases in the observation group were intervened by acupuncture at Shenmen (HT7), Baihui (GV20), Yintang (GV29), Hegu (LI 4), and Taichong (LR 3), once a day, 10 d as a treatment course, totally for 2 courses. Peripheral blood was collected from the two groups before and after intervention to detect the change of 5-HT content. Meanwhile, the Pittsburgh Sleep Quality Index (PSQI), Hamilton Depression Scale (HAMD, 24-item version), Asberg Rating Scale for Side Effects (SERS), and clinicalefficacies were assessed.ResultThe difference in 5-HT content between the two groups was statistically insignificant before intervention (P>0.05); 5-HT contents were significantly increased in both groups after intervention (P<0.05), while the content in the observation group was markedly higher than that in the control group (P<0.05); after treatment, the PSQI and HAMD scores were decreasedin both groups (P<0.05), while the PSQI, HAMD, and SERS scores in the observation group were markedly lower than those in the control group (P<0.05);the total effective rate was 92.6% in the observation group, significantly higher than 81,7% in the control group (P<0.05). ConclusionAcupuncture can significantly down-regulate 5-HT content in insomnia patients, and its total effective rate is higher than that of medication.
9.Expressions of neurotransmitters in patients of insomnia differentiated as liver stagnation transforming into fire treated with acupuncture.

OBJECTIVETo compare the difference in the efficacy between acupuncture and oral administration of trazodone and the expressions of neurotransmitters in patients of insomnia differentiated as liver stagnation transforming into fire.
METHODSSeventy patients of insomnia differentiated as liver stagnation transforming into fire were randomized into an observation group and a control group, 35 cases in each one. In the observation group, acupuncture therapy was adopted at Shenmen (HT 7), Baihui (GV 20), Yintang (GV 29), Hegu (LI 4), Taichong (LR 3), etc. The needles were retained for 20 min each time. The treatment was given once a day, the treatment of 2 weeks made one session. In the control group, trazodone, 100 mg, oral administration, once a day, the treatment of 2 weeks made one session. Two sessions were required in the two groups. The scores in Pittsburgh sleep quality index (PSQI) and Asberg rating scale for side effects (SERS), the levels of neurotransmitters such as 5-hydroxy tryptamine (5-HT) and norepinephrine (NE) and the expressions of protein kinase C (PKC) and brain-derived neurotrophic factor (BDNF) in peripheral blood were observed before and after treatment in the two groups.
RESULTSPSQI score and SERS score after treatment were all decreased compared with those in both groups before treatment (both P<0. 05). After treatment, PSQI score and SERS score in the observation group were lower apparently than those in the control group (both P<0. 05). After treatment NE content and PKC level were decreased; 5-HT content and BDNF mRNA were increased compared with those in both groups before treatment (all P<0. 05). NE content and PKC level in the observation group were lower apparently than those in the control group (both P<0. 05). The serum 5-HT content and BDNF mRNA expression in the observation group were higher than those in the control group separately (both P<0. 05).
CONCLUSIONAcupuncture therapy improves the sleeping quality of patients of insomnia differentiated as liver stagnation transforming into fire, and reduces serum NE level and increases 5-HT content and BDNF expression, which achieves the better efficacy as compared with the oral administration of trazodone. It is one of the effective approaches to the treatment of insomnia differentiated as liver stagnation transforming into fire.

10.Application of Pemberton's osteotomy in the treatment of congenital hip dislocation of chidren

[Objective]To summarize clinical experience of Pemberton's ilium osteotomy in developing hip dislocation and emphasize the combined and individual principles in the clinical practice. [Methods]From February 1996 to April 2007,26 patients(aged 3 to 10 years) with 28 hips were operated on together with soft tissue solution,a pericapsular osteotomy of the ilium (Pemberton) and an optional femoral rotational,shortening or varus osteotomy of the proximal femur. Pre-and post-operative indexes including those of AI angle,CE angle and deep of acetabulum were compared. According to Severin's classification and McKay clinical curative effect evaluation critera,these indexes were evaluated.[Results]According to the Severin's classification,the results were rated as 64.3% excellent,28.6% good and 7.1% fair.On the McKay's critera ,the functional results were graded as 50.0% excellent,35.8% good,7.1% fair and 7.1% poor. On the whole,the radiographic evaluation was superior to that of the functional results.[Conclusion]The Pemberton's osteotomy is safe and effective.The combined surgical treatment can improve the success rate and decrease complications.
