1.The clinical study of orthotopic liver transplantation patients with severe sepsis

Objective To improve the treat effect of the orthotopic liver transplantation patients with severe sepsis. Methods Fif-ty-six post-surgery patients were enrolled in this study. Patients were divided into two groups. One was non-OLT group (A group) and the other was OLT group (B group). Besides general data, the surveillance of blood lactate, the number of failure organs, APACHE Ⅱand MODS were recorded. 28-days survival rate and follow-up were checked. Results The mortality of hospitalization in non-OLT group was 30% and 57.6% in OLT group. The level of blood lactate in OLT group at the 1 st day increased significantly, which was statistically differ-ent with that in non-OLT group (P <0.01). It decreased but kept higher than that in non-group in following seven days. The numbers of failure organs in OLT group were more than in non-OLT group (P <0.01). The continuous APACHE Ⅱ score had no significant difference between two groups. But the continuous MODS score in OLT group was higher than in non-OLT group (P <0.01). Conclusions The 28-days mortality of OLT with severe sepsis is almost two times as much as that of non-OLT. It should cause more attention. The OLT with se-vere sepsis is more likely suffered from failure organs and difficult to recovery. To assess the condition of failure organs in OLT patients with severe sepsis, MODS score is better than APACHE Ⅱ score in this study. It is suggested that the standard of score system could be improved or come up with new score for organ transplantation. It will be better if blood lactate score is included.
2.Determination of chlorogenic acid、paracetmol、Vitamin C and chlorphenamine maleate in Vitamin C Yinqiao Tablets by HPLC

AIM:To establish a method of determining the content of chlorogenic acid,paracetmol,vitamin C and chlorphenamine maleate in Vitamin C Yinqiao Tablets. METHODS: The separation was performed in the C_(18) colunm with the mobile phase of methanol-acetonitril-0.02% phosphate acid(5∶10∶85).The flow rate was(1.0 mL/min) with the wavelength at 219 nm and 310 nm,the temperature of column was 35 ℃. RESULTS: The calibration curves were linear in the ranges of 0.03-0.13 ?g for chlorogenic acid,0.94-4.68 ?g for paracetmol,0.49-2.44 ?g for Vitamin C,0.01-0.05 ?g for chlorphenamine maleate,the average recoveries were not less than 98%,respectively. CONCLUSION: The method is simple,rapid and with satisfactory results.It is suitable for quality control of Vitamin C Yinqiao Tablets.
3.Different effects of airway epithelial cells transfected by elafin on bacterial biofilm

Objective To explore the different effect of elafin incubated by different bacteria on P.aeruginosa(Pa) bioflim. Methods To cultivate the A549 cells in vitro, the pEGFP-N1-elafin eukaryotic expression vectors have been transfected to the cells by LipofectamineTM 2000. Elafin transfected cells incubated by the supernatant of S. epidermidis (S.epidermidis group), Pa (Pa group) and E.coli (E.coli group) respectively for 24 hours,the A549 cells were transfected. Then the levels of elafin were detected by ELISA and Western blot. To establish the Pa biofilm model in vitro,a rapid silver nitrate staining procedure and scanning electronic microscope (SEM)demonstrated bacterial biofilm. After biofilm carriers were put into each group and incubated for 8 hours, we measured the proportion of bacteria biofilm by silver nitrate staining and observed the structure of biofilm by SEM. Results The Pa and E.coli groups(especially Pa) raised the content of elafin in cells and the level of secretion increasing as compared to the normal group, while the S. epidermidis group had no change. Both silver nitrate staining procedure and SEM demonstrated the prestnce of bacterial biofilms. The the proportion of bacteria biofilm and the structure of BF in Pa and E.coli groups were changed, especially in Pa group. Conclusion There was a specificity for bacteria to induce the express of elafin. The inducing effect of Pa was more significant than that of E.coli.
4.Effect of ERCP and endoscopic treatment on the prognosis of acute pancreatitis

5.Periodical quality control and testing of medical equipment

To realize dynamic and standardized periodical quality control of medical equipment. A set of auto reminder module for medical equipment quality control was established by using the user-defined flow of medical equipment operation maintenance system, and the continuous improvement was realized for medical equipment quality control in some hospital. Dynamic medical equipment quality control was formed from the static one, so that the efficiency of medical equipment quality control was enhanced greatly. References were provided for other hospitals in dynamic and informatzied medical equipment quality control and testing.
6.Animal experimental study on Kissing pancreaticojejunostomy

Objective To evaluate the feasibility and safety of the modified panceaticojejunostomy called Kissing panceaticojejunostomy by animal experiment,and to explore the pathological process of primary healing between the pancreatic duct and the jejunal mucous.Methods The tolerance pressure at the anastomotic intestine was measured during surgery to ensure close contact suture of Kissing panceaticojejunostomy.Exploratory laparotomy,pancreatic ductography and pathology was performed at 2,4 weeks after panceaticojejunostomy.Results Kissing panceaticojejunostomy was performed successfully on 6 Beagle dogs.Tolerance pressure in all dogs was 90 cm H2O and above.No bleeding、fistula or contrast extravasation was found,and healing anastomosis was identified by laparotomy and pancreatic ductography.Cicatricial healing was found between pancreatic duct and jejunal mucosa 4 weeks after anastomosis.Conclusions Kissing panceaticojejunostomy is easy to perform,ensuring close-up cicatricial healing without hydrops or abscess between pancreatic duct and jejunal mucosa.
7.Covered retrievable metal stent in the treatment of refractory benign esophageal stricture

Objective To evaluate the efficacy and safety of the covered retrievable metal stent in the treatment of refractory benign esophageal stricture. Methods Six patients with refractory benign esophageal stricture who failed in endoscopic dilation were selected. There were 3 cases of stricture after caustic chemical ingestion, 2 cases of anastomotic stricture after surgery, and 1 case of re-stricture after metal stenting. The shape and diameter of the covered stent were individually designed. Stent was placed across the esophageal stricture. Symptoms after stenting, mucosal hyperplasia at the ends of stent and symptoms after retrieval of stent were followed up. Results Stent placement was performed successfully in all patients. After the stent placement, dysphagia was resolved, and all patients could consume soft food. Three to six months after stenting, all the stents were removed successfully by endoscopy. No mucosal hyperplasia and re-stricture occurred. In 4 cases, after stent had been removed, followed up for 2 to 12 months, symptoms of dysphagia were resolved persistently, and no further treatment was necessary. In the other 2 cases, the stent migrated, and dysphagia recurred within 1 month after removal of the stent. Besides one case of retrostemal pain after stenting, no other complication was noted. Conclusion Individually designed covered retrievable metal stent is a safe and effective way to treat refractory benign esophageal stricture.

9.Study of the drug sensitivity of primary cultured lung cancer cells and the expressions of four drug resistance related proteins

Objective To study the drug sensitivity of chemotherapeutic drugs by MTT method. Methods Twenty five lung cancer specimens obtained by operation were used for primary culture and the sensitivity of the cancer cells to four drugs, adriamycin(ADM), cis dichlorodiamine platinum(DDP), vincristine(VCR) and etoposide(VP16), was tested by MTT assay. TOPOⅡ, p53, bcl 2 and GST ? proteins in 10 cases of lung cancer tissue were determined by immunohistochemistry. Results The successfully obtained results in 21 cases showed that the effective rate of ADM, DDP, VCR and VP16 was 19.0%, 38.1%, 23.8% and 23.8% respectively. Positive expressions of TOPOⅡ, p53, bcl 2 and GST ? proteins were found in lung cancer tissue in the 10 cases which had different multi drug resistance(MDR) to the four drugs. Conclusion It is possible to test the drug sensitivity of the primary cultured lung cancer cells obtained from operation by MTT method and the results are useful for the selection of anticancer drugs. The expressions of TOPOⅡ, p53, bcl 2 and GST ? are factors to determine MDR for lung cancer cells.
10.The size of thyroid nodules contributing to the accuracy of fine-needle aspiration cytology

Objective To evaluate the association of the size of thyroid nodules and accuracy of fineneedle aspiration cytology in diagnose of thyroid nodules.Methods 691 thyroid nodules in 630 patients pathologically confirmed were retrospectively analyzed in our hospital.All imaging data of preoperative ultrasound-guided FNAC were collected in our review.Yields of FNAC were divided into six levels according to the classification criteria of the Bethesda system(level Ⅰ,insufficient material or nondiagnosed;level Ⅱ,benign ; level Ⅲ,atypical hyperplasia; level Ⅳ,follicular neoplasm ; level Ⅴ,suspicious for malignancy; level Ⅵ,malignant),>level Ⅳ was the malignant cytologic criteria for diagnosis of thyroid nodules.According to the maximal diameter of thyroid nodules,the nodules were divided into group A(L≤0.5 cm),group B(0.5 cm<L<1.0 cm) and group C(L≥1.0 cm).Postoperative pathologic results were taken as the gold standard.Results Of 691 nodules,there were 176(25.47%),298(43.13%) and 217(31.40%) in group A,group B and group C respectively.Among the three groups,accuracy of ultrasound-guided FNAC in group B (90.94 %) was higher than in group A(80.11%) and group C(83.41 %),with statistically significant(P <0.05).There was not statistically different between group A and group C(P >0.05).The specificity,positive predictive value and negative predictive value were not statistically different among three groups(P >0.05).Conclusions The size of thyroid nodules was partly associated with accuracy of ultrasound-guided FNAC.
