Objective To explore the clinical effect of blood perfusion combined with penehyclidine hydrochloride injection in the treatment of acute severe organophosphorus pesticide poisoning.Methods 58 patients with severe organophosphorus pesticide poisoning were selected as the research subjects.By using digital table method,they were randomly divided into observation group and control group,29 cases in each group.The two groups were given conventional treatment and blood perfusion treatment after admission,the observation group was treated with penehyclidine hydrochloride injection on this base.Before and after treatment,the myocardial enzymes,liver,kidney function index,blood gas analysis,cholinesterase activity,as well as other treatment of the two groups were observed,and the results were compared.Results Compared with the control group,after treatment,the myocardial enzyme,transaminase,serum amylase and creatinine levels in the observation group were significantly lower,cholinesterase level was significantly increased,the oxygen partial pressure increased,the partial pressure of carbon dioxide decreased obviously,the differences were statistically significant(all P<0.05).The hospitalization time and cholinesterase activity recovery time of the observation group were significantly shorter than those of the control group[(12.57±2.14)d vs.(16.35±2.73)d,(5.46±2.29)d vs.(7.61±3.47)d],and the cure rate of the observation group was significantly higher than that of the control group(100.00% vs.82.76%),the differences were statistically significant(t=2.139,2.208,x2=5.472,all P<0.05).Conclusion Blood perfusion can directly adsorb organophosphorus pesticide,shorten the recovery time of cholinesterase and reduce the case fatality rate,and it combined with penehyclidine hydrochloride injection not only can improve the clinical curative effect of blood perfusion,but also can protect the organ.

In recent year, enzyme, cytokine, glycoprotein, carcinoembrynoic antigen (CEA), prolactin and oncogene protein are studied more, among them CA15-3 and CEA have important clinical value. Looking for hematological test markers which have high sensitivity and speciality is important work.

Objective To clone the human mucin (MUC)5AC gene promoter and construct its luciferase reporter vector for human MUC5AC gene and analyze its transcriptional activity. Methods The 1 348 bp DNA sequence at the human MUC5AC gene 5 end was analyzed by the Vector NTI software.After the target sequence from human A549 cells genomic DNA was amplified by PCR method, and the product of PCR was sequenced.By promoter deletion analysis, 3 promoter segments with diferent lengths were amplified by PCR, then the products were identified by DNA sequencing, and 4 promotor segments were inserted into pGL3- enhancer vectors.Site-specific mutagenesis technique was used to establish mutants of specificity protein (SP)-l and nuclear factor-kappa B (NF-кB) site in MUC5AC gene promoter. The relative luciferase activities were detected in the transfected A549 cells. Results Sequence analysis indicated that there were many cis-acting elements in the regions of 1 348 bp DNA sequence at the human MUC5AC gene 5 end.The 4 reporter gene vectors with promoter segments with different lengths were constructed successfully.Dual-luciferase assay revealed the 372 bp fragment including activity with the minimal fragment. Neutrophil elastase (NE) could increase the expression of luciferase reporter gene plasmid containing mutated NF-кB version (P＜0.05 vs. contro1) of MUC5AC promoter in the transfected A549 cells. The induction by NE decreased markedly when the SP-l element in MUC5AC promoter were mutated. Conclusion This research may provide an important basis for the further study of human MUC5AC gene promoter activity and regulation of gene expression.There is an up-regulative element of gene transcription in the region of -324 to -64 bp in MUC5AC gene upstream. SP-l site of the promotor mediates NE-induced MUC5AC expression in human A549 cells.

Objective To determine the involvement of matrix metalloproteinase-9(MMP-9) in the signal transduction pathway of cigarette smoke-induced airway mucous hypersecretion. Methods The well cultured BEAS-2B airway epithelial cells were incubated with different concentrations of cigarette smoke extract (CSE),also some cells were preincubated with MMP-9 inhibitorⅠbefore CSE or exogenous epidermal growth factor(EGF) was added. The mucin(MUC)5AC mRNA level was analyzed by RT-PCR,MMP-9 and the phosphorylated EGFR(p-EGFR) protein production was assayed by western blot,MUC5AC protein and EGF protein were detected by enzyme-linked immunosorbent assay respectively,and gelatin zymography was used to determine the activity of MMP-9. Results Incubation of BEAS-2B cells with CSE up-regulated MUC5AC gene expression and protein production in a dose dependent manner (P0.05). Conclusion CSE stimulates the activity of MMP-9 and MMP-9 protein,causing the shedding of the EGF and leading to EGFR activation. This activation of EGFR downstream signaling pathway increases MUC5AC gene expression and mucin production.

OBJECTIVE:To evaluate the effects of loading-dose rosuvastatin on vascular endothelial function in patients with non-ST segment elevation acute coronary syndromes (NSTE-ACS) after early interventional therapy. METHODS:Totally of 128 NSTE-ACS patients underwent early interventional therapy were randomly divided into conventional dose group (63 cases) and loading dose group(65 cases). Before operation,all patients were given Clopidogrel sulfate tablets 300 mg and Aspirin enteric-coat-ed tablets 100 mg;on this basis,conventional dose group was given Rosuvastatin calcium tablets 10 mg orally;loading dose group was given Rosuvastatin calcium tablets 20 mg orally. After PCI,both groups were given Rosuvastatin calcium tablets 10 mg orally, qd,and Aspirin enteric-coated tablets(100 mg/d)and Clopidogrel sulfate tablets(75 mg/d),for consecutive 3 months. The blood samples were collected before surgery,8 h and 24 h after surgery. The serum levels of CK-MB,cTn T,hs-CRP,ET and NO were detected. The occurrence of major adverse cardiovascular events was recorded within 3 months after surgery. RESULTS:Compared with before surgery,the serum levels of CK-MB,cTn T,hs-CRP and ET were increased significantly 8 h and 24 h after surgery, while the level of NO was decreased,with statistical significance (P<0.05). However,the change range of loading dose group was lesser than that of conventional dose group,with statistical significance(P<0.05). There was no statistical significance in the incidence of adverse cardiovascular events between 2 groups(P>0.05). CONCLUSIONS:For NSTE-ACS patients underwent ear-ly interventional therapy,loading dose of rosuvastatin can protect the patients and inhibit the injury of vascular endothelial cell in-duced by the surgery.

The concrete methods and experiences for foreign students in case study in the past decades were introduced in this article. We emphasized on teaching students in accordance with their aptitude and step by step, explaining the profound things in a simple way and giving attention to different students' capabilities during teaching. In the practice, we surely felt that case study will bring more advantage than traditional teaching methods, motivate students and improve the capabilities to understand the course content.

BACKGROUND:Due to different experimental methods, the change of cytoskeleton proteins after centrifugal movement is stil controversial.
OBJECTIVE:To establish exhaustive eccentric exercise injury model in rats and to observe cytoskeletal vimentin protein expression at different time.
METHODS:A total of 48 male Sprague-Dawley rats were randomly and equal y divided into six groups:quiet control group, immediately after exercise group, and 12, 24, 48, 72 hours after exercise groups. In the exercise groups, the rats were subject treadmil exercise at the speed of 16 m/min in a-16° slope, for 100 minutes at the interval of 5 minutes. The quiet control group maintained unchanged, without exercise. The cytoskeletal vimentin was detected with immunohistochemical staining using anti-vimentin antibody, and vimentin expression changes were observed at different time after exhaustive eccentric exercise through the changes of target area percentage.
RESULTS AND CONCLUSION:The results of target area percentage showed that, there was no significant difference between quiet control group and immediately after exercise group (P>0.05). Compared with the immediately after exercise group, the target area percentage was slightly increased in 12 hours after exercise group, without significant difference (P>0.05). Compared with the 12 hours after exercise group, the target area percentage was also slightly increased in 24 hours after exercise group, without significant difference (P>0.05). Compared with quiet control group and immediately after exercise group, the target area percentage was increased in 24 hours exercise group (P<0.05). Compared with immediately and 12 hours after exercise groups, the target area percentage was increased significantly in 48 hours after exercise group (P<0.01). Compared with 48 hours after exercise group, the target area percentage began to decline in 72 hours after exercise group (P<0.05), which was higher than quiet control level. After exhaustive eccentric exercise, cytoskeletal vimentin begin to express at varying degrees. The vimentin expression is increased gradual y at 12 hours and reached the peak at 48 hours, then begins to decrease.
